Delayed eruption of the first molar, without a generalized or localized cause, is usually associated with delayed development of the affected tooth. The aim of this study was to investigate the clinical features of the first permanent molar showing delayed development and eruption, and its association with developmental anomalies of other teeth. Panoramic radiographs of 40 healthy children showing delayed development and eruption of first permanent molars were analyzed. The clinical features of affected first molars and developmental anomalies of other teeth (except third molars) were evaluated. Delayed first molars were more frequent in the maxilla. The incidence of bilateral delayed development of first molars was greater than that of unilateral cases in female patients. In contrast, male patients showed unilateral delayed development of the first molar more frequently. A higher incidence of congenitally missing teeth was observed in patients with delayed first molar. In each case, delayed development or congenital absence was observed in the second molar adjacent to the delayed first molar. Overall, delayed first molar seems to be associated with congenital absence of additional teeth. Understanding the developmental mechanisms of this phenomenon requires further studies.

The congenital missing of mandibular second premolars is among the common dental anomaly in children. When a second premolar is diagnosed as congenitally missed, we should consider many factors influencing the treatment plan such as patient's age, states of roots of 2nd primary molar, degree of crowding, skeletal growth pattern, facial profile, procumbency of the incisor and lower facial height, etc. The mineralization of the second premolars begins in the majority of cases at the age of $2\sim2\frac{1}{2}$ years, but this period varies more widely than those for other permanent teeth. Also, mandibular second premolars show the greatest variations in differentiation and calcification. For this reason, aplasia of this group of teeth cannot be diagnosed at early age and with the same degree of certainty. From the clinical studies with 2 cases and some literature review on late development of second premolars, it could be summarized as follows : 1. The 2 cases showed marked delay in the development of mandibular second premolars. 2. After the crypt formation, the speed of calcification seemed nearly normal, suggesting that the delay was due to differentiation rather than calcification. 3. When one is encountered with similar conditions, it would be desirable to consider the possibility of delayed tooth development.

Root resorption of primary teeth usually occurs as the succeeding permanent teeth erupt, which induces differentiation of the hemopoietic cells into osteoclasts. Their root resorption pattern reflects the eruption path of the succeeding permanent teeth, and eventually the primary teeth shed as their succeeding permanent teeth erupt. Even when a permanent tooth germ is congenitally missing, root resorption of the corresponding primary tooth may still occur due to various factors, such as inflammation, traumatic occlusal force, and weakness of periodontium etc. Such congenital missing of permanent teeth is a commonly observed phenomenon in human be ing, and it often accompanies delayed retention of primary teeth. The etiologic factors for congenital missing in elude not only systemic diseases, but also local factors and human evolution process. In the radiographs of the cases in this report, the primary teeth without succeeding permanent teeth show pathologic root resorption. Root resorption progressed about 1/2~3/4 of the roots, and the surfaces of the resorption area were irregular. Considering high susceptibility of the periodontal ligament of primary teeth to root resorption, pathologic root resorption of primary teeth with delayed retention can be explained by the increased masticatory muscle force and abnormal occlusion developed during the mixed dentition. When the primary teeth without succeeding permanent teeth are lost, decision for space maintenance is required and long-term treatment plan for further prosthetic or orthodontic treatment should be establsihed.

Autotransplantation of teeth with complete roots is being increasingly performed for orthodontic treatment or for replacement of unsalvageable teeth, but this procedure has lower survival and success rates than those obtained following transplantation of teeth with incomplete root formation. While previous autotransplantation procedures used only models of the donor tooth and recipient site, surgical guide templates created using 3D-printing technology are now available. They allow shaping of the recipient site in proper direction and to the correct depth, thereby reducing the treatment time and improving the success and survival rates. Herein, we report a case involving autotransplantation of a tooth with complete roots at the site of a congenitally missing permanent tooth using surgical guide templates generated with 3D-printing technology. The procedure resulted in favorable healing of the transplanted tooth.

The purpose of this study was to examine the prevalence of developmental anomalies of permanent lateral incisor and eruption disturbances and analyze the association between two components. Panoramic radiographs of 3984 patients (aged 5 to 13 years) who visited the Department of Pediatric Dentistry of Seoul National University Dental Hospital between November 2016 and October 2017 were screened. The prevalence of developmental anomalies of permanent lateral incisors was 10.2%. The most common developmental anomalies were congenitally missing teeth(66.1%), followed by peg lateralis(33.5%). The prevalence of eruption disturbances was 16.5%. Among the patient with developmental anomalies of permanent lateral incisors, associated eruption disturbances were appeared on 31.1% of patients. Peg lateralis(p < 0.001), underdeveloped lateral incisors(p < 0.001) and dens invaginatus(p = 0.004) were associated significantly with eruption disturbance of permanent teeth. Eruption disturbance in patients with peg lateralis and dens invaginatus was most prevalent in maxillary canine. For patients with underdeveloped lateral incisor, eruption disturbance of maxillary central incisor was most frequent. It is important for patients with developmental anomalies of lateral incisors to detect associated eruption disturbance early through regular checkup. Diagnosis and treatment plan in view of such relationships is important in order to treat appropriately at the optimal time.

The purpose of this study was to find out prevalence of peg-laterals and to evaluate the frequency of dental anomalies in Korean children with peg-laterals. For this study, panoramic radiographs of 3,278 patients (aged 7 to 15 years) from the Department of Pediatric Dentistry admitted from January, 2008 to January, 2013 were selected for an investigation on peg-laterals distribution. The prevalence of peg-laterals was 2.62% (86 subjects). Among the peg-laterals children, the distribution of associated dental anomalies were as follows: congenitally missing teeth (29.1%), dens invaginatus (19.8%), impacted teeth (12.8%), supernumerary teeth (9.3%), and transposition (4.7%). Due to this study showing frequent occurrences of peg-laterals with other dental anomalies, one suggestion is to consider such relationships before deciding on a diagnosis and treatment plan.

In this case, we would like to discuss about the single rooted deciduous mandibular first molar. The deciduous mandibular molar of a five years old boy was shown to have a single root, bilaterally. Ordinarily, the maxilary molars have three roots and the mandibular molars have two roots. However, when the hertwig's epithelial root sheath do not invaginate properly during tooth development, root fusion can occur from the absence of root separation. Molars with fused roots not only have unfavorable crown to root ratio, but also according to many reports, have higher probability of having multiple congenitally missing teeth or dens invaginatus in the maxillary incisors, consequently requiring preventive dental treatment In addition, disorders such as ectodermal dysplasia, syndactyly, clinodactyly, bluish sclera can also be related to this condition. Root fusion is known to be of autosomal recessive inheritance. Up to date, single rooted molars have been reported several times in permanent dentitions but hardly in deciduous dentition, which is the motive for this paper.
