A Detection Method of Hexagonal Edges in Corneal Endothelial Cell Images (각막 내피 세포 영상내 육각형 에지 검출법)

  • In this paper, a method of edge detection from low contrast and noisy images which contain hexagonal shape is proposed. This method is based on the combination of laplacian gaussian filter and an idea of filters which are dependent on the shape. First, an algorithm which has six masks as its extractors to detect the hexagonal edges especially in the comers is used. Here, two tricom filters are used to detect the tricom joints of hexagons and other four masks are used to enhance the line segments of hexagonal edges. As a natural image, a corneal endothelial cell image which usually has a regular hexagonal shape is selected. The edge detection of hexagonal shapes in this corneal endothelial cell is important for clinical diagnosis. Next, The proposal algorithm and other conventional methods are applied to noisy hexagonal images to evaluate each efficiency. As a result, this proposal algorithm shows a robustness against noises and better detection ability in the aspects of the signal to noise ratio, the edge coineidence ratio and the detection accuracy factor as compared with other conventional methods.

Corneal Asphericity for Myopia in Korea (한국인 근시안의 각막 비구면성)

  • We evaluated the contour using corneal asphericity of the myopic cornea in Korea and investigated the relationship between refractive error and other ocular dimensions in Korean myopia, including anterior chamber depth and asphericity. The monoocular asphericity value of 50 myopes with $-4.83{\pm}2.38$ D between early 20 age and early 30 age in adult was included. Cycloplegic refraction, corneal asphericity and anterior chamber depth using corneal topography were examined. The mean asphericity values were$-0.27{\pm}0.13$ and the corneas of 96.0% were prolate ellipse. Refractive error was related to asphericity and anterior chamber depth among myopes. However, asphericity only were significant difference in high and low myopia group. Corneal configuration on the contact lens fits are discussed. We think that the longitudinal study for myopia and asphericity was required.

Mathematical Expression of the Toric Cornea using Corneal Topography Measurements (각막지형도(topography) 각막곡률로부터 토릭 각막형상의 수식화)

  • Purpose: To represent the shape of toric corea in the elliptical function for the determination of curvature distribution and lacrimal thickness between cornea and contact lens when the lens is fitted. Methods: Topography measurements of corneal curvature and curvature equation derived from the assumed elliptical function were evaluated using the Excel program which included the necessary equation derived. Results: Mathematical expressions for the cornea whose ribbon shaped-topography image, in which the center does not coincide with the corneal apex, can be determined. Conclusions: For the application where the higher accuracy on the cornea is not required, such as higher order aberration, the cornea cal be expressed in the simple elliptical function.

The Effects of Corneal Type and Corneal Astigmatism on Tear Volume between Rigid Gas Permeable Lens and the Cornea (각막형상 및 각막난시도가 RGP렌즈와 각막사이의 눈물양에 미치는 영향)

  • Purpose: In the present study, a difference in tear volume between the cornea and the rigid gas permeable (RGP) lens relative to corneal shape and corneal astigmatism was investigated by the alignment fitting status of spherical and aspherical RGP lenses. Methods: Spherical and aspherical RGP lenses were fitted with alignment in 77 subjects (135 eyes) who were in their 20~30s. Tear volume stained with fluorescein was qualitatively analyzed by dividing cornea into center, mid-peripheral and peripheral parts. Results: For the spherical RGP lens fitting, tear volume differences were found in each part in all corneal types. For the aspherical RGP lens fitting, tear volume differences were in each corneal part in symmetric bow tie- and asymmetric bow tie-type corneas. However, the tear was equally distributed from the center to the peripheral part in round- and oval-type corneas. In the group with corneal astigmatism lower than 1.25 D, tear volume between center and peripheral parts, and mid-peripheral and peripheral parts, was different when a spherical RGP lens was fitted. However, tear volume in each part was not different in the group with corneal astigmatism over 1.50 D. Moreover, the tear volumes of the central and mid-peripheral parts were proportionally increased with increasing corneal astigmatism in both spherical and aspherical RGP lenses. Furthermore, aspherical RGP lenses showed greater increments than spherical RGP lenses. Conclusions: The results revealed that the difference in tear volume between aspherical RGP lens and cornea was less than spherical RGP lens, and the difference in tear volume varied according to corneal shape and astigmatism. In addition, the method of measuring relative tear volume between RGP lens and cornea that was established in the present study can be used to evaluate tear volume between contact lens and cornea.

Comparison of Corneal Asphericity with Measuring Range (측정 범위에 따른 각막비구면계수의 비교)

  • Purpose: The purpose of this study is to investigate the changes of asphericity according to diameter and direction of the cornea. Methods: The changes of asphericity according to diameter and direction of cornea is measured by using Pentacam(Oculus Co., Germany), as targeting 57 adults (average $27.0{\pm}4.8$, 20 men, 37 women) without any ophthalmic diseases, which may be effective in eyesight, and refractive surgery. Results: As diameter increases in every direction based on the corneal vertex, asphericity has attentively increased (p<0.05), and the size of asphericity in each direction from every measured diameter showed as superior > nasal > inferior > temporal. In group comparison by nearsightness and astigmatism level, asphericity measured high when nearsightness and astigmatism levels were higher, and this appears to be statistically attentive.(p<0.05). Conclusions: Asphericity of cornea significantly increased according to increase of diameter of cornea, and as measured figure or increased quantity appeared differently in all directions based on corneal vertex, so it helps to understand shape of cornea, and considered that is the part to be considered during manufacturing and fitting of RGP lenses.

A Comparison of the Contact Areas between Cornea and RGP Lenses by Fitting Status (피팅 상태에 따른 RGP 렌즈와 각막과의 간극 비교)

  • Purpose: In this study, the effect of lens fitting status on the contact area between spherical/aspherical RGP lens and the cornea having different astigmatic degree and corneal type was investigated for guiding the proper selection of RGP lens. Methods: Spherical and aspherical RGP lenses were applied on ninety eyes $(25.12{\pm}3.52years)$ having with-the-rule astigmatism by different fitting status. Then, their central, mid-peripheral and peripheral areas of fluorescein pattern were calculated and compared for the quantitative evaluation of the contact area between spherical/aspherical RGP lens. Results: The central and peripheral areas with the alignment fitting was significant different based on lens design. However, the central area didn't show any significant difference by lens design and corneal type when fitted in steep or flat. When analyzed by the corneal shape, both lenses with alignment and flat fitting had significant difference in central and peripheral areas. However, the central, mid-peripheral and peripheral areas with steep fitting didn't show the difference by corneal types. When analyzed by the astigmatic degree, the central and peripheral areas with alignment fitting changed proportionally to the increase of corneal astigmatism regardless of corneal shape. With steep and flat fitting, however, the central, mid-peripheral and/or peripheral areas in round- and symmetric bowtie-typed corneas showed the conflicting result when compared to those of alignment fitting when analyzed by the astigmatic degree. Conclusions: In this study, it was confirmed that the contact areas of cornea and RGP lens fitted steep and flat status were largely affected by the corneal type and corneal astigmatism rather than RGP lens fitted in alignment status. Also, this result commonly occurred in both spherical and aspherical RGP lenses.

Treatment of descemetocele with deep keratotomy combined with a nictitating membrane flap in a dog

  • A 10-year-old, spayed female, Maltese dog presented with a 2-day history of severe left eye squint. Slit-lamp biomicroscopy showed a deep corneal defect stained into a doughnut shape together with hypopyon in the anterior chamber. Based on these results, a diagnosis of descemetocele and uveitis in the left eye was made. Deep keratotomy combined with a nictitating membrane flap effectively resolved the descemetocele without complications. The surgical procedures performed on this patient were easy and effective, and could be used as an alternative to graft surgery for descemetocele treatment.

As a result, this proposed algorithm shows a robustness against noises and better detection ability in the aspects of the output signal to noise ratio, the edge coincidence ratio and the extraction accuracy factor as compared with other conventional methods. At the second step, the lacking part of the thinned image by an energy minimum algorithm is complemented, and then the area and distribution of cells which give necessary information for medical diagnosis are computed.

Cytopathic Change and Inflammatory Response of Human Corneal Epithelial Cells Induced by Acanthamoeba castellanii Trophozoites and Cysts

  • Acanthamoeba castellanii has ubiquitous distribution and causes primary acanthamoebic keratitis (AK). AK is a common disease in contact lens wearers and results in permanent visual impairment or blindness. In this study, we observed the cytopathic effect, in vitro cytotoxicity, and secretion pattern of cytokines in human corneal epithelial cells (HCECs) induced by A. castellanii trophozoites and/or cysts. Morphological observation revealed that panked dendritic HCECs co-cultured with amoeba cysts had changed into round shape and gradually died. Such changes were more severe in co-culture with cyst than those of co-cultivation with trophozoites. In vitro cytotoxicity assay revealed the highest cytotoxicity to HCECs in the co-culture system with amoeba cysts. A. castellanii induced the expression of $IL-1{\alpha}$, IL-6, IL-8, and CXCL1 in HCECs. Secreted levels of $IL-1{\alpha}$, IL-6, and IL-8 in HCECs co-cultured with both trophozoites and cysts were increased at an early incubation time (3 and 6 hr). These results suggested that cytopathic changes and pro-inflammatory cytokines release of HCECs in response to A. castellanii, especially amoebic cysts, are an important mechanism for AK development.

An Ultrastructural Changes of Rat Corneal Epithelium and Stromal layer in Developmental Process (발생과정에 따른 흰쥐 각막의 상피층 및 실질층의 미세구조 변화에 관한 연구)

  • The present study was conducted to investigate the structural changes in rat cornea. Sixty eyes from one-day-old uneyed rats, fourty eyes from 4-weeks-old rats, and foully eyes from 10-weeks-old adult rats were used. With the increase of age, the epithelial layer was thickened by the addition of new successive cellular layers. Then, the new-born rat's epithelial cells formed a pentagonal shape, and the quality of decidual cells showed a high electron-density, although the boundary between cells was distinctive. The newly produced cells showed a low electron-density so that there was the distinctive difference between light and darkness. In Bowman's layer, collagen fibrils demonstrate a regularly arranged structure along with the age. In stroma's layer, the density of keratocytes was decreased and thereby Progressively flattened during the development. The collagenous layer of the adult rats was more distinctive than that of the new-born rats in a form of parallel alignment running vertically and horizontally.
