보건소 고혈압 영양교육 참여자의 식생활 요인 분석 (Analysis of Dietary Characteristics of Participants Attending the Nutrition Education Program for Hypertensive Patients at a Public Health Center)

  • Hypertension is a well-known degenerative disease whose prevalence rate increases with age. Management of high blood pressure is a critical concern in preventive strategies to reduce the morbidity and mortality for cardiovascular disease. The purpose of this study was to examine the dietary characteristics of hypertensive program participants, and to establish strategies based on their nutritional needs. Hypertensive patients were enrolled in the program in a public health center or in a local elderly center, at Suwon, in 1999-2000. Trained dietitians interviewed 62 enrollees(24-hour recall) and related variables. Mean body mass index of the subjects was 25.0kg/m². 30.7% of the subjects had a family history of hypertension. The majority of them ate regularly and partook of all available side dishes. They consumed grains and vegetables regularly, but seldom ate dairy products or food prepared with oil. Male enrollees frequently consumed more processed food and animal fat than did female enrollees(p<0.05). An analysis of the percentage of RDA(Recommended Dietary Allowances of Korea 1995) showed that but for ascorbic acid, enrollees consumed nutrients below the RDA. The food group intake pattern was not diverse, thus only 8.1% of enrollees consumed all food groups in a day. An analysis of eating attitude showed that 64.5% of enrollees always added salt to beef soup. Male enrollees showed low food-related self-efficacy compared to female enrollees, especially with reference to reduction of instant food intake(p<0.01), increase in vegetable intake(p<0.01), reduction of monosodium glutamate(MSG) intake(p<0.01). and not overeating(p<0.05). Their perceived barriers for participating in hypertension nutrition programs included lack of time, program necessity non-recognition, and program comprehension difficulty. These results suggest that nutrition education program necessity non-recognition, and program comprehension difficulty. These results suggest that nutrition education programs for community hypertensive patients should focus on increasing participant consumption of foods, expecially dairy products, and desirable eating attitudes. It also suggests that the program should consider should consider encouraging self-efficacy in changing eating behavior.

Profile of Non-Communicable Disease Risk Factors Among Employees at a Saudi University

  • Background: There is paucity of studies defining the prevalence of non-communicable disease (NCD) risk factors in Saudi Arabia despite the surging epidemic of obesity, change in dietary habits and sedentary lifestyle. Objectives: This cross-sectional study aimed to assess the prevalence of NCDs risk factors among employees at King Faisal University in Al Hassa, Saudi Arabia and to determine the possible correlates for clustering of NCDs risk factors among them. Materials and Methods: All employees were invited to participate; the World Health Organization STEPwise approach was used for data collection which consisted of a personal interview to collect socio-demographic characteristics, NCD history, tobacco use, vegetables and fruit consumption, and physical activity (PA), followed by anthropometric measurements namely weight, height and waist circumference and blood pressure measurements, subjects were finally subjected to biochemical tests with determination of fasting plasma glucose, serum triglycerides, cholesterol and high density lipoproteins. Results: Of the surveyed employees (n=691), daily current smokers accounted for 22.7%. 94.9%, 95.1% and 86% consumed < 5 servings per day of vegetables, fruits and both fruits and vegetables respectively, 73% were physically inactive, 64% were overweight or obese, 22.1% had hypertension, and 21.5% were diabetics. Elevated cholesterol levels were found in 36.6%, low high density lipoproteins in 36.8%, and elevated triglycerides in 36.1%. Only 3% had no NCD risk factors, and 57.6% had ${\geq}3$ factors. Multivariate logistic regression showed that gender (being male, adjusted odds ratio 'aOR'=1.51), aged ${\geq}50$ years (aOR=3.06), < college education (aOR=1.75), current smokers (aOR=2.37), being obese (aOR=6.96) and having a low PA level (aOR=4.59) were the significant positive predictors for clustering of NCD risk factors. Conclusions: Over fifty percent of the studied university's employees had multiple (${\geq}3$) NCD risk factors. Screening and health promotion initiatives should be launched at least targeting the modifiable factors to avert the excessive risk for cardiovascular disease, diabetes mellitus and several types of cancers.

mtDNA D-loop 염기서열 변이를 이용한 토종오리의 계통 분류 (Phylogenetic Analysis using mtDNA D-loop Sequences in Korean Native Ducks)

  • 최근 국내 오리고기 산업이 점진적으로 증가하고 있으나, 국내에서 생산되고 있는 대부분의 오리 종자는 수입에 의존하고 있는 실정이다. 이러한 의존도를 낮추기 위해서 국립축산과학원에서는 토종오리 상용화 연구를 시작하였으며, 이에 토종오리 품종 및 개체 식별의 필요성이 대두되었다. 본 연구에서는 토종오리와 백색실용오리가 다른 야생오리 품종과 얼마나 연관되어 있는지를 알아보기 위해 미토콘드리아 DNA의 D-loop control 영역의 염기서열을 이용하여 계통분석을 수행하였다. 그 결과, 토종오리와 백색실용오리는 대부분의 야생오리 품종과는 구분이 잘 되지만, 청둥오리와는 분류가 되지 않는 것을 확인할 수 있었다. 또한, 토종오리와 백색실용오리 품종간의 분류 또한 이들 집단이 다수의 변이와 haplotype을 공유하고 있음을 확인할 수 있었다. 분석 결과, 11개의 염기 변이가 확인되었으며, 이 변이들은 8개 haplotype으로 구성되어 있었다. 이 중 토종오리에서만 확인된 haplotype 2를 제외한 3개의 haplotype(haplotype number 1, 3, 4)에서 토종오리와 백색실용오리가 동일한 haplotype을 가진 반면, haplotype 5, 6, 7, 8은 백색실용오리에서만 확인되었다. 이상의 결과를 바탕으로 D-loop control 영역은 토종오리와 야생오리의 품종 구분을 위한 기초 자료로 이용될 수 있을 것으로 판단된다.

선체외판부 3.2T 박판에 대한 SAW 용접 적용에 관한 연구 (A Study on the Application of SAW Process for Thin Plate of 3.2 Thickness in Ship Structure)

  • Recently just as in the automobile industry, shipbuilders also try to reduce material consumption and weight in order to keep operating costs as low as possible and improve the speed of production. Naturally industry is ever searching for welding techniques offering higher power, higher productivity and a better quality. Therefore it is important to have a details research based on the various welding process applied to steel and other materials, and to have the ability both to counsel interested companies and to evaluate the feasibility of implementation of this process. Submerged-arc welding (SAW) process is usually used about 20% of shipbuilding. Similar to gas metal arc welding(GMAW), SAW involves formation of an arc between a continuously-fed bare wire electrode and the work-piece. The process uses a flux to generate protective gases and slag, and to add alloying elements to the weld pool and a shielding gas is not required. Prior to welding, a thin layer of flux powder is placed on the work-piece surface. The arc moves along the joint line and as it does so, excess flux is recycled via a hopper. Remaining fused slag layers can be easily removed after welding. As the arc is completely covered by the flux layer, heat loss is extremely low. This produces a thermal efficiency as high as 60% (compared with 25% for manual metal arc). SAW process offers many advantages compared to conventional CO2 welding process. The main advantages of SAW are higher welding speed, facility of workers, less deformation and better than bead shape & strength of welded joint because there is no visible arc light, welding is spatter-free, fully-mechanized or automatic process, high travel speed, and depth of penetration and chemical composition of the deposited weld metal. However it is difficult to application of thin plate according to high heat input. So this paper has been focused on application of the field according to SAW process for thin plate in ship-structures. For this purpose, It has been decided to optimized welding condition by experiments, relationship between welding parameters and bead shapes, mechanical test such as tensile and bending. Also finite element(FE) based numerical comparison of thermal history and welding residual stress in A-grade 3.2 thickness steel of SAW been made in this study. From the result of this study, It makes substantial saving of time and manufacturing cost and raises the quality of product.

농촌 친환경 주거 개발을 위한 이엉지붕 열환경 특성 분석 -신석기시대 이엉지붕 움집을 대상으로- (Analysis of the Thermal Environment Characteristics of Thatched Roof for Eco-friendly Rural Housing Development -Focused on the Neolithic Thatched Roof Dugout Hut-)

  • Due to the development of civilization, the humans is privileged the rich of technologies for housing thermal environment. But, this kind of technological development caused enough trouble of energy excessive consumption. For solve this problem, many researchers strive to exploit the low energy sustainable techniques. For such a reason, the eco-friendly techniques of vernacular house are resurfacing. These traditional techniques are applied to a development of eco-friendly modern housing. They are no longer recognized as outdated products. On this context, this study proposes an scientific analysis on the thermal environment characteristics of Neolithic thatched-roof dugout hut(Um house). So far the several studies have been carried out in viewpoint of the history and structural compositions of the Um house which has been used as the normal housing for about 1000 years in the Neolithic era, however the thermal characteristics analysis of the Um house has never been studied. Um house is not a housing which has been composed by the scientific analysis or architectural design technology, but evolved empirically over a long period. This study on the thermal environment characteristics of Um house would provide basic information for the development of korean eco-friendly rural housing by korean climate characteristics. In this study, the thermal environmental characteristics of the Um house in the Neolithic era was analysed experimentally. The results of this study could be summarized as follows: 1. When the solar insolation and the ambient temperature in the daytime were $420W/m^2$ and $17^{\circ}C$ respectively, the surface temperature of the Um house roof covered with the rice straw was $37^{\circ}C$ and that in the roof $32^{\circ}C$, and in the conditions above the air temperature in the room was $15^{\circ}C$. 2. When the ambient relative humidity was 40%, that in the room of the Um house 50%, and at the ambient relative humidity of 90~100%, that in the room was 60%. 3. Through the experimental analysis, it was verified that the enthalpy and relative humidity is in an inverse relationship. 4. In general the comfort degree in the living space is changed with the seasonal climate, also in this study, the comfort degree in the room of the Um house in October and November was higher than that in May and June.

캠프캐럴 인근 주민에서 다이옥신류 및 유기염소계 농약의 혈중 농도 및 노출력 (Serum Concentration and Exposure History of Dioxins and Organochlorine Pesticides among Residents around the Camp Carroll Area)

  • Objectives: This study was performed in order to evaluate whether 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) could be detected among residents living near Camp Caroll in Waegwan and whether serum concentrations of dioxins, including 2,3,7,8-TCDD, and organochlorine pesticides (OCPs) are associated with length of residence. Methods: Study subjects totaled 113 (for dioxins) and 190 (for OCPs) adults who were selected from participants in a medical investigation. Serum concentrations of dioxins and OCPs were measured using HRGC/HRMS. Information on length of residence was obtained through questionnaires. Results: 2,3,7,8-TCDD was not detected in serum among all subjects. When length of residence was classified as a categorical variable, after adjusting for confounding variables, only residents living in Waegwan for 40 years or longer tended to have high total TEQ values and 2,3,4,7,8-PeCDF with marginal significances. There was no dose-response relation between length of residence and serum concentrations of these chemicals. In multiple regression models with continuous values of the length of residence, total TEQ value and 1,2,3,4,6,7,8-HpCDF were positively associated with length of residence. However, they explained about 3-5% of total variations of serum concentrations of these compounds, while age, consumption of fatty fish, body mass index, alcohol drinking, and cigarette smoking were main variables affecting serum concentrations of dioxins or OCPs. Conclusions: In the current study, high concentrations of certain compounds were mainly observed among persons who lived in Waegwan for at least for 40 years without a dose-response relation. Therefore, it seems difficult to conclude that length of residence meaningfully contributed to the current serum concentrations of dioxins or OCPs among residents in Waegwan. However, considering the half-life of 2,3,7,8-TCDD and indirect exposure routes, the limitations of the current study design should be considered in the interpretation of the study findings.

혈액에서 Sucrose 양성 Vibrio vulnificus 분리 1예 (A Case of Sucrose-Positive Vibrio vulnificus Isolation from Blood Culture)

  • Vibrio vulnificus is a halophilic bacterium frequently involved in human infection of seafood-associated primary septicemia and primary wound infection, mostly in men with over 40-years of age with underlying liver disease. The primary septicemia, which is the most common form of V. vulnificus infection in Korea, is defined as a systemic illness presenting fever or hypotension with recovery of V. vulnificus from blood or tissue without the apparent primary focus of infection. V. vulnificus typically do not produce acid from sucrose, but a case of primary septisemia was found in a patient at Chonnam K hospital in 1993 from whose blood a sucrose-fermenting strain was isolated. The patient was a 62-year-old man, heavy drinker, with underlying liver disease. He consumed a raw seafood dish two days before onset of the present illness. His symptoms were tenderness and swelling on the right foot. He rapidly developed septicemia, resulting in sudden death. V. vulnificus was isolated from the venous blood culture of the patient. On subculture, the isolate formed yellow colonies on TCBS and produced acid from sucrose. Because of these characteristics, species identification was not achieved by the API 20E and was delayed. Other characteristics of the isolate were identical to those of typical V. vulnificus. The isolate was common serotype O4A and possession of V. vulnificus-specific cytolysin gene was detected by PCR. The isolate was susceptible to all the antimicrobial agents tested including tetracycline, but was intermediate to colistin. In conclusion, it is important that microbiologists be aware of the presence of sucrose-positive V. vulnificus when he or she identifies gram-negative bacilli, which is isolated from the blood of patients with a recent history of raw seafood dish consumption.

잠재성 혈우병 환아에서의 편도 적출후 심한출혈의 1치험례 (A Masked Hemophilia B, Severe Bleeding after Tonsillectomy and Adenoidectomy)

  • 편도적출은 우리 이비인후과 임상에서 평소에 다반사로 행해지는 수술의 하나이라 하겠다. 수술술기의 개량, 마취의 발달과 더불어 각종 항생제의 발견등은 편도 적출술에 따르는 여러 가지 합병증을 감소시켰지만, 이중 수술중 또는 수술후에 왕왕 당하는 출혈문제는 가장 흔한 합병증의 하나이며 술자에게는 골치거리의 하나이기도 하여 아직껏 국내외에서 그 보고가 끊어지지 않고 있다. 특히 본증례에서와 같이 본태성 질환으로 인한 출혈의 해결에 있어서는 더욱 복잡곤란한 문제가 많을 것으로 생각된다. 혈우병은 1950년대에 이르러 각 Factor가 발견되어 현재는 혈우병 A.B.C.로 나뉘고 있다. 본증례는 11세의 남아로서 편도 척출술전에는 혈우병의 유무가 인지 못되었던 예로서 수술후 수술창에서 계속되는 출혈로 혈액응고장애를 가진 질환이라고 생각되어 여러 가지 검사결과 P.T.C의 결여로 오는 혈우병 B란 진단을 얻었기에 교훈적이고 귀중한 체험을 얻었다고 생각되어 보고하는 바이다. 환자는 수차의 수혈과 비인강탐폰 등의 방법으로 겨우 지혈되어 수술 20일만에 수술창이 치유되어 퇴원하였다.

대도시지역 일부 초등학생의 치과용 아말감 충전치아와 요중 수은농도의 관련성 (The Relationship between Dental Amalgam Fillings and Urinary Mercury Concentration among Elementary School Children in a Metropolitan Area)

  • 치과용 아말감은 수은, 구리, 아연, 은과 같은 여러 가지 금속으로 이루어진 합금으로 구성되었다. 아말감은 내구성과 진료 시 사용의 편리성과 경제적인 이유로 치과영역에서 가장 흔히 사용되고 효과적인 수복물질로 이용되었다. 하지만 수복물질로서의 대중성과 긴 역사에도 불구하고 아말감에 포함된 수은의 노출로부터 가져올 수 있는 잠재적 부작용 또는 합병증에 대한 염려로 논란이 계속되고 있다. 특히 어린이들의 경우 수은의 독성에 대한 위험성이 더 높기 때문에 우리는 어린이에게서 치과용 아말감과 요중 수은농도의 관련성에 대하여 알아보고자 하였다. 대도시 지역 내 두 개 초등학교 1~4학년 재학생을 대상으로 하여 구강검진과 설문조사를 시행하였으며, 구강검진 당일 채취한 소변시료를 이용하여 요중 수은농도를 분석하였다. 결과는 구강 내 아말감 충전치아가 없는 아동과 비교 했을 때 79개 이상, 10개 이상 갖는 아동의 요중 수은농도가 높아지는 상관관계가 나타났다. 결론적으로 어린이의 구강 내 아말감 충전치면수와 요중 수은농도는 관련성이 있는 것으로 사료된다.

모유 수유아와 생우유를 먹인 아기의 철분결핍에 관한 연구 (A study on cow's milk and nursing method in relation to iron deficiency)

  • 목 적 : 모유수유 기간, 우유병 사용 기간, 생우유를 처음 먹인 시기 및 양을 조사하여 빈혈과의 관련을 알아보고자 하였다. 방 법 : 2002년 8월 1일부터 2005년 7월 31일까지 전주예수병원에 입원한 12개월에서 36개월 환아 930명을 대상으로 빈혈검사 및 설문지를 통해 전향적으로 연구하였다. 결 과 : 빈혈이 발생하는 연령은 30-36개월에서 많이 발생하였으나, 철분결핍성 빈혈은 18-23개월에서 많이 발생하였다. 모유수유 기간이 12개월 이상과 6개월 미만 순으로 빈혈이 많고, 혈청 ferritin이 낮고 철분결핍성 빈혈이 많았다. 우유병 사용 기간과 빈혈, 혈청 ferritin, 철분결핍성 빈혈은 차이가 없었다. 생우유를 먹고 난 후에 부작용은 변비, 설사, 피부발진 순으로 나타났다. 생우유를 처음 먹인 시기와 빈혈, 혈청 ferritin, 철분결핍성 빈혈은 차이가 없었다. 하루에 생우유를 먹인 양과 빈혈, 혈청 ferritin, 철분결핍성 빈혈은 차이가 없었다. 결 론 : 생우유와 우유병 사용 기간과 철분결핍은 관계가 없었으며, 모유수유 기간이 12개월 이상과 6개월 이하 순으로 철분결핍이 많았기 때문에, 철분결핍성 빈혈을 예방하기 위해서는, 적절한 모유수유가 중요한 것으로 판단되었다.
