  • Objectives: To evaluate the effectiveness of an integrated medical service model for a fibromyalgia patient and their caregiver, focusing on reducing pain and improving quality of life. Methods: A single-case study design was employed, involving a fibromyalgia patient and their primary caregiver treated at W University Hospital. The integrated medical service program, based on the Ministry of Health and Welfare's model, included medical consultations and complementary therapies such as psychological counseling, art therapy, music therapy, horticultural therapy, yoga, and meditation. The program was conducted weekly for 8 weeks, with each session lasting up to 100 minutes. Data collection involved both quantitative and qualitative assessments. Quantitative data included demographic surveys, psychological tests, health-related quality of life measures, pain indices, and sleep quality indices. Qualitative data were gathered through feedback evaluations and emotional assessments. Results: The patient showed improvements in mobility, self-care, daily activities, and anxiety/depression, with EQ-VAS scores increasing from 20 to 40 and pain perception decreasing from 67.41 to 42.58. The caregiver reported reduced anxiety/depression and an increase in EQ-VAS scores from 95 to 98. Both patient and caregiver exhibited emotional changes, with decreased depression and increased happiness. However, the patient showed an increase in fear and anger. Conclusions: The integrated medical service model positively impacted the emotional and psychological well-being of the fibromyalgia patient and their caregiver. Despite the limitations of a small sample size and a single-case study design, the findings suggest that an integrated approach can be beneficial. Larger-scale studies are needed to confirm and generalize these results.

  • Objectives: Underpinned by the context of a Korean traditional medicine cohort study on healthy aging, this research primarily aims to guide the selection of Clinical Outcome Assessments (COAs) for elderly healthy aging patient registry research, offering insights into the selection process; and secondly, to streamline the resource-intensive process of obtaining permissions for validated COAs, benefiting future traditional Korean medicine clinical researchers. Methods : In this study, we identified outcomes through a review of previous studies, followed by a process involving expert consultations to select the final outcomes. Subsequently, for the selected outcomes that were Clinical Outcome Assessments (COAs) developed tools, we searched in commercial databases to confirm the availability of Korean versions and the necessity of obtaining permissions. Finally, we obtained permissions for their utilization and, when needed, acquired the original instrument questionnaire through payment. Results: Through a literature review of existing observational studies, a total of 57 outcomes were selected, with 19 of them identified as COA instruments. Upon verifying usage permissions for these 19 instruments, it was found that 17 required author-specific permissions, and among these, 2 needed a purchase as they were commercially available. Conclusion: This study provides a detailed overview of outcome selection and permission acquisition for elderly patient registry research. It underscores the importance of Clinical Outcome Assessment (COA) tools and the rigorous approval process, aiming to enhance research reliability. Continuous verification of COA information is essential, and future research should explore Core Outcome Set (COS) development through consensus-building approaches like Delphi studies.

  • Objectives: The primary aim of this study is to create an Occupational Safety and Health (OSH) guide for high-risk maintenance tasks, specifically one designed for maintenance work (MW) in the electronics industry. Methods: The methodology involved a literature review, field investigations, and discussions. An initial draft of the OSH guide was created and then refined through consultations with experts possessing extensive experience in MW for electronic processes. Results: Specific MW tasks within electronics processing facilities identified as high-risk by the research were selected. A comprehensive OSH guide for these tasks was developed consisting of approximately 11 to 12 components and encompassing about 20-25 pages. Implementing safety and health measures before, during, and after MW is crucial for the protection of maintenance personnel. The guide is enriched with real-case scenarios of industrial accidents and occupational diseases to enhance maintenance workers' comprehension of the OSH principles. For a clearer understanding of and adherence to the safety protocols, the guide incorporates visual aids, including cartoons and photographs. Conclusions: This OSH guide is designed to ensure the protection of workers involved in maintenance activities in the electronics industry. It aligns with global standards set by the International Organization for Standardization (ISO) and Semiconductor Equipment and Material International (SEMI) to ensure a high level of safety and compliance.

  • The purpose of this study was to develop and validate a job-esteem scale that reflects the unique characteristics of academic librarians. The study conducted an analysis of relevant literature and existing scales in the scale development phase, followed by in-depth interviews with 10 academic librarians, resulting in the creation of 100 preliminary measurement items. These items underwent content validity testing, consultations with linguists, and face validity assessments, which culminated in the selection of 52 items for a preliminary measurement scale. A pilot survey was then administered with 300 academic librarians from four-year universities nationwide to collect data, and item analysis and reliability testing were performed with the data. Then, an exploratory factor analysis was conducted, resulting in the elimination of 22 items and the finalization of 30 items. The main survey was conducted with 345 academic librarians, and the study confirmed high reliability of the data through item analysis and internal consistency tests. Through another confirmatory factor analysis, the study finalized 29 items. The final scale met all criteria for convergent, discriminant, and criterion validity. This study marks the first development of a job-esteem scale tailored to the distinct attributes of academic librarians, and its significance lies in its utility for assessing and analyzing the job esteem of academic librarians.

  • Most of oceanic current maps in the secondary school science and earth science textbooks have been made on the base of extensive in-situ measurements conducted by Japanese oceanographers during 1930s. According to up-to-date scientific knowledge on the currents in the Yellow Sea and the East China Sea (YES), such maps have significant errors and are likely to cause misconceptions to students, thus new schematic map of ocean currents is needed. The currents in the YES change seasonally due to relatively shallow water depths, complex terrain, winds, and tides. These factors make it difficult to construct a unified ocean current map of the YES. Sixteen major items, such as the flow of the Kuroshio Current into the East China Sea and its northward path, the origin of the Tsushima Warm Current and its path into the Korea Strait, the path of Taiwan Warm Current, the Jeju Warm Current, the runoff pattern of the Yangtze River flow, the routes of the northward Yellow Sea Warm Current, the Chinese Coastal Current, and the West Korea Coastal Current off the west coast of the Korean Peninsula, were selected to produce the schematic current map. Review of previous scientific researches, in-depth discussions through academic conferences, expert discussions, and consultations for three years since 2014 enabled us to produce the final ocean current maps for the YES after many revisions. Considering the complexity of the ocean currents, we made seven ocean current maps: two representative current patterns in summer and winter, seasonal current maps for upper layer and lower layer in summer and winter, and one representative surface current map. It is expected that the representative maps of the YES, connected to the current maps of the East Sea and the Northwest Pacific Ocean, would be widely utilized for diverse purposes in the secondary-school textbooks as well as high-level educational purposes and even for scientific scholarly experts.

  • Understanding patients' expectation for health visits and providing appropriate care may increase patients' satisfaction with health care, leading to more positive treatment outcome. The study aimed to investigate expectations of the patients with temporomandibular disorders (TMD) at their first visit to university-based orofacial pain clinic and to evaluate any relation with duration of pain, presence of previous treatment, pain severity and pain interference. Inclusion criterion was patients aged over and 18 years old and diagnosed as TMD during 3 months' period from Aug to Nov 2010. They were asked to complete the questionnaires for patients' expectation and the Brief Pain Inventory (BPI) at waiting room prior to consultation. 322 TMD patients participated in the study(M:F=1:1.5, mean age=36 years old). The study indicated that the most important top 3 expectations were 'cure of pain', 'understanding their problem' and 'doctor-patient communication' in order. This finding was not affected by gender, duration and previous treatment history but affected by sub-category of TMD and BPI pain severity and pain interference. 'Pain relief' and 'understanding their problems' were relatively highlighted in the patients with muscle disorders and combination (joint-muscle) disorders of TMD than those with joint disorders who wanted communication and further investigation relatively more (p=0.000). While expectation for pain relief was expected more with increase of pain severity and interference, patients with mild level of pain severity and interference expected communication and further investigation relatively more (p=0.000, 0.017, respectively). Based on the results of the study, though pain relief was the primary concern for TMD patients suffering from pain, their satisfaction with care may be increased by explanation for etiology and mechanism of TMD to make them understand their problems better and doctor-patient communication and collaborative decision-making for treatment. Importance of patient-centered consultations and availability of written material or web sites for patient information should be stressed out.

  • Purpose : Through a survey undertaken among doctors practicing pediatric patients in primary medical facilities on nutrition during infancy, the present study was done to determine whether these doctors were giving appropriate advice on infant nutrition. Methods : We visited pediatric specialists(group one) and non-pediatric specialists or general practitioners(group two) who were practicing in Daegu to ask them to fill out a questionaire on infant nutrition. Results : When 10 points were given to each question for a total of 100 points, the results of the evaluation showed that the score in group one was $73.0{\pm}15.8$ points and $45.1{\pm}18.4$ points in group two, showing a significant difference. No statistical difference was seen in the scores between non-pediatric specialists and general practitioners. Each answer to each question on the feeding period of breast milk, on loose stools during breast milk feeding, methods to deal with jaundice during breast milk feeding, choice of commercial baby food, criteria on directing soy milk, and criteria of guiding infant cereal was different between group one and two. Conclusion : The results of the present investigation showed that specialists in other fields or general practitioners were giving inappropriate advice on nutrition during the infant stage compared with pediatric specialists; thus, we need to confirm methods to prevent inappropriate consultations by specialists of other fields, together with continued education of pediatric specialists.

  • Objectives : When a patient who attempts suicide visits the emergency room, it is important that the departments of emergency medicine, internal medicine, and psychiatry communicate with each other and prioritize treatment. This study was conducted to verify the effectiveness of the multidisciplinary emergency consultation system (ECS) for drug intoxicated patients. Methods : We retrospectively analyzed the data from medical records prior to the ECS, from July 2017 to May 2018, and after the ECS, from July 2018 to May 2019, to verify the effectiveness of the system. Results : After the ECS, admission to open wards was significantly higher than to the intensive care units (χ2=8.567, p=0.014). In addition, the proportion of consultations to the department of psychiatry among patients admitted to other departments tended to increase (χ2=4.202, p=0.053), and the time required for consultation response decreased (Z=-2.031, p=0.042). As a result of the consultation, the proportion of the patients who had been transferred to the department of psychiatry was increased (χ2=4.692, p=0.043), and the time spent to transfer tended to decrease (Z=-1.941, p=0.052). Conclusions : After implementing the ECS for drug intoxicated patients, unnecessary intensive care unit admissions, consultation response time, and the time spent to transfer were reduced, and the rate of consultation referrals and transfer rates increased. This means that the multidisciplinary consultation system rapidly provided essential medical services to patients at lower medical costs.

  • Purpose : Since the infant formula milk has been produced in Korea, it has faced a low rate of breast milk feeding, though breast milk feeding is a little increasing. Therefore, the Korean Pediatric Society launched its website for breast-feeding consultation to provide information to the general public and enhance the health of growing infants. The consultation results were analyzed to identify the problems that mothers encounter during breast-feeding. Methods : From August 1, 2004 to July 31, 2007, 1001 mothers who visited the online consultation webpage (www. pediatrics.or.kr) of the Korean Pediatric Society asked 1,021 questions. The questions were divided into 3 major categories and 14 specific categories. Interesting questions asked more than 100 times were retrospectively analyzed. Results : The results for the major categories were as follows: 413 questions (40.3%) were on how to breast-feed, 315 (30.8%) on problems of feeding mothers, and 293 (28.8%) on problems of the fed babies. In the specific categories, 22.2% of the questions were on how to breast-feed. With the increasing number of working couples and working mothers, many questions were asked on the problems of breast-feeding after returning from work. Conclusion : The author expects that analyses of these consultations will contribute to the enhancement of information on the consultation website, thus enabling to provide clearer answers to people's increased interest in and concerns on breast-feeding. Furthermore, this research will help to establish correct breast-feeding practice.
