Usefulness of ultrasound contrast media for cardiac output measurement with echocardiography

  • The purpose of this study was to determine if contrast media would enhance visualization of the endocardium for assessment of left ventricle (LV) function. In addition, differences between pre- and post-contrast evaluation for the cardiac output measurements including the modified Simpson's method and automated contour tracking (ACT) method were examined. Ten clinically healthy adult beagle dogs (three males and seven females) between 2~3 years old and weighing 6.6~10.8 kg were used. Echocardiographic examinations were performed to compare pre- and post-contrast LV endocardium visualization using a segmental scoring method. Two different methods for measuring cardiac output were also compared. LV visualization was significantly enhanced in post-contrast echocardiography (p < 0.01). Significant differences between pre- and post-contrast measurements for the modified Simpson's method (p < 0.05) were also observed. No significant difference was found for the ACT method. Contrast echocardiography provides better LV chamber opacification and significantly improves wall segment visualization. Furthermore, contrast echocardiography for measuring cardiac output is helpful for the modified Simpson's method.

  • Apical hypertrophic cardiomyopathy is rare disease and a variant of hypertrophic cardiomyopathy with prevalence of 1 in 500 in the general population. Apical hypertrophic cardiomyopathy with apical aneurysm and intramural thrombus is extremely rare. We report a case of apical hypertrophic cardiomyopathy progressing to left ventricular apical aneurysm with intramural thrombus diagnosed by contrast echocardiography.

  • Echocardiography is one of the most frequently used techniques for diagnosing cardiovascular diseases. Over the last twenty years, technological advances have enabled the application of high-quality imaging. Important recent developments have occurred in echocardiography that are already being used clinically. Equipment and hardware is now available to produce reai time three-dimensional and contrast enhanced imaging.. Tissue Doppler and stress echocardiography have provided potential benefit to analyze hemodynamic information of heart. This review discusses each of these new developments and their potential impact on the practice of echocardioaraphy and cardiology in general.

Ebstein anomaly, right-to-left atrial septal defect, and cor triatriatum dexter in a cat: a case report

  • A 6-month-old male Ragdoll cat presented with exercise intolerance. On physical examination, there was a grade 2/6 systolic murmur at the right apex. Diagnostic tests, including SpO2 measurement, blood tests, radiography, echocardiography, contrast echocardiography, and electrocardiography, were performed. Severe right atrial dilation, tricuspid valve leaflets and orifice displacement, right ventricular atrialization, septal leaflet adherence, anterior leaflet tethering, and right atrioventricular junction dilation were noted on echocardiography, alongside a right-to-left atrial septal defect. Cor triatriatum dexter and left ventricular aneurysm were observed. We diagnosed this case as having Ebstein anomaly with rare congenital heart deformities; which is rare in cats.

Correlation between Reverse Redistribution and Subendocardial Myocardial Infarction Observed in Myocardial Contrast Echocardiography (역재분포 소견과 심근조영 심초음파에서 보인 심내막하경색과의 관계)

  • Purpose: The aim of this study is to better understand the pattern and nature of reverse redistribution (RR) in myocardial perfusion imaging. Materials and Methods: In consecutive 20 acute myocardial infarction (MI) patients, frequency of RR was correlated with that of subendocardial MI that was detected by myocardial contrast echocardiography (MCE). RR was judged to be present when there was more than one grade of worsening in perfusion at 24 hr delayed images compared with the initial rest images. MCE evaluated the significant lack of opacification in the subendocardial myocardium relative to the subepicardial myocardium to suggest the subendocardial MI. Kendall's nonparametric correlation coefficiency was calculated. Results: Concordant cases were 15 of 20 (75%) and correlation was statistically significant (p=0.0285). Conclusion: Our results suggested that RR was correlated with MCE-detected nontransmural MI.

  • Echocardiography and catheterization angiography suffer certain limitations in the evaluation of congenital heart diseases in adults, though these are overcome by MRI, in which a wide field-of view, unlimited multiplanar imaging capability and three-dimensional contrast-enhanced MR angiography techniques are used. In adults, recently introduced fast imaging techniques provide cardiac MR images of sufficient quality and with less artifacts. Ventricular volume, ejection fraction, and vascular flow measurements, including pressure gradients and pulmonary-to-systemic flow ratio, can be calculated or obtained using fast cine MRI, phase-contrast MR flow-velocity mapping, and semiautomatic analysis software. MRI is superior to echocardiography in diagnosing partial anomalous pulmonary venous connection, unroofed coronary sinus, anomalies of the pulmonary arteries, aorta and systemic veins, complex heart diseases, and postsurgical sequelae. Biventricular function is reliably evaluated with cine MRI after repair of tetralogy of Fallot, and Senning's and Mustard's operations. MRI has an important and growing role in the morphologic and functional assessment of congenital heart diseases in adolescents and adults.

  • Information about regional wall motion obtained with gated $^{99m}Tc-MIBI$ SPECT was compared in 23 subjects(4 normals) to echocardiography or contrast left ventriculography. Gated SPECT study using 20-30 mCi $^{99m}Tc-MIBI$ were aquired lot 8 frames/RR interval (aqusition time=40min.) and processed with Cine Mode computer protocol. Regional wall motion was assessed by visual analysis. The concordance with echocardiography was 88% and with contrast ventriculography was 78%. We conclude that gated $^{99m}Tc-MIBI$ SPECT was effective in assessing the regional wall motion with the information of regional perfusion in one study.

  • Cardiac papillary fibroelastoma (CPF) is the second or third most common primary cardiac tumor. Although histologically benign, it can cause serious symptoms depending on its location of occurrence, size, and motility. Herein, we report CPF in the left ventricular trabeculation as a potential cause of cerebral infarction.

Right-to-left Shunting Ventricular Septal Defect in a Dog (개에서 발생한 우-좌 단락 심실 중격 결손 증례)

  • A 4-year-old castrated male Dachshund was presented with a chronic history of dyspnea and cyanosis. Complete blood count revealed marked polycythemia with a hematocrit of 79.7%. Thoracic radiographs showed mild right-sided cardiac enlargement. Echocardiography showed a large ventricular septal defect in the left ventricular outflow tract region just proximal to the aortic valve. On Doppler echocardiography, right-to-left shunt flow through the defect was found and confirmed by contrast echocardiography. Based on these diagnostic findings and clinical signs, the dog was diagnosed as right-to-left shunting ventricular septal defect. Phlebotomy and oxygen supplement were performed, but the dog died the day after presentation.
