Reliability and Validity of A Korean Version of the Practice Skills Inventory I (한국어판 사회복지실천기술 척도(Practice Skills Inventory)의 신뢰도와 타당도 평가 I - 의료사회복지사와 정신보건사회복지사를 대상으로 -)

  • Although social work practice skills are one of the important components of social work practice, research on social work practice skills has been very scarce. It seems that the reason might be a lack of standardized instruments used to measure social work practice skills. The purpose of this study is to validate a Korean version of the Practice Skills Inventory(PSI) developed by O'Hare and colleagues. The PSI is the only instrument developed in the field of social work to measure the frequency of social work practice skills that social workers use. A total of 310 social workers working in medical or mental health field were participated in this study. A result of confirmatory factor analysis showed that the Korean version of the PSI is composed of 4 factors like its english version. The Korean version of the PSI was also reliable and valid. Implications for social work research, practice and education are provided.

Comparison of the Activity of Cortical Neurons According to Muscle Contraction Type between Post Stroke Hemiplegic Subjects and Healthy Subjects (뇌졸중 환자와 정상인의 근 수축 유형에 따른 피질 신경원의 활성도 비교)

  • Purpose: In order to examine difference in the activity of potential of spinal neurons and cortical neurons according to muscle contraction type in post stroke hemiplegic subjects and healthy subjects, the present study conducted an experiment as follows. Methods: The subjects in the experimental group were 17 left-side hemiplegic subjects (9 female, 8 male; mean age, 63.41$\pm$9.86 years) with the right hand as the dominant hand selected among post stroke hemiplegic subjects, and 17 age matched healthy control subjects (10 female, 7 male; mean age, 64.12$\pm$12.07 years). Movement-related cortical potentials (MRCPs) were measured using surface electromyography and electroencephalography while concentric and eccentric movements were made alternately. Results: As to the pattern of the activity of MRCP, which indicates the activity of motor cortical neurons, we found that the amplitude is high (p<0.01), the length of excitement is short (p<0.01) and the ascending gradient of amplitude to the peak increases (p<0.05) in post stroke hemiplegic subjects' lesion sites different from healthy subjects. Conclusion: The activity of cortical neurons was no difference in activity according to contraction type was observed in post stroke hemiplegic subjects' lesion sites. This suggests that there is no distinction in the activity of cortical neurons between concentric contraction and eccentric contraction. Accordingly, if post stroke hemiplegic subjects' activity of motor cortex is analyzed by measuring MRCPs, it is considered useful in research on neural plasticity or as a ground of clinical effects in the area of physical therapy for the central nervous system.

Effects of Performing Dual Task on Temporospatial Gait Variables in Subjects With Subacute Stroke (아급성기 뇌졸중 환자의 이중 과제 수행이 보행의 시·공간적 변수에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effects of performing a dual task on gait velocity, temporospatial variables, and symmetry in subjects with subacute stroke. Methods: The study included 14 independent community ambulators with gait velocity of 0.8m/s. The Korean mini-mental state examination, the Berg balance scale, the Trunk impairment scale, and the Fugl-Meyer assessment scale were used to recruit homogeneous subjects. Subjects performed a single task (10m ambulation at a comfortable speed) and a dual task (10m ambulation at a comfortable speed while carrying a water-filled glass). Gait variables were examined with the OptoGait system. Results: The findings of this study were as follows: 1) Gait velocity decreased significantly in the dual-task condition as compared to the single task condition. 2) There were no significant differences between the paretic and non-paretic stances. 3) Paretic swing decreased significantly in the dual-task condition as compared to the single task condition. 4) The non-paretic, double-limb support phase increased significantly in the dual-task condition as compared to the single- task condition. 5) There was no significant difference in temporal symmetry. 6) Non-paretic step length decreased significantly in the dual-task condition as compared to the single-task condition. 7) There was no significant difference in spatial symmetry. Conclusion: Performing dual tasks decreases gait velocity, paretic swing phase, and non-paretic step length, while it increases non-paretic double limb support. In addition, although there is no difference in temporospatial symmetry, there is high inter-subject variability in temporospatial symmetry. Thus, dual tasks should be selected in accordance with the functional level of the hemiplegic patient, and inter-subject variability of the individual should be considered when dual tasks are considered for gait-training of hemiplegic patients.

What's the Disaster Recovery? Types and Preimpact Plan of Recovery (재난복구의 유형과 사전 계획에 대한 이론적 고찰: 미국의 재난복구를 중심으로)

  • The purpose of this paper is to suggest the types of recovery activities and the major contents of preimpact recovery plan through theoretical review. Disaster recovery is to provide immediate support during the early recovery period necessary to return vital life support systems to minimum operation levels, and continuing to provide support until the community returns to normal. Disaster researchers distinguished 4 types in recovering from disaster as follows; an emergency period, a restoration period, a reconstruction period, and a developmental reconstruction period. And recovery measures are both short-term and long-term. Short-term measures are relief and rehabilitation and long-term measures include reconstruction. Finally, to design a preimpact recovery plan, we should define a disaster recovery organization that includes major stakeholders, identify the location of temporary housing, determine how to perform essential tasks, address the licensing and monitoring of contractors and retail price controls to ensure victims are not exploited, determine how recovery tasks will be carried out at historical sites, and recognize the recovery period as a unique time to enact policies for hazard mitigation and incorporate this objecive into the recovery planning process.

A comparison in the evidence-based practice(EBP) awareness of physical therapists versus occupational therapists (물리·작업치료사의 근거중심치료 인식도 차이)

  • Purpose. This study compared the awareness of the evidence-based practice(EBP) in 100 physical and occupational therapists. Methods. A questionnaire on awareness was conducted to examine the attitude toward EBP, the educational experience of EBP, and the performance ability of EBP. A questionnaire consisted of items on the general characteristics, the attitude toward EBP, the educational experience of EBP, and the performance ability of EBP of the subjects. Data analysis was made by IBM SPSS Statistics Ver. 20. The EBP awareness was examined by the independent t-test. Results. For the attitude toward EBP, there was a statistically significant difference in the item of 'Therapists should judge whether they apply study results to individual patient. For the educational experience of EBP, there was no statistically significant difference in all items. For the performance ability of EBP, there was a statistically significant difference in the item about the ability to understand patient's desire for treatment and treatment preference and the item about the ability to determine appropriate treatment process in cooperation with patients. Conclusions. Both two groups showed very low results in the attitude toward EBP, the educational experience of EBP, and the performance ability of EBP. Therefore, it is necessary to enhance EBP education in undergraduate programs and the clinical field.

Diagnosis of Vegetation for the Ecological Rehabilitation of Streams - The Case of the Namhan River - (하천의 생태적 복원을 위한 식생학적 진단 - 남한강을 중심으로 -)

  • This study was designed to present a river model with the aim of restoring the ecosystem and improving the landscape along the urban rivers on the basin of the Namhanl river, a core life channel for the National Capital regin. The revelation of botanical status, transition trend and correlation of plants might lead to providing the urban river restoration projects and ecological river formation projects with basic data for a model of ideal aquatic ecology and landscape. The outcomes of this study could be summed up as follows: 1. The plant communities of river flora found on the basin of the Namhan river could be categorized largely into 39 plant communities 2. Most diverse plants were distributed in the rivers lower reaches such as Unsim-ri where the protected zone of Paldang reservoir for city water borders the body of Jodae swamp where natural streams flow nearby. 3. One of the greatest threats to the biomass of the River Namhan is that the communities of such invasive alien plants as Panicum dichotomiflorum and Ambrosia artemisiifolia var. elatior dominate most parts of the area, a fact that has resulted in a reduced variety of plants and will, sooner or later, be likely to cause an ecological imbalance in the hitherto healthy Aquatic plant life. It is highly advisable to gradually diversify the species of trees and to return the plants bark to their original state since, besides the naturalized plants, plantations afforested with Erigeron canadensis and Erigeron annuus stocks in buckwheat field, Robinia Pseudo-accacia in riverside forest, Pinus rigida in terrestrial forest on the river basin and Larix leptolepis are anticipated to act as interceptors of normal migrations of the fluvial and terrestrial ecosystems. Finally, it seems also desirable to continue to explore the relationship between fluvial and terrestrial ecologies with the purpose of building up a model of natural streams in urban areas based on the surveyed factors for plant life, soil and landscape and, moreover, on the forecasting for overall influences derived from the rotation upon the ecosystem.

The Effect of Balance Task-Related Circuit Training on Chronic Stroke Patients (뇌졸중 환자의 균형 향상 과제 중심 순환 훈련의 효과)

  • Purpose: This study investigated the efficacy of task-related circuit training aimed at improving balance in individuals discharged from rehabilitation facilities following a stroke. Methods: We recruited 12 stroke patients (34-66 years of age) to participate in a task-related circuit training program. Baseline assessment included a history of stroke and an assessment using the Mini-Mental State Examination Korea (MMSE-K). After a baseline assessment, follow-up assessments were administered pre- and post-training. These included Berg Balance Scale (BBS), Functional Reach Test (FRT), and the Time Up & Go Test (TUG). Physiotherapists trained study subjects under the one-to-one supervision of students from the department of physical therapy. Circuit class study participants attended 90-minute treatment sessions, one day a week for 12 weeks (from September to December 2008). The program consisted of a light warm-up period (10 min), physical exercises for improving balance (20 min), tasks focused on improving balance (50 min) and a cool-down period (10 min). Results: Scores for the BBS assessment increased significantly (from 43.2 to 49.7) after the training (p<0.05). Reach distance on the FRT increased substantially (from 27.7 cm to 47.0 cm), although the improvement was not significant (p>0.05). The average time on the TUG test decreased significantly (from 23.7 sec to 19.5 sec) after the training (p<0.05). Conclusion: The task-related circuit training program improved the balance and mobility of subjects, indicating that such a group program is useful for stroke patients who are discharged from the hospital. More such task-related programs set in a community environment should be developed.

Change of Cerebral Motor Area Activity by Anodal Transcranial Direct Current Stimulation (tDCS) (양극 경두개직류자극에 의한 운동관련피질전위의 변화)

  • Purpose: Transcranial direct current stimulation (tDCS) is a useful method for modulating the brain activity. This study compared the effect of continuous and interrupted tDCS using the change in the movement related cortical potential. Methods: Thirty healthy participants (male: 18 and female: 12) were assigned randomly to three groups; sham tDCS, continuous tDCS, which the current continuously flowed for 10 minutes, and interrupted tDCS, which the interrupted current flowed for 10 minutes (repetition: 4sec stimulation and 5sec rest) at an intensity of 1mA with anodal polarity. The effect of tDCS on the right primary motor area was measured from the movement related cortical potential (MRCP) before and after the experiment. MRCP consisted of the bereitshaftspotential (BP) and negative slope potential (NS) at Cz and C4. Results: Continuous and interrupted tDCS showed a significant difference in the changes in the BP, NS at Cz and C4 compared to the sham tDCS. However, there was no significant difference between the continuous tDCS and interrupted tDCS. Conclusion: The change in cortical activity by continuous and interrupted tDCS results from an improvement in the MRCP. An interrupted tDCS may be a safe and useful modality for stimulating the cortical region.

Development of the Expert Education Program for the Management of Mental Health Prevention (정신건강예방관리 전문직 직무교육 프로그램 개발)

  • Mental health service facilities implement an effort to meet the increased mental health demands. However, no specific 'mental health expert education program' have been offered yet. This study aimed to develop of the expert education programs which are general programs(GP), and specific programs(SP) for the management of mental health prevention in 3 kinds of facilities, psychosocial rehabilitation centers, homeless facilities, and domestic violence counselling facilities. Data were gathered from the service providers by structured questionnaire, and experts related with mental health by delphi study. We surveyed the management status and present expert programs. Consequently, domains of the education programs were extracted. As results, we identified the type of provider's job & demands for education. Thus, we gained the informations for developing the expert education programs in mental health facilities. GP were composed by 3 domains and 9 programs, and SP were composed by 3 programs each facilities. This study should be helpful in expanding the government educational operations related with mental health prevention areas. Eventually, community mental health will be promoted and socio-economic burdens by mental health problems will be lessened. In addition, it will be a basic evidence for developing specialized programs in a mental health prevention system foundation.

Effects of guardians' service quality perception on the service perception and behavioral intention of patients in healthcare service (의료서비스 이용에서 보호자의 서비스품질인식이 환자의 서비스품질인식 및 구매행동의도에 미치는 영향)

  • In this study, we examined the effects of guardians' service quality perception(G-SQP) on patients' service quality perception(P-SQP) leading to patients' behavioral intention(P-BI) and the effects of G-SQP on guardians' behavioral intention(G-BI). To investigate the causalities of the variables, we collected national wide samples of 144 hospitals, 1456 patients and 1455 guardians of the patients and analyzed 1146 guardian-patient coupled cases refined with prerequisites such as missing value, outliers, normality and covariance conditions. Followed were contributions of the present study. First, G-SQP was a predictor of P-SQP. Second, in the first contribution statement, there was no statistically significant difference between inpatient and outpatient group. Third, proven was that G-SQP was a predictor of G-BI. Fourth, verified was that P-SQP was a predictor of P-BI. Fifth, G-SQP was a predictor of P-SQP being led to P-BI that meant P-SQP had a mediating role between G-SQP and P-BI. Since the guardians' perception affected patients' buying decision, the present study implied service managers of hospital settings should pay attention to guardians' perception of service quality as well as patients'. With such strategy, hospitals could improve financial performance in long-term.
