Postinfarct Ventricular Septal Defect after Coronary Covered Stent Implantation

  • We report a case of a postinfarction ventricular septal defect caused by an acute recurrent occlusion after the implantation of a covered stent, which was performed as a rescue procedure for the ruptured left anterior descending artery during a percutaneous coronary intervention. Although the emergent implantation of a covered stent for the ruptured coronary arteries such as the left main coronary artery or the origins of the left anterior descending artery can be performed during a percutaneous coronary intervention, and a coronary bypass surgery should be considered in order to decrease the risk of complete occlusion, thus providing a superior long term patency.

  • Diffuse refractory vascular spasms associated with coronary bypass artery grafting (CABG) are rare but devastating. A 42-year-old male patient with a past history of stent insertion was referred for the surgical treatment of a recurrent left main coronary artery disease. A hemodynamic derangement developed during graft harvesting, necessitating a hurried initiation of cardiopulmonary bypass (CPB). Although CABG was carried out as planned, the patient could not be weaned from the bypass. An emergency coronary angiography demonstrated a diffuse spasm of both native coronary arteries and grafts. CPB was switched to the femorofemoral extracorporeal membrane oxygenator (ECMO). Although he managed to recover from heart failure, his discharge was delayed due to the ischemic injury of the lower limb secondary to cannulation for ECMO. We reviewed the case and literature, placing emphasis on the predisposing factors and appropriate management.

  • Constant technological developments in coronary artery disease have contributed to the assessment of both the presence of coronary stenosis and its hemodynamic consequences. Hence, noninvasive imaging helps guide therapeutic decisions by providing complementary information on coronary morphology and on myocardial perfusion and metabolism. This can he done using single photon emission computed tomography (SPECT) or positron emission tomography (PET) and multidetector CT (MDCT). Advances in image-processing software and the advent of SPECT/CT and PET/CT have paved the way for the combination of image datasets from different modalities, giving rise to hybrid imaging. Three dimensional cardiac hybrid imaging helped to confirm hemodynamic significance in many lesions, add new lesions such as left main coronay artery disease, exclude equivocal defects, correct the corresponding arteries to their allocated defects and identify culprit segment. Cardiac hybrid imaging avoids the mental integration of functional and morphologic images and facilitates a comprehensive interpretation of coronaty lesions and their pathophysiologic adequacy by three dimensional display of fused images, and allows the best evaluation of myocardial territories and the coronary-artery branches that serve each territory. This integration of functional and morphological information were feasible to intuitively convincing and might facilitate development of a comprehensive non-invasive assessment of coronary artery disease.

Post-Coronary Artery Bypass Grafting Myocardial Ischemia Caused by an Overgrown Left Internal Thoracic Artery Side Branch

  • We present a patient who developed recurrent angina after coronary artery bypass grafting (CABG). Myocardial single-photon emission computed tomography (SPECT) demonstrated deterioration in the myocardial perfusion, and coronary angiography revealed an overgrown side branch of the grafted left internal thoracic artery (ITA); otherwise, there were no significant changes compared with previous imaging studies obtained after the CABG. After percutaneous embolization of the grafted left ITA side branch, the angina was resolved and myocardial SPECT showed improved perfusion.

동맥전환술: 판상돔맥이식 수기변형과 수술사망의 위험인자 (Arterial Switch Operation: The Technical Modification of Coronary Reimplantation and Risk Factors for Operative Death)

  • 동맥전환술은 대혈관전위증이나 Taussig-Bing 기형의 가장 보편적인 수술법으로 인정되고 있다. 동맥전환술 후 수술사망에 영향을 미치는 위험인자와 함께 관상동맥 이식 수기변형의 유효성을 알아보고자 하였다. 1994년부터 2002년 7월까지 동맥전환술을 동아대학교 병원에서 한 명의 외과의에 의하여 시행된 85예의 대혈관전위증과 양대혈관 우심실기시증 환아를 대상으로 후향적 조사를 하였다. 관상동맥이식 수기변형 등 수술(병원)사망에 관여한다고 생각하는 여러 인자들을 다변량 회귀분석을 통해 수술사망에 미치는 위험인자들을 파악하였다. 전체 수술사망(병원사망)은 모두 17예(20%)였으며 수술사망률이 98년 이전은 31.0% (13/42), 98년 이후는 9.3% (4/43)로 감소하였다. 대동맥궁 기형을 동반하지 않은 경우는 전체 사망률이 12.5% (9/72)였으나 동반된 경우는 61.5% (8/13)로 높은 사망률을 보였다. 관상동맥이식 수기변형을 하지 않은 경우 사망률은 28.1%(18/64)를 보인 반면 관상동맥이식 수기변형을 한 경우는 4.8% (1/21)로 많은 사망률의 감소가 관찰되었다. 다변량 회귀분석에서 심폐기가동시간($\geq$ 250분), 대동맥차단시간($\geq$ 150분), 대동맥궁기형이 동반된 경우, 수술 전 사건(event)이 있었던 경우, 개방 관상동맥 이식술(open coronary reimplantation technique)이 수술사망의 위험인자로 파악되었다. 동맥전환술의 수술사망률은 수술시기가 경과함에 따라 감소하였으며, 대동맥궁 기형의 동반과 술 전 사건(event)이 수술사망의 중요 위험인자였다. 비전형적 관상동맥 형태는 수술사망의 위험요소가 아니었으며 관상동맥 이식의 수기변형이 수술사망을 낮추는 데 매우 유효하였던 것으로 생각된다.

  • We compared the influences of reconstruction methods using $180^{\circ}$ or $360^{\circ}$ data upon contrasts and discriminating capability and diagnostic accuracy in $^{99m}Tc-MIBI$ stress/rest myocardial SPECT. We reviewed SPECT images reconstructed only with $180^{\circ}$ projection data or with $360^{\circ}$ data in 18 patients and in 11 normal subjects. To compare counts of surface structures and deep structures, we measured ape# posterior wall ratios in 11 normal subjects. To compare the contrasts of images, we measured apex/ventricle ratios. To compare contrasts between normal and diseased myocardial segments, we measured count ratios of defect and normal segments in 4 patients who had single coronary artery diseases. To compare diagnostic accuracy, we scored SPECT images made with $180^{\circ}$ and $360^{\circ}$ data segmentally. Sensitivity and specificity for the diagnosis of coronary artery disease and for the revelation of diseased arteries with both $180^{\circ}$ and $360^{\circ}$ SPECT images. If involved coronary arteries had more narrowing than 50% In coronary angiogram, we considered them as diseased arteries Apex/posterior wall ratios were not different significantly in normal subjects. Apex/ ventricle ratios in normal subjects were different significantly between $180^{\circ}$ and $360^{\circ}$ SPECT images. Defect/normal ratios were different significantly between $180^{\circ}$ and $360^{\circ}$ SPECT images in single vessel disease patients. The overall diagnostic accurracy was the same between $180^{\circ}$ and $360^{\circ}$ data collection. Sensitivity was 94% and specificity was 91% for both types of data collection in this sample population. Sensitivity and specificity of each coronary artery territory were not significantly different between the images made with $180^{\circ}$ and $360^{\circ}$ data. The images made with $180^{\circ}$ data had better contrast between ventricle and myocardium and between hypoperfused and normal myocardium, though no difference was found between the ratios of the myocardial counts of surface and deep structures. However, diagnostic sensitivities of diseased artery territories were not different significantly and so were overall diagnostic accuracy between both methods of making images with $180^{\circ}$ and $360^{\circ}$ data.

  • Congenital abnormalities of the coronary arteries are found in 0.6% to 1.3% of patients in coronary angiography. Dual left anterior descending coronary artery (LAD) is a rare coronary anomaly and is incidentally detected during coronary angiography. We report a case of a 65-year-old female with a rare coronary anomaly who was diagnosed with dual LAD via coronary computed tomography and coronary angiography. The imaging studies revealed dual LAD originating from the left main stem and right coronary sinus. These angiographic findings were considered to be consistent with the type IV variety of dual LAD by Spindola-Franco classification. Recognition of dual LAD is important to prevent errors of interpretation of the coronary angiogram and for optimal surgery.

Long Segmental Reconstruction of Diffusely Diseased Left Anterior Descending Coronary Artery Using Left Internal Thoracic Artery with Extensive Endarterectomy

  • In coronary artery bypass grafting, a diffusely diseased left anterior descending coronary artery (LAD) is an obstacle to achieving complete revascularization, consequently leading to the possibility of a poor prognosis. Long segmental reconstruction with or without endarterectomy is a revascularization method for treating diffusely diseased coronary arteries. Herein, we report a successful case of long segmental reconstruction of a diffusely diseased LAD using a left internal thoracic artery onlay patch after endarterectomy.

Transvenous proximal closure of large congenital coronary arteriovenous fistula using the single Amplatzer vascular plug in a 3-year-old girl

  • Congenital coronary arteriovenous fistulas (CCAFs) are rare coronary artery abnormalities in which blood is shunted into a cardiac chamber or great vessel. If the fistula itself is large and tortuous, it is generally recommended to occlude the fistula to prevent several complications. In approaches of transcatheter occlusion, the transvenous approach is preferred over the transarterial approach. The transvenous approach would enable the cannulation of a relatively larger catheter or sheath without potential damage to the femoral vessels or normal coronary arteries, which can occur in the transarterial approach. The transvenous approach may also minimize the blind pouch after releasing the devices. Herein, we report the success of transvenous proximal closure of a CCAF using an Amplatzer vascular plug (AVP) in a 3-year-old patient with cardiomegaly. Complete occlusion was achieved by a single AVP and thrombus formation of the distal aneurysmal portion of the fistula. We suggest that this strategy of closing the proximal end with a dilated fistula using a single AVP by the transvenous approach may be a good option in treating CCAFs in a young child.

Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery in Adulthood: Challenges and Outcomes

  • Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is an extremely rare, potentially fatal, congenital anomaly with a high mortality rate in the first year of life. It occurs rarely in adulthood and may appear with malignant ventricular arrhythmia or sudden death. We report a case of a 49-year-old woman with ALCAPA who presented with dyspnea on exertion. Management was coronary artery bypass grafting to the left anterior descending artery and obtuse marginal arteries, closure of the left main coronary artery ostium, and reestablishment of the dual coronary artery system.
