Purpose: This study is a descriptive research intended to clarify the medication status of community-dwelling elders and to identify factors affecting their medication adherence. Methods: Data were collected using questionnaires and interviews from 101 subjects who had taken prescribed drugs for at least 7 days sampled among elderly people using home care nursing at a general hospital in Seoul. Results: According to the results of this study, medication adherence measured by pill counting was 88.3% and that measured by self-reporting was 94.6%. There were statistically significant differences in medication adherence according to major disease (p=.006), the number of admissions (p=.032), the number of drugs (p=.051), the frequency of medication (p=.026), and depression (r=-.205). In addition, depression was found to be a significant variable explaining the medication adherence with explanatory power 3.8% (p=.035). Conclusion: The presence of depression affected the elderly subjects' the medication adherence. Therefore, more concern and educational approaches are required to encourage elderly people to comply correctly with medication regimens particularly for elderly patients who have a malignant or long-lasting disease or who have to take multiple drugs or maintain a daily dosing frequency.
Purpose: The purpose of this study was to develop and test the psychometric properties of the newly developed instrument, Meaning in Life, for elderly Korean people. Methods: Ten older adults participated in the qualitative research used to develop the initial items. Participants for the psychometric testing were 371 community-dwelling older adults. Validity and reliability analyses included content, construct, and criterion-related validities, internal consistency, and test-retest reliability. Results: The Meaning in Life Scale consisted of 12 items with three distinct factors; value of life, source of life, and will to live, which explained 86.7% of the total variance. A three-factor structure was validated by confirmatory factor analysis. Criterion-related validity was supported by comparison with the Purpose in Life Test (r=.74). Reliabilities were secured with test-retest reliability of Intra-class Correlation Coefficient (ICC) .85 and the Cronbach's alpha coefficient .90. Conclusion: The results of this study indicate that this instrument is useful to measure meaning in life in Korean elders.
Purpose: This study was to examine the effect of electrical muscle stimulation therapy on chronic knee pain and depression for the aged. Method: Design was one-group pretest-posttest design. Samples were 32 elderly of 60 years old and above with chronic knee pain and depression. Measures were the S-F McGill Pain Questuionnaire for chronic knee pain and the Korean Elderly Depression Scale for depression. Electrical muscle stimulation therapy, experimental treatment, was applied for 12 weeks, 3 times/week, 15 min/time. Data were collected from January 2005 to May 2005. Data were analyzed using SPSS PC+ 12 version. Results: After receiving electrical muscle stimulation therapy, chronic knee pain (t=-88.034, P=.000) and depression (t=-114.659, P=.000) were significantly decreased. Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention on chronic knee pain and depression for community dwelling elders.

In this study, we have investigated the effect of perceived health status, sleep, depression and pain on quality of life by Gender in Community-dwelling Older Adults. A cross-sectional descriptive study was conducted with a survey of elderly aged 65 years or older that registered in the health care center located in J city from december 20, 2012 to february 15. Data were analyzed with descriptive statistics, t-test, ANOVA. Pearson correlation coefficients and multiple regression analysis using the SPSS Win 18.0 program. The result of perceived health status on average $9.89{\pm}2.21$ points, sleep $39.72{\pm}8.79$ points, depression, $7.13{\pm}7.54$ points, pain $5.37{\pm}1.90$ points and quality of life, the average was $7.61{\pm}2.25$ points. Perceived health status of low-income that perceived health status, sleep, and pain. explained 44.6% of variance in their quality of life. Based on the findings of the study, low-income elderly in home in order to improve the quality of life of the systematic development of intervention programs for health care and nursing needs to be applied.
Objectives More than half of the elders suffer from chronic sleep disturbances. Moreover, sleep disturbances are more prevalent in patients with depressive disorder than in community dwelling elderly. In this study, we aim to estimate the risk factors of poor sleep quality and its effect on quality of life in patients with late life depression. Methods This study included 159 depressive patients aged 65 years or older who completed Pittsburgh Sleep Quality Index (PSQI). A global PSQI score of 5 or greater indicates a poor sleeper. Structural diagnostic interviews were performed using the Korean version of Mini International Neuropsychiatric Interview (MINI). Depression was evaluated by the Korean form of Geriatric Depression Scale (KGDS). Global cognition was assessed by Mini-Mental State Examination in the Korean version of the Consortium to Establish a Registry for Alzheimer's Disease assessment packet. Quality of life was evaluated by the Korean version of Short-Form 36-Item Health Survey (SF-36). Results The frequencies of poor sleepers were 90.5% in major depressive disorder, 71.8% in minor depressive disorder, 47.1% in subsyndromal depressive disorder, and 73.0% in all types of depressive disorders. Multivariate logistic regression analysis indicated that female [odds ratio (OR) = 2.83, 95% confidence interval (CI) = 1.20-6.67] and higher KGDS score (OR = 1.13, 95% CI = 1.05-1.21) were risk factors of poor sleep quality in patients with late life depression. In the analysis of ANCOVA adjusted for age, gender, education and KGDS score, the mean scores of vitality mental health and mental component summary of SF-36 were lower in poor sleepers than in good sleepers. Conclusions Poor sleepers among patients with late life depression are very common and are associated with female and higher KGDS scores. Poor sleep quality causes a significant negative effect on mental health quality. So researchers and clinicians should be more vigilant in the evaluation and treatment of sleep disturbances in patients with late life depression.

The aim of this study was to determine the physical performance and fear of falls in the community dwelling elderly and to investigate how the factors were connected to one another. The data was collected between January 3rd and March 30th, 2014 from 460 elderly people, aged 65 or older, who agreed to participate in this study. The data was sampled among the elders' assistants as a way of responding to the questionnaire after being read. The study scales were composed of the demographic characteristics, physical function and fear of falls. A total of 139 (30.2%) subjects had experienced a fall. The results showed a positive correlation with the chair rise, standing on the leg, pick up a penny from floor, timed up and go test, and fear of falls. Multiple linear regression analysis showed that a predictor of the fear of falls were chair rise and timed up and go test. A combination of physical performance accounted for 16.9% of the fear of fall. Based on the findings of this study, it is useful to more effectively develop fall prevention and intervention programs in a future study.

BACKGROUND/OBJECTIVES: Probiotics have been suggested as potent modulators of age-related disorders in immunological functions, yet little is known about sex-dependent effects of probiotic supplements. Therefore, we aimed to investigate sex-dependent effects of probiotics on profiles of the gut microbiota and peripheral immune cells in healthy older adults. SUBJECTS/METHODS: In a randomized, double-blind, placebo-controlled, multicenter trial, healthy elderly individuals ≥ 65 yrs old were administered probiotic capsules (or placebo) for 12 wk. Gut microbiota was analyzed using 16S rRNA gene sequencing and bioinformatic analyses. Peripheral immune cells were profiled using flow cytometry for lymphocytes (natural killer, B, CD4+ T, and CD8+ T cells), dendritic cells, monocytes, and their subpopulations. RESULTS: Compared with placebo, phylum Firmicutes was significantly reduced in the probiotic group in women, but not in men. At the genus level, sex-specific responses included reductions in the relative abundances of pro-inflammatory gut microbes, including Catabacter and unclassified_Coriobacteriales, and Burkholderia and unclassified Enterobacteriaceae, in men and women, respectively. Peripheral immune cell profiling analysis revealed that in men, probiotics significantly reduced the proportions of dendritic cells and CD14+ CD16- monocytes; however, these effects were not observed in women. In contrast, the proportion of total CD4+ T cells was significantly reduced in women in the probiotic group. Additionally, serum lipopolysaccharide-binding protein levels showed a decreasing tendency that were positively associated with changes in gut bacteria, including Catabacter (ρ = 0.678, P < 0.05) and Burkholderia (ρ = 0.673, P < 0.05) in men and women, respectively. CONCLUSIONS: These results suggest that probiotic supplementation may reduce the incidence of inflammation-related diseases by regulating the profiles of the gut microbiota and peripheral immune cells in healthy elders in a sex-specific manner.
