Superior Orbital Fissure Syndrome after Repair of Blow Out Fracture (안와골절 정복술 후 발생한 상안와열증후군의 치험례)

  • Purpose: Superior orbital fissure syndrome is a rare neurological complex. Superior orbital fissure syndrome may result from a variety of inflammatory, infectious, neoplastic, iatrogenic, traumatic, vascular cause. The author report a patient who suffered from superior orbital fissure syndrome after inferior orbital wall reduction. Methods: A 26-year-old female suffered from inferior orbital wall fracture with inferior gaze limitation and orbital soft tissue herniation. On posttrauma 10 day, inferior orbital wall was reduced using endoscope and porous polyethylene ($Medpor^{(R)}$) was inserted. On immediate postoperation, she reported that extraocular movement was limited in almost any directions. She underwent exploration surgery to release the presence of extraocular muscle impingement. But, there was no observation of extraocular muscle impingement. On postoperative one day, high-dose steroid therapy was started to release superior orbital fissure syndrome which was defined in postoperative computed tomography. Results: After one month of high-dose steroid therapy, extraocular movement limitations improved progressively in all directions. In four months, extraocular movement recovered completely. Conclusion: Superior orbital fissure syndrome may occur after surgical procedure of orbital wall reduction. Prompt diagnosis and treatment with mega-dose corticosteroid is an effective option for avoiding disaster from compressive syndrome.

  • Retinal detachment is a rare disease in children but cases of serous retinal detachment due to use of systemic corticosteroids have been reported in and out country Rhematogenous retinal detachment has developed in a child with oral and steroid pulse therapy due to nephrotic syndrome. He was treated with laser and buckling in right and left eye respectively. Currently he is under observation through the out patient department. (J. Korean Soc Pediatr Nephrol 2001 ; 5 : 210-12)

  • The idiopathic hypereosinophilic syndrome consists of peripheral blood eosinophilia of $1500/mm^3$ or more without a known cause, plus signs and symptoms of organ eosinophilia. The prognosis of HES without treatment is poor. However, about one third of the patients with this syndrome may respond to corticosteroid thrapy. Morever, the majority of the remainder may have a favorable response to hydroxyurea. We present here a case of hypereosinophilic syndrome without any identifiable causes, involving bone marrow, liver, lungs and cervical lymph node. We tried corticosteroid as a treatment but it showed no response. However the hydroxyurea showed good response.

  • Fifty-one cases of acute herpes zoster infection were analyzed to search for the most effective management strategy of the disease, including pain relief and decreasing the incidence of postherpetic neuralgia. Anti-viral treatment was not included. Analgesics and nerve blocks, such as stellate ganglion block or epidural block, were helpful. Corticosteroid was administered in most of the cases either systemically or epidurally or both. Epidural administration of local anesthetic in combination with corticosteroid seemed to have certain advantages of excellent pain relief as a result of sympathetic blockade and regional plus systemic anti-inflammatory effects of the steroid, although this was not proved by definite statistical evidence.

  • Corticosteroid has been extensively used for the treatment of many medical diseases caused by immune and inflammatory response. And recently it becomes the first choice of treatment for bronchial asthma in a point of it's anti-inflammatory effects. However, this therapy has been associated with many well-known complications including truncal obesity, diabetes mellitus, excerbation of hypertension, delayed wound healing, easy bruisy, atropy of proximal muscles, psychotic symptoms, and/or osteoporosis. We report a case of patient with bronchial asthma who developed an uncommon side reaction, intractable hiccup persisting longer than 48 hours after treatment with oral corticosteroid.

  • Idiopathic thrombocytopenic purpura is an uncommon illness but most common form of thrombocytopenia in pregnancy. Corticosteroids, splenectomy, immunosuppressive drugs, and immunoglobulin therapy have been recommended for management. The optimal method of delivery is controversial. We have experienced a case of idiopathic thrombocytopenic purpura diagnosed previously and managed with corticosteroid and vincristine, which was followed by pregnancy, vaginal delivery and postpartum splenectomy.

  • Eosinophilic pleural effusion (EPE) is defined as a pleural effusion that contains at least 10% eosinophils. EPE occurs due to a variety of causes such as blood or air in the pleural space, infection, malignancy, or an autoimmune disease. Undifferentiated connective tissue disease (UCTD) associated with eosinophilic pleural effusion is a rare condition generally characterized by the presence of the signs and symptoms but not fulfilling the existing classification criteria. We report a case involving a 67-year-old man with UCTD and EPE, who has been successfully treated with a single intrapleural corticosteroid injection.

Study on Blood Cortisol in Patients treated with Oral Corticosteroid Drugs (경구 Corticosteroid계 약물 치료한 환자에 있어서 혈중 cortisol에 관한 연구)

  • This is an analysis of 32 patients who received long continuous corticosteroid drug due to some diseases. Patients were collected from June 1976 to March 1980. Blood cortisol level, variation of diurnal rhythm and side effects were studied. The Result as follows: 1. Side effects were observed in 24 patients(75%) and most common complaint was obesity. 2. Diurnal rhythm analysed by Doe's method shows abnormal diurnal rhythm is 21 out of 32(66%). 3. Mean durations or therapy of abnormal diurnal rhythm and normal diurnal rhythm were $55.7{\pm}4.4$ months and $43.9{\pm}7.0$ months respectively which shows statistically significant difference. 4. Mean cortisol value of steroid treated patients were lower than normal. 5. Reverse diurnal rhythm was 4 out of 21 patients. 6. 8 A.M. cortisol value is lower than 2 times of 8 P.M. in all patients who showed abnormal diurnal rhythm except one.

  • Bacterical tracheitis is a very rare complication in adults after endotracheal intubation. We report a case of bacterial tracheitis associated with endotracheal intubation and corticosteroids. The patient was discharged with a permanent tracheostomy, and a resection and an end-to-end anastomosis of the trachea is planned.

  • Objective : The purpose of this study is to know the effect of Korean medical treatments on sudden sensorineural hearing loss(SSNHL) without systemic or intratympanic steroid treatments.Method : We conducted retrospective review who was diagnosed as SSNHL and treated with Korean medicine in Dept. of Otolaryngology Kyung Hee University Hospital at Gangdong. There were two patients with SSNHL who refused to be treated with western medicine and were treated with Korean medicine including acupuncture, moxibustion, and herbal medicine.Result : One patient recovered completely after eight days of admission, and another patient partial recovered during 30 days through Korean medical treatments.Conclusion : Through these studies, SSNHL is expected to treated more effectively by conventional Korean medical treatments than corticosteroid treatments alone. But the additional study is needed to reveal the exact effects of Korean medical treatments.
