Background: When performing dental treatment under general anesthesia in adult patients who have difficulty cooperating due to intellectual disabilities, anesthesia induction may be difficult as well. In particular, patients who refuse to come into the dental office or sit in the dental chair may have to be forced to do so. However, for adult patients with a large physique, physical restraint may be difficult, while oral sedatives as premedication may be helpful. Here, a retrospective analysis was performed to investigate the effect of oral sedatives. Methods: A hospital-based medical information database was searched for patients who were prescribed oral midazolam or triazolam between January 2009 and December 2017. Pre-anesthesia evaluation, anesthesia, and anesthesia recovery records of all patients were analyzed, and information on disability type, reason for prescribing oral sedatives, prescribed medication and dose, cooperation level during anesthesia induction, anesthesia duration, length of recovery room stay, and complications was retrieved. Results: A total of 97 patients were identified, of whom 50 and 47 received midazolam and triazolam, respectively. The major types of disability were intellectual disabilities, autism, Down syndrome, blindness, cerebral palsy, and epilepsy. Analyses of changes in cooperation levels after drug administration showed that anesthesia induction without physical restraint was possible in 56.0% of patients in the midazolam group and in 46.8% of patients in the triazolam group (P = 0.312). Conclusions: With administration of oral midazolam or triazolam, general anesthesia induction without any physical restraint was possible in approximately 50% of patients, with no difference between the drugs.
Background: Sedation or outpatient general anesthesia is a necessary method for a successful dental treatment for children and the disabled who have a difficulty of cooperation. The aim of this study was to assess the patients who had dental treatment under sedation or outpatient general anesthesia at department of pediatric dentistry, Chonnam national university dental hospital and Gwangju dental clinic for the disabled. Methods: 875 patients who had sedation from January 2009 to September 2013 and 88 patients who had outpatient general anesthesia after May 2011 were assessed for this study. Patient's distribution and treatment patterns were examined. Results: The number of patients has been increasing in both sedation and outpatient general anesthesia. In case of sedation, the proportion of male patients was higher and 4-6 years age group was the highest. Most patients have resided in Gwangju buk-gu and the percentage of restorative treatment was the highest. In case of outpatient general anesthesia, the proportion of male patients was higher than that of sedation. Two groups, 10-19 and 20-29 years age, consisted most of patient. Most patients have resided in Gwangju buk-gu and Jeollanam-do, and the percentage of periodontic and preventive treatment was the highest. Conclusions: Sedation or outpatient general anesthesia for dental treatment has been increasing for children and the disabled who have a difficulty of cooperation. Therefore, it is important to improve treatment environment under sedation and general anesthesia through continuous research and studies.
Purpose: This study was conducted to identify the job competencies of HIV counseling nurses and to figure out the importance, level of difficulty, and performance frequency of the competencies. Methods: The job competencies were drawn from 22 HIV counseling nurses by developing new models and utilizing specialist groups. The importance, level of difficulty, and performance frequency of the competencies were analyzed using the average and deviation. Results: The results of this study identified 6 job competencies such as counseling performance, expertise utilization, cooperation, support, administration, and self-development, 28 sub-competencies, and 48 job descriptions. Out of the 6 job competencies, counseling performance recorded high levels in the importance, level of difficulty, and performance frequency. Conclusion: The study results will be used as the basic data of the training programs for strengthening the job competencies of HIV counseling nurses. Furthermore, it is expected that the results will contribute to the prevention of HIV/AIDS transmission, the core of the national HIV/AIDS control programmes, and the improved health and quality of life in people living with HIV.
Firms prefer in some specific conditions cooperative R&D to non-cooperative for developing technology. Previous studies on the conditions show firms want to choose cost-sharing type of cooperative R&D (the CS-RJV) rather than non-cooperative one when target technology is either 'relatively easy'or 'relatively difficult', and to join multiple-research type of cooperation (the MR-RJV) than to compete each other if technology is only 'relatively easy'. However, by introducing technological spillover as well as difficult of technology, only if there shown that this seemingly contrasted phenomenon almost disappears : the MR-RJV can i3e also preferred by firms even in case of 'relatively difficult'technology only if there exists some extent of a technological spillover.
This paper studied on the relationships between productivity and job satisfaction in small group activities. The researches were practiced with the same method which prof. Rao of Babson university et al.(1987) did on autonomous work groups in U.S.A.. The researcher divided companies of Korea and Japan into similar small groups such as high productive groups and low productive groups in each country and analyzed their degree of job satisfaction. According to the result, high productive groups indicated higher degree of job satisfaction than low productive groups. In the case of Korea, besides activating QC circle activity, it is also necessary to equalize the difficulty of the works, or to set up the pay system taken the difficulty of the works into consideration. In the case of Japan, it is important to encourage good human relationships and member's cooperation and unity.
본 연구의 목적은 라쉬분석을 통해 작업치료사의 전문직 정체성의 수준 및 특성을 파악하고, 추후 작업치료사 정체성에 연구의 기초로 활용하는 것이다. 연구 대상자는 현재 작업치료사로 종사하고 있는 88명을 대상으로 2020년 5월부터 6월까지 구글 온라인 설문조사를 실시하였다. 수집된 자료는 SPSS 22.0 빈도와 기술통계, Winsteps 3.8.0 항목 및 대상자 적합도, 항목난이도, 평정척도, 분리 신뢰도 등 라쉬분석을 하였다. 분석 결과 대상자 중 17명(16.3%)가 부적합, 항목은 25항목 중 5개(20%) 부적합으로 나타났다. 항목난이도는 '나는 체계적으로 작업치료 전문 간행물(소식지, 신문 등)을 읽는다' 항목이 가장 높은 난이도이며, '나는 내가 할 일에 대하여 스스로 결정한다'가 가장 낮은 난이도를 보였다. 평정척도는 3점 척도가 적합한 것으로 나타났고, 분리신뢰도는 .86~.95이었다. 작업치료사의 전문직 정체성에 대한 수준과 특성 파악하고, 항목 및 대상자 적합도, 항목난이도를 라쉬분석을 통해 파악할 수 있었다. 향후 다양한 표본집단을 대상으로 연구가 진행되어야 할 것이며, 작업치료사만을 위한 전문직 정체성 측정 도구 개발 및 업무 특성에 맞는 항목이 제시되기를 기대한다.
본 연구는 암 병동에 근무하는 간호사의 공감피로 경험의 의미와 본질을 이해하기 위해 수행되었다. 연구 참여자는 암 병동 근무기간이 최소 6개월 이상인 11명을 대상으로 하였다. 심층면담을 실시하여 자료를 수집하였으며, Giorgi의 현상학적 방법으로 분석하였다. 연구결과 신체적으로 힘듦, 환자의 상황에 갇힌 마음, 암이라는 병 앞에서 무너짐, 되새겨지는 환자관련 기억, 변화된 자신의 모습을 발견함, 회복의 길을 찾아감의 6개의 범주로 도출되었다.
인터넷이 널리 보급되면서 지능형 검색 에이전트들이 사용자의 요구를 만족시키기 위해 일반화되어 사용되고 있다. 그러나 이러한 지능형 멀티에이전트들은 서로 독립적으로 사용되어 멀티에이전트들 간의 분산된 정보를 원활하고 효율적으로 처리하기 위한 상호 협력 작용이 부족해 정보의 신뢰성이 낮고 동적으로 변화하는 분산 환경에 대처하기가 어렵다. 이런 문제를 해결하기 위해 본 논문에서는 멀티에이전트간의 효율적인 상호 협력과 빠른 정보처리를 위해 브로커 에이전트에 에이전시를 생성하고 신경망을 이용해 멀티에이전트들의 에이전시들을 분류하여 더욱 신속·정확한 정보를 사용자에게 제공하도록 한다. 또한 정보의 신뢰성을 위해서 에이전트 관리기법을 제안하여 기존의 검색 시스템이 가지고 있는 정보갱신문제를 향상시키고, 시뮬레이션을 통해 본 연구의 성능을 평가한다.
Trauma to the lips and tongue can occur by accidental self-biting after dental treatment. After local anesthesia, it is likely that the patient may feel painless even in biting the tongue. In case of young children and disabled patients, the dentists should be careful not to bite the tongue. In this report, we present a case of deep lingual laceration due to biting the tongue in the course of dental treatment under general anesthesia. A 33 year-old male was transferred to our hospital to treat tongue laceration. Before 2 hour on arrival, he had received dental care under general anesthesia at a dental hospital for the disabled because of cooperation difficulty and cerebral palsy. During recovery from general anesthesia, he tried to bite his own tongue involuntary. The doctors and nurses tried to prevent the patient from being injured. Despite these efforts, massive bleeding occurred from the injured sites of the tongue. Because we could not communicate with him, we decided to evaluate the extent of the injury and treat the injured sites under general anesthesia. The laceration wound was sutured for nearly 1 hr general anesthesia. During recovery we inserted mouth prop into the oral cavity to prevent further injuries from tongue biting. After full recovery from general anesthesia he didn't try to bite his tongue. After 4 hour admission, he was discharged without other complications.
본 논문은 코일 제작 시 고려사항인 1, 2차측 코일 인덕턴스, 턴수, 크기, 두께, 차폐지 등을 고려하여 리쯔 와이어로 나선형 코일을 제작하는데 이때 반자동화 권선기를 사용하게 된다. 1, 2차측 코일 사양을 정하여 코일을 제작할 때 다양한 사양의 리쯔 와이어와 나선형 코일을 필요로 하게 되는데, 이때 제작 비용, 기간, 제작 시 발생하는 불량 등으로 인해 코일 샘플 제작에 어려움이 발생하고 원하는 사양의 코일 품질 관리에도 어려움이 생기게 된다. 이를 개선하기 위해 2차측의 정류부를 개선함으로써 코일 설계 및 제작 시 다양한 코일 제작 사양의 범위를 좀더 포괄적으로 할 수 있으며 제작 시 발생하는 비용을 줄이고, 기간 단축 및 코일의 품질 관리를 개선하고자 한다.
