지역사회에서의 가정간호 접근성 제고 방안 - '서울시간호사회' 가정간호사업 분석을 토대로 - (A Study for the Enhancement of Accessibility to Community Home Nursing Care Services - The Home Nursing Care Program of Seoul Nurse Association -)

  • Recently, there has been an increasing need for long-term care and comprehensive health care services in community settings. The Ministry of Health and Welfare introduced the Hospital-Based Home Nursing Care Program in 2000. Before this initiative, there was a Home Nursing Demonstration Center, affiliated with the Seoul Nurse Association, had offered home nursing services with the financial support from the local government. since 1993, the Center's nursing staff has been engaged in a general hospital in an effort to provide home nursing care services within Korea's health care system. The purpose of this study was to analyze and identify characteristics of community-based home nursing care supplied by a community-based home nursing team engaged in a general hospital. Also. visit nursing care services provided by public health centers were evaluated in terms of accessibility and supply versus demand, to enhance the accessibility of low-income patients living in Seoul to home nursing care services. Data were collected from home nursing insurance reimbursement claims submitted by the community-based home nursing care team from March 1 to October 30 in 2001 and a questionnaire survey on home-visit nursing services of 25 public health centers in Seoul. The subjects consisted of 197 patients and 12 public health centers. The result were as follows. First, medical institution's community-based home nursing care program was better in technical quality than health-center-based home-visit nursing care. In addition. the pattern of the subject patients was similar to that of hospital-based home nursing care program. Second, there was a high demand for community-based home nursing care while only a small number of home-visiting nurses served at public health centers in Seoul. As a result, many patients could not receive adequate care. Finally, we suggest that community-based home nursing care program should be introduced in the national health system to meet the at-home care needs of severely ill low-income patients. Furthermore, to better utilize home nursing and visit-nursing care resources and offer continued care for patients in community settings, an efficient referral network should be built among related institutions. This would require improvement of reimbursement system and amendment of the law related to health insurance system and community-based home nursing care services.

재가와상노인 서비스제공 모델 개발을 위한 연구 (Development of an Urban Community Based Comprehensive Service Model for Bed-Ridden Elderly)

  • Purpose: This study aims to identify the ADL and IADL of bed-ridden elderly. Also it is used to show fuctional status, and to investigate the content and the level of nursing services provided. Method: The subjects were 191 elderly who received visiting nurse service through public health centers in the Seoul Metopolitan and Chungnam Province. Data collection was conducted by public health center nurses during four months in 2000. Result: As for daily living activities, 100% of subjects had at least one difficulty in ADL and IADL. Among them, only 0.5% had moderate disabilities and 99.5% had severe disabilities by HFS, 27.9% were in a semi bed-ridden state and 72.1% were completely bed-ridden by JABC. The major service provided was a visiting nurse service which was preferable to the social welfare service. In the visiting nurse service, there was no significant difference according to the elderlys' functional status. In addition there was no standadization about the qualification of the visiting nurse, and single entry point for the nursing service. Conclusion: The researchers urgently suggest that a community based comprehensive service model has to be developed to respond to the needs of the elderly in Korea.

전남 도서지역 보건진료원 업무환경 개선방안 (Improvement for Work Status of Community Health Practitioners in Island Areas)

  • Purpose: The purpose of this study was to assess the work status of community health practitioners in island areas. Methods: Participants in this study were 59 community health practitioners in primary health care posts located on islands in J Province. Data were collected during December, 2011 and analyzed using SPSS 17.0 program. Results: Characteristics of the majority of community health practitioners in the island areas was that they were over 50 years old, married, and had 10 years job experience, The population under their jurisdiction was less than 200 and they had an average of 6-10 patients/day and worked less than 10 hours/day. Health education and practice were the most common of task, and they had various types of paperwork to complete. They visited senior citizens living alone, managed medications, gathered health data for research, waited for night treatment of emergency patients, carried out health promotion programs, and visited seriously ill patients at home after work hours. Conclusion: Results of the study indicate that in order to improve the role and function of the primary health care posts and to boost morale, the practitioners' opinions should be actively accepted by the community health practitioner association and competent government agencies.

연세간호를 태동 시킨 외국 선교사들 (Foreign Nurse Missionaries Starting Yonsei Nursing School)

  • Purpose: Nursing in Korea was started by foreign nurse missionaries. This study was intended to recognize the foreign nurse missionaries contribution with the development of Yonsei nursing education in Korea. Method: This study used an historical study approach. The study target was foreign nurse missionary contributions during 1895~1917. The data was collected from the existing historical nursing studies and Korean Mission Field. Result: Since A.P. Jacobson, the first nurse missionary from the North Mission branch in America in 1895, there were 16 missionaries sent to Korea. Among them, 14 missionaries worked for Severance Hospital Nurse Training School. 7 missionaries served less than 10 years and 5 missionaries stayed in Korea more than 20 years. E.L. Shields served the longest time with 42 years in Korea. They contributed not only in nursing education and services in the hospital, but also public health in the community. Conclusion: Foreign nurse missionaries were a cornerstone for nursing development in Korea. They contributed by not only starting nursing education, but also giving women freedom into Korean society. The historical research regarding foreign nurse missionaries during early 1900s helped to recognize their effort for Korean nursing and society.
