The Effect of Cognitive Rehabilitation Program Combined with Physical Exercise on Cognitive Function, Depression, and Sleep in Chronic Stroke Patients

  • Objective: This study was conducted to verify the effectiveness of a cognitive rehabilitation program consisting of physical exercises and mental activities for patients with chronic stroke with mild cognitive impairment (MCI). We aimed to investigate how this cognitive rehabilitation program affects patients' cognitive ability, depression, and sleep quality. Design: One group pretest-posttest design Methods: The study was conductedon 12 patients who participated in thecognitive rehabilitation complex exercise program for 16 weeks. The Korean version of the Montreal Cognitive Assessment (MoCA-K), Hamilton Depression Rating Scale (K-HDRS), and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate the measured variables before and after study participation. The cognitive rehabilitation complex exercise program included 30 minutes of cognitive exercise and 30 minutes of Brill Exercise. The Wilcoxon signed-rank test was used to compare the variables before and after program participation. Cronbach's ɑ was used to assess the reliability of the test variables. Results: The post-program assessment showed a statistically significant increase in the MoCA-K score, which measures cognitive function (Z=-2.628, P=0.009). For depression ratings, there was a statistically significant decrease in the K-HDRS score (Z=-2.041, P=0.041). For sleep quality, although there was a numerical increase in the PSQI score, the difference was not statistically significant (Z=-0.702, P=0.483). The reliability test confirmed that all the individual test variables exhibited high reliability (cognitive function, 0.859; depression, 0.872; sleep, 0.822). Conclusions: We found that cognitive rehabilitation program used in this study had a positive effect on the cognitive function and depression in patients with chronic stroke with MCI.

  • Objective: This study reports the effectiveness of the Korean medicine treatment of patients with intracerebral and intraventricular hemorrhages suffering unilateral neglect symptoms. Case presentation: The patient was treated using Korean medicine, including acupuncture, herbal medicine, moxibustion, and a limb activation exercise. We measured Unilateral Neglect Symptom progress with a numeric rating scale, Albert's test, Line bisection test, Clock drawing test, and Catherine Bergego Scale. After eight days of combined Korean medicine and rehabilitation exercise, the Unilateral Neglect Symptom intensity reduced from NRS 8 to 6. In addition, Albert's test score decreased from 2.5% to 0%. The Line bisection test score decreased from 19 mm to 12 mm, and the Clock drawing test score improved from 8 to 10. Last, the Catherine Bergego Scale decreased from 12 to 9. No adverse events were observed during treatment. Conclusion: This study suggested that a complex treatment including Jaeumkunbi-tang, acupuncture, moxibustion, and limb activation exercise can be effective for an intracerebral hemorrhage with intraventricular hemorrhage patient suffering from Unilateral Neglect Symptoms. Consequently, more research is needed in the future.

A Systematic Review and Meta-Analysis of Randomized Controlled Trials on Acupuncture for Improving Post-Operative Cognitive Dysfunction (POCD) and Chemotherapy-Related Cognitive Impairment (CRCI)

  • Objectives: This study was conducted to evaluate the efficacy of Korean Medicine Therapy (KMT) for the treatment of Chemotherapy-induced Cognitive Impairment (CICI) through systematic review and meta-analysis of randomized controlled trials (RCTs) as proceeding research to further research the efficacy of KMT for CICI patients. Methods: We conducted a systematic review and meta-analysis according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines (PRISMA). The RCTs on the efficacy of KMT for treatment of CICI were searched by structured search strategies in MEDLINE, EMBASE, Cochrane library, CAJD, KISS, NDSL, KoreaMed, and OASIS. The searched RCTs were screened by inclusion and exclusion criteria. We evaluated the quality of the method in the included studies using the Jadad score and Cochran ROB tool. The efficacy outcomes were the Visual Analogue Scale (VAS) and the Clinical total Effective Rate (CER) of CICI. They were analyzed using mean difference for continuous variable or Relative Risk (RR) for Dichotomous variable in the random-effect model. The RevMan 5.3 program was used for meta-analysis. Results: We included 19 RCTs, including 653 participants, in the systematic review and meta-analysis. There were favorable results for the KMT group after the intervention compared with the pharmacotherapy group, physiotherapy group, and combined treatment group. KMT group showed improvement using CER and VAS compared with exercises, but their heterogeneities were slightly significant. KMT was more effective compared to the Rehabilitation program in CER and the subgroup analysis results showed that KMT had a significant difference compared to other therapies in VAS, to Medication therapy in CER. Conclusions: KMT presented reasonable evidence on improving the Clinical total Effective Rate and Visual Analogue Scale in CICI patients. However, further evaluation in future research is required.

라이프케어 증진을 위한 비만여성에 대한 필라테스의 효과: 문헌 고찰 (A Literatura Review of The Effect of Pilates on Obesity Women for Improvement of Life Care)

  • 본 연구의 목적은 비만여성에게 필라테스를 적용한 연구들을 대상으로 필라테스가 비만여성에게 나타나는 신체적, 정신적 효과에 대하여 조사하는 것이다. 논문의 선정은 데이터베이스 학술연구정보서비스(RISS), Pubmed를 통해 선정기준에 해당하는 총 11편의 논문을 선정하였다. 필라테스로 인한 비만 여성들의 변화로는 허리와 다리의 등속성 근력과 허리둘레의 변화, 비만관 관련된 신체구성성분 중, RBP-4, 인슐린, 글루코스, 코르티솔, 렙틴, 콜레스테롤, 체지방, CRP의 감소와 그렐린, 아디포넥틴 농도의 증가, 혈관 탄성의 유의한 변화를 볼 수 있으며, 자아 존중감 및 삶의 질 지표 변화로 비만으로 인한 부정적 정서에 긍정적 영향을 미친다고 나타내었다. 필라테스는 건강과 관련된 요소에서 효과적이라고 보여지며 비만 여성의 라이프케어에 추천되는 운동으로 생각된다.

근감소성(sarcopenia) 비만 노인의 근지구력 운동과 보충제 섭취가 사지 근육량과 근기능 관련 체력과 혈중지질 및 IGF-1에 미치는 영향 (Effects of Muscular Endurance Exercise and Supplement Intake on Appendicular Skeletal Muscle and Muscle Function-Related Physical Strength, Blood Lipids, and IGF-1 in Obese Older Adults with Sarcopenia)

  • Purpose : This study aimed to introduce a 12-week muscular endurance exercise program and β-glucan consumption to obese elderly people with sarcopenia. We evaluated the program's impact on appendicular skeletal muscle strength and function, muscle function-related physical strength, blood lipids, and IGF-1. The results will serve as foundational data for preventing and improving sarcopenia. Methods : Forty elderly people aged 65 or older were recruited and underwent dual-energy X-ray absorptiometry. Based on criteria related to appendicular skeletal muscle mass (ASM/Height2: less than 5.4 kg/m2) and body fat percentage (at least 30% for women and 25% for men), we selected 24 obese elderly people with sarcopenia and excluded 56 who did not meet the criteria. Variables related to sarcopenia, blood lipids, IGF-1, and muscle function were measured before the 12-week muscular endurance exercise program. Results : In sarcopenic obese elderly participants, the 12-week muscular endurance exercise program significantly increased bone density and muscle mass while decreasing fat mass and percentage (p<.05). This program also improved grip strength, static balance, and SPPB in sarcopenic obese older adults. Furthermore, their 6-minute walk distance significantly increased (p<.05). Conclusion : Muscular endurance exercises combined with supplement intake can increase appendicular skeletal muscle and improve muscle strength and function, thereby improving the daily physical performance of the elderly.

허리 안정화 운동과 병행한 엉덩근육 강화 운동이 제왕절개 분만 후 만성허리통증을 가진 여성의 통증, 기능장애지수, 산후우울증에 미치는 영향 (Effects of the Gluteus Muscle Exercise combined with the Lumbar Stabilization Exercise on Pain, ODI, and Postpartum Depression in Women with Chronic Back Pain After C-section Delivery)

  • Background: This study aimed to investigate the effects of combining the lumbar stabilization exercise with the gluteal muscle strength exercise on pain, the dysfunction index, and postnatal depression in women experiencing chronic low back pain after a cesarean section. Methods: The study used a randomized design with two groups: one group (n=15) performed the lumbar stabilization exercise along with the gluteal muscle strength exercise, while the other group (n=15) only did the lumbar stabilization exercise. The intervention consisted of 30 minute exercise sessions, twice a week, for 6 weeks. The pain was assessed using the visual analog scale (VAS), the dysfunction index with the Oswestry disability index (ODI), and postnatal depression using the edinburgh postnatal depression scale-Korean version (EPDS-K). Results: Both groups showed significant improvements in their VAS, ODI, and EPDS-K scores within their respective groups (p<.05). Additionally, the between-group analysis revealed significant differences in post-test results (p<.05). Conclusion: The combination of the lumbar stabilization exercise and the gluteal muscle strength exercise proved to be more effective in reducing pain, improving the dysfunction index, and alleviating postnatal depression in women with chronic low back pain after a cesarean section. Therefore, incorporating these exercises could be beneficial as an intervention program for women experiencing chronic low back pain after a cesarean section.

원자력 발전소 온배수 피해역 산정을 위한 영향지수 시안 (Thermal Index for the Assessment of the Impacted Area by the Thermal Discharge from Nuclear Power Plant in Korea)

  • 국내 원자력 발전소의 온배수 배출을 둘러싸고 제기된 각 종 수산업 피해 민원이 발생한지도 20년이 가까워 온다. 온배수에 의한 연안환경 및 수산업 피해를 보다 과학적이고 합리적으로 산정하기 위한 지침이 한국 해양학회, 수산학회 공동의 노력으로 마련되었다. 피해역 산정 방안을 보다 합리적으로 마련하기 위하여 기존의 특정 상승온도 기준을 탈피하고 환경과 생태계의 다양한 영향을 포괄적으로 고려할 수 있는 온배수 영향지수를 제안하였다. 이 영향지수는 UNEP(2002)의 권고안에 따른 지침을 수용하고, 현실성과 경제성 및 미래지향적 특성을 갖출 수 있다. 특정 원자력발전소 현장에 적용 가능한 영향지수의 구현을 위하여 예비 실험의 노력이 요구된다.

걷기와 밴드운동이 과체중 및 비만아동의 C-반응성단백질 및 심혈관질환 위험인자의 변화에 미치는 영향 (Effects of Walking and Band Exercise on C-reactive Protein and Cardiovascular Disease Risk Factor in Overweight and Obese Children)

  • 본 연구의 목적은 운동프로그램이 과체중 및 비만아동의 시기에 따른 C-반응성단백질 및 심혈관질환 위험인자의 변화에 미치는 영향을 알아보고자 과체중 및 비만 아동을 대상으로 12주 간의 걷기와 밴드운동을 실시하고, 운동전, 4주 후, 12주 후의 신체조성, 혈중지질, 인슬린, C-반응성단백질를 측정 분석하였다. 연구의 대상자는 연령과 성별에 따른 체질량지수 백분위 수 85 이상(BMI${\geq}$21.3)인 과체중 및 비만아동 16명을 프로그램에 참여하는 운동군에 8명, 대조군에 8명으로 무선배치 하였다. 운동군은 주 2회의 걷기운동 50분과 주 2회의 밴드저항운동을 12주 동안 실시하였다. 연구결과 운동군 내 에서는 체중(p<0.001), BMI (p<0.001), 체지방량(p<0.001), 체지방률(p<0.001), TC (p<0.05), 인슐린(p<0.05), HOMA-IR (p<0.05)이 감소하였고, 제지방률(p<0.001), HDL-C (p<0.01)는 증가하였다. 측정시기를 고려하여 집단간의 차이를 분석한 결과 체중(p<0.001), BMI (p<0.001), 체지방량(p<0.001), 체지방률(p<0.001), 제지방량(p<0.05), 제지방률(p<0.001), 인슐린(p<0.05), HOMA-IR (p<0.05)에서 통계적으로 유의한 차이가 있는 것으로 나타났다. 이상의 연구결과 12주 걷기와 밴드운동에 의해 과체중 및 비만아동의 심혈관질환 위험인자를 일부 개선되지만, C-반응성단백질 농도는 개선되지 않음을 알 수 있었다.

Comparison of the effectiveness of extensor muscle strengthening exercise by itself, exercise with polydeoxyribonucleotide injection, and exercise with extracorporeal shockwave therapy in lateral epicondylitis: a randomized controlled trial

  • Background: Extensor muscle strengthening exercises with counterforce braces (EX) is a conventional conservative treatment for lateral epicondylitis (LE) of the elbow. In addition, polydeoxyribonucleotide (PDRN) or extracorporeal shockwave therapy (ESWT) has been recently used for LE. Methods: Sixty-three patients with chronic LE participated in this study and randomly allocated in three groups (G1: EX, G2: EX+PDRN injection, and G3: EX+ESWT). All of the three groups were taught to perform EX at the first out-patient department (OPD) visit. Group 2 was injected with 3 mL PDRN (5.625 mg/3 mL), while group 3 received ESWT at the first OPD visit. Visual analog scale pain score, Mayo elbow performance score (MEPS), and ultrasonographic examination were checked before, 6 weeks, and 12 weeks after the treatments. Results: Overall functional scores and ultrasonographic findings in all three groups improved after treatment. The mean MEPS in group 2 improved more than groups 1 and 3 at 6 weeks (G1, 56.9>62.4; G2, 54.3>65.0; G3, 55.7>62.6), and more than group 1 at 12 weeks (G1, 56.9>67.9; G2, 54.3>73.6). The mean common extensor tendon depth (CETD) on ultrasonography in group 2 increased more than groups 1 and 3 at 6 and 12 weeks (6 weeks: G1, 0.385>0.386; G2, 0.332>0.392; G3, 0.334>0.357; 12 weeks: G1, 0.385>0.409; G2, 0.332>0.438; G3, 0.334>0.405 [cm]). Conclusions: PDRN injections combined with EX exhibited a greater improvement in mean MEPS and mean CETD compared to EX only or EX combined with ESWT for LE within the 12 weeks follow-up.

혈액투석 환자의 신체 활동 증가가 건강관련 삶의 질과 영양섭취에 미치는 영향 (Effects of Ingressed Physical Activity in Hemodialysis Patients on Health-related Quality of Life and Nutritional Intake)

  • 연구목적은 혈액투석환자를 위한 혈액투석 중 자전거운동과 걷기운동을 개발하여 적용하고, 혈액투석환자의 신체 활동량이 건강관련 삶의 질과 영양섭취에 미치는 영향을 확인하기 위함이다. 연구방법은 혈액투석환자에게 적용한 유산소 운동 프로그램의 효과를 확인하기 위한 비동등성 대조군 전·후 실험연구이다. 연구대상자는 중재군과 대조군에 각각 30명이 배정되어 총 60명이었다. 운동중재는 두 종류의 유산소 운동이 포함되어 있으며, 총 8주간 진행되었다. 첫 번째 운동은 일주일에 세 번씩 혈액투석 중 수행되는 자전거운동이었고 두 번째 운동은 일주일에 최대 7 일 동안 실시되는 걷기운동이었다. 통계분석은 기술통계, 독립표본 t-test, 𝑥2-test와 Mann-Whitney U test를 사용하였다. 영양섭취는 CAN-2.0을 사용하여 평가하였다. 연구결과는 중재군이 중재 후 신체활동량 점수가 유의하게 높았으며, 신체적 건강관련 삶의 질도 중재 후 유의하게 개선되었다. 그러나 영양섭취는 중재 전 후 유의한 차이가 없었다. 비록 그룹 간의 차이는 없었지만, 총 칼로리 섭취는 중재 후에 중재군이 대조군보다 높은 것으로 나타났다. 신체적 건강관련 삶의 질은 동물성 단백질, 망간, 셀레늄과 비타민 C와 유의한 상관관계가 있었다. 본 연구는 혈액투석 중 자전거운동과 걷기운동은 신체활동량과 신체적 건강관련 삶의 질을 향상시키는데 효과적인 것으로 나타났다. 따라서, 본 연구결과는 혈액투석환자의 운동증진을 위한 지침으로 사용될 수 있을 것이다.
