OMOP CDM 구축 시 개인의료정보 보호를 위한 HIPAA PHI 적용 방법 연구 (Study on HIPAA PHI application method to protect personal medical information in OMOP CDM construction)

  • 본 연구에서는 OMOP(Observational Medical Outcomes Partnership) CDM(Common Data Model) 구축시 개인의료정보를 보호하는 방법을 연구하였다. 제안된 방법은 HIPAA(The Health Insurance Portability and Accountability Act) PHI(Protected Health Information)에 대응되는 데이터가 CDM으로 추출 되는 것을 제한하거나 식별 불능 화 처리 하는 것이다. 하지만 한국의 개인정보보호법 및 의료법에는 민감 정보의 처리 제한에 관한 내용은 존재하나 그 민감 정보가 무엇인지에 관한 명확한 규정은 없어 개인의료정보 보호를 위한 민감 정보 선정에 어려움이 있다. 본 연구에서는 이러한 문제를 해결하기 위해 HIPAA PHI를 개인정보 보호법 제23조 민감 정보의 처리 제한 기준으로 정하고 CDM데이터와 매핑 하였다. 본 연구를 통해 CDM구축 시 발생되는 개인의료 정보 보호문제에 대한 해결 방법을 제시함으로써 국내 CDM구축 확산에 기여할 것으로 예상된다.

  • 디지털 헬스케어 서비스 활성화에 따라 디지털 의료 데이터의 양은 매년 급속하게 증가하고 있으며, 의 데이터의 상호 교환과 연동을 위한 다양한 CDM(Common Data Model)이 개발되고 있다. 그러나, 의료 데이터 교류에 대한 요구가 증가하면서, 기존 레거시 시스템의 데이터를 CDM으로 변환하기 위한 추가적인 비용이 소요될 수 밖에 없다. 이에 본 연구에서는 OMOP CDM (Observational Medial Outcomes Partnership Common DataModel) 기반 의료 데이터 ETL (Extract, Transform, Load) 툴을 개발하였다. OMOP CDM ETL 툴은 기존의 레거시 데이터베이스 정보를 CDM으로 변환할 수 있는 효과적인 료인터페이스를 제공함으로써, 디지털 의료 데이터 공유와 관리 및 분석의 효율성을 증대할 수 있을 것이다.

CDM 데이터 공유를 위한 자동화 시스템 (Automation System for Sharing CDM Data)

  • 의료 분야에서 연구 목적을 위해 공유에 대한 필요성이 증가함에 따라 공통 데이터 모델(CDM)의 활용이 증가하고 있다. 하지만 CDM 데이터를 공유할 때 접근 제어와 데이터 내에 있는 개인 정보 보호가 되지 않는 문제들이 존재한다. 본 논문에서는 이러한 문제를 해결하기 위해 블록체인 네트워크에 암호화 방식을 사용하여 CDM 데이터에 대한 접근 제어를 하고, CDM 데이터의 정보를 기록하여 추적이 가능하게 했다. 또한 대용량의 CDM 데이터를 공유하기 위해 IPFS를 이용하였으며, 공유하는 과정을 자동화하기 위해 Celery를 활용하였다. 즉, CDM 데이터 공유에 필요한 정보를 신뢰 기반 기술, 분산 파일 시스템 그리고 자동화를 위한 메시지 큐가 나누어 가진 멀티 채널 자동화 시스템을 제안한다. 이를 통해 CDM 데이터를 공유하는 과정에서 발생하는 접근 제어와 데이터 내에 있는 개인 정보 보호 문제를 해결하고자 한다.

단국대학교병원 EHR 기반 MOA CDM 구축을 위한 용어 매핑 사례와 시사점 (The Case and Implications of Terminology Mapping for Development of Dankook University Hospital EHR-Based MOA CDM)

  • Purposes: The Common Data Model(CDM) is very important for multi-institutional research. There are various domestic and international CDM construction cases to actively utilize it. In order to construct a CDM, different terms from each institution must be mapped to standard terms. Therefore, we intend to derive the importance and major issues of terminology mapping and propose a solution in CDM construction. Methodology/Approach: This study conducted terminology mapping between Electronic Health Record(EHR) and MOA CDM for constructing Medical Record Observation & Assessment for Drug Safety(MOA) CDM at Dankook University Hospital in 2022. In the process of terminology mapping, a CDM standard terminology process and method were developed and terminology mapping was performed by applying this. The constructions of CDM mapping terms proceeded in the order of diagnosis, drug, measurement, and treatment_procedure. Findings: We developed mapping guideline for CDM construction and used this for mapping. A total of 670,993 EHR data from Dankook University Hospital(January 1, 2013 to December 31, 2021) were mapped. In the case of diagnosis terminology, 19,413 were completely mapped. Drug terminology mapped 92.1% of 2,795. Measurement terminology mapped 94.5% of 7,254 cases. Treatment and procedure were mapped to 2,181 cases, which are the number of mapping targets. Practical Implications: This study found the importance of constructing MOA CDM for drug side effect monitoring and developed terminology mapping guideline. Our results would be useful for all future researchers who are conducting terminology mapping when constructing CDM.

OHDSI OMOP-CDM 데이터베이스 보안 취약점 및 대응방안 (OHDSI OMOP-CDM Database Security Weakness and Countermeasures)

  • Globally researchers at medical institutions are actively sharing COHORT data of patients to develop vaccines and treatments to overcome the COVID-19 crisis. OMOP-CDM, a common data model that efficiently shares medical data research independently operated by individual medical institutions has patient personal information (e.g. PII, PHI). Although PII and PHI are managed and shared indistinguishably through de-identification or anonymization in medical institutions they could not be guaranteed at 100% by complete de-identification and anonymization. For this reason the security of the OMOP-CDM database is important but there is no detailed and specific OMOP-CDM security inspection tool so risk mitigation measures are being taken with a general security inspection tool. This study intends to study and present a model for implementing a tool to check the security vulnerability of OMOP-CDM by analyzing the security guidelines for the US database and security controls of the personal information protection of the NIST. Additionally it intends to verify the implementation feasibility by real field demonstration in an actual 3 hospitals environment. As a result of checking the security status of the test server and the CDM database of the three hospitals in operation, most of the database audit and encryption functions were found to be insufficient. Based on these inspection results it was applied to the optimization study of the complex and time-consuming CDM CSF developed in the "Development of Security Framework Required for CDM-based Distributed Research" task of the Korea Health Industry Promotion Agency. According to several recent newspaper articles, Ramsomware attacks on financially large hospitals are intensifying. Organizations that are currently operating or will operate CDM databases need to install database audits(proofing) and encryption (data protection) that are not provided by the OMOP-CDM database template to prevent attackers from compromising.

  • 인공지능 기술을 도입한 의료분야에서 진단 및 예측과 연계한 임상의사결정지원 시스템(CDSS)에 관련된 연구가 활발하게 진행되고 있다. 특히, 인공지능 기술 적용에 가장 많은 이슈를 일으키고 있는 의료영상기반의 질환진단연구가 다양한 제품으로 출시되고 있는 실정이다. 그러나 의료영상 데이터는 일관되지 않은 데이터들로 이루어져 있으며, 그것을 정제하여 연구에 사용하기 위해서는 상당한 시간이 필요한 것이 현실이다. 본 논문은 의료영상 표준인 R_CDM(Radiology Common Data Model)으로 변환하고, 그 데이터를 기반으로 인공지능 알고리즘 개발 연구를 지원하기위한 원스톱 인공지능학습 플랫폼에 대하여 기술한다. 이를 위해 기존 공통데이터모델(CDM : Common Data Model)과 연계에 중점을 두어 DICOM(Digital Imaging and Communications in Medicine) 태그정보를 기반으로 의료영상 표준 모델의 스키마와 다기관 연구를 위한 Report 정보를 포함하여 시스템을 모델링하였다. 이렇게 변환된 데이터 집합을 기반으로 인공지능 학습 플랫폼에서 수행 과정을 결과로 보인다. 제안한 플랫폼을 통해 다양한 영상기반 인공지능 연구에 활용될 것으로 기대하고 있다.

WCO의 CDM과 UCR을 수용한 전자통관시스템 발전 방안 (A Study on Acceptance of CDM/UCR in WCO as e-Customs clearance procedures)

  • A single window platform must be successfully established by implementing CDM/UCR in WCO and UNeDocs in UN/CEFACT, which is part of the framework of standards for secure and facilitate global trade. Core success factors in single window platform for international trade depend on political innovative capacity, advanced leadership of high ranking officials and departments in charge, effective collaborations between the ministries and offices concerned which are supported by supreme executives, self-controlled coordinations between users, convenience and access availability to users, promotion and marketing, identification on predictive impediments, financial raising and revenue model payable, communication's strategy, valid legal basis etc. To settle a stable ubiquitous e-trade platform, firstly, procedures and practices of customs clearance which different from one another mutually must be standardized globally. Secondly, the international standard electronic data exchanges must be introduced to facilitate and simplify customs clearing procedures by holding administrative informations in common between public institutions. thirdly, cooperative work model must be accepted with enhancing outsourcing business between reference groups (export-import firms and IT service providers).

Efficient Idle Virtual Machine Management for Heterogeneous Cloud using Common Deployment Model

  • This paper presents an effective management of VM (Virtual Machine) for heterogeneous cloud using Common Deployment Model (CDM) brokering mechanism. The effective utilization of VM is achieved by means of task scheduling with VM placement technique. The placements of VM for the physical machine are analyzed with respect to execution time of the task. The idle time of the VMis utilized productively in order to improve the performance. The VMs are also scheduled to maintain the state of the current VM after the task completion. CDM based algorithm maintains two directories namely Active Directory (AD) and Passive Directory (PD). These directories maintain VM with proper configuration mapping of the physical machines to perform two operations namely VM migration and VM roll back. VM migration operation is performed from AD to PD whereas VM roll back operation is performed from PD to AD. The main objectives of the proposed algorithm is to manage the VM's idle time effectively and to maximize the utilization of resources at the data center. The VM placement and VM scheduling algorithms are analyzed in various dimensions of the cloud and the results are compared with iCanCloud model.

약물부작용 감시를 위한 공통데이터모델 기반 임상데이터웨어하우스 구축 (Development and Lessons Learned of Clinical Data Warehouse based on Common Data Model for Drug Surveillance)

  • Purposes: It is very important to establish a clinical data warehouse based on a common data model to offset the different data characteristics of each medical institution and for drug surveillance. This study attempted to establish a clinical data warehouse for Dankook university hospital for drug surveillance, and to derive the main items necessary for development. Methodology/Approach: This study extracted the electronic medical record data of Dankook university hospital tracked for 9 years from 2013 (2013.01.01. to 2021.12.31) to build a clinical data warehouse. The extracted data was converted into the Observational Medical Outcomes Partnership Common Data Model (Version 5.4). Data term mapping was performed using the electronic medical record data of Dankook university hospital and the standard term mapping guide. To verify the clinical data warehouse, the use of angiotensin receptor blockers and the incidence of liver toxicity were analyzed, and the results were compared with the analysis of hospital raw data. Findings: This study used a total of 670,933 data from electronic medical records for the Dankook university clinical data warehouse. Excluding the number of overlapping cases among the total number of cases, the target data was mapped into standard terms. Diagnosis (100% of total cases), drug (92.1%), and measurement (94.5%) were standardized. For treatment and surgery, the insurance EDI (electronic data interchange) code was used as it is. Extraction, conversion and loading were completed. R language-based conversion and loading software for the process was developed, and clinical data warehouse construction was completed through data verification. Practical Implications: In this study, a clinical data warehouse for Dankook university hospitals based on a common data model supporting drug surveillance research was established and verified. The results of this study provide guidelines for institutions that want to build a clinical data warehouse in the future by deriving key points necessary for building a clinical data warehouse.

Association of periodontitis with menopause and hormone replacement therapy: a hospital cohort study using a common data model

  • Purpose: The present study was designed to compare the incidence of periodontitis according to menopausal status and to investigate the possible effect of hormone replacement therapy (HRT) on periodontitis in postmenopausal women using a common data model (CDM) at a single institution. Methods: This study involved retrospective cohort data of 950,751 patients from a 20-year database (2001 to 2020) of Ewha Womans University Mokdong Hospital converted to the Observational Medical Outcomes Partnership CDM. One-way analysis of variance models and the χ2 test were used to analyze the statistical differences in patient characteristics among groups. A time-dependent Cox regression analysis was used to calculate hazard ratios and 95% confidence intervals, and P values less than 0.05 were considered to indicate statistical significance. Results: Of the 29,729 patients, 1,307 patients were diagnosed with periodontitis and 28,422 patients were not. Periodontitis was significantly more common among postmenopausal patients regardless of HRT status than among the non-menopausal group (P<0.05). Time-dependent Cox regression analysis showed that the postmenopausal patients had a significantly higher chance of having periodontitis than non-menopausal patients (P<0.05), but after adjustment for age, body mass index, and smoking status, the difference between the non-menopausal and post-menopausal HRT-treated groups was insignificant (P=0.140). Conclusions: Postmenopausal women had a significantly greater risk of periodontitis than non-menopausal women. Additionally, the use of HRT in postmenopausal women could reduce the incidence of periodontitis.
