Purpose: The primary goal of this study was to develop a case management with exercise program for community dwelling elders who live alone, and examine the effects of the program. Method: The design of this research was a one group pre-post test study. The participations were 85 elders diagnosed with hypertension and diabetes mellitus and who lived alone as residents of D city. The case management with exercise program included exercise and counseling as the intervention and was provided for 12 weeks. Data were collected before and after the intervention which lasted from September 3 to November 26, 2009. Collected data were analyzed using descriptive statistics, and paired t-test. Results: There were significant differences in blood pressure (t=-5.24, p<.001, t=-1.94, p= .040), fasting blood sugar (t=-4.41, p<.001), ADL (t=-5.43, p=.022) and cognitive function (t=7.41, p=.008) between pre- and post intervention. Conclusion: These results indicate that the case management program is an important intervention for health promotion for community-dwelling elders, and exercise improves functional status of older persons with diseases. Therefore, now is the time to develop new supportive community-based programs for elders who live alone. However, it is also necessary to do further longitudinal studies to confirm the results of this study.
Objectives: This study aimed to share with experiences of a demonstration program based on a community for prevention and management of hypertension and diabetes mellitus, and to supply the evidence of accessible strategies within the community through the public-private partnershipin the near future. Methods: This study case was "the program of registration and management of hypertension and diabetes mellitus patients" which was conducted in Hogncheon-gun in Gangwon-province, 2012. Results: The infrastructure of this center was constructed with the public-private sector partnership according to the basic model of demonstration program since November, 2012. So, the total registered rate of hypertension and diabetes mellitus patients were 26.6% in comparison with suspected patients (to the result of 2011 Korea National Health and Nutrition Examination Survey), 37.8% in comparison with the real number of outpatient (to the claims data of 2011 Branch Honcheon-gun, National Health Insurance Corporation), and 107.8% in comparison with the project goal, sequentially. To the patients who were not treated for 30 and 60 days among the registered patients, a recall service was conducted. Through this intervention, it was monitored that this program has enhanced the consecutive treatment rate of the registered patients. Conclusions: To improve the continuous management of hypertension and diabetes mellitus patients, we are gotten to know that the community need the joint participation and mutual cooperation with public-private sector partnership.
Objectives: This study was conducted to investigate practical experiences of telemedicine of community health practitioners(CHPs). Methods: Qualitative data were collected by in-depth interviews from 10 CHPs who have experiences in managing telemedicine system. All interviews were recorded and transcribed according to qualitative conventional content analysis processes. Results: As a result, 32 themes were deduced and 11 theme clusters and 3 categories were formed and each coding categories were derived directly from the text data. 11 theme clusters derived from the 32 meaningful themes were as follows: Human resources, equipments and systems, computer program (Input resources), human resource management, patient registration and management, medication, laboratory test (Progress), benefits in telemedicine system managing, difficulties in telemedicine system managing, complains in telemedicine system managing, client responses to telemedicine system (Outcome evaluation). 3 categories derived 11 theme clusters were 'input', 'progress', and 'outcome evaluation'. Conclusions: This study has contributed to the understanding of operation of telemedicine by CHPs in community health posts. For more systematic and comprehensive management, further study should be conducted to reflect experience and positions of public health center physicians, collaborative hospital physician and patients.
Objectives: Busan had the highest mortality and the shortest life expectancy at birth among 16 provinces in Korea in 2008 and there were considerable health inequalities within the region. This study was performed to build up a priority setting framework in Healthy City Busan project. Methods: Analytic hierarchy process was used to determine the relative priority weight for different strategic and program dimensions along with the consistency of response. An on-site workshop-based meeting (calculating importance) and online survey (calculating risk) were conducted to obtain data from 8 experts. Results: The results showed that in strategic criteria "active health promotion & diseases prevention" and "building infrastructure for the Health City project" were two most important factors. In program criteria, considering both importance and risk scores, "making a healthy community" and "building community health centers" in disadvantaged areas were a top priority group. In addition, "enacting an ordinance for the Healthy City", "building the infrastructure for health impact assessment" and "making health care safety net for vulnerable population" were also higher priorities group. Conclusions: Our findings suggest that the Healthy City project in Busan should be focused on strengthening health equity and building infrastructure for sustainability of the project.
A liver fluke, Opisthorchis viverrrini (OV), is the major cause of the high incidence of cholangiocarcinoma in North-eastern Thailand. The prevalence of OV infection remains high in various parts of the country, especially in wetland rural areas where a large proportion of the community work in agriculture and continue the traditional practice of eating raw or uncooked cyprinoid fish products. The national control program seems to have had little impact in many of these areas, and it has been difficult to make precise assessments of the overall effectiveness of the program. Therefore there is a need for a community-based approach to prevent infection with the parasite, ideally involving as many players as possible. Here we document an attempt to assess the best means to prevention on the basis of a community intervention in three villages in north-east Thailand, with participation of representatives of Health Promotion Hospitals of the Ministry of Public Health with dedicated staff, but also school teachers, independent government sponsored village health volunteers, and housewives responsible for cooking and diet selection. An action plan was followed, allowing detailed discussions of practical proposals, their introduction and then repeated reflection and further proposals at the individual village level.
The purposes of this research were to understand the realities of health management and to analyze the subjective, functional, psychological, and social health effects of the application of an exercise and education program to manage health on an elderly population residing in an agricultural community. The specific purposes of this research were: 1. to determine the subjective, functional, psychological health conditions, and performance level of instrumental activities of daily living of both the experimental and control groups before introducing a health management program, and then 2. to compare these with both the experimental and control groups' subjective, functional, psychological health condition and performance level of instrumental activities of daily living after applying the health management program. Objective : This study was conducted to development of public health promotion programs for menopausal women using Traditional Korean Medicine (TKM). Method : 4 domestic data bases were used to derive TKM intervention that could improve women's menopausal symptoms. Based on this, the program was organized through consultation with experts from the academic community. Results : Program was composed of 3 categories, lecture, therapy and practical exercise. The lecture was made with 'Understanding of menopause', 'Understanding of meditation', and 'Dietary therapy on menopause'. In therapy, 少府(HT8), 合谷(LI4), 內關(PC6), 三陰交(SP6) and 加味逍遙散(Gamisoyo-san) were chosen by expert discussion. Meditation, aromatherapy, walking and palate test were selected for practical exercise. Conclusion : Programs could be used to improve women's health in menopause.

Purpose: Women are more vulnerable to post-traumatic stress (PTS) than men, causing several health problems. Nurses should understand and work with women who have experienced trauma and provide interventions to promote their physical, social, and mental health. Methods: This quasi-experimental pilot study used a one-group pre-test/post-test design. Data were collected from 14 women recruited between December 2019 and May 2020 from a self-sufficiency support center in South Korea for sexually-exploited women who had experienced trauma. The program consisted of six one-on-one intervention sessions per week for six weeks. Each session averaged 60~120 minutes. Participants were assessed at pre-test, post-test, and one-month follow-up. Changes in outcome variables over time were analyzed using the Wilcoxon signed-rank and Friedman tests. Results: The caring program for health promotion was divided into six sessions: understanding the self, sharing traumatic events and negative emotions, reframing the meaning of traumatic events, identifying thoughts and physical and emotional responses, developing health promotion activities, and maintaining a positive attitude during the process of change. As a result of the caring program, PTS (F = 36.33, p < .001), depression (F = 24.45, p < .001), health-promoting behaviors (F = 7.06, p = .004), and self-esteem (F = 19.74, p < .001) among the participants differed significantly at pre-test, post-test, and follow-up. Conclusion: This study provides foundational information for the implementation of a theory-driven program by nurses in clinical and community settings to provide comprehensive care for women who have experienced trauma.
Forming health care management model in small-scale enterprises was the purpose of this study. For the purpose, we tried to investigate the characteristics of small-scale enterprises and analyzed the pattern of their health care management. The results are as follow: 1. The strength of health managing agency and technical supporting program lies in team approach by specialized manpower. However, if the liaison between each part of the organization is not smooth, the overall management will be very difficult. 2. Small scale enterprises are characterized by their short life after the establishment, use of rental building, lack of welfare facilities, weakness in sanitary management and aggregation of factories of similar type of industry. Because of these characteristics, it is very difficult to solve problem basically, such as improvement of working environment. Therefore, it is important to focus on health education and community based approach. 3. Many workers in small-scale factories are in middle and old age. They have health problems mainly related to personal habits. Implementation of an appropriate health promotion program is needed. 4. The number of workplaces, which should be managed by health managing agent. is increasing rapidly. But the number of health managing agent is limited. In the aspect of the requirement of manpower and equipment, training personal agent is more urgent than founding institutional agent. 5. The uniform method of health management hampers the choice of employer and workers. The types of provision of health management should be diversified. 6. For an efficient management, a frequent visit of personal agent and the following referral to a specialist should be done. The specialists in charge of secondary management are from the field of occupational medicine, occupational hygiene, ergonomics, etc. 7. The health management of small-scale facilities should have six components. They are community-based approach, multi-disciplinary cooperative system, program based on the need of recipient, forming partnership of employer and worker, change of lifestyle, and evidence-based program.
Minority populations in the United States have a higher prevalence of non-insulin-dependent diabetes mellitus (NIDDM) and more persons die of the disease than white persons. This study was to review and compare risk factors and prevalence rates of NIDDM in African Americans, Hispanic s, Korean Americans and Native Americans in the United States. The risk factors of NIDDM, including family history of diabetes, obesity, physical inactivity, diet and age, were reviewed in the minority populations. Risk factors such as obesity, physical inactivity and family history of diabetes occurred to a greater extent in some minority populations than in the white population. Diabetes should be treated as a public health problem for minority populations. Due to the increase of older populations and the increased prevalence of obesity and sedentariness, NIDDM in minorities is nearing epidemic proportions. Good diet and regular exercise can reduce the incidence of NIDDM but an understanding of the cultural aspects of diabetes is imperative in order to provide adequate community health education programs because those programs involve diet and behavior changes, characteristics that are often culturally determined. In summary, it is important to plan a community health education program targeted on NIDDM in a culturally adapted manner that will be received with both comprehension and acceptability. In particular, the program for high-risk populations should be stressed so to prevent diabetes. Preventive approaches to diabetes should be considered because they can be both therapeutic and cost effective.
