농어촌 개발과 의료보장 개혁에 따른 보건진료원의 책임 (The role and responsibility of community health practitioner based on the rural community development and the reform of health care system)

  • This study was conducted by community survey of 215 community health practitiner's posts and literature review of official documents. The result was as follows ; 1. The role and responsibility of community health practitioner's post must be studied by the community health practitioner and the community health leader, and on the basement of community health needs, community development plan, and reforom of health care system. 2. Comprehensive health care of community is very important role and responsibility of community health practitioners. However, it was supervised by the senior community health practitioner in provincial government. 3. The community health practitioner must be trained by formal inservice educational program focused on comprehensive health care. 4. The community health practitoner must be the health guider and health leader as the member of community.

전남 도서지역 보건진료원 업무환경 개선방안 (Improvement for Work Status of Community Health Practitioners in Island Areas)

  • Purpose: The purpose of this study was to assess the work status of community health practitioners in island areas. Methods: Participants in this study were 59 community health practitioners in primary health care posts located on islands in J Province. Data were collected during December, 2011 and analyzed using SPSS 17.0 program. Results: Characteristics of the majority of community health practitioners in the island areas was that they were over 50 years old, married, and had 10 years job experience, The population under their jurisdiction was less than 200 and they had an average of 6-10 patients/day and worked less than 10 hours/day. Health education and practice were the most common of task, and they had various types of paperwork to complete. They visited senior citizens living alone, managed medications, gathered health data for research, waited for night treatment of emergency patients, carried out health promotion programs, and visited seriously ill patients at home after work hours. Conclusion: Results of the study indicate that in order to improve the role and function of the primary health care posts and to boost morale, the practitioners' opinions should be actively accepted by the community health practitioner association and competent government agencies.

보건진료원의 업무 및 보건진료소 운영에 관한 고찰 (Duties of Nurse Practitioners in the Community and Management of Primary Health Care Posts)

  • By the rural area health care special law in 1980, Primary health care posts were established in rural areas as fundamental elements of the national health system. Nurses have been deployed to the posts after taking an education course mandated by the special law. However, health care posts have confronted environmental changes over the past 30 years such as an aging and decreasing rural population and advanced traffic systems, which make it necessary to reshape their form and role. Therefore, some guidelines are suggested for future role enlargement of health care posts by analyzing their current management and duties. The guidelines are as follows: 1) enlarging the portion of prevention and management of chronic degenerative diseases, 2) development and practice of diverse health promotion programs, 3) extension of primary health care for the increasing older population, 4) development of health programs for married immigrants, 5) practice of timely maternal child health programs, 6) development of adequate health care posts for low-income people in rapidly urbanizing rural areas and in poor areas in big cities, and 7) revision of laws and institutional arrangements for the role enlargement of health care posts to match social changes and customer needs.

보건진료소 고혈압 관리사업의 실태 (A Study on Hypertension Management of Community Health Practitioner Posts)

  • 본 연구는 농촌지역의 고혈압 관리실태를 파악하기 위하여 전국의 보건진료소 1,849개소를 모집단으로 하여 비례층화표출법에 의해 700개소의 보건진료소를 선정하여 자가보고식 설문지를 2003년 3월 13일부터 5월 13일까지 두 달간 실시하여 수집된 205개의 설문지를 분석하여 다음과 같은 결론을 얻었다. 첫째, 보건진료원의 평균 연령은 42세였고, 근무경력은 11년에서 20년 사이가 62%를 차지하였고 20년 이상의 근무경력자도 17.2%로 나타났다. 교육수준은 간호대 3년제 졸업이 61.3%이고 대학원이상이 7.0%였다. 둘째, 조사대상지역의 생업별 지리적 특성은 농촌인 곳이 77.3%, 어촌이 14.7%이고, 행정적 관할인구는 501-1,000명이 44.3%였고, 실제적 관할인구는 501-1,000명이 53.3%였다. 65세 이상인구의 비율은 평균 26.7%이고, 담당하고 있는 리는 평균 4개이고, 마을건강원의 수는 평균 6명인 것으로 응답하였다. 셋째, 기타 고혈압 사업과 관련된 특성으로는 월평균 혈압측정자 비율은 평균 25.4%, 월평균 방문가구 중 고혈압 관리를 위해 방문하는 가구 비율은 평균 42.4%, 월평균 내소자 중에서 혈압으로 인해 보건진료소를 내소하는 비율은 평균 15.6%였다. 고혈압 사업을 하는데 있어서 운영협의회의 역할이 있다고 응답한 곳은 15.7%였고, 사업을 수행하는데 마을건강원이 도움된다고 응답한 곳이 52.7%이고 보건진료원 자신이 고혈압 사업을 수행하는데 능력이 충분하다고 응답한 곳이 76.2%였다. 보건지소와의 협조는 잘 된다고 한 곳이 43.4%, 안 된다고 한 곳이 56.7%인 것으로 나타났다. 넷째, 일반주민을 대상으로 한 고혈압환자 예방사업 중에서 보건교육과 관련된 항목 중 보건교육내용선정과 교육시간선정은 보건진료원의 판단 하에 결정하는 경우가 50% 이상이었고, 1년 동안 4회 이상의 보건교육을 실시하는 곳이 56.7%이며, 교육방법은 강의와 시청각 매체를 이용하고 주로 사용하는 시청각 매체는 비디오라고 83.6%가 응답하였다. 교육자료는 보건소에서 배부 받아 사용하는 경우가 64.5%였고 교육 후에 평가를 실시하는 곳은 22.1%였다. 고혈압환자 조기발견은 96.1%가 보건진료소를 내소하는 경우에 혈압측정을 통해 발견하였고, 89.3%는 가정방문을 통해 환자를 발견, 49.1%는 지역사회 보건의료기관 및 일반자원으로부터 의뢰를 통해 환자를 발견하였다. 그 외에 집단 행사를 통해 환자를 발견하는 경우는 39.5%이고, 기존 자료를 통해 환자를 발견하는 경우는 35.7%였다. 다섯째, 고혈압환자를 대상으로 한 고혈압환자 등록 및 관리사업에서 고혈압 환자만을 위해 전용기록지를 사용하는 곳이 35.6%이고, 50% 이상이 타의료기관에서 관리받는 환자까지 보건진료소에 등록해서 관리하고 있었다. 고혈압관리수첩은 사용하지 않는 경우가 34.8%였고 고혈압환자 관리를 위해 전산프로그램을 사용하는 경우는 68.5%이고, 사용용도는 월보작성이나 환자치료 및 추구관리를 위해 사용한다고 60%이상이 응답하였고 10.6%는미치료자를색출하는데사용한다고응답하였다. 여섯째, 모든 의뢰환자에게 의뢰서를 발급하는 경우가 22%이고, 반드시 회신서를 확인해서 환자기록지에 보관하는 경우가 26.2%였다. 그외에 민간의료기관과의 공식적, 비공식적인 의뢰체계를 구축한 곳이 64.7%였다. 고혈압환자 치료 및 추구관리에서 투약관리와 혈압 상태관리는 95% 이상이 수행하고 있었으나 생활요법 중에서 스트레스나 비만관리, 식이관리는 약 7%정도가 전혀 확인을 안 하고 있다고 응답하였다. 또한 작년 한해 동안 고혈압환자 및 가족을 대상으로 교육을 실시한 경우가 약 30%였고, 고혈압 환자만을 대상으로 교육을 실시한 곳은 70.7%였다.

강원도 보건진료소 고혈압 환자의 원격관리시스템 적용 효과 (The Effect of the Telemedicine Service System Application for the Patients with Hypertension at Community Health Practitioner Posts in Gangwon Province)

  • Objectives: This study is to identify the effects of utilizing a telemedicine service system on patients with hypertension at home in rural areas. Methods: The study was designed to be a retrospective case-control study. The subjects of this study were 152patients with hypertension who were managed by community health practitioner posts; using telemedicine service system group (n=76), usual care group (n=76). The data was collected through EMR (electric medical record) from September to December, 2013, and analyzed by descriptive statistics of $x^2$/t-test and ANCOVA. Results: The analysis showed that telemedicine service system was an effective way to deal with body weight (F=4.723, p=.031) and BMI (F=5.225, p=.024). Conclusions: This study provides evidence for utilizing the telemedicine service system based on information technology as intervention method in the hypertension management.

보건진료소 정보시스템을 이용한 보건진료원의 업무 분석 (Service Analysis of Community Health Nurse Practitioner using Information System)

  • The purpose of this study was to analyze the activities of Community Health Nurse Practitionses using the Community Health Post Information System(CHPIS). The information system that have been introduced in 1994 and used by 400 Community Health Posts(CHPs) since 1997, which is about $20\%$ of the total CHPs nationwide. Twenty-five CHPs from two provinces participated in the analysis. Seventy-two percent of the CHPs among the participating CHPs started using the system since 1996. The degree of utilization of the information system was classified into three groups (i. e., high. medium, and low). The results revealed that only $48\%$ utilized the system with high level. The areas of analysis of the information system included characteristics of community residents, environmental attributes, and job analysis of Community Health Nurse Practitioners(CHNPs). The study results indicated that primary health care and drug demand and supply system showed the highest level of satisfaction in utilizing the information system by CHNPs.

일 지역 보건진료소 건강증진사업 실태와 관련요인 분석 (Health Promotion Programs in Primary Health Care Posts in Rural Areas and Factors Influencing Service)

  • Purpose: The purpose of this study was to identify health promotion services in rural areas and factors influencing this service. Method: From March to April, 2007, a structured questionnaire on services in 2006 was used to collect data from community health practitioners in all of the Primary Health Care Posts (PHCP) in North Chungchong Province. Collected data were analyzed using SPSS 12.0 Win program. Results: The most frequently offered programs were health gymnastics, walking exercise, bathing and vaccination service, and hypertension management. The main obstacles to these health services were lack of adequate space, insufficient budget, and overwork. The most frequently offered health education programs were education on hypertension, exercise, diabetes, volunteer work, and smoking cessation. The main obstacles to health education were lack of adequate space, insufficient education materials and equipment, and lack of cooperation from the citizens. Improvement and reinforcement of health promotion programs should include support of specialist, development of appropriate methods of service delivery, and education materials, and increase ease in using community resources. Conclusions: The research results show that a new model of health promotion must be developed for efficient health promotion programs in rural PHCP.

보건진료소 보건사업 현황 분석 (Analysis of the Results of Health Service Programs in the Primary Health Care Posts)

  • Purpose: To analyze health programs of the PHCP (Primary Health Care Posts) Method: From August 2006 to July 2007, data on the general quality and health program of the PHCP was requested by official letter and replies were received via E-mail. From December 8 to December 30, 2007, data from 1,268 (66.8%) PHCP out of 1,897 PHCP were analyzed using SPSS 12.0 Win program. Results: The average population covered by each PHCP is 878.3 people. For the health and special programs, Community Health Practitioners report high motivation for programs on health promotion, management of chronic illness, social welfare (40-50%). Demand by the residents was reported at 10% and increases in the health of the residents were attributed to high interest and demand. Volunteer work was 83.3% for bathing, 54.5% for equipment support and 46% for exercise programs. As elders make up 30% of the population in rural areas, there is an increasing demand for volunteer work in bathing programs. Conclusions: As the number of elders in the population increases and there is an increased need for more medical treatment for older people who are sick, the role of PHCP must be strengthened to include visits to homes of community residents. Where financial support for the PHCP is difficult, it is necessary to develop sound data on demographic characteristics of the population in order to develop efficient and effective health promotion programs. The finding that 54.7% of the population need management of chronic illness has difficulty in seeing a physician indicates a need to enhance the health care delivery system by strengthening the role of the Community Health Practitioners and including them in the civil service system to ensure stability of the PHCP.

Evaluation of a Community Health Practitioner Self-care Program for Rural Korean Patients with Osteoarthritis

  • Purpose: The purpose of this study was to evaluate a self-care program for elders with osteoarthritis managed by primary health care workers, Community Health Practitioners (CHPs), in rural Korea. Methods: The self-care program, consisting of 7 areas, was evaluated with a randomized experimental study for patients over age 60 with osteoarthritis in which 150 participants in the experimental group and 140 in the control group were compared. The self-care program was implemented for six weeks, 2 hours per week, at community health posts by CHPs. Data were collected using an interview questionnaire given by the CHPs and laboratory tests before and after the intervention for both groups. Propensity score matching analysis was done to test effectiveness after controlling for confounding variables. Results: The intervention group showed a significant decrease in the number of painful joints (p<.001) and a significant increase in self-care ability (p<.05) compared to the control group. Conclusion: Study results indicate that training and utilizing primary health care workers in rural areas is valuable in increasing the generalization and continuity of intervention programs. As arthritis should be managed life-long, CHP directed self-care programs are useful interventions for rural elders with arthritis to learn self-care management.

노인건강문제와 간호의 방향 (Geriatric Health Problems and Directions for Nursing Improvements)

  • The elderly population will increase from 5.8% in 1996 to 12.5% in 2020. The related problems of health will also become a very important issue in the future. Therefore it is important to address the problems of geriatric nursing and geriatric health. 87.6% of the aged were ill with chronic degenerative diseases in 1994. In 1995, hospital admission rates (86.8) for those aged 60 or older were higher than that (56.3) of the total age group. Such high medical utilization will increase national health costs. For the development of geriatric nursing, active nursing intervention in various settings combined with education and research should be developed. Considering the health and welfare of the aged and the present status and views in Korea, I suggest the following: First, the health needs of the elderly in the institution, must be met by a plan that fosters geriatric nurses and programmed service development. Second, health service for the residential elderly must be provided in day care centers, short stays, nursing homes and geriatric hospitals. Geriatric nursing services should be provided in home residential areas, public health centers, public health subcenters as well as having, community health practitioners in primary health care posts and home health nurses. Third, geriatric nursing curriculum must be developed adjust to situations and culture of Korea and be included in the nursing curriculum. And gerontological nurse practitioner or geriatric specialist must be fosteraged to provide the professional care for the aged. Geriatric nursing research should be also achived for geriatric nursing improvements.
