The Effect of Quercetin in Corneal Opacity Induced by Mitomycin-C

  • This study examined the effects of quercetin on corneal opacity caused by corneal edema by suppressing the damage on corneal endothelial cell, which was induced by mitomycin-C (MMC). In the MMC-treated group, the number of keratocytes was noticeably fewer compared to that of other groups. Although this group showed normal amount of fiber in the corneal stroma, the thickness was shown to be very thick and the alignment of the corneal endothelial cells that worked as the barrier against aqueous humor was irregular. According to such results, it was known that corneal opacity induced by MMC is not caused by proliferation of keratocytes, but by corneal edema triggered by the infiltration of aqueous humor. In the MMC+quercetin and quercetin+MMC-treated groups, the number of keratocytes was higher and polymorphonuclear leukocytes infilteration was lower significantly compared to that of the MMC-treated group. Although the amounts of fiber and endothelioid cell arrangement were normal, there was more space observed in the corneal stroma. Nonetheless, these groups showed significantly lower stromal thickness compared to that of the MMC group. In conclusion, quercetin has the effect on the reduction of corneal opacity caused by corneal edema that work MMC-induced damage to the corneal endothelial cells.

  • Hybrid optical-electronic procedures are introduced for the automated estimation of cell parameters (e.g., size, size variation, and shape). Two different optical Fourier analysis procedures are applied to high contrast cell boundary patterns obtained from specular micrographs of the endothelial layer. In one case, a large number of cell patterns are illuminated to extract average cell size information. Once the average cell size information has been obtained, individual cells are illuminated to extract shape information.

각막 내피 세포 영상내 육각형 에지 검출법 (A Detection Method of Hexagonal Edges in Corneal Endothelial Cell Images)

  • 본 연구에서는 육각형상을 포함하는 잡음이 많은 저 대비 영상으로부터 에지를 검출하는 방법을 제안한다. 이 방법은 라플라시안-가우시안 필터의 조합과 형상에 의존하는 필터의 아이디어에 기초하고 있다. 먼저, 모퉁이에서 특히 육각형의 에지를 검출하기 위한 검출기로서 6 개의 마스크를 갖는 알고리즘을 사용한다. 여기에서 두 개의 삼각화살 모양의 필터는 육각형의 삼각화살 모양의 접속부를 검출하기 위해 사용되고, 나머지 네 개의 마스크는 육각형 에지의 선성분을 강조하기 위해 사용된다. 자연영상으로서 보통 규칙적인 육각형상의 각막 내피 세포를 선택하며, 이 각막 내피 세포내 육각형상의 에지 검출은 임상 진단에 있어서 중요하다. 그 다음, 에지 검출법의 유효성을 평가하기 위해 제안 알고리즘과 기존 방법을 잡음을 포함하는 육각형 영상에 적용한 결과 본 제안 방법이 다른 방법에 비해 신호 대 잡음비와 에지의 일치율 및 검출 정확도에서 잡음에 대한 강인성과 양호한 검출 능력을 나타낸다.

뉴질랜드 흰 토끼의 각막 두께, 내피세포 및 안압에 관한 조사 (Survey of Corneal Thickness, Endothelial Cell and Intraocular Pressure in New Zealand White Rabbits)

  • Rabbit's cornea is the most common thing that used for researches in ophthalmology, so many experimentations have done. The purpose of this study was to survey the corneal pachymetry, tonometry and specular microscopy in New Zealand white rabbits. Three months old male rabbits ($2.9{\pm}0.2\;kg$) were examined with contact the specular microscope to evaluate the endothelial cell density, coefficiency of variation and the percentage of hexagonal cells in the central cornea. For the pachymetry, ultrasonic pachymeter was used for central cornea in each twenty rabbits, and tonometry was performed forty rabbit's eyes to evaluate the intraocular pressure. Corneal endothelial cell density in New Zealand white rabbit was $3304{\pm}354\;cell/mm^2$. Coefficiency of variation was $45.8{\pm}2$ and the percentage of hexagonal cells in the central cornea was $44.4{\pm}11%$. The pachymetry of cornea was $346.1{\pm}21\;{\mu}m$ and tonometry of eye was $14.3{\pm}2\;mmHg$. It is considered that normal New Zealand white rabbit's pachymetry, specular microscopy and tonometry were useful basic data of rabbit's cornea for research and clinic in veterinary ophthalmology.

Attenuation of Extracellular Acidic pH-induced Cyclooxygenase-2 Expression by Nitric Oxide

  • Corneal endothelial cells play an important role in maintaining the transparency and ionic balance of the cornea. Inflammation causes many changes in the intracellular and extracellular environment of the cornea, including acidosis. We examined the relationship between changes in extracellular pH and expression of cyclooxygenase-2 in cultured bovine corneal endothelial cells. When extracellular pH ($[pH]_o$) was reduced to pH 6.4, COX-2 mRNA increased, with a peak at 2 h. This was blocked by pretreatment with actinomycin D and incubation with spermine NONOate (SPER/NO, a nitric oxide donor). Exposure to the $H^+$ ionophore, carbonyl cyanide m-chlorophenylhydrazone (CCCP), also raised COX-2 mRNA levels. CCCP-induced COX-2 mRNA expression was also reduced by SPER/NO. These results were confirmed immuno-cytochemically. These data demonstrate that COX-2 expression is stimulated by the lowering of extracellular pH that could result from bacterial infection, and that this is countered by over-production of nitric oxide, which could also result from bacterial infection.

Edge Complement of the Cornea's Endothelial Cell Using Energy Function

  • An area distribution of Corneal Endothelial Cell(CEC) include important clinical information. In this paper, we present a two-step processing method of contour complement for the CEC. In the first step; we apply not only conventional Laplasian Gaussian filters(LGF) but also three-arrow-shaped LGFs which is newly developed to extract vertices of hexagonal shapes. In the second step; we complement the lacking part of CEC by using an energy minimum algorithm. Using the results, we measure areas of CEC.

"Letter-Box" Conjunctival Flap in a Dog with Severe Corneal Edema after Phacoemulsification

  • A two-year-old, spayed female, Bichon Frise, was referred for severe corneal edema and corneal ulcer in the left eye (OS). The cornea had gradually swelled over one week after phacoemulsification performed a month prior, and that was refractory to 5% sodium chloride eye drop instillation or temporary partial tarsorrhaphy. A complete ophthalmic examination was performed. Severe corneal edema with intrastromal bullae and moderate anterior chamber flare was found on slit-lamp biomicroscopy in the OS, which obstructed the fundus examination. Corneal thickness was measured using high-resolution ultrasound biomicroscopy. The thickness of the OS cornea was 2.74 mm. The "letter-box" conjunctival flap was planned. Dorsal and ventral superficial keratectomy followed by a hood conjunctival flap was performed. Topical and systemic antibiotics and 5% sodium chloride eye drops were prescribed. Decreased corneal thickness was observed at one week, two weeks, and two months postoperatively (1.53 mm, 1.32 mm, and 0.92 mm, respectively). There were no postoperative complications, such as ocular discomfort or recurrent corneal ulcers. The "letter-box" conjunctival flap, a type of superficial keratectomy and conjunctival advancement hood flap, effectively relieved the severe irreversible corneal edema. This could be a simple but effective surgical intervention for patients with endothelial cell damage especially after phacoemulsification.

각막 내피 세포 영상내 육각형 경계의 검출과 보완법 (Extraction and Complement of Hexagonal Borders in Corneal Endothelial Cell Images)

  • 본 연구에서는 저 명암 대비 영상에서 잡음이 많은 육각형을 포함하는 윤곽 검출과 보완의 2단계 처리방법을 제안한다. 이 방법은 라플라시안-가우시안 필터(LGF)의 조합과 형상에 의존하는 필터의 아이디어에 기초한다. 먼저, 1단계에서는 모서리에서 특히 육각형상의 에지를 검출하기 위한 검출기로서 6개의 마스크를 갖는 알고리즘을 사용한다. 여기에서 두 개의 삼각화살 모양의 필터는 육각형의 삼각화살 모양의 접속부를 검출하기 위해 사용되고, 기타 네 개의 필터는 육각형 주변의 에지를 검출하기 위해 사용된다. 자연영상으로서 보통 규칙적인 육각형상의 각막 내피 세포를 선택하며, 이 각막 내피 세포의 형상을 자동적으로 계측하는 것은 임상진단에 있어서 중요하다. 그 유효성을 평가하기 위해 제안 방법과 기존 방법을 잡음을 포함하는 육각형 영상에 적용한다. 그 결과, 제안 알고리즘이 기존의 다른 방법에 비해 잡음에 대한 강인성과 출력 신호 대 잡음비, 에지 일치율 및 검출 정확도의 면에서 보다 양호한 검출률을 나타냈다. 다음으로, 2단계에서는 에너지 최소화 알고리즘에 의한 세선화 영상의 결손 부분을 보완한 후 임상진단에 필요한 정보를 제공하는 세포의 면적과 분포를 계산한다.

거짓비늘증후군에서 개방각녹내장 병합 유무에 따른 전안부 소견과 생체 계측치에 대한 비교 (Comparison of Anterior Segment Features between Groups with or without Glaucoma in Pseudoexfoliation Syndrome)

  • 목적: 거짓비늘증후군에서 개방각녹내장 병합 유무에 따른 전안부 소견과 생체 계측치들을 비교 분석하여 거짓비늘증후군에서 녹내장 발생과 관련된 인자들을 알아보고자 하였다. 대상과 방법: 본원에서 거짓비늘증후군으로 진단된 96명(115안)을 대상으로 하였으며, 단순거짓비늘증후군 64명(76안)과 녹내장 병합 거짓비늘증후군 32명(39안)의 두 군으로 나누어 나이, 성별, 기저질환, 거짓비늘물질이 관찰된 부위 및 홍채의 변화, 핵백내장 정도, 산동 정도, 각막내피세포 수, 중심각막두께, 전방깊이, 안축장, 각막곡률, 안압을 비교하였다. 결과: 두 군 간 나이(p=0.694), 성별(p=0.161), 당뇨(p=0.440), 고혈압(p=0.238), 거짓비늘물질의 관찰 위치로 전낭, 동공연(p=0.700, 0.210), 홍채의 변화 양상으로 동공연 탈색소, 주름 소실(p=0.526, 0.708), 핵백내장 정도(p=0.617), 산동 정도(p=0.526), 중심각막두께(p=0.097), 전방깊이(p=0.283), 안축장(p=0.095), 수평, 수직 각막곡률(p=0.066, 0.306)에서 유의한 차이를 보이지 않았으며, 녹내장병합거짓비늘증후군에서 안압이 높고(p=0.026) 동공연의 막형성이 많으며(p=0.047), 각막내피세포 수의 감소(p=0.048)가 유의하게 관찰되었다. 결론: 거짓비늘증후군에서 개방각녹내장이 병합된 경우 높은 안압, 각막내피세포 수 감소, 동공연의 막형성 소견이 유의하게 비교 관찰되므로 이러한 변화 양상이 관찰될 때 녹내장 병합 가능성이 높음을 인지하여 경과관찰 시 세심한 주의가 필요할 것으로 사료된다.

Image Analysis Algorithm for the Corneal Endothelium

  • The number of the living endothelial cells and the shape of those are very import clinical parameters for the evaluation of the quality of cornea. In this paper, we developed the automated endothelial cell counting and shape analysis algorithm for a confocal microscope. Since, the endothelial images from the confocal microscope has a non-uniform illumination and low contrast between cell boundaries and cell bodies, it is very difficult to segment the cells from the endothelial images. To cope with these difficulties, we proposed the new two stage image processing algorithm. At first stage algorithm, we used a high-pass filter and histogram equalization to compensate the non-uniform brightness pattern and a morphological filter and a watershed method are applied to detect the boundary of cells. From this stage, we could count the number of cells in an endothelial image. At second stage algorithm, we used a Voronoi diagram method to classify the shape of cells. This cell shape analysis and the percent of hexagonal cells are very sensitive in detecting the early endothelium damage. To evaluate the performance of the proposed system, we p개cessed seven endothelial images obtained using a confocal microscope. The proposed system correctly counted 95.5% cells and classified 92.0% of hexagonal cell shapes. This result is better than any others in this research area.
