Current concepts of craniofacial fibrous dysplasia: pathophysiology and treatment

  • Fibrous dysplasia is an uncommon genetic disorder in which bone is replaced by immature bone and fibrous tissue, manifesting as slow-growing lesions. Sporadic post-zygotic activating mutations in GNAS gene result in dysregulated GαS-protein signaling and elevation of cyclic adenosine monophosphate in affected tissues. This condition has a broad clinical spectrum, ranging from insignificant solitary lesions to severe disease. The craniofacial area is the most common site of fibrous dysplasia, and nine out of 10 patients with fibrous dysplasia affecting the craniofacial bones present before the age of 5. Surgery is the mainstay of treatment, but the technique varies according to the location and severity of the lesion and associated symptoms. The timing and indications of surgery should be carefully chosen with multidisciplinary consultations and a patient-specific approach.

Analysis of Strategies for Quality Assurance in Online Education: The Implications of the Role of an Instructional Design Team to Support Faculty

  • This study investigates faculty support for quality assurance in online education, and offers suggestions for its improvement based on feedback from Instructional Design (ID) staff working at a public university in the U.S. Qualitative research using semi-structured interviews was conducted with seven ID staff in order to examine their perceptions regarding faculty support related to quality assurance in online education. The results of the data analysis indicate that four types of faculty support-quality assurance reviews using Quality Matter (QM) standards, templates, individual consultations with ongoing support, and monitoring-were offered for faculty. Faculty support for quality assurance in online education could be improved by developing specific quality assurance standards, recruiting external experts, examining learning effects, developing a quality assurance management system, and sharing documents among ID staff. This study highlights the necessity of quality assurance in online education and provides cases of faculty support in a real higher education setting.

  • The present study examines the relative roles of external recommendations and internally felt satisfaction in influencing patient loyalty to a doctor. It establishes that recommendations do result in preliminary loyalty formation in the patient to the doctor, but only until the formation of own experiences; the significance of external sources of recommendation in the determination of patient loyalty becomes insignificant thenceforth. Implications for the doctors are that they should strive at fostering bonds of emotional attachment in their present patients so that they become strongly loyal and spread positive word-of-mouth which could result in the doctor getting new patients as well. Probable extensions of this research are also discussed later in the paper.

Consultations to Department of Dentistry for Child and Adolescent Inpatients with Dental Trauma (치과적 외상이 있는 소아청소년 입원환자의 치과 협진 의뢰)

  • The purpose of this research is analyzing present condition, and pursuing developmental direction of consultation to department of dentistry for medical inpatient with dental trauma. This research conducted a retrospective analysis of consultation to department of dentistry for medical inpatient at Wonju Severance Christian Hospital from March 2011 to February 2017. This research analyzed chief complaint and dental diagnosis, referring department, time of consultation after dental trauma, relation between hospitalization period and time of consultation after dental trauma, relation between dental trauma and admission in the medical department due to trauma, cause and age of dental trauma occurrence, treatment of dental trauma, and revisiting rate of inpatient with dental trauma after discharge. Among all the chief complaint and dental diagnosis, dental trauma was the highest. Among all the referring departments, departments of surgery were the highest. In relation to being delayed of consultation term after dental trauma, it turned out that there's pretty high mutual relation between the hospitalization period and the consultation term after dental trauma, and the hospitalization period had the quantitative influence on the consultation term. Statistically, dental trauma and admission in the medical department due to trauma had significant relations. In case of those patients due to car accidents, they had dental trauma without exception. Statistically, patient's group with dental trauma in hospital room was younger than patient's group with dental trauma in non-hospital room. Statistically, dental trauma and treatment in hospitalization had no significant relations, and dental trauma and revisiting department of dentistry after discharge had no significant relations. For the child and adolescent inpatients hospitalized for car accidents, there is high possibility of accompanying dental trauma, and dental consultations can be delayed. For the young child inpatients, the possibility of dental trauma occurrence in hospital room is high. Moreover, treatments during hospitalization and revisiting department of pediatric dentistry after discharge are not performed well for inpatients with dental trauma. Regarding these results, it can be an alternative that improving of consultation system, dedicated workforce placement and having dental facilities and equipment in the hospital ward, education to medical doctor and dentist, communication between medical doctor and dentist. This research can be used as a study material of medical and dental departments, and it's expected to be committed to developing of consultations to department of dentistry for child and adolescent inpatients with dental trauma.

A Study on the Classification of Income on University's Industrial Consultations (대학 산업자문료 소득 구분에 관한 연구)

  • Industrial consultation is a kind of personal service for companies. The Industry-Academic Cooperation Foundation sets up a consultation contract in which a professor performs the consultation as the person in charge. Recently, there is an issue regarding taxation of the consulting fee paid to the professor; in short, there is no standardized practice for the imposition of income tax. In this study, we examine the characteristics of industrial consultation and present an acceptable taxation rule based on related laws. First, it is not appropriate to regard consulting fees as wage income, considering that there is no employment relationship between the Industry-Academic Cooperation Foundation and the professor. Considering that the base consulting fee amount is the same as the invention compensation, according to accounting practices, and that an employee invention is apt to be derived in the consultation, it is reasonable that the consulting fee should be regarded as wage income similar to employee invention compensation. As treating the consulting fee as wage income could end up reducing industrial consultations, the government should amend the income tax law to include industrial consultation as a type of other income.

Analysis of Coastal Area Utilization by Consultation of Marine Environmental Impact Assessment Related-Systems (해양환경영향평가 관련 협의에 의한 연안이용분석)

  • This study presented politic proposals and diverse utilization-type in coastal areas by analyzing results of reviewed related-statements of marine environment during the one year period of 2007 by Marine Environmental Impact Assessment Center. Total of 358 cases were reviewed, which was a significant increase from 270 in 2006. Consultation on the utilization of sea areas (CUSA) accounted for the largest number of 165 (46.1%) and it was followed by 104 cases of environmental impact assessment (EIA) (29.0%) and 89 cases of prior environmental review (PER) (24.9%). As such, evaluation statements (EIA+PER) related to consultations of the Ministry of Environment accounted for approximately 54% of the entire cases reviewed. To analyze the overall results of reviewing marine-related evaluation statements, utilization and planning were conducted by 47.9%, 38.4% and 13.7% in the South Sea, West Sea and East Sea of Korea, respectively. In evaluation statements (EIA+PER), port construction, industrial complex construction, urban management plan and road construction took up most of the part by 40.9%, 20.2%, 10.4% and 7.3%, respectively. In terms of CUSA-statement, it was evaluated that consultations were mostly carried out on use and reclamation of public water surface in coastal areas and on sea aggregate extraction process in EEZ. The largest number of plans for coastal use were established for Jeollanam-do, followed by Gyeongsangnam-do, Chungcheongnam-do, Gyeongsangbuk-do and Gyeonggi-do. In particular, development plans were concentrated on Jeollanam-do and Gyeongsangnam-do with stable marine environment and outstanding view of the nature. In most cases, these regions are adjacent to the areas designated as a sea area for environmental management and fisheries resources protection zone. Therefore, conflicts exist between development and preservation. Also, rather than random development, more detailed marine environmental impact assessment, gathering of public opinions and politic harmony are essentially required. For efficient coastal management and environmentally sound and sustainable development, fulfilling consistent and transparent coastal policies as well as active and reliable decision making to center on coastal environment by management bodies will be important.

A Study of Teacher's Satisfaction Regarding Oriental Medical Doctor's Student Health and Wellness Program (한의학 교의사업 시범시행 후 교사들의 교의만족도에 관한 연구 - 성남시 중고등학교를 대상으로)

  • Objectives This study aims to build the baseline data for school health care program by analyzing the questionnaires about satisfaction regarding Oriental Medical Doctor's (Korean Medicine) student health and wellness program. Methods The association of Korean medicine doctor in Sung-nam city conducted Oriental Medical Doctor's student health and wellness program for 12 middle schools and 8 high schools in Sung-nam from August to December, 2015. The participating Oriental Medical Doctor visited each school for 8 times during 5 months period, and conducted health consultations, health education classes and Korean medicine treatment for the school students and school employees. Teachers and administrators from the participating schools answered the self-reported satisfaction questionnaires and the satisfaction questionnaires results were analyzed. Results 85 people responded the program satisfaction questionnaires; 69% responders were very satisfied and 28% were satisfied about the program specifically about the student's health check-ups, informatory brochures for the parents, good participation enrollment process. Responders were pleased about doctor's consultations regarding 'Advising students about their general health', 'Ways to cure sick students fast', 'Providing students with useful information about health', and 'Teaching students how to manage their health in a good shape'. 92.94% of participants agreed to the needs of the school health and wellness program. Conclusions One of the fundamental national health policies is to provide students opportunities to maintain their good health. We have developed a successful pilot program called, "Oriental Medical Doctor's student health and wellness program". We got positive feedback from the participating school teachers regarding our program. Further studies based on this study are needed to show the benefit of the program to broaden its use.

  • The purpose of this study was to analyze the parental consultations, patients' age and consulters. Also it was to improve the tendency of consultation homepage The subjects were 2485 questions on Korean Academy of Pediatric Dentistry Homepage except deleted questions which were not related with consultations. The examined contents were categorized into patients' age, counseling contents, consulters. The results were as follows: 1. Patients mostly belonged to preschool children(64.29%). 2. In the subjects, questions were mostly about growth and development, restorative and pulp treatment. 3. In the subdivided subjects, great part of questions were about tooth eruption, restorative and pulp treatment and trauma. 4. The 93.72% of consulters were mothers. 5. We should reinforce with FAQ about growth and development, restorative and pulp treatment and trauma. Also we should improve on quality of information and childcare related contents and Homepage related pediatric dentistry.

Need to Pay More Attention to Attendance at Follow-Up Consultation after Cancer Screening in Smokers and Drinkers

  • Background: Follow-up clinical consultations could improve overall health status as well provide knowledge and education for cancer prevention. Materials and Methods: This is the cross-sectional study using the Korean Community Health Survey (KCHS) 6th edition for 2012, with 115,083 respondents who underwent cancer checkups selected as subjects. Associations between the presence of consultation and the socioeconomic status were determined using statistical methods with the SAS 9.3 statistical package (Cary, NC, USA). Findings: Among the recipients, 32,179 (28.0%) received clinical consultations after cancer screenings. Those in rural areas (odds ratio, OR=0.71, 95% confidence interval (CI), 0.69-0.73) visited follow-up clinics less frequently than did those in urban areas. Starting at the elementary school level, as the education level increased to middle school (OR=1.26, 95% CI: 1.19-1.34), high school (OR=1.29, 95% CI: 1.23-1.36) or college (OR=1.76, 95% CI: 1.65-1.89), the participation rates also increased. When compared with the lowest quartile group, the quartile income level showed a statistical trend and difference as follows: second lowest quartile (OR=1.11, 95% CI: 1.07-1.16), third lowest (OR=1.12, 95% CI: 1.07-1.17) and highest quartile income (OR=1.29, 95% CI: 1.23-1.35). In addition, the people with economic activities (OR=0.87, 95% CI: 0.84-0.90) visited follow-up clinics less frequently than did the others. Current smokers (OR=0.93, 95% CI: 0.89-0.98) and inveterate drinkers (OR=0.88, 95% CI: 0.85-0.94) had a tendency to visit less often than did non-smokers and other drinkers with all cancers combined. Interpretation: We suggest primary prevention through lifestyle modifications including smoking and drinking, and environmental interventions may offer the most cost-effective approach to reduce the cancer burden.

Psychiatric Consultation for 5 Years in a University Hospital (일 대학병원에서 5년간 시행된 정신과 자문의뢰 분석)

  • Objective: This study aimed to investigate the pattern of psychiatric consultation-liaison in a new general hospital over a 5-year period. Method: Retrospective chart review was conducted for psychiatric consultations performed from May 2004 to December 2008 in a new general hospital in Hwasun, Korea. Results: A total of 1,852 patients who were referred to the Department of Psychiatry for consultation were included in the analysis. The main reasons for psychiatric consultation were changes in mental status(20.5%), depression(16.8%), insomnia(12.8%), and anxiety(7.9%). Psychiatric consultations were conducted with diagnoses of delirium(39.7%), depressive disorder(28.2%), adjustment disorder(7.9%), and anxiety disorder(4.1%). Patients with delirium were significantly older than were those with other psychiatric diagnoses(p<0.001). Delirium was more common in male patients than in female patients(47.1% vs. 29.9%, respectively), and depression was more common in female patients than in male patients(48.3% vs. 33.9%, respectively)(p<0.001). Delirium was more common in patients who underwent surgical operation than those who did not(p=0.010). Conclusion: Delirium was the most common diagnosis for psychiatric consultation followed by depression in a university hospital. Delirium was more prevalent in men than in women, while depression was more common in women.
