Therapeutic Results of Postoperative Radiation Therapy for Uterine Cervical Cancer (자궁경부암의 수술후 방사선치료 결과)

  • This is a retrospective analysis of 64 patients who was treated with postoperative radiation therapy after radical hysterectomy and bilateral pelvic lymphadenectomy (53 patients) or total abdominal hysterectomy(11 patients) for uterine cervix cancer between May 1980 and September 1991 at the Department of Radiation Oncology, Kyung Hee University Hospital. Most patients were FIGO IB (31 Patients) and IIA (25 patients), and median period of follow-up was 5.1 years. Of these patients,24 received adjuvant whole pelvis irradiation of 6000 cGy and 40 received 5000-5500 cGy whole pelvis irradiation and/or intracavitary radiation (7 Patients). The actuarial overall and relapse free 5 year survival rate were $71.0\%$, $68.3\%$ respectively. The survival rates by stage were $79.1\%$ in stage I, and $61.2\%$ in stage II. Treatment failure was noted in 18 of 64 patients ($28.1\%$), Iocoregional failure in 8 ($12.5\%$), distant metastasis in 8 ($12.5\%$), paraaortic node metastasis in 1 and one patient and concurrent locoregional and distant metastasis. The univariate analysis of prognostic factors affecting to overall survival rate represented lymph node status, the number and site of metastatic lymph node, parametrial invasion, the thichness of cervical wall invasion, and size of cancer mass. Histology, vessel invasion, endometrial extension, hemoglobin level. resection margin status, age, radiation dose were not significant prognostic factors. Complication relating to operation and postoperative radiation were variable according to radiation therapy method: 6000 cGy RT group 8/24($33.3\%$), 5000-5500 cGy+ICR 3/7 ($42.9\%$), 5000-5500 cGy external RT only group 3/33 ($9.1\%$). In conclusion, the results suggest that postoperative radiotherapy is necessary in high risk patients for locoregional control and improving survival rate, and higher dose does not improve results but only increases complication.

  • Error control is one of major concerns in many electronic systems. Experience shows that most malfunctions during system operation are caused by transient faults, which often mean abnormal signal delays that may result in violations of circuit element timing constraints. This paper presents a novel CMOS-based concurrent timing error detector that makes a flip-flop to sense and then signal whether its data has been potentially corrupted or not by a setup or hold timing violation. Designed circuit performs a quiescent supply current evaluation to determine timing violation from the input changes in relation to a clock edge. If the input is too close to the clock time, the resulting switching transient current in the detection circuit exceeds a reference threshold at the instant of the clock transition and an error is flagged. The circuit is designed with a $0.25{\mu}m$ standard CMOS technology at a 2.5 V supply voltage. The validity and effectiveness are verified through the HSPICE simulation. The simulation results in this paper shows that designed circuit can be used to detect setup and hold time violations effectively in clocked circuit element.

The Study of Malodor Reduction after Periodontal Treatment (치주질환 치료 후 구취 감소에 대한 연구)

  • Bacterial byproducts and volatile sulfur compounds(VSC) have been found to be the leading intra-oral agents, specifically, the byproducts of gram negative anaerobic bacteria have been implicated as primary factors of halitosis in patients presenting with periodontal disease. The objective of this study was to determine the correlation between periodontal treatment and the subsequent reduction in the level of halitosis. Forty-three subjects presenting with periodontal disease were examined before periodontal treatment, one week after treatment, one month after treatment, and finally, two months after treatment, using a portable sulfide monitoring $Halimeter^{(R)}$ to measure the VSC concentrations at the prescribed intervals. The results of the study were as follows: 1. Significant decreases in the mean VSC concentration were observed at the one week, one month, and two month post-op intervals relative to the pre-op measurement. (p<0.05) 2. Significant decreases in the mean VSC concentration were observed in subjects after completion of flap operations. Significant decreases in the mean VSC concentration were observed at the one and two month post-flap operation measurement relative to the VSC concentration at one week (p<0.05), but no significant differences between the one month and two month VSC concentrations were found. (p<0.05) 3. Significant decreases in the mean VSC concentration were observed in subjects after completion of subgingival curettage (p<0,05). Significant decreases were found between the one week and one month measurements and between the one month and two month measurements, but significant differences were not observed between the one week and two month measurements. (p<0.05) The results of this study show significant decreases in VSC concentration in test subjects after periodontal treatment. It can be inferred from the results above, that periodontal disease is a significant contributing factor of halitosis, and that treatment of periodontal disease can been an effective means of reducing VSC concentration in patients presenting with halitosis concurrent with periodontal disease.

Usefulness of indirect open reduction via a transconjunctival approach for the treatment of nasal bone fracture associated with orbital blowout fracture

  • Background: Nasal fracture and orbital blowout fracture often occur concurrently in cases of midface blunt trauma. Generally, these multiple fractures treatment is surgery, and typically, the nasal bone and orbit are operated on separately. However, we have found that utilizing a transconjunctival approach in patients with concurrent nasal bone fracture and orbital blowout fracture is a useful method. Methods: The participants in the present study included 33 patients who visited the Plastic Surgery outpatient department between March 2014 and March 2017 and underwent surgery for nasal fracture and orbital blowout fracture. We assessed patients' and doctors' satisfaction with surgical outcomes after indirect open reduction via a transconjunctival approach for the treatment of nasal bone fracture with associated orbital blowout fracture. Results: According to the satisfaction scores, both patients and doctors were satisfied with transconjunctival approach. Conclusion: We presented here that our method enables simultaneous operation of nasal fracture accompanied by orbital blowout fracture, rather than treating the two fractures separately, and it allows precise reduction of the nasal fracture by direct visualization of the fracture site without any additional incisions or difficult surgical techniques. Also, by preventing the use of excessive force during reduction, this method can minimize damage to the nasal mucosa, thereby reducing the incidence of nasal bleeding.

A preliminary study on the excavation sequence of a room-and-pillar underground structure by the drill-and-blast method (발파 굴착에 의한 주방식 지하구조물의 굴착공기 분석 연구)

  • A room-and-pillar underground structure is characterized by its grid-type array of room and pillar. As a result, its construction and economical efficiency can be governed by excavation sequence. In this study, the construction period by the drill and blast method which can be treated as a main sequence for excavation was examined by considering the regulation for blasting and construction standard of estimation in Korea. To evaluate the construction period for the room-and-pillar underground structure constructed in 4 kinds of square-type area ($30{\times}30{\sim}57{\times}57m$), the concurrent excavation pattern which was suggested in the previous researches was used. From the suggested condition, the total construction period by drill-and-blast method can be estimated with the consideration of the construction area, number of jumbo drill and faces in operation.

  • Multiple segment osteotomy orthognathic surgery serves to combine the total or segmental maxillary and mandibular correction of the dentofacial deformities with concurrent procedures to provide immediate repositioning to the dento-osseous elements. In addition, splitting the palate may often be necessary to correct a functionally poor relationship of the maxilla to the mandible or the facial skeleton by realigning the maxillary arch. In this case, the discrepancy in a bimaxillary horizontal relationship and the space between the 2nd premolar and 2nd molar was retained after lengthy preoperative orthodontic treatment. However, we could correct these dento-osseous discrepancies immediately by performing midpalatal expansion, anterior segmental osteotomy and symphyseal osteotomy with bimaxillary osteotomies. If the blood supply to each segment segments was maintained and primary closure of the operation site was feasible, multiple segment osteotomy was considered as a very effective technique for treating dentofacial deformities in vertical, transverse, and sagittal dimensions with differential repositioning of all segments.

Efficiency Evaluation of Nozawa-Style Black Light Trap for Control of Anopheline Mosquitoes

  • House-residual spraying and insecticide-treated bed nets have achieved some success in controlling anthropophilic and endophagic vectors. However, these methods have relatively low efficacy in Korea because Anopheles sinensis, the primary malaria vector, is highly zoophilic and exophilic. So, we focused our vector control efforts within livestock enclosures using ultraviolet black light traps as a mechanical control measure. We found that black light traps captured significantly more mosquitoes at 2 and 2.5 m above the ground (P<0.05). We also evaluated the effectiveness of trap spacing within the livestock enclosure. In general, traps spaced between 4 and 7m apart captured mosquitoes more efficiently than those spaced closer together (P>0.05). Based on these findings, we concluded that each black light trap in the livestock enclosures killed 7,586 female mosquitoes per trap per night during the peak mosquito season (July-August). In May-August 2003, additional concurrent field trials were conducted in Ganghwa county. We got 74.9% reduction (P<0.05) of An. sinensis in human dwellings and 61.5% reduction (P>0.05) in the livestock enclosures. The black light trap operation in the livestock enclosures proved to be an effective control method and should be incorporated into existing control strategies in developed countries.

Meniscal Repair with Meniscal Arrows (반월상연골 화살을 이용한 반월상연골 봉합술)

  • Purpose: The results of meniscal repair of peripheral tear of meniscus with meniscal arrow was analysed. Materials and Methods: Peripheral tears of posterior 1/2 of menisci of 25 cases including 18medial, 6 lateral and 1 both menisci were repaired with meniscal arrows. One to six meniscal arrows were used for each meniscus according to the length of tears. Concurrent tears of ACL were noted in14 cases. The follow-up period was 3.4 years in average. The findings of physical examination and MRI which were taken once or twice in 1$\~$5 years after operation were evaluated. Results: Healing of the tear was achieved in all cases. However, a vertical longitudinal tear of body occurred in two medial menisci along the line of insertion of arrows. An arrow migrated subcuta-neously in two cases, respectively. Conclusions: The meniscal arrow was effective in the treatment of the peripheral tear of menicus. However, there was a chance of occurrence of longitudinal tear of the meniscus in case of insertion of arrows in a row. Migration of the arrow may occur.

Enhancement Technique of Discharge Measurement Accuracy Using Kalesto Based on Index Velocity Method in Mountain Stream, Jeju Island (지표유속법 기반 제주 산지형 하천 Kalesto 유량 정확도 향상 기법)

  • In the mountain streams in Jeju Island, strong turbulence and roughness usually made it nearly impossible to utilize most of intrusive instrumentation for streamflow discharge measurements. Instead, a non-intrusive fixed electro-magnetic wave surface velocimetry (fixed EWSV: Kalesto) became alternatively popular in many representative streams to measure stream discharge seamlessly. Currently, Kalesto has shown noteworthy performance with little loss in flood discharge measurements and also has successfully provided discharge for every minute. However, Kalesto has been operated to regard its measured one-point velocity as the representative mean velocity for the given cross-section. Therefore, it could be highly possible to potentially encompass discharge measurements errors. In this study, we analyzed the difference between such Kalesto discharge measurements and other alternative concurrent discharge measurements such as Acoustic Doppler Current Profiler (ADCP) and mobile EWSV which were able to measure velocity in multi-points in the cross-section. Consequently, Kalesto discharge deviated from ADCP discharge in amount of 48% for relatively low flow, and more than 20% difference for high flow compared with mobile EWSV discharge measurements. These results indicated that the one-point velocity measured by Kalesto should be used as a cross-sectional mean velocity, rather it should be accounted for as an index-velocity in conjunction with directly measured cross-sectional mean velocity by using more reliable instrumentations. After inducing Kalesto Discharge Correction Coefficient (KDCC) that actually means relationship between index velocity and cross-sectional mean velocity, the corrected discharge from Kalesto was significantly improved. Therefore, we found that index velocity method should be applied to obtain better accuracy of discharge measurement in case of Kalesto operation.

  • Since Chrysler Motor Co. had experienced the digital development system in the beginning of 1990's, most of leading automobile companies are trying to apply a digital information system for their own business process reengineering based upon concurrent engineering system from product planning phase. This is called as virtual DMU(Digital Mock-Up) system instead of the traditional PMU(Physical Mock-Up) system. By using the virtual prototype, all of the design requirements and system specifications can be checked, changed and optimized more quickly and more efficiently. This paper consists of five chapters for the DMU information system. In the 1$^{st}$ chapter, the principle of digital design system is suggested by using four basic modules such as product design module, process design module, manufacturing system design module and central control module. The basic scheme of DMU is introduced with the benefits of application in the chapter 2. In the chapter 3, a digital design process of new car development is explained with the detailed DMU design and design review processes. In the chapter 4, the practical DMU manufacturing techniques and applications are introduced as CAD/CAM analyses, DPA(Digital Pre-Assembly)reviews for development, production, operation and maintenance phases, digital tolerance analyses and digital factory analyses for assembling line simulation, automated robot welding processes, production jig & fixtures and painting process simulation. Finally, the activities of digital design support; CAS-styling, CAE-engineering and CAT-testing are summarized for design optimization in the chapter 5. As today's automobile manufactures and related business organizations are struggling to compete in the global marketplace, they are concentrating on efficient use of DMU information system to reduce the new car development cost, to have shorten the delivery schedule and to improve product design quality. To meet the demand of those automobile industries on digital information systems, the CALS(Computer aided Acquisition and Logistics Support) and EC(Electronic Commerce)initiative has been focused as a dominant philosophy in defense & commercial industries, specially automobile industries.s.
