Remodeling is a commonly encountered term in the field of cardiothoracic surgery that is often used to describe various pathophysiological changes in the dimension, structure, and function of various cardiac chambers, including the aorta. Stanford type A or DeBakey type 1 aortic dissection (TAAD) is a perplexing pathologic condition that can present surgical teams with the need to navigate a maze of complex decision-making. Ascending or hemi-arch replacement leaves behind a significant amount of distal diseased aortic tissue, which might have a persistent false lumen or primary or secondary intimal tears (or communications between lumina), which can lead to dilatation of the aortic arch. Unfavorable aortic remodeling is a major cause of distal aortic deterioration after the index surgery. Cardiac surgeons are aware of post-surgical cardiac chamber remodeling, but the concept of distal aortic remodeling is still idealized. The contemporary literature from established aortic centers supports aggressive management of the residual aortic pathology during the index surgery, and with continuing technical advancements, endovascular stenting options are readily available for patients with TAAD or for complicated type B aortic dissection cases. This review discusses the pathophysiology and treatment options for favorable distal aortic remodeling, as well as its impact on mid- to long-term outcomes following TAAD repair.

This study confirms that transactions of personal medical records through My Data can be made. Moreover, it confirms that there is a need for a mediating role between stakeholders. This study lacks statistical access in the process of performing stakeholder roles. However, personal medical records will be traded safely in the future, and new subjects will enter the market. Furthermore, the domestic bio-industry will develop. Through this study, various problems were derived in establishing Medical MyData in Korea. Moreover, it looks forward to continuing various studies in the health care sector in the future.
This paper evaluates Korea's achievements in implementing the WTO Information Technology Agreement (ITA1), which has been referred to as one of the most successful stories in trade liberalization. ITA1 and other agreements for trade liberalization were evaluated based on quantative statistics, and the implication that is not shown as numbers has often been ignored. Based on in-depth interview methods, results of this paper provides academic importance in filling the gap in knowledge regarding the success of trade liberalization from a qualitative perspective. As well, it has practical importance in exploring implications for continuing the success in implementing the ITA2 and further implementing other agreements for trade liberalization.
The 11th Congress of the Vietnamese Communist Party(VCP) held in January 2011 was the forum to reconfirm the gradual reform. By analysing the documents submitted to the Congress, it can be realized that the VCP's reform policy reveals the compromise between conservative and progressive perspectives by "the Socialist-oriented Market Economy". The VCP proposes to proceed the balanced reform for the sustainable development by multi-sector economy to achieve the basis of the modern state by 2020 on the one hand and also urges to strive against "the peaceful evolution" to defend the socialist regime from abroad as well as within the party itself on the other hand. Globally, Fintech, which acts to replace existing intranational payment methods, is actively used, and such Fintech payments are widely accepted in international B2C trade. However, Fintech for B2B trade has not been released to the market thus far, and previous attempts at electronic international payment systems, such as Bolero, TradeCard, and Identrus, have not succeeded, and most of them have completely disappeared. On the other hand, there are continuing attempts to introduce Fintech technologies such as blockchain to international payments, and these attempts are more widely pursued in foreign countries. If international payments using Fintech are marketed through foreign services and such Fintech services become the de-facto standard for international payments, then Korea in which dependency on international trade is high will be vulnerable due to a dependency on foreign services such as relying on foreign SNS platforms like Facebook and Instagram. Therefore, this paper proposes a business model that can be used as a legitimate B2B trade international payment system on the basis of domestically provided services, focusing on various services afforded by K-SURE, which is the Korea Trade Insurance Corporation.
One of the central and primary doctrine of the law of marine insurance is that the contract of indemnity entered into by assured and insurer is a contract of the utmost good faith. The notion of utmost good faith is a well established doctrine derived from the celebrated case of Carter v. Boehm(1766), decided long before the inception of the Marine Insurance Act(MIA). With the codification of the law, the principle found expression in sections $17{\sim}20$ of the MIA 1906. In section 17 is presented the general duty to observe the utmost good faith, with the following sections introducing particular aspects of the doctrine, namely, the duty of the assured and brokers to disclose material circumstances, and to avoid making misrepresentations. It is somewhat surprising that section 17, being a long founded doctrine, has not attracted the attention of the courts until very recently. Given that the most significant manifestations of uberrimae fidei are non-disclosure and misrepresentations, fulfillment of the obligation of utmost good faith was, not unreasonably, for a long time perceived in terms of the duty to disclose and not to misrepresent. However, Black King Shipping Corporation v. Massie, 'Litsion Pride'(1985) has clarified that the duty of disclosure stems from the duty of utmost good faith, and not vice versa. The duty of utmost good faith is an independent and overriding duty, with the ensuring sections on disclosure and representations providing mere illustrations of that duty. It is now clear that there are important questions with regard to the general doctrine and as to the nature and scope of any duty of good faith continuing after the contract of insurance is made which require separate and fuller discussion. The purpose of this paper is to review the nature and scope of the duty of utmost good faith.
The present review focuses on the effects of energy intake on performance, changes in body tissue during lactation, and metabolic parameters in dairy cows. Especially, pre-partum nutrition and its influence on lactation are emphasized. In recent decades the increase in genetic potential of dairy cows has increased milk yield. This fact sharpens the problem of a negative energy balance in early lactation because the amount of energy required for maintenance and milk production exceeds the amount of energy cows can consume. Around parturition, reduced feed intake reinforces the situation. Continuing negative energy balance causes decreasing milk yield, fertility problems, and incidence of metabolic diseases. Hence, the cow has to rely on body reserves that were stored in late lactation and the dry period. It is evident that the nutritional status pre-partum acts as the key factor for milk yield and fertility parameters in the following lactation. Cows overfed during the foregoing gestation and which have gained large quantities of body fat have lower dry matter intake along with the need to mobilize larger quantities of body reserves in lactation. The milk yield in the following lactation is lower than in cows fed according to their requirements. Cows restrictively fed in late gestation have a higher feed intake in lactation and a lower mobilization of body reserves. The effect of energy intake post-partum plays only a minor role for performance parameters in lactation. Lipid mobilized from body reserves makes a substantial contribution to the energetic cost of milk production in early lactation and adipose tissue undergoes specific metabolic alterations. Adipose tissue is degraded to free fatty acids, which are used in liver for energy purposes. High lipid mobilisation promotes the development of a fatty liver and therefore a reduced gluconeogenesis.
Over the past decade, hospice and palliative care in Japan have progressed rapidly under the national policies supported by the Cancer Control Act. The numbers of palliative care units/inpatient hospices, hospital palliative care teams, and clinics with a home hospice function have been steadily increasing. The increasing numbers of physicians, nurses, and pharmacists have been certified as specialists in palliative care by national associations. Collaborative efforts have been made to standardize and disseminate educational programs and training opportunities in undergraduate, postgraduate, and continuing medical education. Research activities in Japan have markedly contributed to the growing body of evidence, especially in the fields of terminal delirium, terminal dehydration, palliative sedation, care for dying patients, prognostication, communication, psycho-oncology, and regional palliative care programs. This review focuses on major palliative care settings, specialty, national associations, education, and research in palliative care in Japan.
Objectives We are going to analyze patient's medical and Korean medicine use trends after lumbar surgery, and examine the percentage of use of Korean medicine after surgery and its relevance to the medical care outcome after lumbar surgery. Methods Using 3% patients' sample data of the Health Insurance Review and Assessment Service, two groups were compared the treatment progress of the Korean Medicine treatment group and the untreated group after lumbar surgery by hierarchical logistic regression analysis. After hierarchical logistic regression analysis(including propensity scores), two groups were compared after lumbar surgery, the Korean Medicine treatment group within 50days and untreated group within 50days. Results Lumbar surgery was performed in 2750 patients in 2015. It was 3.72 that the risk(odds ratio) of finished treatment of patients treated without Korean Medicine, compared to patients with Korean Medicine. It was 0.12 that the risk of continuing treatment(odds ratio) of patients treated with Korean Medicine within 50 days, compared to patients treatment more than 50 days. Conclusions The ratio of Korean Medicine treatment after lumbar surgery was 14.8%. The group that did not have Korean Medicine showed a higher possibility of treatment termination than the group who did not. Among the groups treated with Korean Medicine, the early treatment group was more likely to end treatment than the late treatment group. Considering various situations in the medical environment, further studies such as prospective studies and long-term data analysis are considered to be necessary.
The purpose of this study is to investigate various opinions on changes and revisions in the Medical Technician Act, to compare the curriculum of radiological technologist and physicians or dentists, and to compare the definitions and scope of work of radiological technologist in Korea and abroad. From the information, The goal is to review whether the phrase 'guidance of a doctor or dentist' specified in the definition of the 'Act on Medical Technicians, etc.' is realistically appropriate. radiological technologist receive specialized college education on radioligical science & medical imaging. The training hours for radiolgical science student are greater than medical students. In addition, radiological technologists are continuously developing their competencies for new knowledge and skills through continuing education in clinical fields. In particular, radiological technologist are making steady research efforts to reduce patient exposure and improve medical image quality. As a result of this investigation, it is considered that the term "guided by a doctor or dentist" as currently defined in the 'Act on Medical Technicians, etc.' may need to be revised in consideration of the professionalism of the radiological technologist.
