슬관절의 측면 방사선 촬영에서 보조기구를 이용한 검사방법의 정확성과 편의성에 대한 연구 (A Study on the Accuracy and Convenience of Imaging Method Using Support Device in Knee Joint Lateral Radiography)

  • 슬관절의 측면 촬영 검사방법에서 기존에 권고되고 있는 정 측면 자세에서 중심 X선의 입사 각도를 머리 쪽 $5{\sim}7^{\circ}$로 조정하는 방법을 검사방법 A, 중심 X선이 슬관절 강을 향해 검사대의 수평면과 수직으로 입사하는 방법을 검사방법 B, 경골 원위부(distal tibia)의 바깥 면(lateral side)을 방사선 투과성 물질(radiolucent materials)로 보상한(compensated) 상태에서 중심 X선을 관절 강에 수직 입사하는 방법을 검사방법 C로 분류하였다. 연구 대상자들을 분류하여 3가지 검사방법을 각각 시행한 후, 검사영상에서 관절 강 거리(joint space distance)와 양측과의 거리(both condyle distance)를 정량적 지표로 설정하여 이를 측정하고 비교하였다. 또한 각 검사방법의 편의성을 검사자(Practician)들을 대상으로 설문조사를 통해 확인하였다. 정량적 지표의 측정 결과 검사방법 A와 C는 통계적 분석 결과 특징적인 차이가 없었지만(p>0.05), 검사 방법 B는 A와 C 모두에서 차이를 보였다(p<0.05). 검사방법의 편의성을 평가한 결과, 검사방법 A가 모든 항목에서 검사방법 B, C 보다 상대적으로 점수가 낮게 평가되었고, 검사방법 B와 C는 근소한 차이로 B가 높은 평가를 받았다. 본 연구를 통해 제안한 간단한 보조기구(support device)를 이용한 슬관절 측면 촬영 검사방법은 다소 복잡한 준비과정 없이도 기존의 권고 방법과 큰 차이가 없는 관절 강의 묘사가 가능하고, 검사의 진행과정에서 검사자의 편의성을 증가시킬 수 있었다.

  • In the progression of the Temporomandibular Joint Disorder(TMD), not only deformation and perforation of disc occur. But also fibrotic adhesion and inflammatory changes to the retrodiscal tissue can be seen in addition to the condylar degenerative change (e.g. osteoarthritis). However, the correct diagnosis,?planning for appropriate treatment, and prediction of prognosis are limited, because there are no means to stage the progression of the disorder. In this study relative signal intensity of retrodiscal tissue in MRI and the synovial fluid concentration of matrix metalloproteinase-2 (MMP-2), MMP-9, and Interleukin-6(IL-6) in the 23 temporomandibular joints(TMJ), from 17 patients with TMD were evaluated as a possible diagnostic marker. The relative signal intensity of retrodiscal tissue was referenced to brain gray matter with same region of interest(ROI) size. The concentrations of MMP-2, MMP-9, and IL-6 were evaluated by Enzyme Linked Immunosorbent Assay (ELISA). The collected data were compared with condylar degenerative change, joint effusion and disc position observed in MRI. The relative signal intensity of the retrodiscal tissue was increased significantly when degenerative changes were present. In addition, there was significantly high signal intensity in the presence of a disc displaced without reduction. The concentration of IL-6 was significantly increased when condylar degenerative change was no observed. And there were no changes in the levels of IL-6 according to disc position and joint effusion measurement. Moreover, there were no significant relevance between the concentration of total MMP-2 and active MMP-9 in synovial fluid, relative to degenerative changes in the mandibular condyle, to joint effusion, and to disc position observed on MRI images. In conclusion, the relative signal intensity of the retrodiscal tissue can be regarded as a mean of diagnosing the procession of TMD in a non-invasive manner. But more additional studies are required for the levels of MMP-2. MMP-9, and IL-6 to determine their potentials as a diagnostic marker for TMD.

하악후퇴증 환자에서 골신장술과 하악지 시상분할 골절단술의 술 후 안정성에 관한 비교 연구 (Comparison of Postoperative Stability between Distraction Osteogenesis and Bilateral Sagittal Split Ramus Osteotomy in Mandibular Retrognathism)

  • Purpose: In general, the surgical treatment for mandibular retrognathism is represented by two methods, distraction osteogenesis (DO) and mandibular osteotomy surgery. The DO is mostly preferred when the degree of advancement of mandible is large. However, the postoperative stability of mandibular advancement using DO have not been actively investigated. Therefore, in the present study we have compared the postoperative stability between DO and bilateral sagittal split ramus osteotomy (BSSRO) in mandibular retrognathism. Methods: Seven patients who had been treated by DO and thirteen patients with BSSRO were included in this study. Serial lateral cephalograms were analyzed by manual tracing and the amount of the mandibular elongation was measured. To evaluate the postoperative stability, positional changes of the condylar position and B point were analyzed. Results: Mean amount of mandibular advancement was $6.51{\pm}3.57mm$ for BSSRO group and $12.43{\pm}4.35mm$ for DO group, respectively. There was no significant difference in age between the two groups (P>0.05). Mean follow up periods were 10.77 months for BSSRO group and 11.28 months for DO group, respectively. After mandibular advancement, mean positional changes in the condyle were $0.56{\pm}1.43mm$ horizontally and $0.72{\pm}1.61mm$ vertically for BSSRO group and $0.53{\pm}1.56mm$ horizontally and $0.56{\pm}1.75mm$ vertically for DO group, respectively. Mean change of distance from B point to Y-axis was $-1.76{\pm}0.83mm$ for BSSRO group and $-2.14{\pm}1.82mm$ for DO group, respectively. According to the condylar position and B point, there were no significant differences in postoperative stability between the two groups (P>0.05). Conclusion: There was no significant difference in postoperative stability between DO and BSSRO group according to condylar position and B point. Based on the results of the present study, it is hypothesized that DO would be a good treatment choice for severe mandibular retrognathism because DO could achieve more mandibular advancement and concurrent soft tissue elongation.

심한 퇴행성 악관절 질환 환자에서 보철성 과두를 이용한 하악과두 재건술의 치험례 (A CLINICAL CASE OF ALLOPLASTIC CONDYLE REPLACEMENT WITH CONDYLAR PROSTHESIS IN SEVERE DEGENERATIVE JOINT DISEASE)

  • This is a case report about patient who had suffered from degenerative joint disease and treated by TMJ reconstruction with condylar prosthesis. The patient visited Korea University An-am hospital on 2007 complaining symptom about both TMJ pain, mouth opening limitation and open bite. From CT view there was severe resorption of both condylar head, therefore condylar prosthesis reconstruction was planned. After 3D RP model analysis for preparation, the patient was operated under general anesthesia for condylar prosthesis reconstruction and the symptom was alleviated. (increased mouth opening, reduced anterior open bite, full mouth occlusal contact achieved) Follow up was carried out monthly, but after this, patient refused follow up. After 26 months from the operation, the patient revisited for anterior open bite. In clinical evaluation, occlusal contact was remained, but anterior open bite was relapsed. From cephalometry analysis, severe resorption of glenoid fossa was found. Therefore, Autogenous disc reconstruction with alloplastic material was planned on August 2009. After another surgery, condylar prosthesis was regained its normal position in glenoid fossa, and occlusion was recovered properly.

골격성 3급 부정교합 환자에서 양측 상행지 시상분할 골절단술을 이용한 하악 후방이동 시 이동량에 따른 회귀현상 (Evaluation of Relapse according to Set-back Degree of the Mandible at Bilateral Sagittal Split Ramus Osteotomy in Mandibular Prognathism Patients)

  • Purpose: The purpose of this study was to examine the appropriate degree of set-back of the mandible by evaluating the rate of relapse after surgery. Methods: Among the patients who visited our hospital from January 2002 to January 2007 and who underwent orthognathic surgery, of the patients available for follow-up observation, the rate of relapse after surgery was investigated according to the set-back degree. The patients were divided into groups by the degree of set-back, and relapse was evaluated by the radiographs performed the day after surgery, 6 months after surgery, 1 year after surgery, 2 years after surgery and 3 years after surgery. Results: In cases that exceeded the limit of posterior movement of the mandible (13 mm) or that had the wrong position of the condyle, a greater tendency toward relapse was shown. Conclusion: Based on the results of this study, among the cases that required a large amount of posterior movement of the mandible, two jaw surgeries accompanied by bilateral sagittal split ramus osteotomy (BSSRO) and LeFort I osteotomy are recommended.

전방십자인대 재건술후 대퇴골 경골핀 고정의 실패 - 증례보고 - (Failure of Cross-Pin Femoral Fixation after Anterior Cruciate Ligament Reconstruction - A Case Report -)

  • 34세 남자 환자로 등산중 실족으로 발생한 전방십자인대의 완전파열로 자가 슬괵건을 이용한 경핀 고정법으로 전방십자인대 재건술을 시행하였다. 술후 경과는 양호하였으나 술후 3개월째 굴신 신전 운동 시 슬부 후측면의 동통과 지속적인 관절내 부종 소견을 보였으며 수차례의 관절천자와 약물투여에도 반응이 없었다. 컴퓨터 단층 촬영 상후방 피질 골의 천공이 관찰되어 술후 6개월째 이차관절내시경 수술을 시행하였다. 내시경 소견상 경핀고정물의 전방부 1/3에 일치되는 투명한 이물질과 대퇴경골관절면의 연골의 손상이 관찰되었으며 이물 제거 후 증세는 호전되었다. 경핀 터널의 잘못된 위치가 실패의 원인이라고 생각되었으며 경핀고정법을 이용한 전방십자인대 재건술시, 슬관절 과굴곡 상태에서 예각의 대퇴 터널을 만들어 후방 피질 골을 두껍게 남기고, 핀 홀을 만들 때 경핀가이드를 횡상과축보다 $10\~20$도 외회전시켜야 대퇴골 후방피질골의 천공을 방지할 수 있어 조기 실패를 예방할 수 있다고 생각된다.

악관절 후방부착부에 있어서 MRI 신호변화에 대한 고찰 (A STUDY ON THE CHANGES OF MR SIGNAL INTENSITY OF POSTERIOR ATTACHMENT OF THE TEMPOROMANDIBULAR JOINT)

  • Magnetic resonance imaging has been used in the temporomandibular joint(TMJ) primarily to define morphology and positional relationship of associating structures. This report examines signal intensity characteristics of the posterior attachment as they related to the severity of internal derangement. Fifty six joints in 35 patients with a history of TMJ dysfunction were imaged writ MR using $T_1$-weighted spin echo sequence. According to disk position, ability to reduction, and the presence of osteoarthritis, the joints were categorized into three groups. A group 1 was anterior disk displacement with reduction; a group 2 was anterior disk displacement without reduction; a group 3 was anterior disk displacement without reduction and condyle had osteoarthritic change. The control group was determined by the clinical absence of any signs or symptoms of current or past TMJ pain and dysfunction. Calculated the relative value of MR signal intensity in posterior attachment and disk to cerebral cortex of temporal lobe by means of computer program, we have compared them with each groups. The result showed statistically little significant difference of disk signal intensity among each groups. but, signal intensity from posterior attachment in group 2 and 3 were significantly(p<0.05) decreased than control group. this might reflect an fibrosis or hyalinization of posterior attachment, which was part of remodeling process that occurs in disk displacement without reduction. However, this study could not demonstrate histologic confirmation of the decreased signal intensity in the posterior attachment. So, further investigation could be needed to understand the association between them.

  • This study was performed to measure the mandibular movement and the changes of masicatory and sternocleidomastoid muscle activity reflected by occlusal disturbance during habitual chewing. For this study, 18 subjects(14 males and 4 females with an average age of 24.0) were selected. The impression of each subject were taken for measuring intermolar distance on lower dentition. The activities of masticatory and sternocleidomastoid muscle and the mandibular movement were recorded and analyzed during habitual chewing by means of E.M.G.(electromyograph), E.G.N.(electrognathograph), rotate program in BioPak analyzing system(BioResearch Inc.). 다발성 안면골절환자의 교합회복 증례 (Full mouth rehabilitation of a panfacial fracture patient with bilateral condylar fracture)

  • 다발성 안면골절 시 연조직 및 경조직의 소실로 인해 적절한 과두-디스크의 기능을 회복할 위치를 찾는데 어려움이 있다. 다발성 안면골절 및 양측 과두골절과 부족한 수직고경 및 안면비대칭을 가진 환자가 내원하였다. 교합안정장치를 이용하여 골절된 과두를 정복하고 과두-디스크 복합체에 새로운 기능적 위치를 선정하였다. 추적조사를 통해 과두의 위치가 편안감과 안정성이 적절하다고 판단한 뒤 임플란트와 고정성 보철물을 이용한 완전구강회복을 시행하였다. 환자는 심미성과 기능에 만족하였으며 안정적 교합상태를 보였지만 추가적인 교합변화의 가능성을 배제할 수 없기 때문에 야간안정장치 및 정기적인 관리가 필요할 것으로 사료된다.

골격성 III급 부정교합자의 측두하악관절에 관한 방사선학적 연구 (A radiographic study of temporomandibular joints in skeletal class III malocclusion)

  • Purpose : To investigate the differences between the position of the mandibular condyles in temporomandibular joints of patients presenting with normal occlusion and skeletal class III malocclusion. Materials and Methods: Forty-two subjects with normal occlusion and thirty-seven subjects exhibiting skeletal class III malocclusion prior to orthodontic treatment were included in the study. Transcranial radiographs of each subject were taken at centric occlusion and 1 inch mouth opening. The positional relationship between the mandibular condyles with articular fossae and articular eminences at two positional states were evaluated and analyzed statistically. Results: The mandibular condyles of the skeletal class III malocclusion group were found to be located more anteriorly from the center of the articular fossae compared to the normal occlusion group in centric occlusion. The mandibular condyles of the skeletal Class III malocclusion group were located more superiorly from the middle of articular height than those of the normal occlusion group in centric occlusion. However, these differences were not statistically significant. At 1 inch mouth opening, the mandibular condyles of the skeletal class III malocclusion group were placed more posteriorly from the articular eminences than those of the normal occlusion group. The mean angle of the articular eminence posterior slope were 56.51 ° ± 6.29° in the normal occlusion group and 60.37° ± 6.26° in the skeletal Class III malocclusion group. Conclusions: The mandibular condyles of the skeletal Class III malocclusion group were placed more anteriorly at centric occlusion and more posteriorly at 1 inch mouth opening when compared with those of the normal occlusion group.
