Left Ventricular Ejection Fraction Predicts Poststroke Cardiovascular Events and Mortality in Patients without Atrial Fibrillation and Coronary Heart Disease

  • Background and Objectives: It is controversial that decreased left ventricular function could predict poststroke outcomes. The purpose of this study is to elucidate whether left ventricular ejection fraction (LVEF) can predict cardiovascular events and mortality in acute ischemic stroke (AIS) without atrial fibrillation (AF) and coronary heart disease (CHD). Methods: Transthoracic echocardiography was conducted consecutively in patients with AIS or transient ischemic attack at Soonchunhyang University Hospital between January 2008 and July 2016. The clinical data and echocardiographic LVEF of 1,465 patients were reviewed after excluding AF and CHD. Poststroke disability, major adverse cardiac events (MACE; nonfatal stroke, nonfatal myocardial infarction, and cardiovascular death) and all-cause mortality during 1 year after index stroke were prospectively captured. Cox proportional hazards regressions analysis were applied adjusting traditional risk factors and potential determinants. Results: The mean follow-up time was $259.9{\pm}148.8days$ with a total of 29 non-fatal strokes, 3 myocardial infarctions, 33 cardiovascular deaths, and 53 all-cause mortality. The cumulative incidence of MACE and all-cause mortality were significantly higher in the lowest LVEF (<55) group compared with the others (p=0.022 and 0.009). In prediction models, LVEF (per 10%) had hazards ratios of 0.54 (95% confidence interval [CI], 0.36-0.80, p=0.002) for MACE and 0.61 (95% CI, 0.39-0.97, p=0.037) for all-cause mortality. Conclusions: LVEF could be an independent predictor of cardiovascular events and mortality after AIS in the absence of AF and CHD.

Complete Versus Culprit-Only Revascularization for ST-Segment Elevation Myocardial Infarction and Multivessel Disease in the 2nd Generation Drug-Eluting Stent Era: Data from the INTERSTELLAR Registry

  • Background and Objectives: We aimed to compare outcomes of complete revascularization (CR) versus culprit-only revascularization for ST-segment elevation myocardial infarction (STEMI) and multivessel disease (MVD) in the $2^{nd}$ generation drug-eluting stent (DES) era. Methods: From 2009 to 2014, patients with STEMI and MVD, who underwent primary percutaneous coronary intervention (PCI) using a $2^{nd}$ generation DES for culprit lesions were enrolled. CR was defined as PCI for a non-infarct-related artery during the index admission. Major adverse cardiovascular event (MACE) was defined as cardiovascular (CV) death, non-fatal myocardial infarction, target lesion revascularization, or heart failure during the follow-up year. Results: In total, 705 MVD patients were suitable for the analysis, of whom 286 (41%) underwent culprit-only PCI and 419 (59%) underwent CR during the index admission. The incidence of MACE was 11.5% in the CR group versus 18.5% in the culprit-only group (hazard ratio [HR], 0.56; 95% confidence interval [CI], 0.37-0.86; p<0.01; adjusted HR, 0.64; 95% CI, 0.40-0.99; p=0.04). The CR group revealed a significantly lower incidence of CV death (7.2% vs. 12.9%; HR, 0.51; 95% CI, 0.31-0.86; p=0.01 and adjusted HR, 0.57; 95% CI; 0.32-0.97; p=0.03, respectively). Conclusions: CR was associated with better outcomes including reductions in MACE and CV death at 1 year of follow-up compared with culprit-only PCI in the $2^{nd}$ generation DES era.

Revascularization Strategies in Patients With ST-Segment Elevation Myocardial Infarction and Multivessel Disease: Is FFR-Guided Strategy Still Valuable?

  • Several studies have shown the benefit of complete revascularization (CR) over culprit-only percutaneous coronary intervention (PCI) in patients with ST-segment elevated myocardial infarction (STEMI) and multivessel disease (MVD). Nevertheless, optimal strategy to select targets for non-culprit PCI has not been clarified. In this paper, we critically discuss and compare the safety and efficacy of different strategies for CR in patients with STEMI and MVD using a Bayesian network meta-analysis including all previous randomized controlled trials (RCTs). In Bayesian network meta-analysis of 13 RCTs, culprit-only PCI was associated with higher risk of major adverse cardiac events (MACE), compared with angiography-guided or fractional flow reserve (FFR)-guided CR strategies. However, there was no significant difference between angiography-guided and FFR-guided CR strategies in the risk of MACE and its individual components including all-cause death, cardiac death, myocardial infarction (MI), and revascularization. These evidence support that both angiography-guided and FFR-guided complete revascularization strategies would be reasonable treatment option in patients with STEMI and MVD. If the non-culprit lesion is severe on visual assessment, angiography-guided PCI can be considered. If the non-culprit lesion is intermediate in severity or unclear based on visual assessment, FFR-guided strategy can be used as a reliable and objective tool, providing similar benefits with less stents compared with an angiography-guided strategy. Further RCT is needed to evaluate direct comparison between angiography-guided and FFR-guided CR strategies in patients with STEMI and MVD. Ongoing FRAME-AMI trial (NCT02715518) will provide more evidence regarding this issue.

Deep Learning-Based Lumen and Vessel Segmentation of Intravascular Ultrasound Images in Coronary Artery Disease

  • Background and Objectives: Intravascular ultrasound (IVUS) evaluation of coronary artery morphology is based on the lumen and vessel segmentation. This study aimed to develop an automatic segmentation algorithm and validate the performances for measuring quantitative IVUS parameters. Methods: A total of 1,063 patients were randomly assigned, with a ratio of 4:1 to the training and test sets. The independent data set of 111 IVUS pullbacks was obtained to assess the vessel-level performance. The lumen and external elastic membrane (EEM) boundaries were labeled manually in every IVUS frame with a 0.2-mm interval. The Efficient-UNet was utilized for the automatic segmentation of IVUS images. Results: At the frame-level, Efficient-UNet showed a high dice similarity coefficient (DSC, 0.93±0.05) and Jaccard index (JI, 0.87±0.08) for lumen segmentation, and demonstrated a high DSC (0.97±0.03) and JI (0.94±0.04) for EEM segmentation. At the vessel-level, there were close correlations between model-derived vs. experts-measured IVUS parameters; minimal lumen image area (r=0.92), EEM area (r=0.88), lumen volume (r=0.99) and plaque volume (r=0.95). The agreement between model-derived vs. expert-measured minimal lumen area was similarly excellent compared to the experts' agreement. The model-based lumen and EEM segmentation for a 20-mm lesion segment required 13.2 seconds, whereas manual segmentation with a 0.2-mm interval by an expert took 187.5 minutes on average. Conclusions: The deep learning models can accurately and quickly delineate vascular geometry. The artificial intelligence-based methodology may support clinicians' decision-making by real-time application in the catheterization laboratory.

관상동맥-폐동맥 이상 기시증에 대한 수술의 조기 결과 (Early Result of Surgical Management of the Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery)

  • 배경: 관상동맥 폐동맥 이상 기시증(Anomalous origin of the left coronary artery from the pulmonary artery; ALCAPA)은 드문 선천성 심장병으로, 아이들에게 있어 심근경색 및 허혈성 심장병의 가장 흔한 원인 중 하나이다. 치료하지 않을 경우 출생 1년 내에 높은 사망률을 보이며, 진단 즉시 이중관상동맥 체계로 수술적 교정을 해주는 것이 치료 원칙이다. 아직 국내의 임상 경험 및 결과에 대한 보고가 드물어 본원의 경험을 공유하고자 한다. 대상 및 방법: 서울아산병원에서는 1989년 6월부터 2003년 7월까지 수술을 시행한 6예의 좌관상동맥 폐동맥 이상 기시증과 1예의 우관상동맥 폐동맥이상 기시증 환아를 대상으로 하여 심전도, 단순흉부촬영 및 심초음파 소견 등을 후향적으로 검토하였다. 결과: 환아들 중 남아는 3명이고, 여아는 4명이었으며, 수술 당시 환아의 연령은 중앙값 5.4개월(3$\∼$33개월)이였다. 환아의 몸무게는 평균 6.7$\pm$2.6 kg (3.7$\∼$11.3 kg)이었고, 진단은 3명만이 초기에 좌 관상동맥 폐동맥 이상 연결증으로 정확히 진단되었고, 4명은 다른 진단으로 입원하였다. 수술은 이중관상동맥 체계의 형성을 원칙으로 직접 좌관상동맥-대동맥 이식술을 시행하였다. 체외순환 시간은 114$\pm$37분, 대동맥차단 시간은 55$\pm$22분이었다. 이차성 심방중격결손의 단순 봉합이 1예, 승모판 성형술이 2예에서 동반 시술되었다. 중환자실 재원일은 평균 5일, 인공호흡기 보조시간은 평균 38시간이며, 술 후 입원기간은 평균 12일이었다. 심전도와 흘부 단순 촬영 소견은 만기 사망한 한 예를 제외하고 모두 호전되었으며, 좌심실 기능의 회복도 거의 정상적으로 회복되어, 수술 후 1년 내에 실시한 심초음파 검사에서 좌심실 박출률(EF: Ejection fraction)은 평균 41.2$\pm$10.3$\%$에서 평균 $59.8{\pm}13.9\%$로, 좌심실 수축률(Shortening Fraction)은 평균 23.6$\pm$4.7$\%$에서 37.4$\pm$7.9$\%$로 호전되었고, 좌심실의 수축기말의 용적지수(LVEDDI: Left ventricular end-diastolic dimension index to body surface area)는 수술 전100.8$\pm$25.6 mm/$m^{2}$에서 79.3$\pm$ 15.8 mm/$m^{2}$로 감소한 소견을 보였다. 승모판 성형술은 전 승모판엽 탈출증이 있는 두 환아에서 동시에 시행하였다. 수술 후 1년 내 시행한 심초음파에서 모든 환아에서 단지 경등도 이하의 승모판 폐쇄 부전 소견을 보였다. 수술 후 조기 사망은 없었으며, 합병증으로는 유미흉이 한 명에서 있었다. 술 후 10개월째 허혈성 확장성 심근증이 호전되지 않아 Dor 술식을 시행한 후 사망한 예를 제외한 나머지 6명은 특이 증상 없이 정상 생활 중이다 결론: 좌관상동맥 페동맥이상 기시증은 드물기는 하나, 영유아기에 심근경색 및 허혈성 심근증 또는 선천성 승모판 폐쇄 부전등을 초래하는 심각한 선천성 심질환이다. 그러나 진단 즉시 직접 좌관상동맥-대동맥 이식술로 수술적 교정을 해줌으로써 좋은 성적을 기대할 수 있음을 보여주었다.

심근경색에서 측부순환 유무에 따른 $^{99m}Tc$-MIBI 심근 SPECT 소견 (The Relation between Collateral Circulation and $^{99m}Tc$-MIBI Heart SPECT)

  • 심근경색후 측부순환의 존재는 심근경색의 크기를 감소시키고 생존가능한 심근의 양을 늘려 심근기능의 보존에 기여하며 심실류 혈성을 예방하는 것으로 알려져 있다. 본 연구에서는 심근경색후 측부순환의 분포와 측부순환에 따른 심근 SPECT소견을 알아보고자 시행하였다. 급성심근경색으로 진단된 환자중 2주내에 관동맥 조영술을 시행하여 TIMI grade 0, 1인 환자 56예를 대상으로 하여 측부순환이 좋은 I군과 측부순환이 나쁜 II군으로 분류하여 양군간의 임상양상 및 좌심실기능과 심근 SPECT 소견을 조사하여 다음의 결과를 얻었다. 1) 경색관련 동맥에로의 측부순환은 우관동맥 15예, 좌전하행동맥 10예, 좌회선동맥 50예 순이 었다. 2) 측부순환 경로는 좌전하행동맥에서 우관동맥으로 13예(40.6%), 우관동맥에서 좌전하행동맥으로 9예(28.1%)로 우관동맥과 좌전하행동맥 사이에 흔하였다. 3) 최고 CK 활성도는 I군에서 낮은 경향이 있었다. 4) $^{99m}Tc$-MIBI 심근 SPECT상 측부순환로가 발달한 I군에서 경색부위 가역성 관류결손 부위 빈도가 높았다 (83.4% vs 15.3%, p<0.05). 5) LVEDV, LVESV, EF는 두 군간에 차이가 없었다. 관동맥 조영술상 측부순환로의 존재는 $^{99m}Tc$-MIBI 심근 SPECT의 가역성 관류결손 부위와 유의한 연관성이 있었으며, 심근 SPECT는 기능적 측부순환을 관찰하는데 유용한 검사로 생각된다.

Trends and Outcomes of Type 2 Myocardial Infarction During the COVID-19 Pandemic in the United States

  • Background and Objectives: There is limited data on the impact of type 2 myocardial infarction (T2MI) during the coronavirus disease 2019 (COVID-19) pandemic. Methods: The National Inpatient Sample (NIS) database from January 2019 to December 2020 was queried to identify T2MI hospitalizations based on the appropriate International Classification of Disease, Tenth Revision-Clinical Modification codes. Monthly trends of COVID-19 and T2MI hospitalizations were evaluated using Joinpoint regression analysis. In addition, the multivariate logistic and linear regression analysis was used to compare inhospital mortality, coronary angiography use, and resource utilization between 2019 and 2020. Results: A total of 743,535 patients hospitalized with a diagnosis of T2MI were identified in the years 2019 (n=331,180) and 2020 (n=412,355). There was an increasing trend in T2MI hospitalizations throughout the study period corresponding to the increase in COVID-19 hospitalizations in 2020. The adjusted odds of in-hospital mortality associated with T2MI hospitalizations were significantly higher in 2020 compared with 2019 (11.1% vs. 8.1%: adjusted odds ratio, 1.19 [1.13-1.26]; p<0.01). In addition, T2MI hospitalizations were associated with lower odds of coronary angiography and higher total hospitalization charges, with no difference in the length of stay in 2020 compared with 2019. Conclusions: We found a significant increase in T2MI hospitalizations with higher in-hospital mortality, total hospitalization costs, and lower coronary angiography use during the early COVID-19 pandemic corresponding to the trends in the rise of COVID-19 hospitalizations. Further research into the factors associated with increased mortality can increase our preparedness for future pandemics.

Association Between Body Mass Index and Clinical Outcomes According to Diabetes in Patients Who Underwent Percutaneous Coronary Intervention

  • Background and Objectives: We evaluated the effect of diabetes on the relationship between body mass index (BMI) and clinical outcomes in patients following percutaneous coronary intervention (PCI) with drug-eluting stent implantation. Methods: A total of 6,688 patients who underwent PCI were selected from five different registries led by Korean Multicenter Angioplasty Team. They were categorized according to their BMI into the following groups: underweight (<18.5 kg/m2), normal weight (18.5-24.9 kg/m2), overweight to obese (≥25.0 kg/m2). Major adverse cardiac and cerebrovascular events (MACCE), defined as a composite of death, nonfatal myocardial infarction, stroke, and target-vessel revascularization, were compared according to the BMI categories (underweight, normal and overweight to obese group) and diabetic status. All subjects completed 1-year follow-up. Results: Among the 6,688 patients, 2,561 (38%) had diabetes. The underweight group compared to normal weight group had higher 1-year MACCE rate in both non-diabetic (adjusted hazard ratio [HR], 2.24; 95% confidence interval [CI], 1.04-4.84; p=0.039) and diabetic patients (adjusted HR, 2.86; 95% CI, 1.61-5.07; p<0.001). The overweight to obese group had a lower MACCE rate than the normal weight group in diabetic patients (adjusted HR, 0.67 [0.49-0.93]) but not in non-diabetic patients (adjusted HR, 1.06 [0.77-1.46]), with a significant interaction (p-interaction=0.025). Conclusions: Between the underweight and normal weight groups, the association between the BMI and clinical outcomes was consistent regardless of the presence of diabetes. However, better outcomes in overweight to obese over normal weight were observed only in diabetic patients. These results suggest that the association between BMI and clinical outcomes may differ according to the diabetic status.

Role of Myocardial Extracellular Volume Fraction Measured with Magnetic Resonance Imaging in the Prediction of Left Ventricular Functional Outcome after Revascularization of Chronic Total Occlusion of Coronary Arteries

  • Objective: The purpose of this study was to prospectively investigate the value of the myocardial extracellular volume fraction (ECV) in predicting myocardial functional outcome after revascularization of coronary chronic total occlusion (CTO). Materials and Methods: Thirty patients with CTO underwent cardiovascular magnetic resonance (CMR) before and 6 months after revascularization. Three baseline markers of functional outcome were evaluated in the dysfunctional segments assigned to the CTO vessels: ECV, transmural extent of infarction (TEI), and unenhanced rim thickness (RIM). At the global level, the ECV values of the whole myocardium with and without a hyperenhanced region (global and remote ECV) were respectively measured. Results: In per-segment analysis, ECV was superior to TEI and RIM in predicting functional recovery (area under receiver operating characteristic curve [AUC]: 0.86 vs. 0.75 and 0.73, all p values < 0.010), and it emerged as the only independent predictor of regional functional outcome (odds ratio [OR] = 0.83, 95% confidence interval [CI]: 0.77-0.89; p < 0.001) independent of collateral circulation. In per-patient analysis, global baseline ECV was indicative of ejection fraction (EF) at the follow-up examination (β = -0.61, p < 0.001) and changes in EF (β = -0.57, p = 0.001) in multivariate regression analysis. A patient with global baseline ECV less than 30.0% (AUC, 0.93; sensitivity 94%, specificity 80%) was more likely to demonstrate significant EF improvement (OR: 0.38; 95% CI: 0.17-0.85; p = 0.019). Conclusion: Extracellular volume fraction obtained by CMR may provide incremental value for the prediction of functional recovery both at the segmental and global levels in CTO patients, and may facilitate the identification of patients who can benefit from revascularization.

Ticagrelor Monotherapy After 3-Month Dual Antiplatelet Therapy in Acute Coronary Syndrome by High Bleeding Risk: The Subanalysis From the TICO Trial

  • Background and Objectives: Identifying patients with high bleeding risk (HBR) is important when making decisions for antiplatelet therapy strategy. This study evaluated the impact of ticagrelor monotherapy after 3-month dual antiplatelet therapy (DAPT) according to HBR in acute coronary syndrome (ACS) patients treated with drug eluting stents (DESs). Methods: In this post-hoc analysis of the TICO trial, HBR was defined by 2 approaches: meeting Academic Research Consortium for HBR (ARC-HBR) criteria or Predicting Bleeding Complications in Patients Undergoing Stent Implantation and Subsequent DAPT (PRECISE-DAPT) score ≥25. The primary outcome was a 3-12 months net adverse clinical event (composite of major bleeding and adverse cardiac and cerebrovascular events). Results: Of the 2,980 patients without adverse events during the first 3 months after DES implantation, 453 (15.2%) were HBR by ARC-HBR criteria and 504 (16.9%) were HBR by PRECISE-DAPT score. The primary outcome rate was higher in HBR versus non-HBR patients (by ARC-HBR criteria: hazard ratio [HR], 2.87; 95% confidence interval [CI], 1.76-4.69; p<0.001; by PRECISE-DAPT score: HR, 3.09; 95% CI, 1.92-4.98; p<0.001). Ticagrelor monotherapy after 3-month DAPT was associated with lower primary outcome rate than ticagrelor-based 12-month DAPT regardless of HBR by ARC-HBR criteria, with similar magnitudes of therapy effect for HBR and non-HBR patients (p-interaction=0.400). Results were consistent by PRECISE-DAPT score (p-interaction=0.178). Conclusions: In ACS patients treated with DESs, ticagrelor monotherapy after 3-month DAPT was associated with lower rate of adverse clinical outcomes regardless of HBR, with similar magnitudes of therapy effect between HBR and non-HBR.
