Arteriovenous fistula formation following disk surgery (추간원판절제술후 발생한 동-정맥루공 수술치험 1례)

  • A vascular complication caused by lumbar disc surgery is not infrequent till recently after the first report by Linton & White in 1945. In October 1980, we experienced one case of arteriovenous fistula following lumbar disc surgery in the department of thoracic surgery, CAFGH. The A-V fistula was situated between left common iliac artery and vein, which was confirmed by angiography easily. The A-V fistula was corrected surgically by Taylor`s method successfully without complication.

  • Pneumothorax after needle electromyography is a rare complication, which usually associated with examination of diaphragmatic and intercostal muscles. However, by the literatures, it can also occur with supraspinatus, serratus anterior and paraspinal muscles. We experienced a case of pneumothorax after cervical paraspinal muscle needle electromyography. From the anatomical vulnerability of pneumothorax during needle insertion, we emphasized the importance of avoiding this complication.

  • The bronchopleural fistula is the most common and serious complication of postpneumonectomy empyema. We experienced one case of postpneumonectomy empyema with bronchopleural fistula which treated with Abruzzini operation using residual long bronchial stump. Median sternotomy was used with extension about 3cm incision toward cephalic side. We ligated and divided the innominate vein. We did not open the pericardium with extrapericardial approach. Stapler was used to distal bronchial side and additional interupt sutures were used on proximal side.

  • In an attermpt to aviod the deleterious effects of cardiopulmonary by pass such as pulmonary complication neurologic complication and renal failure off-pump CABG has been rediscovered and developed. We experienced off-pump CABG in 2 cases with unstable angina complicated with COPD and report herein the cases with review of literature.

  • A 45-year-old woman was diagnosed as having chylothorax after a mitral valve replacement for mitral stenosis. direct injury of lymphatics in thymus a ramification of thoracic duct was presumed to be responsibe for this complication. Four weeks of conservative treatment failed and surgical treatment was performed, We report a case of surgical treatment for chlyothorax after and open heart surgery.

  • Cephalohematoma with epidural hematoma[EDH] is a known complication of birth trauma and is usually best managed by observation only. However, this complication may jeopardize a neonatal life because of cranial compromise and hemodynamic instability. It should also be recognized that surgical intervention confers undoubted benefits on patients in some cases. We report a case of massive EDH with cephalohematoma and linear skull fracture, successfully treated with a craniotomy and evacuation of the hematoma.

  • Percutaneous epidural neuroplasty (PEN) is a known interventional technique for the management of spinal pain. As with any procedures, PEN is associated with complications ranging from mild to more serious ones. We present a case of intracranial subdural hematoma after PEN requiring surgical evacuation. We review the relevant literature and discuss possible complications of PEN and patholophysiology of intracranial subdural hematoma after PEN.
