사업장 보건관리자 공동채용모형개발(II)-사업주와 간호사의 의견 비교- (Developing the Joint Employment System of Occupational Health Nurse Practitioners for the Minor Enterprises in Korea (Part 2) - Comparison of Opinions between Employers and Occupational Health Nurses -)

  • This study is to develop the model on the joint employment system of occupational health nurse practitioners for medium or small sized enterprises in Korea based on the comparison opinions between employers and occupational health nurses (OHN). The data were collected by questionnaires from Oct. to Nov. in 1997. The number of subjects was 210 OHNs and 176 employers, response rate was 47.1% for OHNs and 23.2% for employers. The SAS PC program was used for the descriptive statistics. The results were as follow: 1. More than three years career was preferred for qualification of OHN by employers and OHNs. OHNs preferred that all instruments and materials should be equipped in each industry, but employers preferred that they should be equipped by OHN. 2. For the limit number of industries and employees per one OHN, employers preferred average 2.25 industries and 307 employees, OHNs preferred average 3.0 industries and 448 employees. Both of them preferred two times visit per week for all day long. 3. OHNs wanted that the local office of Ministry of Labor could act as mediator, but employers preferred direct contact individually. For the average wage, employers preferred 887,000 won per month, but OHNs preferred 1,960,000 won per month. 4. OHNs preferred the Korean Association of Occupational Health Nurse as the recruit agency. Finally, employers and OHNs have the opinions that they can choose this system depending on the condition. There is in need of development of effective strategy for this new system such as manpower development, cost-benefit analysis, establishment of occupational health service standard, advertisement for employers, and government supported project.

노인의 건강상태, 여가 만족 및 외로움이 생활만족도에 미치는 영향 (Effects of perceived health status, leisure satisfaction and loneliness on life satisfaction for elderly people)

  • Purpose: This study was done to identify the relationship of perceived health status, leisure satisfaction, loneliness and life satisfaction and to identify predictors of life satisfaction for elders. Method: The research design for this study was a descriptive survey design using a convenience sample. Data collection was done using self-report questionnaires with 297 elders from 6 community health practitioner posts. Data were analyzed using ANOVA, t-test, Pearson correlation coefficients and hierarchical multiple regression with the SPSS Win 18.0 Program. Results: Life satisfaction had significant negative correlations with age (r=-.31, p<.001) and loneliness (r=-.73, p<.001), and positive correlations with monthly spending money (r=.44, p<.001), health status (r=.63, p<.001), and leisure satisfaction (r=.62, p<.001). The explained variance for life satisfaction was 72%. Variables of monthly spending money, health status, leisure satisfaction, and loneliness significantly predicted the degree on life satisfaction of these elders. Conclusion: Findings of this study provide a comprehensive understanding of life satisfaction and related factors for elders in Korea. However, further studies with a larger random sample and more detailed research design are necessary.

보건간호사의 의사결정 유형과 지식 유형에 관한 실증연구 (Public Health Nurses유 Decision Making Models and Their Knowledge Structure)

  • The purpose of this study was to describe decision making model of 180 public health nurses in Korea and their knowledge structure for decision making. The differences of decision making models by nurse's knowledge structure were also tested. Research concepts were measured using the instrument based on systemic and interpretive decision making approaches that were developed by Lauri & Salantera (1995). The results were as follows. 1. The public health nurses turned to, most commonly, a mixed practical-theoretical knowledge structure (45.9%), followed by practical knowledge (32%) and theoretical knowledge (22.1%). 2. The six different decision making models were identified. These were named for decision making theories and nursing process. These were client-oriented decision making, rule-oriented systemic decision making, wholistic and intuitive decision making, decision making depending on subjective view and experience, systemic decision making for defining problems. 3. The public nurses who had practical and practical-theoretical knowledge structure and community health practitioner (CHP) retold that decision making depends on subjective view and experience. Also the public health nurses who had 5~19 years clinical experience represented hypothetico-deductive decision making for defining problems.

노인복지관 간호사를 위한 간호사정조사도구 개발 및 적용 (Development and Application of a Nursing Assessment Tool for Senior Center Nurses)

  • Purpose: The purposes of this study were to develop a nursing assessment tool for senior center nurses, and to test its feasibility and content validity. Methods: The study utilized a psychometric test design. Preliminary items were developed based on geriatric health needs and Gordon's 11 domains of functioning health. Initially, the tool was evaluated for content validity and feasibility. Then, it was administered among 195 older adults in a senior center by a gerontological nurse practitioner. Data were analyzed to describe the nursing problems of the senior center older adults. Results: The final version of the nursing assessment tool consisted of 27 items. The internal consistency, measured with the Cronbach's ${\alpha}$, was .74. The result of the assessment showed that senior center older adults had high nursing needs in the area of management of chronic diseases (hypertension, diabetes, and lipids), dental care, community relations, safety, elder abuse, health behaviors (such as drinking), mental health (depression, suicide, and cognitive function), and health consultations. Conclusion: The new tool was feasible for use with senior center participants, and it was evaluated as having high content validity by senior center nurses.

보건진료전담공무원의 전문직업성에 미치는 영향요인 (Influential Factors for Professionalism of Community Health Practitioners(CHPs))

  • 본 연구는 보건진료전담공무원의 개인 심성, 직무연속성, 직무만족 및 전문직업성의 관계를 파악하고 나아가 전문직업성에 미치는 요인을 규명하기 위한 서술적 조사연구이다. 본 연구는 2017년 8월 20일부터 9월 25일까지 G도에 소재하고 있는 보건진료소에 근무하는 보건진료전담공무원을 대상으로 하였으며, 153부를 최종 분석에 사용하였다. 수집된 자료는 IBM SPSS Statistics 20.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Scheffe test, Pearsons's correlation coefficients, stepwise multiple regression을 통해 분석하였다. 연구결과 대상자의 일반적 특성에 따른 전문직업성 정도의 차이는 월소득(F=6.951, p<.001), 근무경력(F=5.245, p=.002), 보건진료전담공무원 선택동기(F=3.676, p=.004)에서 통계적으로 유의한 차이를 나타냈다. 주요 변수 간의 관계에서 전문직업성은 개인 심성(r=.573, p<.01), 직무연속성(r=.496, p<.01) 및 직무만족(r=.539, p<.01)과 유의한 정적 상관관계를 나타내었다. 일반적 특성, 개인 심성, 직무연속성, 직무만족이 전문직업성에 미치는 영향을 파악하기 위해 다중회귀분석을 실시한 결과, 보건진료전담공무원의 전문직업성과 가장 높은 관련요인은 직무만족(${\beta}=.320$, p<.001), 개인 심성(${\beta}=.291$, p<.001), 월 소득(${\beta}=.283$, p<.001) 및 직무연속성(${\beta}=.176$, p=.009) 순으로 나타났고, 이들 변인들의 총설명력은 49.6%이었다. 본 연구결과를 바탕으로 보건진료전담공무원의 전문직업성을 강화시킬 수 있는 개인차원과 조직차원에서의 융복합적 프로그램 개발과 정책적 지원이 바람직하다고 본다.

보건진료소에 등록된 고혈압 환자의 순응도 연구 (A Study on Compliance of Hypertensive Patients Registered at Community Health Practitioner Post)

  • 본 연구는 고혈합 진단을 받고 일개 도농복합 2개 지역 12개 보건진료소에 등록된 고혈압 환자 중 연구에 동의한 304명을 대상으로 2004년 6월 21일 ~ 7월 16일까지 4주간에 걸쳐 고혈압 환자들의 약물치료 및 생활요법에 대한 순응정도와 이에 미치는 요인을 파악하고자 실시되었다. 순응군은 약물요법을 꾸준히 하면서 생활요법 측정 점수가 9점 이상인 군으로 하였고, 약물요법은 꾸준히 하지 않거나 생활요법 점수가 9점 미만인 군을 비순응군으로 분류 하였고, 일반적인 특성, 건강신념모델변수, 고혈압에 대한 지식, 자기효능감 등을 조사 하였다. 또한 남자와 여자의 특성이 다를 것으로 생각하고 남녀 각각에 대해 순응군과 비순응군으로 분류하여 통계적인 분석을 실시하였으며, 주요 결과를 요약하면 다음과 같다. 1. 규칙적으로 약물치료를 받는다고 응답한 경우 남자(90.3%), 여자(93.3%), 생활요법을 실행 한다고 응답한 경우 남자(452%), 여자(56.4%)로 나타났다. 2. 남자는 교육수준이 높을수록 고혈압에 대한 순응군이 높았으며 통계적으로 유의하였다(p<0.05). 3. 남녀순응군에 있어서 자기효능감 점수가 통계적으로 유의하게 높았다(p<0.05). 여자순응군에 있어서 고혈압에 대한 지식 점수가 유의 하게 높았다(p<0.05). 4. 건강신념모델 변수와 관련해서는 남자에서는 감수성과 유익성, 여자에서는 심각성, 유익성, 장애도가 유의한 변수였다(p<0.05). 5. 순응군과 비순응군을 종속변수로 한 다중로지스틱 회귀분석 결과 남자에 있어서는 교육수준, 자기효능감이 여자에 있어서는 고혈압에 대한 지식, 자기효능감, 유익성이 통계적으로 유의한 변수로 나타났다(p<0.05). 고혈압 환자의 관리에 있어 약물치료 못지않게 중요한 것은 생활습관의 개선이다. 보건진료소에 등록된 고혈압 환자의 경우 대부분 규칙적인 약물치료를 받고 있어, 보건진료원은 이에 대한 정기적인 확인과 더불어 잘못된 생활습관을 고칠 수 있도록 하고 효과적인 보건교육을 통해 고혈압에 대한 올바른 지식을 습득하도록 하여야겠다. 또한 환자들을 대상으로 자기효능감을 높일 수 있는 방안을 모색하여 생활습관 개선을 통한 합병증 예방 등 순응에 따른 유익성에 대해 보다 적극적으로 인지시켜야겠다.

보건간호사 인력수요 추계에 관한 연구 (A Study on the Projected Workforce for Public Health Nurses in Korea)

  • Purpose: Recently there have been many changes in health care environments in Korea. To perform public health programs effectively and efficiently, it is necessary to analyze and identify the demand and supply for the public health nurses. Method: The study analyzed experts' opinions regarding the supply of public health nurses, as well as national and foreign statistical data on workforce supply of public health nurses. Two methods for estimating the amount of demand for public health nurses were used: one was applying the indicators of developed countries for public health nurses based on population: the other was to refer to regulations and/or recommended guidelines in Korea. Result: 1) The number of public health centers, public health sub-centers, and primary health care posts have decreased between 1990 and 2001, from 260 to 242, 1318 to 1270, 2038 to 1907, respectively. 2) Between 1997 and 2002, the number of public health nurses has also decreased from 5572 to 5112. 3) In the case of applying regulations, the number (5112) of existing public health nurses falls shortly by 942. 4) In 2001, the Korean population per one public health nurse was 9262. 5) In the case of applying regulations, the number of public health nurses required to meet the demand for health services in 2001 and 2020 is estimated at 5932 and 6347, respectively. 6) In the case of applying the indicators of developed countries, the number of public health nurses required to meet the demand for health service in 2001 and 2020 is estimated at 9.469 and 10.310, respectively. Conclusion and suggestions: Because of the importance of public health industry, public health nurses have been approved as a field specialist and specialized nurse practitioner by the newly revised legal regulation, there have been absence of approval of their role differentiation and capability. In addition, organizational activity and insufficient number of the public health nurses have contributed to the inactive utilization of them. As community public health is focused on caring individuals as well as organizations, it requires more autonomy and special skills than other fields. Therefore, public health nurses need to enhance the capability as health educator, consultant, and information management persons through advanced education course for public health nurses. Public health nurses need to be prepared as advanced nurse practitioners by receiving advanced education courses and field experiences.

농촌지역사회 보건요원의 교육을 통한 주민의 보건복지향상에 관한 사회의학적 연구 (Socio-Medical Approach to the Welfare of Rural Residents Through the Education of Community Health Personnel)

  • In this county, the gap between the urban 'haves' and the rural 'have-nots' continues to be an increasing problem. WHO and UNICEF see primary health care(PHC) as the key to achieving an acceptable level of health throughout the world as a community development. PHC is essential health care made accessible to individuals and families in the community by means acceptable to them. It is the first level of contact of individual, the family, and community with the national health system. It includes at least education on health system. It includes at least education on health problems, promotion of food supply, MCH including family planning, immunization against infectious diseases, control of endemic diseases, treatment of common diseases and injuries, promotion of mental health, and provision of essential drugs. However, of the aboves, education concerning of mental health problems and the methods to identify, prevent, and control them is the principal step of establishment. In Korea, the category of PHC worker includes the physician as public doctor and nurse as primary health care practitioner and community health leader as village health worker. PHC workers of the aboves will thus function best if they are appropriately trained to respond to the health needs of the community. However in this country, since the national PHC service project launched in 1980, the government has not developed and performed appropriate and enough education and training activities. In light of above reasons, several categories of health education activities had been planned and performed being aimed at above specific target groups and the main focus was on the village health workers for about one year from July 1991 to July 1992 in Yeoju Kun of Kyonki Province. At the end of the period, evaluation of education input was carried out to measure the improvement of healthful life of people in terms of awareness, attitude, and practice. The totals of 80 village health workers, 13 public health practitioners and 9 public docters took in the course of health education for a few hours at every month and the evaluation works of educational effect were taken. The results the study were as follows. 1) Number of persons who realized the maxim "health care of the people is a duty of the government" increased after the education course, On the other hand, the rate of satisfaction on the effort of government for health promotion of the people decreased. 2) Public doctors and primary health care practitioners(nurses) liked and enjoyed the education schedule as a meeting of peer group. It provided chances of communication with staffs of Korea University Hospital. It was said that lectures covered great deal of knowledge and technic they urgently needed in the field. 3) After finishing the education course, more of village health workers(VHW) thought they adapted themselves to their roles and functions showing increased number of home visit and contact with primary health care practitioners by month. 4) In case of patient refer, VHW preferred primary health care practitioners to public doctors. 5) Capability of VHWs in most of their functions increased dramatically after when the education course finished except tuberculosis control.

농어촌 보건지소 진료실적과 관련 요인 (Factors Related to the Medical Service Performance of Rural Health Sub-centers)

  • 본 연구는 농어촌 보건지소에서 수행하는 진료실적에 영향을 미치는 관련 요인을 파악하여 향후 보건지소 기능을 활성화할 수 있는 방안을 마련하고자 하였다. 연구의 분석단위는 읍 면 지역에 위치한 보건지소이며, 2009년 12월 31일 현재 설치 운영 중인 1,242개소를 대상으로 하였다. 보건지소간 진료실적의 차이는 지역간 인구의 차이를 보정한 인구당 진료실적을 산출하여 분석하였다. 그 결과 보건지소 진료실적은 관할지역 내 민간 병 의원과 보건진료소가 없는 경우, 보건지소에서 보건소 및 가장 가까운 응급의료기관까지 거리가 먼 경우, 65세 이상 인구 비율이 높을수록 유의하게 많았다. 반면 0-4세 인구 비율과 공중보건의사 인턴 수료자 배치 유무는 유의한 영향요인이 되지 못하였다. 이상의 결과를 보면 읍 면에 소재한 보건지소의 진료실적은 관할지역 인구와 보건의료환경 특성에 의해 유의한 영향을 받고 있어 이러한 특성을 바탕으로 하여 차별화된 서비스 제공전략을 마련할 필요가 있겠다.

미국 전문간호사(NP)의 역할과 교육과정에 관한 고찰 (Nurse Practitioner Roles and Curriculums in the United States)

  • Based on literature, status and role of the NP in America was reviewed. The process of developing NP program in America suggests us many things. In America, nurse practitioners have sustained a mutually beneficial status with their patients for over thirty years. Excel fence in academic education and clinical training will enable nurse practitioners to continue to provide quality health care. The magnitude changes in the health care system of the United States, the challange of providing real access of health care continues. Lack of access to adequate primary care was the driving force in the initial 1965 Federal Involvement in developing the NP role. In 1993 President Bill Clinton's health care reform initiative provided policy support for NPs as primary care providers. The Institute of Medicine explicitly recognized NPs as an integral part of the primary care team. In addition, several national reports recognized NPs as affordable, accessible, high-quality care providers. The recent passage of direct Medicare reimbursement for NPs reflected public policy statements coincided with and likely contributed to a growth spurt in the NP workforce. From 1965 to 1977 NP programs offered traditional primary care clinical tracks(adult, family, woman's health, and pediatrics) for relatively small clusters of students in a variety of institutional settings. From 1978 to 1990 these educational programs were incorporated into graduate schools of nursing. By 1990 the majority of NPs received educational preparation in master's-level nursing programs. A new emphases was placed on postmaster's NP programs designed for master's prepared clinical nurse specialists and nurse managers. he the health care system shifted hospital nursing resources toward community-based care, these master's -level nurses sought additional NP preparation. NP educational programs are defined as the educational structure in which one or more NP clinical tracks are offered. NP clinical tracks, in turn, offer curriculum and supervised clinical experiences that match standards in specific practice areas such as family(FNP), adult(AUP), geriatrics(GNP), pediatrics(PNP), women's health (WHNP), neonatal (NNP), and acute care(ACNP). There were indications that NP practice was expanding into new clinical areas as evidenced by new types of tracks, particularly in acute care and psychiatry. The increase in acute care NP students likely reflects the increased demand from hospitals and other acute care settings. In Korea, change of nurse's role into nurse practitioner's role may have many difficulties. The need of health consumer, policy support of government, approval of medical care team are all essential component. Every nursing personnel make effort to planning the new health care delivery system.
