The delay which is one of the quality of service parameters is considered to be a crucial factor for the effective usage of real-time audio and video streams in interactive multimedia collaborations. Among the various causes of the delay, we focus in this paper on the local delay concerned with the schemes which handle continuous inflow of encoded data from constant or variable bit-rate audio and video encoders. We introduce two kinds of implementation approaches, pull model and push model. While the pull model periodically pumps out the incoming data from the system buffer, the push model receives events from the device drivers. From our experiments based on Windows NT 3.51, it is shown that the push model outperforms the other for both constant and variable bit-rate streams in terms of the local delay, when the system suffers reasonable loads. We interpret this experimental data with M/G/1 multiple vacation queuing theories, and show that it is consistent with the queuing theoretic interpretations.

This paper describes a theoretical framework for rate allocation to maximize the power efficiency of overall heterogeneous wireless networks whose users are assumed to have multihoming capabilities. Therefore, the paper first presents a power consumption model considering the circuit power and radio transmission power of each wireless network. Using this model, two novel power efficient rate allocation schemes (PERAS) for multihoming services are proposed. In this paper, the convex optimization problem for maximizing the power efficiency over wireless networks is formulated and solved while guaranteeing the required quality of service (QoS). Here, both constant bit rate and variable bit rate services are considered. Furthermore, we extend our theoretical framework by considering the sleep-operation management of wireless networks. The performance results obtained from numerical analysis reveal that the two proposed schemes offer superior performance over the existing rate allocation schemes for multihoming services and guarantee the required QoS.
Massive broadcasting contents such as UHD(Ultra High Definition) TV which requires multi-channel capacity for transmission has been introduced in recent years. A transmission scheme with channel bonding has been considered for transmission of massive broadcasting contents. In HFC(Hybrid Fiber Coaxial) networks, DOCSIS 3.0(Data Over Cable Service Interface Specification 3.0) has already applied channel bonding schemes for up/downstream of data service. A method unlike DOCSIS 3.0 is required to introduce a channel bonding scheme in the broadcasting service having unidirectional transmission with a downstream. Since a massive broadcasting content requires several channels for transmission, VBR(Variable Bit Rate) transmission has been emerging for the bandwidth efficiency. In addition, research on channel allocation and resource scheduling is required to guarantee QoS(Quality of Service) for the broadcasting service based on VBR. In this paper, we propose a transmission method for mass broadcasting service in HFC network and show the UHD transmission simulator developed to evaluate the performance. In order to evaluate the performance, we define various scenarios. Using the simulator, we assess the possibility of channel bonding and VBR transmission for UHD broadcasting system to provide mass broadcasting service efficiently. The developed simulator is expected to contribute to the efficient transmission system development of mass broadcasting service.

In ATM networks CBR data traffic is sent in constant bit rate, and VBR data traffic in variable bit rate. Therefore unused bandwidth at network capacity may exist. To avoid waste of network resourcesm, ABR traffic utilizes the unused bandwidth to the utmost after CBR and VBR data traffic being first served. In this paper, a transfer rate-based congestion control algorithm is proposed for efficient ABR service in ATM networks. In the proposed algorithm the ATM switch first calculates bandwidth according to variable cell transfer rate in an ABR source. records this value in ER field in a RM cell, and then transmits a RM Cell to an ABR source. In this way the proposed algorithm dynamically allocates bandwidth to each ABR source, and the switch also can rapidly adapt to a change of a transfer rate of an source. The performance simulation of the proposed algorithm has batter performance in terms of source condition and link utilization.

Background: Previous research has shown that paramedics are exposed to risks in the form of injuries to the musculoskeletal system. In addition, there are studies showing that they are also at increased risk of cardiovascular disease, cancer, and psychiatric diseases, which can partly be explained by their constant exposure to stress. The aim of this study is to evaluate whether the use of shoulder straps decreases physical effort in the form of decreased heart rate and cortisol concentration. Methods: A stretcher with a dummy was carried by 20 participants for 400 m on two occasions, one with and one without the shoulder straps. Heart rate was monitored continuously and cortisol samples were taken at intervals of 0 minutes, 15 minutes, 30 minutes, 45 minutes, and 60 minutes. Each participant was her or his own control. Results: A significant decrease in heart rate and cortisol concentration was seen when shoulder straps were used. The median values for men (with shoulder straps) at 0 minutes was 78 bpm/21.1 nmol/L (heart rate/cortisol concentration), at 15 minutes was 85 bpm/16.9 nmol/L, and at 60 minutes was 76 bpm/15.7 nmol/L; for men without shoulder straps, these values were 78 bpm/21.9 nmol/L, 93 bpm/21.9 nmol/L, and 73 bpm/20.5 nmol/L. For women, the values were 85 bpm/23.3 nmol/L, 92 bpm/20.8 nmol/L, and 70 bpm/18.4 nmol/L and 84 bpm/32.4 nmol/L, 100 bpm/32.5 nmol/L, and 75 bpm/25.2 nmol/L, respectively. Conclusion: The use of shoulder straps decreases measurable physical stress and should therefore be implemented when heavy equipment or a stretcher needs to be carried. An easy way to ensure that staff use these or similar lifting aids is to provide them with personalized, well-adapted shoulder straps. Another better option would be to routinely sewn these straps into the staff's personal alarm jackets so they are always in place and ready to be used.
A mobile terminal will expect a number of handoffs within its call duration. In the event of a mobile call, when a mobile node moves from one cell to another, it should connect to another access point within its range. In case there is a lack of support of its own network, it must changeover to another base station. In the event of moving on to another network, quality of service parameters need to be considered. In our study we have used the Markov decision process approach for a seamless handoff as it gives the optimum results for selecting a network when compared to other multiple attribute decision making processes. We have used the network cost function for selecting the network for handoff and the connection reward function, which is based on the values of the quality of service parameters. We have also examined the constant bit rate and transmission control protocol packet delivery ratio. We used the policy iteration algorithm for determining the optimal policy. Our enhanced handoff algorithm outperforms other previous multiple attribute decision making methods.
Background: This study aims to compare the clinical outcomes of steroid injections during the rehabilitation period after arthroscopic rotator cuff repair (ACRC). Methods: Among patients who underwent ARCR, 117 patients who met the inclusion and exclusion criteria were enrolled. Pain and range of motion (ROM) recovery at the 3-, 6-, and 24-month follow-up visits and functional outcome at the 24-month follow-up were compared between 45 patients who received ultrasound-guided subacromial steroid injection at postoperative week 4 or 6 and 72 patients who did not. Functional outcome was assessed using the American Shoulder and Elbow Surgeons (ASES) score and Constant score. Healing of the repaired tendon and retear were observed at the 6-month follow-up via magnetic resonance imaging (MRI) or computed tomography (CT) arthrography. Results: At the 3-month follow-up, the steroid injection group showed lower visual analog scale scores than the control group (p<0.05) and showed faster recovery of forward flexion and internal rotation (p<0.05). From the 6-month follow-up, the two groups did not show differences in pain and ROM, and the ASES score and Constant score also did not significantly differ at the 24-month follow-up. The two groups did not differ in retear rate as determined by MRI or CT arthrography at the 6-month follow-up. Conclusions: This study demonstrated that ultrasound-guided subacromial steroid injection at 4 or 6 weeks after ARCR leads to quick pain reduction and ROM recovery until 3 months after surgery. Therefore, subacromial steroid injection is speculated to be an effective and relatively safe method to assist rehabilitation.
Many recent studies have been conducted involving the transport of constant and variable bit rate MPEG-2 video in asynchronous transfer mode (ATM) networks. In this study, the traffic control of supporting MPEG-2 video communications in ATM networks under unloaded or loaded network conditions, in which the generated traffic sources are bursty in nature, are considered. We analyse about MPEG-2 traffic and design a model, which makes use of the ATM OAM funcation in order to support the traffic control functions. To implement the model, we propose a scheme, which combines the performance management OAM function and the bandwidth allocation function. Especially, we design this scheme to control the VBR (Variable Bit Rate)MPEG-2 video traffic.

In this paper, we discuss a novel reservation-based, asynchronous MAC protocol called 'Multi-rate Multi-hop MAC Protocol' (MMMP) for multi-hop ad hoc networks that provides QoS guarantees for multimedia traffic. MMMP achieves this by providing service differentiation for multirate real-time traffic (both constant and variable bit rate traffic) and guaranteeing a bounded end-to-end delay for the same while still catering to the throughput requirements of non real time traffic. In addition, it administers bandwidth preservation via a feature called 'Smart Drop' and implements efficient bandwidth usage through a mechanism called 'Release Bandwidth'. Simulation results on the QualNet simulator indicate that MMMP outperforms IEEE 802.11 on all performance metrics and can efficiently handle a large range of traffic intensity. It also outperforms other similar state-of-the-art MAC protocols.

In this paper, we present a new macroblock-based bit allocation scheme in multiple MPEG-1 video encoders system and a single multiplexer over a single channel. The proposed scheme models the relations between fate(Bits/MB) and distortion(MSE/MB), rate and quantizer parameter(QP), distortion and quantizer parameter, respectively, in the same form. By using these relations, we minimize the Larangian cost function to obtain a bit allocation scheme based on macroblock unit. Experimental results show that the proposed scheme can reduce MSE compared to other conventional buffer-based rate control methods, i.e. independent buffer control method and shared common buffer control one. And we confirmed, through computer simulation, that the proposed scheme can be effectively modified to maintain the objective quality of a specific video service at a constant level.
