서울에서의 개의 안과질환에 대한 역학적 특징과 임상적인 특징에 대한 연구(2009년에서 2013년) (Epidemiological and Clinical Features of Canine Ophthalmic Diseases in Seoul from 2009 to 2013)

  • 서울에서 개의 안과질환에 대한 역학적인 특징에 대해 2009년 1월부터 2013년 12월까지우성동물메디칼센터에 안과질환으로 내원한 초진환자들의 자료를 분석하였다. 총 1253두의 안과환자들이 초진 내원하였으며, 평균나이는 7.93년령 (SD, 4.06)이었다. 환자중에 Shih Tzu (29.4%)가 가장 많았으며, Maltese, Yorkshire Terrier, Cocker Spaniel, toy Poodle (P < 0.01)이 뒤를 따랐다. 눈에서 질병이 호발하는 곳은 수정체 관련 질환이 가장 많았으며 (28.3%), 안검질환, 망막질환, 비루관질환, 포도막질환, 유리체질환, 시신경계 질환이 그 뒤를 따랐다(P < 0.01). 수정체, 안검, 망막, 비루관, 포도막, 유리체, 시신경계 질환들은 주로 양측성 질환(P < 0.01)으로 발생하는 경향을 보였다. Shih Tzu에서는 각막질환이 가장 많이 발생하였으며, 수정체, 안검, 망막, 녹내장 질환이 그 뒤를 따랐다. Maltese에서는 수정체, 안검, 망막, 각막질환의 순으로 질병이 발생했다. Yorkshire Terrier, Cocker Spaniel, 그리고 toy Poodle 종은 수정체 질환이 가장 많았다(P < 0.01). 안과질환으로 병원에 내원하는 주된 환자들은 Shih Tzu, Maltese, Yorkshire Terrier등과 같은 소형견들이었고, 대부분 수정체 질환과 같은 품종소인이 있는 질환이 대부분이었다.

  • The aim of this study was to show the clinical aspects in outpatients who visited Oriental Neuropsychiatry Clinic. Method : We studied 419 patients who visited Oriental Neuropsychiatry Clinic of Dong-Sea Medical Center from march, 2007 to August, 2007. Results & Conclusion : 1. The ratio of male to female patients was 1:2 and age distribution was most frequent in 50s. 2. From onset, 32% of patients visited Oriental Neuropsychiatry Clinic in a week, 58% of patients visited in a month. 3. Oriental Neuropsychiatry diseases were 80%, Non-Oriental Neuropsychiatry diseases were 20%. The most common Oriental Neuropsychiatry diseases were Somatoform Disorders. 4. The most common diagnosises of the Oriental Neuropsychiatry diseases were Psychosomatic diseases. 5. The most common cheif corn1ains were Headache. 6. The most commonly used herbal medicine was Gamiondam-tang and Guibi-tang

Genetic testing in clinical pediatric practice

  • Completion of the human genome project has allowed a deeper understanding of molecular pathophysiology and has provided invaluable genomic information for the diagnosis of genetic disorders. Advent of new technologies has lead to an explosion in genetic testing. However, this overwhelming stream of genetic information often misleads physicians and patients into a misguided faith in the power of genetic testing. Moreover, genetic testing raises a number of ethical, legal, and social issues. Diagnostic genetic tests can be divided into three primary but overlapping categories: cytogenetic studies (including routine karyotyping, high-resolution karyotyping, and fluorescent in situ hybridization studies), biochemical tests, and DNA-based diagnostic tests. DNA-based testing has grown rapidly over the past decade and includes preandpostnatal testing for the diagnosis of genetic diseases, testing for carriers of genetic diseases, genetic testing for susceptibility to common non-genetic diseases, and screening for common genetic diseases in a particular population. Theoretically, once a gene's structure, function, and association with a disease are well established, the clinical application of genetic testing should be feasible. However, for routine applications in a clinical setting, such tests must satisfy a number of criteria. These criteria include an acceptable degree of clinical and analytical validity, support of a quality assurance program, possibility of modifying the course of the diagnosed disease with treatment, inclusion of pre-and postnatal genetic counseling, and determination of whether the proposed test satisfies cost-benefit criteria and should replace or complement traditional tests. In the near future, the application of genetic testing to common diseases is expected to expand and will likely be extended to include individual pharmacogenetic assessments.

Different Point of View to the Autoimmune Diseases and Treatment with Acupuncture

  • Objectives: It was aimed to investigate the basic action mechanism of the autoimmune diseases and common features of all diseases. Autoimmune disease are classified organ specific and systemic. Methods: These diseases are seen systemic and disease start locations, origins seem differently. This makes learning and understanding difficult. Autoimmune diseases investigated for easier understanding. It was noticed that, autoimmune diseases' starting places are specific and same all of them. This remarkable point is very important for acupuncture also. So; whole literatüre was researched and important point was found. Results: Whole autoimmune diseases are attack to mesodermal layers and mesodermal origin organs of the body's. The common property of all these disease are same; Diseases start from the mesoderm and mesodermal layer even though their organ origins' belongs to different germ layer. From this point of view, we were able to classify autoimmune diseases simply and it was planned how can we effect body in this context with acupuncture. Conclusion: And, when immunity comes into question, induction of adaptive immunity is depend on antigen presentation to T cells and this situation take place in the lymph node (LN) and also in the skin.When we sank the acupuncture needle into skin, signals create and start mesodermal contacts, during this time mesenchymal origin' autoimmune cells are regulated with this signals.

호흡기내과의사를 위한 직업성 폐질환 리뷰 (Clinical Year-in-Review of Occupational Lung Disease)

  • Occupational lung disease (OLD) is a group of lung diseases caused and/or aggravated by organic and inorganic inhaled dust, fumes, and mist. OLD can develop under various occupational situations. Therefore, occupational history should be considered when evaluating respiratory symptoms. Once OLD is developed, it may not be treated and may even progress after exposure to the causative agents has stopped. The best ways to treat OLD are prevention and early detection by controlling the working environment and conducting regular surveillance of workers. Common OLDs in Korea are coal worker's pneumoconiosis, asbestos-related diseases, and occupational asthma. Recent aspects of these common OLDs in Korea will be described based on recently published studies.

Occupational Lung Diseases: Spectrum of Common Imaging Manifestations

  • Occupational lung diseases (OLD) are a group of preventable conditions caused by noxious inhalation exposure in the workplace. Workers in various industries are at a higher risk of developing OLD. Despite regulations contributing to a decreased incidence, OLD remain among the most frequently diagnosed work-related conditions, contributing to significant morbidity and mortality. A multidisciplinary discussion (MDD) is necessary for a timely diagnosis. Imaging, particularly computed tomography, plays a central role in diagnosing OLD and excluding other inhalational lung diseases. OLD can be broadly classified into fibrotic and non-fibrotic forms. Imaging reflects variable degrees of inflammation and fibrosis involving the airways, parenchyma, and pleura. Common manifestations include classical pneumoconioses, chronic granulomatous diseases (CGD), and small and large airway diseases. Imaging is influenced by the type of inciting exposure. The findings of airway disease may be subtle or solely uncovered upon expiration. High-resolution chest CT, including expiratory-phase imaging, should be performed in all patients with suspected OLD. Radiologists should familiarize themselves with these imaging features to improve diagnostic accuracy.

  • Chronic diseases such as cancer, cardiovascular diseases, are the leading cause of death and disability in Korea since 1970 due to lifestyle change introduced by urbanization & industrialization. The type of cancer and cardiovascular diseases changes as lifestyle becomes westernized. These diseases account for 4 of every 10 deaths and affect the quality of lift of Koreans. Although chronic diseases are among the most common and costly healthy problems, they are also preventable. Adopting healthy behaviors such as quitting smoking, being physically active, eating right with moderate alcohol drinking, and maintaining healthy weight can prevent or control the effect of these diseases.
