급성 편두통 치료를 위한 글루탐산 수용체 길항제의 임상적 유효성에 대한 메타분석 (Meta-analysis on the Efficacy of Glutamate Receptor Antagonists for Acute Migraine Treatment)

  • Background: Glutamate is implicated in the pathophysiology of migraine, a common neurological disorder. Therefore, glutamate receptor antagonists (GluRAs) have been suggested as a novel migraine treatment that are able to overcome the limitations of triptans. Objective: The aim of this study was to perform a meta-analysis to assess the efficacy of GluRAs for patients with migraine. Method: The PubMed, Cochrane Library, CINAHL, and Clinical Trial.gov databases were searched for randomized placebo-controlled trials of the efficacy of GluRAs for patients with migraine conducted up to August 2019. Two independent reviewers screened the literature according to inclusion and exclusion criteria and performed quality assessment and data extraction. Review Manager 5.3 software was used for the meta-analysis. Results: Three studies involving a total of 206 patients were included in the final analysis. Compared with placebo, GluRAs significantly improved the pain-free response at 2 hours (odds ratio [OR]=3.85, 95% confidence intervals [CIs]=1.63-9.09) and the 24-hour sustained pain freedom (OR=7.40; 95% CIs=2.36-23.20). The use of rescue medications with GluRAs was lower compared to that with placebo, but the difference was not significant (OR=0.39, 95% CI=0.10-1.47). Conclusion: Our meta-analysis showed that GluRAs were more effective than placebo for patients with migraine.

메타분석을 통한 중풍 위험인자 연구 (The Meta-Analysis Study on the Risk Factors of Stroke)

  • Objective : This study was conducted to integrate the results of studies which show the relationship between risk factors(smoking, drinking, intaking coffee, exercise, BMI, family history, hypertension, DM, hyperlipidemia, heart disease, TIA) and the incidence of stroke in Korean adults. Methods : We surveyed 17 stroke-related articles, published between 1995 and 2007, and conduct meta-analysis. And we estimated common odds ratio(OR). Results : The overall OR of risk factors associated CVD were as follow : Smoking had a 2.488 OR, no regular exercise had a 1.942 OR, $BMI{\geq}25$ had a 1.848 OR, hypertension had a 6.053 OR, DM had a 2.931 OR, heart disease had a 5.455 OR, and TIA had a 5.238 OR. Conclusion : The result of this study suggest that smoking, no regular exercise, $BMI{\geq}25$, hypertension, DM, heart disease, and TIA are important risk factor of stroke.

남자 고등학생의 과민대장증후군 유병률, 증상 유형 및 위험요인 (Prevalence, Subtypes and Risk Factors of Irritable Bowel Syndrome among Male Adolescents)

  • Purpose: The purpose of this study was to identify the prevalence, subtypes and risk factors of Irritable Bowel Syndrome (IBS) among male adolescents. Methods: The survey was conducted on male adolescents (n=441) attending a high school. Rome IV criteria was used to diagnose IBS. Data on health promoting behaviors, psychological distress, sleep hygiene practices were collected using self-administered structured questionnaires. Results: The prevalence of IBS was 11.1% and the subtypes were mixed (77.5%), unclassified (10.2%), diarrhea-predominant (8.2%), and constipation-predominant (4.1%). Risk factors influencing IBS were sleeping time (Odds Ratio [OR]=1.00, 95% Confidence Interval [CI]: 0.99~1.00), anxiety, a subfactor under psychological distress (OR=1.12, 95% CI: 1.02~1.22), and arousal-related behaviors, a subfactor under sleep hygiene practices (OR=1.10, 95% CI: 1.03~1.17). Conclusion: Our findings indicated that IBS is a common problem in male adolescents. It is necessary to develop school-based intervention strategies addressing sleeping time, anxiety and arousal-related behaviors in order to prevent and improve IBS in male adolescents.

난임 및 정상 여성의 생식건강증진행위 (Reproductive Health Promotion Behavior of Infertility Women and Normal Women)

  • Purpose: To compare reproductive health promoting behaviors (RHPBs) of infertile women with those of normal women and identify effects of RHPB on infertility. Methods: A total of 148 females (73 infertile women and 75 normal women) were enrolled in this study. Measurements included their general characteristics and RHPB using self-report questionnaires. Data were analyzed with descriptive statistics, ${\chi}^2$ test, ANCOVA, and hierarchical logistic regression using SPSS. Results: There were significant difference in incomes, number of family, number of term deliveries, and number of abortions according to infertility diagnosis. Mean duration of infertility was 32.16 months. Only 12.32% women had known cause of infertility. The most common cause of infertility was unknown. Mean RHPB score was 3.98 for infertile women and 4.41 for normal women. In logistic regression, total RHPB (odds ratio [OR], 0.21) and safe sex of RHPB (OR, 0.66) were significant factors influencing infertility. Infertile women's total RHPB and subcategories of RHPB (safe sex behavior and sexual transmitted disease [STD] prevention) were lower than those of normal women. Conclusion: For infertility women, RHPB-related intervention programs are needed, especially information about safe sex behavior and STD prevention.

Association between Sociodemographic Factors and Diarrhea in Children Under 5 Years in Rwanda

  • Diarrheal disease is the second leading cause of mortality and morbidity in children under 5 years old worldwide, and is the most common cause of malnutrition in sub-Saharan Africa. In Rwanda, diarrhea is the third leading cause of death in children under 5 years old. This study examined the association between sociodemographic factors and diarrhea in children under 5 years using the data of 7,474 households in the 2014-2015 Rwanda Demographic and Health Survey. Overall prevalence of diarrhea in this study was 12.7% in children. An increased risk for diarrhea was found for children aged 12-23 months (odds ratio (OR)=4.514), those with a low economic status (OR=1.64), those from the Western province (OR=1.439), those with poorly-educated mothers (OR=5.163), and those with families engaged in agricultural activities (OR=1.624). In conclusion, sociodemographic factors significantly affect the risk of developing diarrhea in children under 5 years in Rwanda. Designing and implementing health education promoting awareness of early interventions and rotavirus vaccination are essential to reduce diarrheal diseases for the Rwandan community.

Incidence and Associated Factors of Infantile Colic in Thai Infants

  • Purpose: Infantile colic, a common functional gastrointestinal condition, causes distress and frustration in families. Its prevalence and incidence vary from community to community. The purpose of our study was to demonstrate the incidence of and factors associated with infantile colic in Thai infants. Methods: We conducted a prospective analytic study to explore the incidence and factors associated with infantile colic in 386 Thai infants aged between one month and six months. Caregivers were interviewed using a questionnaire about infants' symptoms of colic based on the definition from the Rome IV criteria. Family background and potential precipitating factors of colic were also evaluated. Results: The incidence of colic in infants younger than 6 months was 6.5%. All infants' colic started within 12 weeks of life and lasted approximately 6 weeks. Sex, gestational age, birth weight, delivery route, birth order, family factors, and parental factors were not correlated with the occurrence of colic. Infants who were exclusively breastfed for the first 2 months of life had a lower incidence of infantile colic than those who were mixed- or formula-fed (odds ratio=3.0; 95% confidence intervals=1.3 to 7.2). Conclusion: The incidence of infantile colic in Thai infants in our study was 6.5%, which is similar to that in other reports. Being exclusively breastfed for the first two months was the only risk factor in our cohort.

Postoperative infection after cranioplasty in traumatic brain injury: a single center experience

  • Purpose: To determine the incidence and risk factors of postoperative infection after cranioplasty in patients with traumatic brain injury (TBI). Methods: Data of 289 adult patients who underwent cranioplasty after TBI at a single regional trauma center between year 2018 and 2021 were reviewed retrospectively. Patient characteristics and various procedural variables, such as interval between craniectomy and cranioplasty, estimated blood loss, laterality and materials of the bone flap, and duration and classification of perioperative antibiotics usage were analyzed. Results: Postoperative infection occurred in 17 patients (5.9%). Onset time of infectious symptom ranged from 9 days to 174 days (median, 24 days) after cranioplasty. The most common cultured organism was Staphylococcus aureus (47.1%), followed by Klebsiella pneumoniae (17.6%) and Enterococcus faecalis (17.6%). Patients with postoperative infection were more likely to have diabetes (odds ratio [OR], 6.96; 95% confidence interval [CI], 1.92-25.21; P=0.003), lower body mass index (OR, 0.81; 95% CI, 0.66-0.98; P=0.029), and shorter duration of perioperative antibiotics (OR, 0.83; 95% CI, 0.71-0.98; P=0.026). Conclusions: For TBI patients with diabetes, poor nutritional status should be managed cautiously for increased risk of infection after cranioplasty. Further studies and discussions are needed to determine an appropriate antibiotics protocol in cranioplasty.

한국 성인 음주 남성의 고위험 음주 관련 요인: 2차 분석 연구 (Factors associated with high-risk drinking among men in Korea: a secondary analysis)

  • Purpose: This study was conducted to identify factors associated with high-risk drinking in Korean men. Methods: This study was a secondary analysis using data from the Korea National Health and Nutrition Examination Survey (KNHANES VIII-2), 2020. Data were downloaded from the KNHANES website. The subjects of this study were 1,653 alcohol-drinking men between 19 and 64 years of age. Data were analyzed using the Rao-Scott chi-square test and complex sample logistic regression statistics. Results: The high-risk drinking rate among alcohol-drinking men was 27.1%. High-risk drinking was more common in men who smoked (odds ratio [OR] = 2.11, p < .001), men with a middle school education or lower (OR = 1.91, p = .016), men who lived with a spouse (OR = 1.61, p = .025), men who slept less than 6 hours on weekends (OR = 1.51, p = .016), and men who perceived stress (OR = 1.30, p = .044), while it was lower in men who were underweight (OR = 0.19, p = .006). Conclusion: To reduce the rate of high-risk drinking, it is necessary to provide an intervention that considers factors associated with high-risk drinking among men. In particular, smoking was the most important factor associated with high-risk drinking, implying that the integrated management of drinking and smoking is necessary to reduce high-risk drinking.

Oral Squamous Cell Carcinoma and Associated Risk Factors in Jazan, Saudi Arabia: A Hospital Based Case Control Study

  • Background: Oral cancer is the third most common malignancy in Saudi Arabia, the highest incidence of which is reported from Jazan province. The objective of this study was to evaluate the association of various locally used substances, especially shamma, with oral cancer in the Jazan region of Saudi Arabia. Materials and Methods: A hospital-based case-control study was designed and patient records were scanned for histologically confirmed oral cancer cases. Forty eight patients who were recently diagnosed with oral cancer were selected as cases. Two healthy controls were selected for each observed case and they were matched with age (+/- 5 years) gender and location. Use of different forms of tobacco such as cigarettes, pipe-smoking and shamma (smokeless-tobacco) was assessed. Khat, a commonly used chewing substance in the community was also included. Descriptive analysis was first performed followed by multiple logistic regression (with and without interaction) to derive odds ratios (ORs) and 95% confidence interval (CIs). Results: Mean age of the study sample (56% males and 44% females) was 65.3 years. Multinomial regression analysis revealed that shamma use increased the odds of developing oral cancer by 29 times (OR=29.3; 10.3-83.1). Cigarette (OR=6.74; 2.18-20.8) was also seen to have an effect. With the interaction model the odds ratio increased significantly for shamma users (OR=37.2; 12.3-113.2) and cigarette smokers (OR=10.5; 2.88-3.11). Khat was observed to have negative effect on the disease occurrence when used along with shamma (OR=0.01; 0.00 - 0.65). Conclusions: We conclude that shamma, a moist form of smokeless tobacco is a major threat for oral cancer occurrence in the Jazan region of Saudi Arabia. This study gives a direction to conduct further longitudinal studies in the region with increased sample size representing the population in order to provide more substantial evidence.

Psychosocial Factors and Musculoskeletal Pain Among Rural Hand-woven Carpet Weavers in Iran

  • Background: Musculoskeletal pain (MSP) is a common and disabling problem among carpet weavers and is linked to physical and psychosocial factors of work. This study aimed to determine the prevalence of MSP, its psychosocial risk factors, and association of pain in each pair of anatomical sites among carpet weavers. Methods: A cross-sectional study was performed among 546 hand-woven carpet weavers in rural small-scale workshops of Iran. Data were collected by using parts of a standardized CUPID (Cultural and Psychosocial Influences on Disability) questionnaire focused on MSP in 10 body sites, including the low-back, neck, both right and left shoulders, elbows, wrists/hands, individual, physical and psychosocial risk factors. Statistical analysis was performed applying logistic regression models. Results: Prevalence of MSP in at least one body sitewas 51.7% over the past month. The most common sites were low back and right shoulder pain 27.4% and 20.1%, respectively. A significant difference was found between the mean number of painful anatomical sites and the level of education, age, physical loading at work, time pressure, lack of support, and job dissatisfaction. In pairwise comparisons, strongest association was found between pain in each bilateral anatomical site (odds ratio = 11.6-35.3; p < 0.001). Conclusion: In home-based workshops of carpet weaving, psychosocial factors and physical loading were associated with MSP. This finding is consistent with studies conducted among other jobs. Considering the preventive programs, the same amount of attention should be paid to psychosocial risk factors and physical loading. Also, further longitudinal studies are needed to investigate the relationship of psychological factors.
