Colonic Leiomyoma in Two Dogs (두 마리의 개에서 결장 평활근종의 증례)

  • Two dogs were referred with the clinical signs of depression and constipation, respectively. One dog with depression had hypoglycemia on biochemistry. On survey abdominal radiographs, a soft tissue density round mass with calcified spots was identified cranial to the urinary bladder. On ultrasonographs, a solitary oval heterogeneous complex-echoic mass with well-defined margin was found. On surgery, the mass was pedunculated and originated from the colonic wall. The resected mass was confirmed as colonic leiomyoma. The other dog with constipation had normal CBC and biochemistry values. On survey abdominal radiographs, there was a soft tissue density round mass dorsal to the descending colon and rectum. The descending colon was displaced ventrally and narrowed by the mass. On ultrasonographs, a well-defined round heteroechoic mass compressed the colon. The mass was also pedunculated and originated from the colonic wall on surgery and confirmed as colonic leiomyoma. Two dogs recovered without any clinical signs after surgery and showed permanent resolution of the clinical signs, respectively.

Effects of Fructans and Isomaltooligosaccharide on Large Bowel Mass and Plasma and Fecal Immunoglobulin A in Rat

  • There are increasing evidences that prebiotics can modulate various properties of the immune system. This study was conducted to investigate effects of three kinds of fructans (chicory inulin, chicory inulin oligosaccharide and fructooligosaccharide) and a glucose oligomer(isomaltooligosaccharide) in large bowel mass and innnunoglobulin A (IgA) in rats. Forty five Sprague-Dawley male rats weighing about 1909 were randomly sorted to receive one of the five treatments, which were control diet, control diet+6% isomaltooligosaccharide (IMOS), control diet+6% fructooligosaccharide (FOS), control diet+6% chicory inulin oligosaccharide (CIOS), or control diet + 6% chicory inulin (CI). Rats were pair-fed and received the experimental diets for 5 weeks. Cecal and colonic wall weights were significantly higher in fructan (FOS, CIOS, CI)-fed groups compared with control and IMOS groups, and the length of colon was elevated in FOS and CIOS groups compared with control group. Fecal concentrations of acetic acid and total short-chain fatty acids (SCFAs) were significantly elevated in fructan-fed groups. Plasma and cecal levels and fecal excretion of immunogiobulin A (IgA) in rats were not significantly different among groups. However, fructooligosaccharide tended to increase IgA level in cecum. Cecal IgA level was significantly negatively correlated with pH of cecal content (r=-0.337), positively correlated with acetic acid level (r=0.310). Fecal IgA excretion was positively correlated with total SCFA (r=0.311) and propionic acid (r=0.400) level in feces. These results indicate that fructooligosaccharide and chicory inulin oligosaccharide exerted trophic effects in large bowel wall, increased production of SCFAs and decreased pH, which were conditions positively associated with cecal and colonic IgA secretion.

Development on the colon of fetuses and neonates in Korean native goats (한국재래산양의 태아 및 신생아의 결장 발달에 관하여)

  • The development of colon in fetuses between 60, 90, 120 days of gestation and neonates of Korean native goats was investigated by light scanning electron microscopy. The results were summarized as follows : 1. The colonic wall appeared to be differentiated into the epithelium, lamina propria, submucosa, tunica muscularis and serosa in 60-day-old fetus. 2. The epithelium of the colonic villi was stratified in some areas and simple columnar in others at 60- and 90day-old fetus but was only observed simple columnar epithelium at 120-day-old fetus. 3. The goblet cells and the intestinal glands appeared in the colonic mucosa at 90 days of gestation and continued gradually to increase in number during gestation. 4. The well-developed villi of the colon appeared in columnar shape at 60 days of gestation and increased in length and width until 90 days of gestation but the villi appeared to be shorter and degenerated after 120 days of gestation. At birth only the rudiment trace of the villi remained. 5. The tunical muscularis of the colon was continuously developed during gestation period. The inner circular and outer longitudinal muscle layers were distinguishly observed in the colon of 90-day-old fetuses. 6. Scanning electron microscopically, the colonic villi developed in columnar shape or finger-like of fetuses at 60 days of gestation. The of the colonic villi became degenerated after 120 days of gestation. At birth only the rudiment trace of villi remained.

Mechanisms of Motility Change on Trinitrobenzenesulfonic Acid-Induced Colonic Inflammation in Mice

  • Ulcerative colitis is an inflammatory bowel disease (IBD) characterized by recurrent episodes of colonic inflammation and tissue degeneration in human or animal models. The contractile force generated by the smooth muscle is significantly attenuated, resulting in altered motility leading to diarrhea or constipation in IBD. The aim of this study is to clarify the altered contractility of circular and longitudinal smooth muscle layers in proximal colon of trinitrobenzen sulfonic acid (TNBS)-induced colitis mouse. Colitis was induced by direct injection of TNBS (120 mg/kg, 50% ethanol) in proximal colon of ICR mouse using a 30 G needle anesthetized with ketamin (50 mg/kg), whereas animals in the control group were injected of 50% ethanol alone. In TNBS-induced colitis, the wall of the proximal colon is diffusely thickened with loss of haustration, and showed mucosal and mucular edema with inflammatory infiltration. The colonic inflammation is significantly induced the reduction of colonic contractile activity including spontaneous contractile activity, depolarization-induced contractility, and muscarinic acetylcholine receptor-mediated contractile response in circular muscle layer compared to the longitudinal muscle layer. The inward rectification of currents, especially, important to $Ca^{2+}$ and $Na^+$ influx-induced depolarization and contraction, was markedly reduced in the TNBS-induced colitis compared to the control. The muscarinic acetylcholine-mediated contractile responses were significantly attenuated in the circular and longitudinal smooth muscle strips induced by the reduction of membrane expression of canonical transient receptor potential (TRPC) channel isoforms from the proximal colon of the TNBS-induced colitis mouse than the control.

Effects of a soluble dietary fibre NUTRIOSE$^{(R)}$ on colonic fermentation and excretion rates in rats

  • The resistant dextrin NUTRIOSE$^{(R)}$, developed from starch, is expected to act as a prebiotic. The aim of this study was to determine the effects of NUTRIOSE$^{(R)}$ on cecal parameters, short-chain fatty acid (SCFA) concentrations, and fecal excretion in rats. In an initial experiment, twenty-four male Fischer F344 rats were randomly assigned to one of the following four treatments for 14 days: G0 (control diet), G2.5 (control diet+2.5% of dextrin), G5 (control diet + 5% of dextrin), and G10 (control diet + 10% of dextrin). After 14 days, total cecal weight, cecal content, and cecal wall weight were significantly increased in G5 and G10 compared to G0. At the same time, cecal pH was significantly lower in G10 compared to G0. Total SCFA concentration was significantly higher in G10 than in G5, G2.5, and G0, and significantly higher in G5 than in G0. Acetate, butyrate, and propionate concentrations were significantly increased in G5 and G10 compared to the controls. In a second trial based on a similar design, eighteen male Fischer F344 rats were treated with a control diet supplemented with 5% of dextrin or 5% of fructo-oligosaccharide. The results obtained with NUTRIOSE$^{(R)}$ were similar to those obtained with the fructo-oligosaccharide. In a third experiment, two groups of 5 Fischer F344 rats were orally treated with 100 and 1,000 mg/kg NUTRIOSE$^{(R)}$, respectively, and from 18% to 25% of the dextrin was excreted in the feces. The results of these three studies show that the consumption of NUTRIOSE$^{(R)}$, by its effects on total cecal weight, cecal content, cecal wall weight, pH, and SCFA production, could induce healthy benefits since these effects are reported to be prebiotic effects.

Available phosphorus levels modulate gene expression related to intestinal calcium and phosphorus absorption and bone parameters differently in gilts and barrows

  • Objective: Dietary phytase increases bioavailability of phytate-bound phosphorus (P) in pig nutrition affecting dietary calcium (Ca) to P ratio, intestinal uptake, and systemic utilization of both minerals, which may contribute to improper bone mineralization. We used phytase to assess long-term effects of two dietary available P (aP) levels using a one-phase feeding system on gene expression related to Ca and P homeostasis along the intestinal tract and in the kidney, short-chain fatty acids in stomach, cecum, and colon, serum, and bone parameters in growing gilts and barrows. Methods: Growing pigs (37.9±6.2 kg) had either free access to a diet without (Con; 75 gilts and 69 barrows) or with phytase (650 phytase units; n = 72/diet) for 56 days. Samples of blood, duodenal, jejunal, ileal, cecal, and colonic mucosa and digesta, kidney, and metacarpal bones were collected from 24 pigs (6 gilts and 6 barrows per diet). Results: Phytase decreased daily feed intake and average daily gain, whereas aP intake increased with phytase versus Con diet (p<0.05). Gilts had higher colonic expression of TRPV5, CDH1, CLDN4, ZO1, and OCLN and renal expression of TRPV5 and SLC34A3 compared to barrows (p<0.05). Phytase increased duodenal expression of TRPV5, TRPV6, CALB1, PMCA1b, CDH1, CLDN4, ZO1, and OCLN compared to Con diet (p<0.05). Furthermore, phytase increased expression of SCL34A2 in cecum and of FGF23 and CLDN4 in colon compared to Con diet (p<0.05). Alongside, phytase decreased gastric propionate, cecal valerate, and colonic caproate versus Con diet (p<0.05). Phytase reduced cortical wall thickness and index of metacarpal bones (p<0.05). Conclusion: Gene expression results suggested an intestinal adaptation to increased dietary aP amount by increasing duodenal trans- and paracellular Ca absorption to balance the systemically available Ca and P levels, whereas no adaption of relevant gene expression in kidney occurred. Greater average daily gain in barrows related to higher feed intake.

Two Cases of Perforated Typhlitis in Acute Lymphocytic Leukemia (급성 림프구성 백혈병에 합병된 천공성 typhlitis)

  • The authors, over the last 6 months, have treated 2 patients with perforated typhlitis complicating acute lymphocytic leukemia (ALL) with good outcome. The first patient was a 13-year-old male who developed intermittent high fever, abdominal pain, abdominal distention and diarrhea during the course of maintenance chemotherapy. The peripheral leukocyte ranged from 230-470/$mm^3$. Serial ultra sonograms and CT scans demonstrated irregular thickening of the cecal and ascending colonic walls and subsequent ragged perforation of the posterior wall of the cecum. He survived after treatment by right hemicolectomy and aggressive supportive measures. The patient case was a 3 year-old female who developed intermittent high fever, right lower abdominal pain, a mass, and watery diarrhea during the course of maintenance chemotherapy. Serial ultra sonograms and CT scans demonstrated irregular thickening of the cecal wall (6-15mm in thickness) and subsequent small perforation of the posterior wall of the cecum with thick-walled localized abscess. She has recovered completely after aggressive medical management. We learned two lessons from our experience treating these patients:1) early diagnosis provided by a high index of suspicion and the use of ultra sonogram or CT scan is essential. And 2) although perforation is one of the surgical indications for the treatment of typhlitis, it is possible to manage the perforation nonoperatively in selected cases with localized abscess.

A Case Report of Colonic Mucinous Adenocarcinoma in 27 Year Old Patient (27세 남자환자에서 발견된 대장의 점액선암종 1례)

  • Mucinous adenocarcinoma occurs in 1.6-25.4% of patients with colorectal cancer. We report a case of a 27-year-old man with negative findings on initial colonoscopic biopsy, but finally diagnosed with mucinous adenocarcinoma of the colon. After undergoing an abdominal CT due to persistent abdominal pain, he was transferred to our hospital. The abdominal CT showed a diffuse and irregular wall thickening in the distal transverse colon. Due to the edema and stenosis of colonic wall, it was difficult to insert the colonoscope into the proximal region; a biopsy revealed chronic colitis with lymphofollicular hyperplasia. Transverse colectomy and lymph node dissection were performed. The diagnosis was mucinous adenocarcinoma of approximately 20×4.5 cm. Compared to adenocarcinoma, mucinous adenocarcinoma is found in a younger population with an advanced stage and is less responsive to palliative chemotherapy. Therefore, recalcitrant abdominal pain even in young people warrants early detection through appropriate examinations such as abdominal CT and colonoscopy.

A Case of Metastatic Ampulla of Vater Cancer Achieving Cure (고형물 삼킴장애로 내원한 환자 1례)

  • We report a case with dysphagia for solids. A 51-year-old man with benign esophageal stricture was transferred for endoscopic treatment. He had lye ingestion history at 9 years old and underwent esophagectomy with right colonic interposition for the treatment of the benign esophageal stricture. But his symptom was acting up 2 years ago and lasted afterward even though he had underwent endoscopic treatments for dysphagia several times, including balloon dilation and stent insertion. He had polypoid enhancing wall thickening around anastomosis site of stomach with perigastric soft tissue density and suspicious nodular extension to omentum on the small bowel computed tomography. So he had a surgical resection of small bowel and jejunojejunostomy, and the pathological result was adenocarcinoma, intestinal type with soft tissue infiltration. Later he underwent total gastrectomy with segmental resection of interpositional colon and segmental resection of duodenum and ileo-colic anastomosis revision. And recently he has been on chemotherapy.

MR Images of Bowel Wall Thickening in Patients with Minimal to Moderate Cirrhosis: Comparison with Clinical Findings (경증과 중등도 간경화 환자에서 장관 비후의 MR 영상: 임상소견과의 비교)

  • Purpose : To evaluate the MR imaging findings of bowel wall thickening in patients with minimal to moderate cirrhosis, and analyze their clinical significances comparing with laboratory findings. Materials and Methods : We assessed retrospectively the MRI findings of 123 patients with minimal to moderate cirrhosis, and compared these with the clinical laboratory findings. We evaluated the involved sites and MR image findings of thickened bowel wall, as well as the presence of collateral vessels, ascites, and splenic size. These were compared with serum albumin and bilirubin levels, and prothrombin time. Results : Gastrointestinal wall thickening was detected at 37 sites in 25 patients (20%), and more frequently detected in moderate cirrhosis (29%) than in minimal cirrhosis (17%). Jejunum and ascending colon were the most common sites of bowel wall thickening; each was involved at 22 and 9 sites, respectively. Ascending colonic wall thickening was more commonly detected in moderate cirrhosis than in minimal cirrhosis. The thickened bowel wall showed symmetric contour, high signal intensity on T2-weighted images, mixed iso- and low signal intensity on T1-weighted images, and homogeneous or target-like enhancement. Serum albumin level was significantly lower in patients with bowel wall thickening ($3.3{\pm}0.9$ g/dl vs. $3.9{\pm}0.7$ g/dl; p=0.0024). Serum bilirubin level was significantly higher in patients with bowel wall thickening ($1.7{\pm}1.0$ mg/dl vs. $1.4{\pm}1.2$ mg/dl; p=0.0160). Bowel wall thickening did not significantly correlate with the presence of collateral vessels, ascites, splenic size, and prolongation of prothrombin time. Conclusion : In minimal to moderate cirrhosis, the MR imaging evaluation of bowel wall thickening was useful for estimating the severity of cirrhosis and laboratory findings.
