Purpose: This study was performed to analyse the trends of risk factor management programs for patients with coronary artery disease. Methods: Using PubMed, 35 intervention studies related to risk factor management programs among randomized controlled trials searched with the key words of coronary artery disease and coronary heart disease. Collected studies were analysed according to the characteristics of studies and participants, method and content of intervention, and outcome indicator and its effects. Results: The mean period of intervention was $28.7{\pm}26.8$ weeks, the mean frequency was $3.0{\pm}2.0$ times per week, and the duration of one session was below 60 minuets in 65.8% of the reviewed studies. The interventions were counselling, exercise, education, and cognitive behavior therapy. Counselling was applied most frequently in previous studies. The outcomes of intervention had been measured with anthropometric, physical, physiological, psychological, behavioral, and cognitive aspects, but the effect of the intervention was inconsistent among the studies. Conclusion: Based on the results of this study, systematic and comprehensive cardiac rehabilitation program consisted of counselling, exercise, and education should be developed and performed for health management and relapse prevention of patients with coronary artery disease.
Mutations in the factor Ⅴ gene are major risk markers for venous thrombosis. Several factors for blood coagulation have been related with cardiovascular disease. Ⅰ investigated genotype distribution for three mutations (G1691 A, A2379G and G2391 A) of the factor Ⅴ gene in the Korean population. Objectives : To determine obesity for the screening of individuals at high risk of coronary heart disease in urban areas. Methods : Data were obtained from 4,137 adults between 19 and 85 years of age (2,372 males, 1,765 females), not recognized as taking medicines for cardiovascular diseases, who underwent a health check-up at the health promotion center of university hospitals in cities between Jan. 2003 and Dec. 2004. The variables studied were divided into two broad categories, and their relationships examined. obesity indices and risk factors for coronary heart disease. To reveal the relation between each of the obesity indices and the proportion of individuals at risk of coronary heart disease, the obesity indices were stratified and odds ratios obtained after age adjustment. Results : From a gender comparison of anthropometric measures, men were found to have significantly greater heights, weights, and waist and hip circumferences than women. From a gender comparison by the obesity indices, women were found to have significantly higher BMI, %Fat, waist to hip and waist to stature ratios than men. As obesity indices, the waist to stature ratio and the waist circumference were strongly correlated with coronary risk factors, both in men and women. The age-adjusted odds ratio of coronary risk factors increased significantly with increasing waist circumference, BMI, %fat, waist to hip and waist to stature ratios, and were highest specifically for the waist to stature ratio and the waist circumference. Conclusions : The study results showed that the waist to stature ratio and the waist circumference, as obesity indices, were most closely correlated with coronary risk factors. It is suggested that the waist to stature ratio and, specifically, the waist circumference can be effectively used in the field of health management for screening those with high levels of coronary risk factors.
관상동맥 석회화는 심혈관질환 위험인자와 연관이 있으며, 동맥경화반의 양과 밀접한 관계가 있음을 선행 연구에서 보고되고 있다. 이에 본 연구에서는 2020년 12월부터 2021년 2월까지 대구시 소재 S 영상의 학과 건강검진센터를 방문하여 관상동맥 Calcium scoring CT를 시행한 109명을 대상으로 혈액인자가 관상동맥 석회화 수치에 미치는 영향을 알아보고자 하였다. 연구결과 허리둘레 인자에서는 비정상 군이 정상군에 비해 석회화 발생 위험도가 1.113배 증가하였고, 공복혈당 인자에서는 비정상 군이 정상 군에 비해 석회화 발생 위험도가 1.036배 증가하였으며, 중성지방 인자에서는 비정상 군이 정상 군에 비해 석회화 발생 위험도가 1.008배 증가하였으므로 허리둘레 인자와 공복혈당인자, 중성지방 인자가 관상동맥 석회화 수치에 영향을 주는 인자임을 알 수 있었다. 석회화 수치 발생 위험도는 허리둘레 인자와 공복혈당인자, 중성지방 인자와 유의한 관계가 있으므로 석회화 수치와 연관성이 있는 위험 변수들의 수치를 줄이기 위하여 평소 건강관리에 관심을 갖고 건강검진을 통하여 사전에 예방하기 위해 노력한다면 심혈관질환의 발생률을 낮추고, 심혈관질환과 관련된 의료비를 줄이는데 도움이 될 수 있을 것이라 기대한다.
Purpose: Cardiovascular risk factor modification is important for patients with coronary artery disease to prevent poor progression of the disease. Without an understanding and an awareness of risk factors, patients with coronary artery disease are not able to reduce their risk by the lifestyle modification. The aims of this study were to assess patient's awareness of risk factors and to identify predictors of awareness of risk factors. Methods: A descriptive correlational study using a cross-sectional survey method was performed. The sites of the study were three hospitals in Seoul, Korea. The sample consisted of 214 subjects. The awareness of risk factors and other subjects' characteristics were measured by a questionnaire developed for this study. Results: A range of subjects (gender: 72.0%; hyperlipidemia: 51.9%; hypertension: 40.7%; obese: 37.4%; age: 35.5%; smoking: 22.4%; diabetes: 19.2%; family history: 18.2%) were not able to accurately identify the risk factors. Among predictors, gender (${\beta}$=-.17) and diabetics (${\beta}$=-.25) had statistically significant influences on awareness of risk factors. Conclusion: Our findings highlight the lack of awareness of risk factors for coronary artery disease. The findings have important implications for nursing practice in terms of guiding educational strategies for the modification of risk factors for coronary heart disease.
PURPOSE: The purpose of this study is to understand the difference in the risk factors of coronary artery disease (CAD) between Korean and American CAD patients to determine the discriminant factor for each group, as well as to provide useful information to be reflected in the national concern of health. METHODS: Data were collected from 248 Korean and 107 American CAD patients who underwent either percutaneous coronary intervention or coronary artery bypass grafting. By using t-test and $X^2$-test, risk factors were compared between the Koreans and Americans. To elucidate which risk factor was the most discriminant for each group, logistic regression analysis was performed. RESULTS: All risk factors, except diastolic blood pressure, showed a significant difference between the two groups. $X^2$-test showed statistical significance with respect to the smoking rate between the female groups. Moreover, there was a statistically significant difference between the two groups regarding blood total cholesterol (TC) and triglyceride, and between the male groups, here was a statistically significant difference with respect to blood high-density lipoprotein cholesterol (HDL). Diabetes mellitus (DM) was the most discriminant factor for Korean patients while TC/HDL is the most discriminant for the Americans. CONCLUSION: The characteristics of CAD risk factors were determined to be different between Koreans and Americans in this study. TC/HDL was a discriminant factor for Americans while DM was a discriminant factor for Koreans. This result implies that DM should primarily be given attention to prevent CAD in Korean adults.
본 연구는 건강검진 수검자들에 대한 관상동맥질환 위험인자의 유병률을 파악하고자 대전광역시에 있는 C 대학교병원 건강검진센터에서 종합건강검진을 받았던 30세 이상의 일반성인 3,345명을 연구대상으로 하였다. 연구결과, 관상동맥질환 위험인자의 군집별 유병률을 보면, 위험인자를 1개 갖고 있는 사람이 27.6%, 2개 갖고 있는 사람이 26.5%, 3개 갖고 있는 사람이 16.9%등의 순이었으며, 적어도 1개 이상 관상동맥질환 위험인자를 갖고 있는 사람이 80.0%나 되는 것으로 나타났다. 따라서 관상동맥질환 발생 위험을 줄이기 위한 위험인자의 군집별 위험군에 근거한 차별화 된 건강증진프로그램의 개발과 보급이 필요하다고 본다.
Genetic factors are important in the pathogenesis of coronary artery disease (CAD). Angiotensin converting enzyme (ACE) gene insertion(I)/deletion(D) polymorphism is one of the genetic factor found to be related with CAD. We investigated the association between I/D polymorphism of the ACE gene and the presence of CAD. Threehundred and seven patients (187 males and 120 females, aged between 35-80, mean $54.3{\pm}9.8$ years) who underwent diagnostic coronary angiography were included in the study. ACE I/D polymorphism was detected by polymerase chain reaction. Of the 307, 176 had CAD. The most frequently observed genotype in all subjects was ID (47.9 %). However, in patients with CAD the frequency of II genotype was lower whereas DD genotype was higher compared to the controls (p < 0.05). The number of D allele carrying subjects were also higher (p < 0.05) in CAD patients. The logistic regression analysis indicated that the ACE D allele is an independent risk factor (odds ratio = 1.48, 95% CI = 1.01-2.18, p < 0.05). In conclusion, the I/D polymorphism of ACE gene (carrying D allele) is an independent risk factor for CAD in the studied Turkish population.
Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of metabolic syndrome (MS), and to examine factors affecting the severity of coronary artery diseases in patients with acute myocardial infarction (AMI). Methods: A total of 894 patients who had admitted C national university hospital from 2008 to 2010 participated in this study. Collected data were lipid profiles, abdominal circumference, blood pressure, fasting blood sugar (FBS) level, participants' demographic data and other risk factors by interview, measurement, and review of participants' medical records. MS was defined according to modified National Cholesterol Education Program Adult Treatment Panel III and Asia-Pacific Criteria. Results: The participants' mean age was 64.7 (${\pm}11.0$) years and 65% was male patients. The participants' with MS was 37.6% in men and 71.4% in women. According to binary logistic regression analysis, high FBS (95% CI 1.7-2.0) and lower high-density lipoprotein (HDL) cholesterol (95% CI 1.1-1.9) were independent predictors of severe coronary artery disease. Conclusion: These risk factors of severe coronary artery disease will be utilized as an important basic data in part of management, education, and countermeasure of patients with both MS and AMI.
.저자들은 1994년 7월부터 1996년 6월까지 관상동맥우회 술을 시행한 환자 103명을 심실구축력 40%를 기준 으로 심실기능저하군 24명과 심실기능정상군 79명으로 분류하여 환자의 술전,수술변수 18개를 분석하였다. 분석결과 환자의 특성과 수술사망에 영향을 미치는 위험인자에 대해 다음과 같은 결과를 얻었다. 1. 술전 변수중 심비대(p=0.0012), 혈청 크레아티닌 이상(p=0.0166), IABP 실시(p=0.0096)는 기능저하군이 기능 정상군보다 유의하게 높은 빈도를 보였다 2. 수술변수중 기능저하군은 수술적응증에 있어서 심근경색후 협심증의 빈도가 많았고(p=0.00003), 내흉동맥 의 사용은 유의하게 적었다(p=0.00416). 3. 조기사망률은 기능저하군이 기능정상군보다 약간 높았으나 통계적으로 유의한 차이는 없었다(기능저하군 8.3%, 기능정상군 5.1%, p=0.5492). 4. 합병증발생률은 기능저하군 50%(12/24), 기능정상군 33%(79/25)로 두군간에 유의한 차이는 없었다(p=0.1007). 5. 심실기능정상군에서는 연령(고령)(p=0.041)이 수술후 조기사망에 유의한 위험인자로, 심실기능저하군에서 는 연령(p=0.018)외에도 술전 IAaP 실시(p=0.0036), 고콜레스테롤혈증(p=0.00 7), 응급수술(p=0.0036)이 유의 한 위험인자로 작용하였다. 이상의 결과를 통해 심실기능이 저하된 관상동맥질환 환자는 심실기능이 정상인 환자에 비해 많은 술후 조기사망의 위험인자를 갖고있으나.술후 조기사망률과 합병증 발생률은 두 군간에 유의한 차이를 보이지 않 음으로써 심실기능이 저하된 환자에 있어서도 증상의 개선과 장기생존률의 향상을 얻기 위하여 좀더 적극적인 관상동맥우회 술의 실시가 필요할 것으로 생각된다.
