Impact of Computerization on Direct and Indirect Nursing Time (병원전산화에 따른 직접 및 간접간호 양상의 변화)

  • The purpose of this study was to examine impact of hospital computerization on the nursing practice. The study was conducted using the repeated measured design. The subjects were 166 nurses working at surgical nursing care units in a tertiary teaching hospital. A questionnaire was developed based on the Burke's study, and reviewed by a committee for content validity. Eighty-eight nurses responded the questionnaire throughout the study. Indirect nursing care time significantly decreased(p<0.05) after the computerization. These decrease related to time saving in checking physicians' order and writing nursing records. However, the change in direct nursing care time was not clear. In conclusion, the computerization led to the decrease in indirect nursing care time but it did not link to increase in direct nursing care time. Therefore, nurse managers should take into account redesign of the nursing works after computerization.

  • Purpose: The research studied to survey the degree of utilization, satisfactions and problems of the computerization in nursing practice. Method: The subjects were 176 nurses who are working at university hospital in Seoul. The research was operated by 75 questionaries which were developed and modified from precedence study of Kim(1998), No(1996) and Pang(1996). The data was processed by SPSS for window 11.0 and the results analyzed with number, percentile, average, t-test and ANOVA. Results: As the way of using computer system in nursing, the most nurses used that to Dr's order check, treatment activity, laboratory, barcode sticker, medication in order that were connected with order system mutually to other parts. The average degree of contentment from the computerization in nursing practice was presented as 3.855 in the full mark 5. The facility of stock management got the highest points(4.98). The problem was marked 3.12 and the biggest problem is interruption of work from the computer system error(4.03). The contentment of computerization in nursing practice by duty year was reflected a significant difference(F=5,457 P=0.005) and was not related with computerization and computer education(F=0.028 P=0.867, F=2.178 P=0.142).

A Survey Study of Nursing Diagnosis Use in Clinical Practice (간호진단의 임상적용 활성화를 위한 기초조사 연구)

  • The purpose of this study was to identify the degree to which nursing diagnoses accepted by NANDA are used and to identify problems in application of nursing diagnoses in clinical practice. With the expanding potential for computerization of nursing diagnosis, the survey also included data on the present status of hospital computerization and willingness to use computerized nursing diagnoses. The data collection was done from July 1 to August 3, 1996 using structured questionnaires. The questionnaires were mailed to 1,126 head nurses working in 44 hospitals with on occupacy of over 500 beds located in Korea, of these, 883 were returned from 40 hospitals. Among the 883 questionnaires, 867 were used for the analysis. The results of the analysis are as follows 1. Among 109 nursing diagnoses, pain, constipation, diarrhea, hyperthermia, high risk for infection, sleep pattern disturbance, and anxiety, chronic pain, altered urinary elimination, and altered nutrition : less than body requirements were the ten most frequently used diagnoses. 2. The primary problem in the use of nursing diagnoses was lack of time and personnel. Others were lack of knowledge and motivation, absence of protocols and absence of the appropriate methods to apply nursing diagnoses. 3. Among the 40 hospitals, 27 hospitals used a computerized system and expressed willingness to utilize the computerized system of nursing diagnoses that is planned for the future.

A Survey on Nurses' Utilization of Computerization in Nursing Practice (간호사의 간호업무 전산화 활용에 대한 조사연구)

  • I have practiced this reserch for the purpose of proposition of basic data for amendment and addition of computer system after I surveyed the degree of abilities and satisfaction on the computerization in nursing\ practice. Subjects were collected from September 1 to September 30, 1998. Study subjects are 151 clinical nurses who are working at university hospital in Chonbuk area. Study instrument consists of 116 questionnaire which was developed by Kim H. K(1998). The data were analized number, mean ,t-test, ANOVA by using SPSS. The study results are as follows: 1. Most of the nurses who answered the questions were educated computerizing(96%). The abilities of computer program utilities were the most in inputting data(74%). The program nurses wanted computerizing education was wordprocess, dealing internet and statistics. 2. As for applying parts of computer system in nursing, the most nurses were Dr's order check (96%), treatment activity(91.4%), medication(89.4%), lab. test(87 .4%), transfering department(85.4%), transfering room(79.5%), diet(71.8%), discharge (70.2%), Barcord Sticker(70.2%) , reservation(62.2%) in order that were all conected order system mutually to other part. It showed that computer system was not applied for treating the original nursing work such as nursing record (13%), duty scheduling(6.0%) , nursing process(4.6%) , Q.I(1.3%), nursing research(1.3%), education(1.3%), 3. As for the benefit when computer system is applied for, the most respondence was promptness of work and convenience(90.6%) , exactness of work (82.8%), offering information exactily and efficiently(36.3%), offering good quality of nursing (6.0%) in order. 4. The degree of contentment on computerizing in nursing practice showed average 3.24%. The best marking item is that they are eager to participate in the computer education. Next, the automatic output of label and giving the number automatically is to prevent loss and mentioned items of patient registration from being changed(3.95) and to prevent mistakes possibly happening because of doctor's difficult order(3.85). 5. The problems which may happen to in nurses' utilization of computerization for the nursing practice showed average 3.18. The most problem is that the practice may stop because of the problem of computer itself(3.67), and the next, the indication of act can be imperfect(3.66) , manual training may overlap because of incomplete computerization (3.60), practical education for the computerizing is lack(3.41), and the computer literate nurses are lack(3.40) . 6. Study subject's contentment by age was significant difference(F=3.10,P=0.0119). Study subject's contentment by job posision was significant difference(F=6.001P=0.0034) I will propose the following according to the above results. The domain of original nursing practice is urgernt. Manual and indication of act should be made before long in computer obstacle. Nursing department should support the nurses so that they could receive the education needed for the nurses themselves.

  • This study was conducted to develop a home care nursing management system based on the validated and useful data base found through literature review. The contents and structure according to a development procedure for a computer system were as follows. 1. A data base on home care nursing patients was accumulated by putting data respectively in both steps and fields - from selection criteria. basic information. prescription. plan of home visits. to application of nursing process. 2. Accumulated data was classified and designed to search by basic information. drug/injection prescription. examination prescription, treatment prescription. supply. and a record of the nursing process. 3. Various forms of retrieval including graphs were elaborated in terms of diagnosis and intervention aspects.

Impact of Applying 'Nursing Process Program' to Nursing Practice - Based on I.C.U Nursing Practice - (임상실습에서의 간호과정 전산프로그램 활용결과 분석 - 일 간호대학의 중환자 실습을 중심으로 -)

  • This study was done to identify the effectiveness of applying a 'Nursing Process Program' to the nursing students' clinical practice. The data collection period was from September 2000 to January 2001, and the subjects of the study were 39 students who were doing clinical practice in the ICU. The results are as follows : 1. Most subjects had a personal computer, had experience in using computers for one year and usually did word processing and internet, and wanted computerization of nursing work. The average results of the questions on the need for, and the effectiveness of applying a 'Nursing Process Program' to clinical practice were, on five point scales, 4.07 and 4.21 respectively. 2. There were no significant differences in the mean scores for suhjects' attitudes toward computers and computer anxiety between before and after using a 'Nursing Process Program'. 3. The total case study frequency using a 'Nursing Process Program' was 68, and 'Risk for infection' was the most frequent nursing diagnosis.

A Study on the Development of a Computerized Standardized Nursing Care Plan (표준화된 간호계획의 전산 시스템 개발을 위한 연구)

  • Computerization has been applied in most areas of health care in recent years. But computerized standardized nursing care plans have not yet been adequately developed even though they can significantly contribute to an improvement in the quality of nursing care. Therefore, this study was done to develop computerized nursing care plans. For this purpose, the study had the following objectives ; 1) to develop a computerized program for standardized nursing care plans which include defining characteristics, relating factors, expected out-comes and nursing interventions 2) to test applicability of the computerized pro-grams to the actual clinical situation of the clinical nurse. Application of the system development life cycle by davis was used as the study methodology. There are five stages in this method : strategy planning development, system analysis, system design, system practice and system evaluation. The first four steps in this methodology were used in this project. Two clinical nurses were selected to apply this computerized program in the clinical situation. The results of the study are as follows ; 1) Ten standardized nursing care plans including defining characteristics, relating factors, expected outcomes and nursing interventions which had been proven to have validity in the clinical setting were computerized using the Foxbase Program. 2) These programs were applied to the actual clinical situation by two clinic친 nurses working at Severance Hospital. For further study, continuous revision should be done by the users. Evaluation related to the utilization of the program should also be done.

A Survey on Nurses' Attitudes for Computerization in Nursing Practice in Kyung In Province (경인지역 임상간호사의 간호업무 전산화활용에 관한 태도 조사연구)

  • This study attempted to increase quality of nursing care by assessing the nurses' attitudes for computerization in nursing practice and ensuring right recognition and attitude for computerized system. Study subjects are 311 nurses who are working at 4 general hospitals. Those hospitals are selected according to the degree of computerized in Kyoung In Province. Study instrument consists of 20 items and 12 general questionnaire which was developed by Brodt and Stronge(1985) and modified by Lee(1991). Data collection was done from april 19th in 1995 to april 29th. Data gathered were analized number, t-test, ANOVA by using SPSS. The study results are as follows : 1. Study subject's attitude for computer application showed positive response as mean score of 3.44 especially nurses who had experience using computer showed 100% pro-response. 2. Study subject's attitude by age was significant statistically(F=3.9589, p=.0038), among them age 25-30 revealed the highest positive scores, and age 40 showed the lowest positive scores. 3. Study subject's attitude by education level showed no significant difference(F=1.4182, 2437), and those who are more educated showed more positive response. 4. Study subject's attitude by duty years showed no significant difference(F=2.2648, p=.0622), and those who had 7- 9 duty years had the highest positive attitude however those who have 10 duty years showed lowest positive attitude. 5. Study subject's attitude by position revealed no significant difference(F=2.0017, p=.1369), and the charge nurse groups who were using computer showed the highest scores. 6. Study subject's attitude by computerized type showed significant difference(F=23.9665, p=.0000), and G hospital which had computer system in nursing area obtained the highest positive scores. 7. study subject's attitude by presence of computer education showed no significant differenec(F=.9002, P=.3435), and non computer educated groups showed the highest positive response. 8. Study subject's attitude by computer education place revealed no significant difference(F=4.3723, p=.0020). and the groups who were taught individually or in the institution obtained the highest positive scores. 9. Study subject's attitude by duty year computer use showed no significant response(F=.9591, P=.3282), and groups of computer use showed the high degree of positive response. 10. Study subject's attitude by duty year using computer showed significant difference(F=5.9174, p=.0009),and the groups using computer for 6 years or group of 4-6 years showed the highest degree of positive response and the groups using computer for 1 year showed the lowest degree of positive response.

Design of Knowledge Model of Nursing Diagnosis based on Ontology (온톨로지에 기반한 간호진단 지식모델의 설계)

  • Nurses have performed their nursing practice according to the standard guidelines such as NANDA, NIC, and NOC, and recorded the information on nursing process into EMR system. In particular, NANDA, nursing diagnosis taxonomy, has difficulty expressing nursing diagnosis in detail because it represents abstract concepts of nursing diagnosis. So, the hospitals in KOREA have developed and used the list of nursing diagnosis on their own without referring the international standard terminologies, and it caused the delay of computerization of nursing records. Therefore, we proposed a ontology development methodology on nursing diagnosis based on NANDA and SNOMED-CT. The developed ontology, systematically developed with the frequently used nursing diagnosis terminologies in each hospital, based on the proposed methodology enables knowledge expansion and interoperable exchange of nursing records between EMR systems. We developed an ontology using the 112 nursing diagnosis terms defined by extracting and refining information on nursing diagnosis recorded in Kyungpook National University Hospital. We also confirmed the content validity and the usefulness of the developed ontology through expert assessment and experiment.

A study for innovation of Adult nursing assessment tool (간호정보조사지 개선을 위한 연구)

  • This study tried to suggest the basic materials that can be efficiently applied in clinical cases by understanding problems through a content analysis of an adult nursing assessment tool and opinion agreement about nurse's practical usage presently used in the hospital. The study was carried out in 36 attached hospitals in nationwide universities from May to December, 1999, the two hundred and twenty five reports were for analysis. The contents of the collected nursing assessment tool were analyzed. It was found that the tool had been used with various names and content and there were instances of partial omission of a number of items, such as documentation and time records. Other results revealed that they mostly had a systematic classification of items, formation of details a form of a check list, and the effect of saving time. In spite of the adult nursing assessment tool, it was suggested that its style be subdivided according to the specialized of a department and that standardized style be amended and supplemented. The respondents also answered that there had to be education about continuous and sufficient health assessment skills on the physical examination record. The most frequently suggested items to be added were: past history, marital status, patient and caregiver's address and telephone number. It was found that a patient's education career, economic status, religion, hygienic practice, sexual life and hobby were the most frequently omitted items on record. The reason given wes because the items were associated with his/her privacy. These results highlight the importance of analyzing the content with an complete data collection, supplying basic content for a more accurate nursing record, computerization, sharing information and standardization of the form.
