The Change of Corneal Refractive Power by Everted Silicone Hydrogel Contact Lens Wear (역방향 실리콘 하이드로겔 렌즈 착용에 따른 각막 굴절력 변화)

  • Purpose: The purpose was to study the corneal refractive power changes associated with the wearing of everted silicone hydrogel soft lenses. Methods: The corneal refractive power and corneal astigmatism were measured using corneal topographer (CT-1000, Shin-nippon Co., Japan) for checking change of corneal refractive power and objective refractive error was measured by auto-refractometer (Natural vision-K 5001, Shin-nippon Co., Japan). We measured at baseline and 1 week after lens wearing. Results: The correcting of corneal refractive power could be effective in low myopia. It's more effective to the higher power of greatest meridian of cornea and the more corneal astigmatism. 73% of subjects' refractive error was decrease less than 1 D and 17% of the subjects had an reverse effect (increase) occurs. The reduction of objective refractive error was more effective when cornea refractive power was great or corneal astigmatism was much. Conclusions: Pressure which the everted silicone hydrogel lens to the cornea could be caused. It occurred as the degrees of corneal power, corneal astigmatism and objective refractive error differences. Selection of an appropriate subject is important considering difficulty of changing the parameters of the lens.

Spherical and Aspherical RGP Lens Fitting to Epithelium and Endothelium of Rabbit's Cornea -Scanning Electron Microscopy (구면과 비구면 RGP 렌즈 장기 착용시 각막 상피, 내피에 미치는 영향-주사전자현미경적 관찰)

  • To investigate the comparative effect of spherical and aspherical RGP lens were worn during 3 weeks on rabbit's cornea. Four white rabbits were worn right eyes with spherical lens and 4 white rabbits were worn right eyes with aspherical RGP lens. Left eyes were served as control. The rabbits were sacrificed at 3 weeks after fitting and observed morphological changes by scanning electron microscopy and also investigate proliferation rate of the corneal epithelium with RGP wearing. After spherical RGP lens wearing, the epithet layer damaged compared to aspherical lens. The superficial cell layer strip off seriously, cell size significantly changed abnormal. Both spherical and aspherical RGP lens fitting group showed so many bacteria and back surface of lens was found like a fern shape. The aspherical RGP lens original material type was some formal than spherical lens. We thought that these pattern was significantly altered with spherical lens by prohibited transmitter oxygen from atmosphere therefore the epithelium shape was changed. This suggested wearing the aspherical lens might be less physiologic than shperical lens fitting.

The Analysis of Study Trends of Wild ginseng, Bovis Calculus, Ursi Fel, Moschus and The Study on Applied Possibility as Obesity medications (산삼, 우황, 웅담, 사향의 연구 동향 분석 및 비만 치료제로써의 응용 가능성 탐색 연구)

  • Objectives: After organizing segmentations related to obesity from classics and each studies, we would like to examine the value of wild ginseng, bovis calculus, ursi fel and moschus as a obesity medicine. Methods: We found out 4 drugs which are wild ginseng, bovis calculus, ursi fel and moschus from the classics, such as "Shin-Nong-Bon-Cho-Kyung", "Myoung-Eui-Byul-Lok", "Bon-Cho-Gang-Mok", "Bon-Cho-Jong-Shin", "Bon-Cho-Bi-Yo", "Bon-Cho-Gu-Jin" and "Deuk-Bae-Bon-Cho", we searched websites such as RISS, Korean traditional knowledge portal, the society of Korean medicine for obesity research, Korean pharmacopuncture institute, the Korean academy of oriental rehabilitation medicine and Korean acupuncture & moxibustion society as well. Results: 1. We couldn't find the contents related to the efficacy of improving obesity through wild ginseng, bovis calculus, ursi fel and moschus from the classics. 2. Wild ginseng had a study dealing with the efficacy of anticancer, obesity, toxicity, immunity, sulfation, diabetes, plasma proteins, depression, hypertension, endometriosis and hair loss. Especially among 6 studies related to obesity, apart from the study that treated extract of wild ginseng directly to the cell or oral administration to the animals, also have studies that had an efficacy of improving obesity through injecting pharmacopuncture to acupuncture point and vein, etc. 3. Bovis calculus had a study about efficacy related to arthritis, liver, inflammation of cornea, obesity, anticancer, immunity, antacid, heart, lumbar pain, hypertension, toxicity, a digestive organ and sasang constitution, especially in case of obesity, had each studies about oral administration of complex prescription and about cell unit experiment of pharmacopuncture. 4. Ursi fel had a study about efficacy related to liver, arthritis, inflammation of cornea, immunity, obesity, anticancer, antacid, lumbar pain, digestive organ, toxicity, cell protection, skin ailment, etc. Especially in case of obesity, had each studies about oral administration of animal experiment and about cell unit experiment of pharmacopuncture. 5. Moschus had a study about efficacy related to liver, the central nervous system, arthritis, inflammation of cornea, immunity, obesity, anticancer, hypertension, heart, lumbar pain, etc. Especially in case of obesity, had one study about oral administration of complex prescription. Conclusion: Through the result of probability as a obesity medicine of wild ginseng, bovis calculus, ursi fel and moschus, we could identify the possibility of each drug as a obesity medicine. Also in the future, it is considered that studies about one or complex prescription and pharmacopuncture as a obesity medicine will be needed.

A Relationship between Corneal Eccentricity and Stable Centration of RGP Lens on Cornea (각막 이심률과 RGP 렌즈의 중심안정위치와의 상관관계)

  • Purpose: The present study was performed to investigate a relationship between the stable centrations of spherical RGP lens and aspherical RGP lens on cornea and corneal eccentricity. Methods: Two RGP lenses with different designs were fitted in alignment, steep or flat on total 84 eyes having corneal eccentricity of 0.28~0.78. The stable centration of lenses on cornea was analyzed by taking photographs with a high-speed digital camera. Results: The stable centrations of spherical and aspherical RGP lenses in horizontal direction were decentrated to temporal side. More centration to median side was shown when corneal eccentricity was larger. The difference between the stable centrations of spherical and aspheric RGP lenses according to corneal eccentricity was bigger when the fitting state was flatter. The difference in the stable centrations of aspherical RGP lens was smaller than that of spherical RGP lens regardless of fitting status. The stable centrations of spherical and aspherical RGP lenses in vertical direction were located below corneal apex regardless of fitting status however, there was no significant difference analyzed by the variation of corneal eccentricity. However, there were many cases that RGP lenses were in upper eyelid with increasing corneal eccentricity. Conclusions: The consideration of corneal eccentricity is required for RGP lens fitting and manufacturing aspherical RGP lens since the stable centration of spherical RGP lens as well as aspherical RPG lens' centration was changed depending on corneal eccentricity.

The Investigation and Development of Astigmatism Correction Treatments by Finite Element Method and Animal Experiments (유한요소법과 동물실험을 통한 난시교정술의 고찰 및 개발)

  • The purpose of this study is to investigate the effects of various factors in keratotomy for astigmatism correction on surgical outcomes by finite element method as well as animal experiments. Three kinds of surgical techniques were mechanically investigated : arcuate, straight, and inverse arcuate keratotomy. Among the three techniques the arcuate keratotomy is the most popular one while the other two techniques are being investigated in this area. The arcuate keratotomy was found to be more controllable and effective in reducing the refractive power than the others. In arcuate keratotomy it was found most effective when the incision was located in the middle position between the apex and the edge of the cornea from the results of experiment as well as finite element study. Regarding to the range of the corneal incision in arcuate keratotomy, the incision angle of 90$^{\circ}$ was found th be most effective in reducing refractive power than other angles even it was incised up to 150$^{\circ}$. Therefore, it was concluded that 90$^{\circ}$ of incision angle results in the largest decrease in refractive power in arcuate keratotomy. However, other important findings were that the effect of the surgery decreased with time so the visco-effect of the cornea and auto-healing process. Therefore, these factors should be considered in future studies.

Effect of Intense Regulated Pulse Light on Thickness of Tear Film Lipid Layer and Dry Eye Syndrome (조절펄스광선조사가 눈물막 지질층 두께 및 안구건조증에 미치는 효과)

  • Purpose: To evaluate the clinical effect of intense regulated pulse light (IRPL) and changes in tear film thickness in dry eye patients. Methods: A retrospective study was conducted on 25 patients with Meibomian gland dysfunction who had subjective dry eye discomfort and whose tear film break-up time (TBUT) was < 10 seconds. All patients were treated with IRPL three times on days 1, 15, and 45. TBUT, Schirmer 1 test results, cornea staining score with fluorescein, ocular surface disease index (OSDI), and lipid layer thickness were measured and compared before and after the procedure. Results: TBUT was significantly increased from $3.7{\pm}1.2s$ to $4.4{\pm}2.1s$ after IRPL (p = 0.03). Schirmer 1 test and cornea staining scores changed significantly (p < 0.01 and p = 0.01, respectively). OSDI also showed a statistically significant improvement from $39.5{\pm}17.5points$ to $30.4{\pm}18.7points$ (p = 0.01). However, no significant difference in lipid layer thickness was observed (p = 0.49). Conclusions: IRPL is an effective treatment modality to improve TBUT, Schirmer 1 test scores, cornea staining scores, and subjective dry eye symptoms.

Edge Complement of the Cornea's Endothelial Cell Using Energy Function

  • An area distribution of Corneal Endothelial Cell(CEC) include important clinical information. In this paper, we present a two-step processing method of contour complement for the CEC. In the first step; we apply not only conventional Laplasian Gaussian filters(LGF) but also three-arrow-shaped LGFs which is newly developed to extract vertices of hexagonal shapes. In the second step; we complement the lacking part of CEC by using an energy minimum algorithm. Using the results, we measure areas of CEC.

Analysis of Corneal Higher-order Aberrations after Myopic Refractive Surgery

  • This study was performed to analyze the optical aberrations of the cornea induced by myopic refractive surgery. Corneal total higher-order aberrations, spherical aberration and coma for 4-mm and 6-mm pupils were measured using a wave-front analyzer. The amount of aberrations of the oblate corneal optics by the achieved correction was found to be larger than for the prolate corneal shape with complete eye, in an emmetropia control group. The change in corneal shape acts as an optical factor that degrades the quality of the retinal image; it seems to be one of the important factors related to quality of vision.
