NOVEL TREATMENT MODALITY OF IMPLANT-RETAINED OVERDENTURE FOR IMMEDIATE FUNCTION IN COMPLETELY EDENTULOUS PATIENTS (무치악 환자에서 하악 임플랜트 지지 피개의치에 의한 즉시 하중 부담에 관한 연구)

  • Statement of problem: Conventional mandibular complete denture lacks in support and stability for edentulous patients to use. Purpose: To enhance support and stability of mandibular complete denture, 2 implants were inserted on the inter-mental foramina space of edentulous patients. With 2 magnetic attachments on the inner side of the overdenture, we gave immediate loads and evaluated the stability of the implants, the responses of the supporting tissues, and the satisfaction of the patients. Material & methods: 6 edentulous patients (5 male, 1 female) each recieved 2 SLA implants in the inter-mental foramina region and implants were immediately loaded with overdenture with magnet attachments. To evaluate the implants stability and the peri-implant tissues, clinical exams, RFA tests and radiographic exams were preformed at the 1 week, 2wks, 6wks, 12wks, and 24wks. post- surgery. Results: The mean surgery time was $45.7{\pm}7.7mins$., while the denture delivery time was $45.5{\pm}12.6mins$. Only 2 of 14 implants were failed. Survived implants that remained were clincally and radiographically stable. Mean ISQ values were relatively stable, showing $69.71{\pm}5.55$, $69.00{\pm}9.48$, $67.92{\pm}7.86$, $67.92{\pm}9.58$, $70.08{\pm}7.61$, $71.92{\pm}6.43$ at the 1 wk, 2 wks, 6 wks, 12 wks, 24 wks. follow up check. Crestal bone changes were $-1.18mm{\pm}0.68mm$, $-1.35{\pm}0.69mm$, $-1.47{\pm}0.68mm$ at the 6wks, 12wks, 24wks. follow up check. Bleeding on probing(BOP index) was not significant. Conclusion: Mandibular ovedentures with 2 magnetic attachments over two interforaminal implants on edentulous patients for immediate function is a recommendable novel treatment for edentulous patients which shows stability on the implants and supporting tissue.

  • Implant fixed prosthesis for the complete edentulous maxilla provides significant benefits in the aspects of functions and esthetics compared with the conventional denture. Implant supported fixed prosthesis are totally supported by implant, and thus stabilizes the prosthesis to the maximum degree as possible. Also, the improved retention and stability of fixed prosthesis enhance patients' psychological and psychosocial health. This clinical presentation describes a maxillary full arch implant-supported fixed prosthesis in complete maxillary edentulous patient who showed vertical and horizontal alveolar bone resorption in the anterior ridge. To rehabilitate the esthetics and proper lip support, the zirconia framework was fabricated and the pink porcelain was veneered to reproduce the natural gingival tissue. After 9 months of follow up, the restorations were maintained without complications and the patient was satisfied with the restoration both functionally and esthetically.

  • A patient failed to use conventional complete dentures due to severe mandibular bone resorption and soft tissue hyperplasia on lips, tongue and oral vestibule from vascular malformation. The treatment of choice would be an implant-retained overdenture, however, he refused owing to high cost. The closed mouth impression technique using BPS principle, CR record registration with gothic arch tracing and monoplane occlusion using non-anatomic teeth were used to fabricate complete dentures for this patient. Patient satisfaction, the retention and the stability of complete dentures were highly improved as a result.

  • It is difficult to provide prosthodontic treatment to fully edentulous patients with severe alveolar bone resorption, because this makes patents hard to endure functional load, and to adapt to the dentures, which result in emotional stress to patients. Implant supported overdenture can be chosen to solve these problems. Among several types of them, the implant retained and tissue supported overdenture, is available to the patients of low masticatory force with the reduced cost. Attachments also can be used for increasing retention and esthetics in dentures. Especially, $Locator^{(R)}$ system needs a small vertical interarch space for restoration and is able to compensate the difference of angle between fixtures. In this study, we restored maxilla with conventional complete denture, and mandible with implant and tissue supported overdenture and $Locator^{(R)}$ system.

  • In the case of fully edentulous patients with severe alveolar bone resorption, the consideration of 'All-on-X' implant-supported fixed prosthesis after placing four or more implants in the anterior maxilla is possible. Recent advancements in digital dentistry have enabled systematic and predictable treatment in all phases, including diagnosis, surgery, and prosthesis fabrication. By incorporating digital dentistry techniques such as digital complete denture, implant surgical guides, facial scanning into the conventional restoration process, it is possible to reduce the complexity of the prosthesis fabrication and effectively achieve the transition from provisional prosthesis to definitive prosthesis in terms of both aesthetics and function.

  • The tongue is one of the most common sites of oral cancer. Glossectomy is known as the gold standard for tongue cancer treatment. However, surgical removal can lead to reduced mobility of the tongue and the patients may have difficulty performing normal oral functions like swallowing and pronunciation. Therefore, additional prosthetic consideration to supplement the function of the impaired tongue is needed for oral rehabilitation of such patients. Palatal augmentation prosthesis helps the tongue to reach the palate by lowering the position of the palatal polished surface. The oral functions of the patients with limited tongue mobility can be improved by the prosthesis. In this case, palatal augmented maxillary denture and conventional mandibular denture were fabricated for the completely edentulous patient with reduced tongue mobility after glossectomy due to tongue cancer. As a result, the oral functions of the patient were improved with the prosthesis.

Prediction accuracy of incisal points in determining occlusal plane of digital complete dentures

  • PURPOSE. This study aimed to predict the positional coordinates of incisor points from the scan data of conventional complete dentures and verify their accuracy. MATERIALS AND METHODS. The standard triangulated language (STL) data of the scanned 100 pairs of complete upper and lower dentures were imported into the computer-aided design software from which the position coordinates of the points corresponding to each landmark of the jaw were obtained. The x, y, and z coordinates of the incisor point (XP, YP, and ZP) were obtained from the maxillary and mandibular landmark coordinates using regression or calculation formulas, and the accuracy was verified to determine the deviation between the measured and predicted coordinate values. YP was obtained in two ways using the hamularincisive-papilla plane (HIP) and facial measurements. Multiple regression analysis was used to predict ZP. The root mean squared error (RMSE) values were used to verify the accuracy of the XP and YP. The RMSE value was obtained after crossvalidation using the remaining 30 cases of denture STL data to verify the accuracy of ZP. RESULTS. The RMSE was 2.22 for predicting XP. When predicting YP, the RMSE of the method using the HIP plane and facial measurements was 3.18 and 0.73, respectively. Cross-validation revealed the RMSE to be 1.53. CONCLUSION. YP and ZP could be predicted from anatomical landmarks of the maxillary and mandibular edentulous jaw, suggesting that YP could be predicted with better accuracy with the addition of the position of the lower border of the upper lip.

A case of maxilla implant overdenture using Pekkton telescopic attachment with severe alveolar bone resorption (심한 치조골 소실이 있는 상악 무치악 환자에서 Pekkton telescopic attachment를 이용한 임플란트 피개의치 증례)

  • It is necessary to set the correct occlusal plane and to distribute the occlusal force uniformly considering the state of the opposing dentition during the prosthetic of the single edentulous patient with severe alveolar bone resorption. Implant supported overdenture is superior to complete denture in terms of maintenance and stability, and limited implants are used in fully edentulous patients with high alveolar bone resorption. Telescopic attachments using a newly introduced material based on poly-aryl-ether-ketone (PAEK) have the advantages of typical telescopic copping, excellent abrasion resistance, and are lighter and more economical than conventional implant overdentures. In this case, we restored maxillary arch with a implant retained overdenture using the telescopic attachment made of Pekktonand the mandible was restored with fixed implant prosthesis. Through these procedures esthetic aspects and functional outcomes were satisfactorily achieved.

A STUDY ON THE CHANGES IN RETENTION OF CLIPS USED TO RETAIN IMPLANT-SUPPORTED OVERDENTURE (임플랜트지지 overdenture용 clip의 유지력 변화에 관한 연구)

  • Implant-supported overdenture is known as a useful appliance, instead of using the conventional complete denture, for better retention and stability. In this study 4 types of materials such as, gold bar/plastic clip(group AuP), gold bar/metal clip(group AuM), palladium bar/plastic clip(group PdP), and palladium bar/metal clip (group PdM) were used to evaluate the retention forces according the type of clips and alloys used for bar fabrication, in the Hader bar system. Repeated insertions and removals of overdenture were conducted in each group. and the retention forces were measured and compared the data of each group according to the number of insertion and removal. The obtained results were as follows, 1. In the comparison of retention forces according to type of bar-clip, retention was increased in the order of group AuM, PdM, PdP AuP. and the retention force of group AuM was significantly increased compared with those of others (p<0.05). 2. In the comparison of retention forces according to the number of insertion, only group PdP showed significant decrease in retention(p<0.05). 3. In the comparison of retention forces according to the type of bar and clip. there was no significant difference in the type of bar, but the retention of plastic clip was significantly higher than that of metal clip when Au bar was used(p<0.05). 4. In the observation of the bar surface, group AuM using Au bar and metal clip showed the most scratches among bar groups.

  • Overdenture using dental implants could improve the problems of conventional complete denture function which are pain during mastication, insufficient retention and stability. Locator attachment used widely for implant-retained overdenture has advantages that it needs the smallest vertical space and also its nylon male cap allows personalized retention for each case. However its retention force decreases rapidly with function rather than the bar and ball attachment. So, implant fixture should be positioned as parallel as possible.
