A Study on Visiting Nursing Service in Primary Health Care Units (일차보건의료조직에서의 방문간호사업 실태조사연구)

  • The visiting nursing service is an essential part of public health. The purpose of this study was to analyze the visiting nursing service in primary health service centers. The data were collected from visiting nurse records in Wonju City Health Center, Myun Health Center and Community Health Subcenters. The period of data collection was from April 6 to July 15, 1998. The major findings were as follows: 1. Characteristics of 36 service providers. 1) Age : Over 40 years old - 66.7% 2) Educational level: The proportion of registered nurses was 47.2% and nurse aids 52.8% 3) Career: The proportion of providers who worked over 11 years in Public Health Service was 7.8% 2. Characteristics of Subjects 1) The major health problems were cerebro vascular attack, hypertension, D. M., arthritis, gastrointestinal problems and psychiatric problems. The prevalence of chronic health problems increased with age, except for mental illness. 2) The prevalence of cancer was 4.3/1000. 3. Contents of Home Visiting Nursing Services. 1) The major service was education and counseling. 2) The other services were Direct Care(ROM exercise, wound care, physical therapy, basic nursing care etc,) as 56.5%, of the work involved Indirect Care(teaching, counseling, emotional support, etc,) 30.3%, medication - 11.7%, and referral to hospitals - 1.5%.

A Study on the Establishment of Clinical Nurse Specialist (우리나라 전문간호사제도 개선방안에 관한 연구)

  • Our medical care system is trying to diversify in order to meet the client's needs, and to adjust to a medical environment which is changing very rapidly. Because current nursing theory and practice focus on holistic care, health care management, education, and research, contrary to the traditional emphasis on only assisting a physician, more autonomy and specialization for the implementation of nursing are required. Considering these trends and actual needs, the category of clinical nurse specialist should be established as soon as possible. In order to develop strategies for implementing this new professional specialty, the authors conducted a field survey and literature review of the current system in Korea. As a result, various obstacles and constraints were discovered as follows : 1) There are few accredited educational programs for the training of CNS's. 2) Several hospitals already have staff designated as clinical nurse specialist (CNS) even though the term CNS is not yet standardized or adopted in nationwide. 3) The role of the CNS is not clearly understood by the medical societies, or even nursing societies. A nurse who works in specific nursing areas such as central supply, kidney dialysis, intensive care, coronary care, etc. for a long time, considers herself /himself a CNS. Based upon the above findings, the following alternatives are recommended. 1) The role of the CNS should be defined according to specified functions and authority : professional autonomy ; counselling and educating patients and their familes, nurses, and even other medical personnel ; research on improvement of nursing ; and management of the nursing environment including medical resources, information, and cases. 2) the qualification of CNS should be attained only by a nurse who has an RN license and clinical experience of more than 3 years in a specific nursing field: passes a qualifying examination; and contributes to the professional development of peers, colleagues, and others. A master's degree should only be optional, because of the insufficient of graduate programs which are well designed for the CNS. 3) The CNS should initially be a head nurse rather than line staff in order to deal with as wide an experience base as possible. 4) The nursing specialty could be divided into two areas such as a clinical field and a community field. The clinical field could then be categorized by the Styles' classification such as diseases and pathogenics, systems, ages, acuity, skills/techniques, and function/role ; the community field could be classified according to work site.

Factors Influencing on Nurse's Image and Career Identity of High-school Students who Participated Job Experience (진로체험 참여 고등학생의 간호사 이미지와 진로 정체감에 미치는 요인)

  • Objectives: This study was conducted in order to examine the correlation between the image of nurse and career identity among high school students participating in a vocational experience program of a nursing department, to find influential factors, and ultimately, to improve the image of nurse and promote the establishment of career identity for the development of nursing career and to obtain strategic information for developing nursing departments' vocational experience programs. Methods: Data for this study were collected using a structured self-report questionnaire from 90 high school students participating in a vocational experience program executed at the Nursing Department of W University in G Province during the period from April 26 to July 19 in 2014. Results: According to the results of data analyses, factors influencing the image of nurse were career identity(${\beta}=0.339$, p=0.002) and satisfaction with the program(${\beta}=0.153$, p=0.043) in order of significance(F=9.51, p<0.001), and factors influencing career identity were the image of nurse(${\beta}=0.301$, p=0.002), satisfaction with the program(${\beta}=0.254$, p=0.010), and gender(${\beta}=0.231$, p=0.015) in order of significance(F=12.06, p<0.001). Conclusions: These results suggest that when high school students participating in a vocational experience program of a nursing department are satisfied with the program, the program is more effective in improving the image of nurse and promoting the establishment of career identity. Based on this study, further research is necessary to get strategic information for developing various vocational experience programs, to provide high school students with vocational experiences related to nursing through the programs, and to examine the effects of such programs.

A Study on the Nursing Profession as Stipulated by Health & Medical Laws of Korea (우리나라 보건의료법령에 명시된 간호에 관한 연구)

  • The purpose of the study is to find out how laws related to the nursing profession can be improved by analyzing the rules and regulations concerning nursing. Furthermore, to help settle legal matters in the process of doing nursing work. The data used for the study are the Health and Medical Act, the Maternal and Child Health Act, the School Health Act, the Special Act for Health and Medical Service in Rural Areas, the Industrial Health & Safety Act and the Notice on Nursing Professional Courses analyzed by age and content. The results of the study are as follows : First, basic nursing practice includes 'nursing care for recuperation and assistance in medical treatment and in special areas including the pre-vention of disease, maintenance of health, control of environment, and other therapeutic activities. It is suggested that the phrase 'assistance in medical treatment' should be eliminated as it limits the basic nursing practice to the assistance of the medical treatment. Second, Article 56 of the Health & Medical Act prescribes a special nurse but it does not prescribe a specific job. Accordingly, the new provison concerning the specific jobs of a special nurse should be added or a job guide should be inseated. Third, it is prescribed that those who have completed the training course after obtaining a license are qualified to be a midwife, a special nurse and a nurse practitioner working in special areas. However, school nurses, occupational health nurses and maternal and health workers are required to obtain a nurse license, but not to take an additional training course. Nurses working in special areas should be legally recognized as nurse specialists. The regulations to control various qualification standards consistently should be established. Fourth, the qualifications and types of nurses by area prescribed by Article 54 of the Health and Medical Act are not consistent with those of special nurses as recognized by affiliated organizations of the Korean Nurse Association and some hospitals. Accordingly, the qualifications and types of special nurses should be adjusted in consideration of special nurses. Fifth, as Article 16, Paragraph 2 of the Health and Medical Act does not prescribe the type and scope of first - aid treatment that nurses can provide, the first-aid treatment of nurses might be considered as an unlicensed practice. The specific regulations regarding these matters should be established. Sixth, the contents of the nursing record, which are prescribed by Article 21 of the Health and Medical Act as a duty, include 1) matters concerning body temperature, pulse, breath and blood pressure 2) matters concerning drug prescription 3) matters concerning input and output 4) matters concerning the treatment and nursing care (Article 17 of the Enforcement Regulations, Health and Medical Act). However, these matters are limited to basic nursing care and assistance in medical treatment. The new recording methods on nursing process are suggested to be adopted legally. Seventh, the prescription right entrusted to nurses which are prescribed by the School Health Act, the Special Act on Health and Medical Service for Rural Areas, and the Industrial Health and Safety Act are not consistent with the rights of nurses as prescribed by the Health and Medical Act. New regulations prescribing the partial right for medical treatment entrusted to nurses in consideration of the restraint of time and place in emergency situations should be established.

A Study on Occupational Care Diver Though Nurse's Experiences in General Hospitals (일부 종합병원 간호사들이 경험한 간병인에 대한 연구)

  • This study attempted to analize nurse's experiences about care giver including nurse's recognition of occupational care giver's existence, the helping part of occupational care giver's activities, problems between occupational care giver and nurse, and correlations between reasons of problems and each variable. The subjects of the study are 218 nurses who are working at general hospitals. The study informations gathered by using structured questionnaire which consisted of 38 items. The reliability of the instrument was cronhach's 0.8310. The collected data was analized by spss program for the statistics of percent, t-test, ANOVA, and Pearson's correlation. The study results were as follows : 1. For the nurse's recognition of occupational care giver's existence 70.2% of nurses(153 nurses) who felt occupaitional care giver's existence was helpful. The result showed 2.20 when transfered to score. 2. Occupational care giver's activities which were helpful to nurses were in order of following : simple serving, personal hygiene, companion of talking, room arrangement, and saftey management. 3. Among 10 items of the problems which nurses having experiences with occupational care givers, 4 items were related with nurses directly. 88.4% of the reasons of the problems were due to lack of occupational care giver's eduction. 4. For the relationships of general characteristics and nurse's reason of problems there was the significant difference in the nurse's status and education. There was the significant difference showed in the group of over status of charge nurse and over college education(T=-2.08, P<0.05, T=-2.13, P<0.01) 5. Correlation between nurse's recognition of occupational care giver's existence and the problems between care giver and the nurse revealed weak relationship (r=0.2002, P<0.01). However, correlation between nurse's recognition of occupational care giver's existence and degree of occupational care giver's helping and also relationship of degree of care giver's helping and problems between these two groups were not statistically significant.

  • Purpose: This study was attempted to provide quality improvement and revitalization of visiting nurse services by Importance-Performance Analysis of persons being provided with the services. Methods: The subjects were 350 people being provided with visiting nurse services from a long term home care institution in the kangwon Province during the period of data collection between Aug. 16 and Sep. 15, 2011. Data analysis was conducted through t-test, ANOVA, and IPA by using the SPSS/WIN 12.0 program. Results: As a result of the Importance-Performance Analysis of visiting nurse services, it turned out that the strength on reliability, assurance, and empathy should be maintained with good work and the aspects of responsiveness appeared to be intensively improved, and the tangibility proved to be subject to improvement. Conclusion: For the activation of visiting nurse services, methods for improving the performance in the intensive care area and management strategy establishment which highlights the advantages of strength maintenance area are needed.

Parish Nursing : A New Challenge for Primary Health Care (지역교회간호(Parish Nursing) - 일차건강간호를 위한 새로운 도약)

  • ursing as a profession is characterized by its holistic, mind-body-spirit approach to the patient. Also, nurses have historically been the leaders in health education and promotion. Parish nursing has a great potential for providing primary preventive health care. services as well as assisting people to access the health care system. While working in the community, parish nurses see the church as the new arena for delivering health care services. The parish nurse program was introduced by Granger Westberg in 1984. The concept of parish nursing is based on several beliefs; health is multidimensional and affects all aspects of an individual-physical, psychological, social, and spiritaul being. Parish nursing is one model in which churches can cooperatively work with health care institutions to address the needs of their parishioners. The role of the parish nurse is emphasized in four basic area: a) health education, b) health counseling, c) referal services, and d) facilitation and organization of support groups within the congregation. The parish nurse programs work chiefly in congregation or commuity where a certain language of faith is ready at hand. This means that the parish nurse works in an ecology of meanings and care which encourages the drawing on the message of God's grace, the practices and habits it encourages. The parish nurse may be involved in the church's health ministries and may work on either paid or volunteer basis; however, one of the most important qualification of the parish nurse is to have the nursing knowledge and skills to practice within the standards of Nursing Practice Act. The completion of standards of practice for professional nurses practicing as parish nurses had been identified as a priority by the HMA Executive Board (1996, HMA). In conclusion, parish nursing promotes health and healing by empowering the faith community, family, or individual to incorporate health and healing practices. There are several preconditions that should proceed to establish the foundation for successful development of the parish nursing program in Korea. First, reciprocal relationship with home health nursing should be considered. Second, correct terms and concepts of parish nursing should be studied and understood. Third, systematic study and investigation should be followed for further development of parish nursing. Fourth, strengths and weaknesses of different models should be studied to develop proper model of parish nursing for Korean situation. Finally, consensus of standardized education program and corporation with various religious communities as well as health institutions should be established. When these preconditions are met, the role of parish nursing as a new program for the promotion of holistic health will be established.
