  • Takayasu's arteritis is a chronic inflammatory disease, involving mainly the aorta and its main branches, which can cause stenosis or occlusion. It involves the bracheocephalic, carotid, subclavian, vertebral, and renal, as well as the coronary and pulmonary arteries. The clinical manifestations range from asymptomatic to catastrophic, with dizziness, hypertension, claudication, cerebral infarction, chest pain and dyspnea. Takayasu's arteritis involving the pulmonary arteries, but not the aorta and its main branches, is very rare. Herein, a case of isolated pulmonary Takayasu's arteritis combined with pulmonary thromboembolism, and hyperthyroidism is reported, with a review of the literature.

  • Metabolic syndrome (MetS) is a disease that is accompanied by various metabolic related problems and refers to a disease in which various adult diseases occur along with obesity. These metabolic syndromes appear according to the individual's genetic background. APOA5-ZPR1-BUD13, a gene cluster belonging to chromosome 11q23.3, is well known for its risk of plasma triglycerides and coronary artery disease. Recently, the GWAS results for metabolic syndrome were published in Koreans. The results included the APOA5-ZPR1-BUD13, and the SNPs that first appeared in Koreans in the ZPR1 and BUD13 were also discovered. In this study, the reproducibility was investigated for the newly discovered ZPR1 (rs964184) and BUD13 (rs2075295, rs1558861) using The Health Examinees (HEXA) cohort and showed significance. In addition, BUD13 (rs117548857, rs10488698, rs149527022, rs10790162), ZPR1 (rs2075290, rs145796806, rs201247587), APOA5 (rs12791103, rs1263173, rs7396835, rs17520254) were additionally discovered and significant results were obtained. For the SNPs that showed significant results, the effect on protein expression and the effect of expression quantitative trait loci (eQTL) were also confirmed. This study is expected to contribute to the prevention and treatment of diseases with differences in onset based on individual genetic patterns as well as presenting the effect of genetic mutations in the APOA5-ZPR1-BUD13 on metabolic syndrome and blood lipid levels.

Job performance, empowerment, and satisfaction of regional dental hygienists according to systemic disease-related knowledge of regional dental hygienists in South Korea (일부 지역 치과위생사의 전신질환 관련 지식에 따른 업무수행능력과 임파워먼트 및 직무만족)

  • Objectives: The purpose of this study was to investigate the level of systemic disease-related knowledge among dental hygienists and analyze the effect on their job performance, empowerment, and satisfaction to provide basic data for expanding education on systemic diseases among dental hygienists and improving their job performance, empowerment and satisfaction. Methods: A survey was conducted among dental hygienists working in Busan and Gyeongsangnam-do region, South Korea, from October 27 to November 10, 2020, with a total of 245 questionnaires included in the final analysis. Results: Results of analysis revealed an average score for systemic disease-related knowledge among dental hygienists of 16.53±3.33 points and the higher the systemic disease-related knowledge, the higher the job performance, empowerment, and satisfaction. The higher the coronary artery disease knowledge and respiratory and infectious disease knowledge, the higher the job performance, and the higher the respiratory and infectious disease knowledge, the higher the job empowerment and job satisfaction. Conclusions: This study revealed that the higher dental hygienists' level of systemic disease-related knowledge, the higher their job performance, empowerment, and satisfaction. Therefore, this study suggests that dental hygienists' education on systemic disease-related knowledge should be expanded, and diverse systemic disease education programs should be developed for application in clinical practice.

  • Cardiovascular diseases (CVDs), including coronary artery disease, stroke, heart failure, and hypertension, are the global leading causes of death, accounting for more than 30% of deaths worldwide. Although the risk factors of CVDs have been well understood and various treatment and preventive measures have been established, the mortality rate and the financial burden of CVDs are expected to grow exponentially over time due to the changes in lifestyles and increasing life expectancies of the present generation. Recent advancements in metagenomics and metabolomics analysis have identified gut microbiome and its associated metabolites as potential risk factors for CVDs, suggesting the possibility of developing more effective novel therapeutic strategies against CVD. In addition, increasing evidence has demonstrated the alterations in the ratio of Firmicutes to Bacteroidetes and the imbalance of microbial-dependent metabolites, including short-chain fatty acids and trimethylamine N-oxide, play a crucial role in the pathogenesis of CVD. However, the exact mechanism of action remains undefined to this day. In this review, we focus on the compositional changes in the gut microbiome and its related metabolites in various CVDs. Moreover, the potential treatment and preventive strategies targeting the gut microbiome and its metabolites are discussed.

The Relationship between health related physical fitness and self-perceived health status (건강관련 체력과 주관적 건강인식에 관한 연구)

  • Modern technology has lessened the physical demand of every activities. Thus, physical inactivity has led to a rise in ‘Hypokinetic diseases’. The prefix, hypo, means lack of and kinetic refers to movement. Individuals who do not exercise regularly are at greater risk of developing hypokinetic diseases such as coronary heart disease, hypertension, hyperlipidemia, obesity, musculoskeletal disease. Systematic exercise program defenses development of hypokinetic diseases. Exercise program bases on basic physical fitness test. The physical fitness evaluation is used to degree of Wellness of the individuals. It includes the evaluation of Health-Related Physical Fitness -musclular strength, muscular endurance, flexibility, cardiorespiratory endurance, body composition - and skill or sports related fitness -agility, balance, coordination, reaction time, speed. In present study, the authors investigated health-related physical fitness test result and questionnaire for 133 male and 71 female from Jannuary 16 to March 15, 1997. The purpose of this study is to observe relationship health related physical fitness and self-perceived health status. The results are as follows. 1. There were statistically significant differences in BMI, grip strength, back strength, muscular endurance, cardiorespiratory endurance, and flexibility between male and female subjects. Flexibility is higher in female than male subject, but Others are higher in male than female subjects. 2. There were statistically significant differences in grip strength, muscular endurance between 30s, 40s, and 50s group In both gender subjects, and In cardiorespiratoryendurance between the groups especially in Female subjects. 3. For male subjects, flexibilitywas measured lower in group who classified low level in self-perceived health status than group of others. Also, for female, subjects, Muscular endurance & flexibility were measured. 4. It was shown that grip strength, back strength, and muscular endurance were significantly lower in group who have chronic diseases than normal group for Male subjects. But, For female subjects, all the component were statistically insignificant results between normal and disease group. 5. Both male and female subjects, there were Positive correlation among grip strength, back strength, muscular endurance, cardiorespiratory endurance, and flexibility but, negative correlation for age. Therefore, health-related physical fitness is very important component for Heath Promotion & Wellness. Physical Fitness test is valuable test in health evaluation, health management and health promotion, so available for hospital, sports center, community health center, industrial field, school etc.

Lack of Association between the Klotho Gene and COPD

  • Background: Although the aging process and features of chronic obstructive pulmonary disease (COPD) have several similarities, the relationship between aging and COPD pathogenesis remains incompletely understood. The klotho gene was found to be related to premature aging and emphysematous changes in an animal model. We investigated whether klotho gene polymorphisms are related to COPD susceptibility and emphysema severity. Methods: A total of 219 COPD subjects from the Korean Obstructive Lung Disease Cohort and 305 control subjects were genotyped for two single nucleotide polymorphisms (SNPs) of the klotho gene associated with coronary artery disease. Logistic regression was performed to determine the association of these SNPs with COPD susceptibility and linear regression was performed to investigate their association with emphysema severity in COPD subjects. Results: The mean age of the COPD subjects was 66 years and their mean FEV1 was 1.46 L. There were no associations between either SNP or COPD susceptibility (p=0.6 and 0.2, respectively) and there were no associations with emphysema severity. Conclusion: Genetic polymorphisms of the klotho gene were not associated with COPD in a Korean population.

The Functionalities and Active Constituents of Olive Oil (올리브유의 기능성과 활성성분)

  • Olive oil is widely consumed in Korea, and is a representative fat source in the Mediterranean diet, known to be effective in the prevention of coronary artery disease. In addition, diverse functionalities have been reported, such as anti-cancer, anti-oxidation, and anti-inflammation effects. In this review, the status of production and variety were investigated with respect to the functionalities of olive oil. The main functional constituents of olive oil are oleic acid, known to improve blood cholesterol, and the minor constituents are polyphenol, tocopherol, squalene, and phospholipid, the concentrations of which can be used to distinguish pressed from refined olive oil. A number of studies of the functionality of olive oil have dealt with the minor constituents, and the beneficial functionalities, such as anti-oxidation, anti-inflammation, and improving blood circulation have been reported. This review intensively investigates the functionalities and the responsible components, and suggests that continual studies on olive oil are necessary for the prevention of various metabolic diseases.

Assessment of the Cardioprotection Offered by Fisetin in H2O2-induced Zebrafish (Danio rerio)-Tg (cmlc2: egfp)

  • The aim of this study was to evaluate the protective function of fisetin, a natural flavonoid in zebrafish heart for the treatment of myocardial infarction in coronary and ischemic heart disease. For this purpose, we induced oxidative stress zebrafish (Danio rerio)-Tg (cmlc2: egfp) by $H_2O_2$ and then administered fisetin, the protective effect of fisetin was determined by measuring the heart rate following fisetin administration. After testing the toxicity of fisetin, we found that the heartt increased in a concentration-dependent manner, however there was no difference between the heart rates of embryos and adults. The improved heart rate demonstrated the cardioprotective effect of fisetin. The result showed that fisetin, at concentration of 3and $5{\mu}M$, significantly increased heart rate compared with the heart with $H_2O_2$ alone. This indicates that fisetin plays an important role in the prevention of heart damage and treatment of cardiovascular diseases caused by oxidative stress due to ischemia / reperfusion.

The Case-Control Studies Between The Single Nucleotide Polymorphisms of the Human SA and MTHFR Gene and Hypertension in Korean Population

  • The role of the kidney in initiating hypertension has been much debated. The SA gene is expressed in the kidney and is association with hypertension in man and in experimental animal models. Also, increased plasma concentrations of homocysteine have been found in patients with coronary artery disease (CAD) and hypertension. The genetic variation of methlene tetrahydrofolate reductase (MTHFR) gene is related to its enzyme activity and to the plasma homocysteine concentration. In view of the effect of SA and MTHFR as risk factor for cardiovascular diseases, we investigated the Pst I RFLP of the SA gene and C667T mutation of the MTHFR gene in the Korean patients with hypertension. There were no significant differences in the allele and genotype frequencies of these polymorphisms between normotensive and hypertensive subjects. Therefore, our results do not support a possible role of these genes on hypertension in Korean population.

Effects of Plasma Lipoproteins on Expression of Vasular Cell Adhesion Molecule- in Human Microvasuclar Endothelial Cells (혈관내피세포에서 Vascular Cell Adhesion Molecule-1 발현에 대한 혈장 지단백의 효과)

  • Although an elevated plasma level of high density lipoprotein (HDL) is known as a protective component against the development of atherosclerosis and ensuing coronary heart diseases, the related mechanisms are still not established . It has been clearly demonstrated in the early stages of atherogenesis that adhesion of monocytes and lymphocytes to the vascular endothelium is enhanced via adhesion molecules, and that monocytes and macrophages accumulate in the subendothelial space. The present study has investigated whether isolated plasma HDL plays a role in protection against atherogenesis by inhibiting the expression of vascular cell adhesioin molecule-1(VCAM-1) on the endothelial cells. Effects of plasma native low density lipoprotein (LDL) and ac ethylated LDL(AcLDL) on VCAM-1 expression were also examined by using an immunocytochemical technique. While plasma HDL did not alter the basal expression of VCAM-1 , lipopolysaccharide(LPS) induction of this adhesion modlecule was markedly inhibited at a phyaiological concentration of HDL. In contrast, 30$\mu\textrm{g}$ protein/ml AcLDL increased sifnificantly both basal VCAM-1 expression and its LPD induction , suggesting that this modified LDL enhances leukocyte adhesiion to endothelial cells. Unlike AcLDL , plasma native LDL inhibited significantly VCAM-1 expression. This indicates that LDL did not undergo oxidative modificantion while incubated with endothelial cells. These results suggest that plasam HDL may inhibit atherogenesis by reducing the expression of adhesion molecules, which is a protective mechanism independent of tis reverse cholesterol transport function . Modified LDL is a potent iducer for adhesion molecules in vascular endothelical cells and could play a role in the pathogenesis of atherosclerosis by adhering to blood cells.
