Kawasaki disease (KD) is an acute systemic vasculitis that predominantly affects children, and can result in coronary artery lesions (CAL). A patient with KD who is resistant to treatment with intravenous immunoglobulin (IVIG) has a higher risk of developing CAL. Incomplete KD has increased in prevalence in recent years, and is another risk factor for the development of CAL. Although the pathogenesis of KD remains unclear, there has been increasing evidence for the role of genetic susceptibility to the disease since it was discovered in 1967. We retrospectively reviewed previous genetic research for known susceptibility genes in the pathogenesis of KD, IVIG resistance, and the development of CAL. This review revealed numerous potential susceptibility genes including genetic polymorphisms of ITPKC, CASP3, the transforming growth factor-${\beta}$ signaling pathway, B lymphoid tyrosine kinase, FCGR2A, KCNN2, and other genes, an imbalance of Th17/Treg, and a range of suggested future treatment options. The results of genetic research may improve our understanding of the pathogenesis of KD, and aid in the discovery of new treatment modalities for high-risk patients with KD.
Familial hypercholesterolemia (FH) is a genetic disease that is not well known or diagnosed in Korea. This disease is associated with persistently high levels of low-density lipoprotein cholesterol (LDL-C), which increase the risk of coronary artery disease at a young age. Therefore, early diagnosis and treatment are important; however, there are no global consensus diagnostic criteria. In Korea, the Dutch Lipid Clinic Network diagnostic criteria, and the Simon Broome diagnostic criteria were used for diagnosis of FH according to the agreement announced at the Korean Society of Lipid and Atherosclerosis (KSoLA) in 2022. Recently, the absence of coronary artery calcification has been considered a good prognostic factor, even among patients with very high LDL-C levels who are considered to be at high risk for atherosclerotic cardiovascular disease. We describe throughout this paper the diagnosis and treatment of FH in a young male without coronary artery calcification.
This study was performed to examine the effect of excess vitamin A feeding on lipid profile and vitamin A content of rats. 12 male Sprague-Dawley rats. weighing 114.1$\pm$0.9g, were divided into control and excess vitamin A group, and fed experimental diet for 6 weeks. In the excess vitamin A group, the level of vitamin A was 400, 000 1.U./kg diet(100 times of standard level). It was observed that vitamin A excess group had higher levels of plasma total lipid, total cholesterol, and trighlyceride and lower HDL ratio, indicating a high risk to coronary heart disease. In the liver, total CHOL and TG content were also high, and fatty liver was produced by excess Vitamin A. Vitamin A contents of organs were higher when excess vitamin A was given. These results suggested that excess vitamin A could be a dietary factor increasing the lipid contents of plasma and liver. Therefore, it could be said that vitamin A megadose can be one of the risk factors to coronary heart disease.
Objectives: Dyslipidemia in diabetes mellitus is a significant risk factor for the development of cardiovascular complications. The aim of this study was to evaluate the effect of the ethyl-acetate fraction of an ethanolic extract from Streospermum suaveolens on lipid metabolism in streptozotocin (STZ)-induced diabetic rats. Methods: Diabetes was induced by intraperitonial injection of STZ (50 mg/kg). Diabetic rats were treated with an ethyl-acetate fraction orally at doses of 200 and 400 mg/kg daily for 14 days. On the $15^{th}$ day, serum lipid profiles, such as total cholesterol (TC), triglycerides (TG), low-density lipoprotein (LDL), and high-density lipoprotein (HDL), were estimated in experimental rats. The atherogenic (AI) and the coronary risk (CRI) indices were also evaluated. Results: The ethyl-acetate fraction at doses of 200 and 400 mg/kg significantly (P < 0.001) and dose-dependently reduced serum cholesterol, triglycerides and LDL, but increased HDL towards near normal levels as compared to diabetic control rats. The fraction also significantly (P < 0.001) lowered the atherogenic index (AI) and coronary risk index (CAI) in a dose-dependent manner. Conclusion: The present study demonstrated that the ethyl-acetate fraction of Stereospermum suaveolens exhibits a potent antihyperlipidemic activity in hyperglycemic rats and suggests that the plant may have therapeutic value in treating the diabetic complication of hyperlipidemia.
Purpose: The purpose of this study was to explore the knowledge and learning needs on cardiac rehabilitation of coronary artery bypass graft(CABG) patients. Method: The subjects consisted of 100 CABG patients at A hospital in Seoul. Data were collected by the two different kind of questionnaires which measure knowledge and learning needs on cardiac rehabilitation of CABG patients. The subjects responded the questionnaire on knowledge before CABG and that on learning needs before their discharge. Result: The mean score of knowledge on cardiac rehabilitation was 68.54. Knowledge on risk factor, nature of disease, diet, daily activity, medication, post operative care were great in order. The mean score of learning needs on cardiac rehabilitation was 4.28. Learning needs on diet, medication, nature of disease, post operative care, daily activity, risk factor were great in order. There were significant differences in knowledge according to occupation, economic status and family history(p=.021, p=.017, p=.023). There was a positive correlation between knowledge and learning needs(r=.3009, p=.002). Conclusion: Level of knowledge on cardiac rehabilitation of CABG patients is low and knowledge on postoperative care is the lowest, and learning needs are great in ail categories.
1. Objective The purpose of this study was to prove the correlation between risk factor of cardiovascular disease and sasang constitution. 2. Methods We analyzed the data of 200 patients who visited Cheonan oriental medical hospital from Apr 1, 2007 until September 31, 2007, for medical examination, which includes sasang constitution examination, blood chemistry test and physical examination. And we classified their results into 3 groups based on with NCEP-ATP III diagnostic criterions. 3. Results and Conclusion The results were as follows : 1. The frequency of dyslipoproteinemia(Hypertriglycemia) was significantly higher in Teaumin group than other constitutions. 2. The frequency of Metabolic syndrome was significantly higher in Taeumin group than other constitutions. 3. The risk of coronary heart disease was higher in Teaumin than other constitutions.
Purpose: The purpose of this study was to identify the impact that health motivation, stage of change and cardiac risk factors have on health behaviors in Korean patients with coronary artery disease(CAD) admitted to hospital. Method: The participants in this study were 127 patients with CAD hospitalized between May 2008 and July 2009. A structured questionnaire with personal interviews and chart reviews were used to collect data. Data were analyzed using the SPSS. Results: Of the relatively significant factors, including a stage of change, self efficacy, modifiable risk factor score, and perceived barriers, stage of change was found to be the most significant predictor of health behavior in patients with CAD. These variables accounted for 48.2% of the variance in health behavior. Conclusion: The results of the study indicate the necessity of developing a cardiac rehabilitation program for use in their daily lives after discharge from the hospital. Stage of change should be assessed for all patients with CAD being discharged from acute care hospitals.
A total of 40 patients having a diagnosis of atherosclerotic coronary arterial disease were analysed on the operative outcomes according to variables as follows: 1) preoperative risk factors such as age, sex, CCS (Canadian Cardiovascular Society) functional class, type of angina, number of diseased vessel, presence of left main coronary artery stenosis, previous history of habitual smoking and presence of other medical diseases (diabetes mellitus, essential hypertension), 2) preoperative management such as intravenous infusion of nitroglycerine, preoperative IABP (intra-aortic balloon pump) support and whether the operation was scheduled as emergency or not, 3) intraoperative variables such as infusion method and composition of cardioplegic solutions, number of distal anastomosis, use of internal mammary artery, total cardiopulmonary bypass time and total cross clamp time. Complications included operative death in 12.5%, perioperative myocardial infarction in 25.0% and perioperative arrhythmia in 17.5%. Nineteen perioperative variables were analyzed to identify risk factors for these end points. For operative death, presence of left main coronary artery stenosis (p = 0.056) and cardiopulmonary bypass time (p = 0.029) were significant in the univariate analysis, but presence of left main coronary artery lesion (p = 0.011, $\chi$$^2$= 6.45) and abscence of preoperative of IABP support (p = 0.069, $\chi$$^2$ = 3.30) were independent predictor in multivariate analysis (stepwise linear logistic regression).
1994년 7월부터 1995년 8월까지 서울대학교병원 흉부외과에서 시행한 관상동맥 우회술 78례 중 49례 (62,8%)에서 수술 1년 후 관상동맥 조영술을 시행하여 이식 혈관의 개존성을 조사하였으며, 수술 전,후 및 수술과 관련된 여러 위험 요소들이 개존성에 미치는 영향에 대하여 분석하였다. 관상동맥 우회술 후 평균 13.4$\pm$2.1개월 째에 관상동맥 조영술을 시행하였다. 내유동맥으로 문함을 시행한 60개소 중 3개소에서 string sign을 보였으나, 폐쇄 없이 전례에서 개존성이 유지되었으며, 요골동맥으로 문합을 시행한 4개소는 모두 개존성이 유지되었다. 복제정맥으로 문합을 시행한 81개소 중 69개소(85.2%)에서 개존성이 유지되었으며, 12개 소(14.8%)에서 폐쇄되었다. 개존성에 미치는 위험요소로 수술 전,수술 중 그리고 합병증 등의 수술 후 요소로 나누어 일원적 및 다원적으로 분석하였으며, 내유동맥의 협착 과 복재정맥의 개존성, 폐쇄에 미치는 요소들을 분석하였다. 내유동맥은 환자의 연령(60세 이상),수술 후 대동맥 내 풍선펌프의 삽입,수술 후 출혈, 및 급성신부전 등 합병증이 이식 혈관의 협착과 관련된 위험 인자였으며, 복제정맥은 우회술을 시\ulcorner 받은 관상동맥의 직경이 1.5mm 이하인 경우가 이식 혈관의 폐쇄 위험성과 관련된 위험 인자로 분석되었다(p<0.05). 관상동맥 우회술 1년 후 이식 혈관의 개존성은 내유동맥이 복재정맥보다 우수함을 보였으며 환자 연령 수술 후 대동맥 내 풍선펌프의 삽입여부, 출혈, 급성 신부전 등의 합병증 및 문합된 관상 동맥의 직경 등이 개존에 관련된 요소로 분석되었다. 이러한 결과를 토대로 수술 후 이식혈관의 폐쇄 여부에 대한 11측과 수술 방법이나 수술 후 환자의 추적 관찰에 도움을 줄 수 있으리라 생각된다.
