An Overview on Severe Acute Respiratory Syndrome(SARS) in WHO Clinical Reports (중증급성호흡기증후군(SARS)의 한약 치료에 관련된 WHO 임상논문 보고에 대한 개관)

  • Objective: In the winter of 2002, severe acute respiratory syndrome(SARS) began to spread throughout the world. More than 5,000 cases were reported in China, including over 1,700 cases in Hong Kong Special Administrative Region(Hong Kong SAR). The total number of cases reported from Canada and Singapore was more than 200. The total number of SARS cases world-wide reached 8,437 with incidences in 29 counties. Mortality from SARS is estimated at $10{\sim}12%$. When the SARS outbreak occurred in China, the State Administration of Traditional Chinese Medicine of China immediately initiated clinical research projects on the use of integrated herbal medicine and Western medicine for treating SARS. and, in Hong Kong SAR, research on the use of herbal medicine for the prevention and treatment of SARS. Reports were released during convalescence. The objective of this study is to overview twelve clinical SARS reports of WHO on the treatment of SARS with herbal medicine and evaluate the efficacy and safety of treatment of SARS with herbal medicine, and further to share experiences and knowledge of the treatment of SARS. Methods: Twelve clinical reports about SARS from the WHO were selected, overviewed and evaluated for efficacy and safety of treatments of SARS. Results and Conclusion: Twelve clinical reports about SARS showed that the integrated treatment may have advantages, and the advantages are reflected in the following findings: Firstly, herbal medicine is not targeted only at a specific etiology or a certain pathological link, but also at the pathological status of the patients at that particular time. Therefore, comprehensive readjustment was made through various angles, targets and channels to restore the balance of the body. Secondly, there are advantages in the differentiation of the disease and the treatment. Based on the various symptoms, herbal medicine enables the physician to adopt the most suitable principle, provide individual treatment, and to administer medicine in accordance with the actual process and nature of the illness. Thirdly, there are advantages in the results of the treatment; herbal medicine can relieve symptoms, promote absorption of lung inflammation, improve the degree of blood oxygen saturation, regulate immunological functions, reduce the required dosage of glucocorticoid and other Western medicines, and reduce case fatality rate, in addition to lowering the cost of treatment.

Efficacy of Wood Preservatives Formulated with Okara and Its Microscopic Analysis (두부비지 방부제의 방부효능 및 현미경적 분석)

  • As a substitute for CCA, which is inhibited due to its environmental pollution and human harmfulness, and CuAz and ACQ with a high cost, okara-based wood preservatives were formulated with okara hydrolyzates using copper sulfate and/or borax as a metal salt. The efficacy of the preservatives and X-ray microanalysis of wood specimens treated with the preservatives were examined to confirm the potential of the okara-based wood preservatives. Most of the preservatives showed excellent decay resistance against brown-rot fungi, Postia placenta and Gloeophyllum trabeum. The efficacy was improved when the acid concentration and temperature used for the hydrolysis of okara increased. In addition, when borax was added into copper sulfate/okara hydrolyzates preservative formulations, any decay was not found in the specimens. From the microscopic observation of the specimens treated with okara-based wood preservatives, it seems that okara is contributed to the fixing of metal salts in wood blocks. Therefore, it is speculated that okara-based wood preservatives can effectively protect wood against fungal attack as CuAz, and that the preservatives are sufficient to use as an alternative wood preservative of CCA, ACQ and CuAz.

Predictors of Intention to Undergo Mammography among Underutilizers (유방암 검진 미수검자의 검진의도 관련 요인)

  • Objectives: To identify the factors associated with the intention to undergo mammography among Korean women without a prior screening experience. Methods: Among 1,039 women of the general population, we selected 145 women (mean age: 54.2 years, age range : 40-78 years) without any prior experience with mammography. They were recruited for the 'Cancer Information Needs Assessment Survey' by using the method of random multi-stage cluster sampling. Data on the socio-demographic characteristics, intention to undergo mammography based on the Precaution Adoption Process Model, level of self belief and self efficacy for breast cancer screening, motivation for decision to undergo breast cancer screening were obtained by conducting a household survey. Results: Of the study subjects, 49.7% were classified as "unengaged" and "decided not to act" regarding breast cancer screening. Women with the intention to undergo mammography were more likely to be younger (OR 0.11, 95%CI 0.04-0.36), to have been recommended to undergo screening by others (OR 3.27, 95%CI 1.36-7.87), to have a high level of perceived sensitivity (OR 3.15, 95%CI 1.27-7.82), and to have a high level of self efficacy (OR 1.09, 95%CI 0.97-1.23). Exposure to campaigns and information regarding breast cancer screening, whether cancer patients are or not in around, perceived severity, perceived benefit, and perceived cost were factors that were not significantly associated with the intention to undergo mammography. Conclusion: It is necessary to develop tailored intervention strategies for women who have never undergone breast cancer screening on the basis of their demographic characteristics and factors that positively influence the intention to undergo mammography.

Modelling protection behaviour towards micronutrient deficiencies: Case of iodine biofortified vegetable legumes as health intervention for school-going children

  • BACKGROUND/OBJECTIVES: Despite successes recorded in combating iodine deficiency, more than 2 billion people are still at risk of iodine deficiency disorders. Rural landlocked and mountainous areas of developing countries are the hardest hit, hence the need to explore and advance novel strategies such as biofortification. SUBJECTS/METHODS: We evaluated adoption, purchase, and consumption of iodine biofortified vegetable legumes (IBVL) using the theory of protection motivations (PMT) integrated with an economic valuation technique. A total of 1,200 participants from three land-locked locations in East Africa were recruited via multi-stage cluster sampling, and data were collected using two, slightly distinct, questionnaires incorporating PMT constructs. The survey also elicited preferences for iodine biofortified foods when offered at a premium or discount. Determinants of protection motivations and preferences for iodine biofortified foods were assessed using path analysis modelling and two-limit Tobit regression, respectively. RESULTS: Knowledge of iodine, iodine-health link, salt iodization, and biofortification was very low, albeit lower at the household level. Iodine and biofortification were not recognized as nutrient and novel approaches, respectively. On the other hand, severity, fear, occupation, knowledge, iodine status, household composition, and self-efficacy predicted the intention to consume biofortified foods at the household level; only vulnerability, self-efficacy, and location were the most crucial elements at the school level. In addition, results demonstrated a positive willingness-to-pay a premium or acceptance of a lesser discount for biofortification. Furthermore, preference towards iodine biofortified foods was a function of protection motivations, severity, vulnerability, fear, response efficacy, response cost, knowledge, iodine status, gender, age. and household head. CONCLUSIONS: Results lend support for prevention of iodine deficiency in unprotected populations through biofortification; however 'threat' appraisal and socio-economic predictors are decisive in designing nutrition interventions and stimulating uptake of biofortification. In principle, the contribution is threefold: 1) Successful application of the integrated model to guide policy formulation; 2) Offer guidance to stakeholders to identify and tap niche markets; 3) stimulation of rural economic growth around school feeding programmes.

Concurrent Chemoradiation with Weekly Cisplatin for the Treatment of Head and Neck Cancers: an Institutional Study on Acute Toxicity and Response to Treatment

  • Background: Concurrent chemoradiation with three weekly high dose cisplatin is the non-surgical standard of care for the treatment of locally advanced head and neck cancers. Although this treatment regime is efficacious, it has high acute toxicity, which leads not only to increased treatment cost, but also to increased overall treatment time. Hence, the current study was undertaken to evaluate the acute toxicity and tumor response in head and neck cancer patients treated with concurrent chemoradiation using $40mg/m^2$ weekly cisplatin, which has been our institutional practice. Materials and Methods: This single institution retrospective study included data for 287 head and neck cancer patients treated with concurrent chemoradiation from 2012 to 2014. Results: The mean age of the patients was 48.8 years. The most common site of involvement was oral cavity. Most of the study patients presented with advanced stage disease. The mean overall treatment time was 56.9 days. Some 67.2% had overall complete response to treatment as documented till 90 days from the start of treatment. According to the Radiation Therapy Oncology Group (RTOG) acute radiation morbidity scoring criteria, mucositis was seen in 95.1% of the patients. Dermatitis and emesis were observed in 81.9% and 98.6%, respectively. Regarding haematological toxicity, 48.8% and 29.6% suffered from anaemia and leukopenia, respectively, during treatment. Acute kidney injury was assessed using the Common Terminology Criteria for Adverse Events (CTCAE), and was found in 18.8% of the patients. Conclusions: Concurrent chemoradiotherapy with weekly cisplatin is an effective treatment regime for head and neck cancers with reasonable toxicity which can be used in developing countries, where cost of treatment is so important.

Production of Recombinant Human Interleukin-11 (IL-11) in Transgenic Tobacco (Nicotiana tabacum) Plants

  • Interleukin-11 (IL-11) is a cytokine that plays a key regulatory role in the immune system. Recombinant human IL-11 (rhIL-11) exerts a preventative effect against apoptotic cell death and inhibits preadipocyte differentiation. IL-11 also is used to stimulate the bone marrow to produce platelets in order to prevent low platelets that may be caused by chemotherapy. Unfortunately, the high production cost of IL-11 associated. In this study, we investigated the feasibility of transgenic plants for the cost-effective production of rhIL-11. Production of rhIL-11 proteins in whole-plant expression system will be more economical when compared to the current E. coli based expression system. The human rhIL-11 gene was codon optimized to maximize plant host system expression. IL-11 expression vector under the control of a constitutive cauliflower mosaic virus 35S (CaMV 35S) promoter was introduced into tobacco by Agrobacterium-mediated transformation. The 5'-leader sequence (called ${\Omega}$) of tobacco mosaic virus (TMV) as a translational enhancer was added to construct. Transgenic tobacco plants expressing various levels of rhIL-11 protein were generated. Western blotting of the stably transformed lines demonstrated accumulation of the appropriately sized rhIL-11 protein in leaves. This research demonstrated the efficacy of using tobacco as an expression system for the production of rhIL-11.

Staple Line Coverage with a Polyglycolic Acid Patch and Fibrin Glue without Pleural Abrasion after Thoracoscopic Bullectomy for Primary Spontaneous Pneumothorax

  • Background: This study was conducted to determine the efficacy of staple line coverage using a polyglycolic acid patch and fibrin glue without pleural abrasion to prevent recurrent postoperative pneumothorax. Methods: A retrospective analysis was carried out of 116 operations performed between January 2011 and April 2013. During this period, staple lines were covered with a polyglycolic acid patch and fibrin glue in 58 cases (group A), while 58 cases underwent thoracoscopic bullectomy only (group B). Results: The median follow-up period was 33 months (range, 22 to 55 months). The duration of chest tube drainage was shorter in group A (group A $2.7{\pm}1.2day$ vs. group B $3.9{\pm}2.3day$, p=0.001). Prolonged postoperative air leakage occurred more frequently in group B than in group A (43% vs. 19%, p=0.005). The postoperative recurrence rate of pneumothorax was significantly lower in group A (8.6%) than in group B (24.1%) (p=0.043). The total cost of treatment during the follow-up period, including the cost for the treatment of postoperative recurrent pneumothorax, was not significantly different between the two groups (p=0.43). Conclusion: Without pleural abrasion, staple line coverage with a medium-sized polyglycolic acid patch and fibrin glue after thoracoscopic bullectomy for primary spontaneous pneumothorax is a useful technique that can reduce the duration of postoperative pleural drainage and the postoperative recurrence rate of pneumothorax.

The Relationship between Adoption of Innovation and Diffusing Intention for ICT Convergency Industry among Farmers (농업인의 혁신기술 수용 및 저항 요인과 농식품 ICT 융복합사업 확산의도와의 관계)

  • This study explored the effects of adoption of innovation and resistant attitude on farmers' perspectives and also was deigned for developing prompt spread of ICT and facilitating stronghold support center in the convergence agricultural field. To closely examine the variation for exploitation intent of stronghold support center in the ICT convergence agriculture, 110 farmers who had experienced introduction for ICT hybrid environmental control system were participated in the study using the questionnaire. The results revealed the several findings. Firstly, there was no significant differences between the self-efficacy such as confidence of application technology for farmers and adoption of innovative technique (p=.075). Secondly, suitability of using ICT hybrid environmental control system showed a significant difference in the adoption of innovation (p=2.750) while complexity of using ICT hybrid environmental control system indicated the negative effects on the adoption of innovation (p=3.591). Thirdly, introduction cost of ICT hybrid environmental control system showed the negative effects in the adoption of innovation (p=2.278), whereas adoption of innovation indicated a significant difference in the stronghold support center in the convergence agricultural field (p=5.500). Finally, resistance of ICT adoption of innovative technique revealed the negative effects on the stronghold support center in the ICT convergence agriculture. This study, therefore, demonstrated that educational assistance for acquiring ICT technique, suggestion of influences for productivity, and development for skills were needed to extend ICT convergence technology. Additionally, the study indicated the strategies related with promotion as well as ways of minimizing introduction cost.

  • Photovoltaic (PV) modules are environmentally friendly energy-conversion devices to generate electricity via the photovoltaic effect of semiconductors on solar energy. One of key elements in PV modules is "Backsheet," a multi-layered film to protect the devices from a variety of chemicals including water vapor. A representative Backsheet is composed of polyvinyl fluoride (PVF) and poly(ethylene terephthalate) (PET). PVF is relatively expensive, while showing excellent resistance to chemical attacks. Thus, it is necessary to develop alternatives which can lower its high production cost and guarantee lifetime applicable to practical PV modules at the same time. In this study, PET films with certain levels of crystallinity were utilized instead of PVF. Since it is well known that PET is suffering from trans-esterification and hydrolysis under a wide pH range, it is needed to understand decomposition behavior of the PET films under PV operation conditions. To evaluate their chemical decomposition behavior within a short period of times, accelerated decomposition test protocol is developed. Moreover, electrochemical long-term performances of the PV module employing the PET-based Backsheet are investigated to prove the efficacy of the proposed concept.

Review on Patient Outcomes Research (환자진료결과연구에 대한 고찰 -미국의 PORT 연구를 중심으로-)

  • Background : In this paper patient outcomes research is going to be reviewed and described, to be compared with relevant studies, and to consider the application in Korea. Methods : We compiled and reviewed the articles and materials related to patient outcomes research especially by PORT(Patient Outcomes Research Team) and rearranged them for seeking the main point and comparing with relevant studies. Results : Patient outcomes research emphasizes patient outcomes as well as conventional clinical outcomes. It is prospective study observing effectiveness in real situation instead of efficacy in ideal condition. Patient outcomes research comprises of 6 fields ; literature review and meta-analysis, use of claims data, decision modeling, outcomes assessment, cost of care, dissemination of research findings. SAA(small-area variations analysis), appropriateness study and clinical practice guideline are connected with patient outcomes research. Conclusion : In view of the fact that current medical policy in Korea is shifting its focus from accessibility to the improvement in quality and cost containment, and is stressing patient-based research, patient outcomes research is one direction for which the medicine is accountable and assessable. Considering that the number of patient receiving medical treatment in Korea is higher than the West, patient outcomes research has competitive power as against the West.
