A Study on the Pattern of 'Heat Entering The Blood Chamber' (열입혈실증(熱入血室證)에 대한 소고(小考))

  • Objective : One of the unique syndromes in Asian traditional medicine named 'heat entering the blood chamber(熱入血室, HEBC)' first appeared in Shanghanlun(傷寒論) and Jinguiyaolue(金匱要略) written by Zhangzhongjing(張仲景) who is the most famous doctor in ancient China. Method : Through comparison between Huangdineijing(黃帝內經), Shanghanlun(傷寒論), Jinguiyaolue(金匱要略) and other medical texts, the correct meaning, causes and mechanisms of HEBC can be analysed and organized to suggest new view of HEBC in modern society. Result : This syndrome is usually occurred in women during catching cold and menstruation, accompanying alternation of chillness and fever, pseudo-malaria, delirium, raveled chest(結胸), uterine hemorrhage, etc. The main sign of this syndrome, delirium belongs to the category of liver disease and fever in Huangdineijing(黃帝內經) which is a document more early published than Shanghanlun. Although there are still many other comprehensions about what blood chamber is, it could be the same as uterus according to Huangdineijing, it is relevant to the control of menstruation and emotions, and the function of liver and thoroughfare vessel(衝脈). Conclusion : HEBC is a syndrome exclusive to women, caused by their unique physical and psychological characteristics. It's beginning can be found in Huangdineijing, and by Shanghanlun and Jinguiyaolue, its concept as a single disease pattern becomes established. In other words, HEBC is a complex disease related to menstruation and its related hormonal dysfunctions, closely related to PMS, menopausal syndrome of today. Physical symptoms accompanied by psychological anxiety and fear is characteristic of this condition. Therefore gynecological approaches as well as socio-cultural issues related to women in modern society must be adopted when dealing with HEBC.

The Relationships between Type of Localized Fat and Traditional Diagnostic Method in Korean High-school girls (서울지역 여고생의 국소지방 침착의 양상과 변증과의 연관성)

  • Background : There are many korean women who have localized fat despite of they have normal or slightly over weight. The relationships of traditional diagnostic method with total body fat and body mass index had been reviewed in other study, but there is no study regarding the relationship of traditional diagnostic method and localized fat. So we designed this study to find out the relationships between localized fat and traditional diagnostic method among Korean high-school girls. Methods : This research was performed in 21 high-school girls with localized fat during April 2007. We measured body mass index, percent of body fat, waist circumference, hip circumference and means of both thigh circumference. Phlegm questionnaire, blood stasis questionnaire, cold-heat questionnaire, Korean eating attitude test-26 and questionnaire of Sasang constitution classification Ⅱ+ have been administered. Results : There were significant correlations between body mass index, percent of body fat and body size. Regardless of body mass index, they had high phlegm and heat pattern. Most of them are classified as Tae-um type person. Conclusions : High-school girls with localized fat had pattern of high phlegm, heat and characteristics of Tae-um person.

A Literature Study on the Combination of acupoints of HANGYEOK and YEOLGYEOK in the Saam Acupuncture Method (사암침법 한격과 열격의 경혈구성에 관한 고찰)

  • Objectives: The objective of this study is to review the methods of combining acupoints of HANGYEOK(Cooling Mode) and YEOLGYEOK(Heating Mode) in Saam Acupuncture Method. Methods: We searched and investigated the journals and literatures by OASIS(Korea Institute of Oriental Medicine) and relevant searching engines with the key word "Saam, Hangyeok, Yeolgyeok". Unfortunately there was no relevant articles on this theme except six literatures. Six literatures with the key words relevant to Saam and acupuncture were selected and analyzed according to each composition of acupuncture prescription especially focusing on HANGYEOK and YEOLGYEOK. Results: Analyzing HANGYEOK and YEOLGYEOK of 12 meridians, two different methods emerged. In HANGYEOK to treat Heat Pattern, one method is to tonify Water points and directly sedate Fire points. The other is also to tonify Water point, but sedate Earth points instead of Fire points. It is assumed that Earth sedation method is to eradicate the underlying Fire. In only Heart meridian and Pericardium meridian there is one method to treat Heat Pattern: Water tonification and Fire Sedation. In YEOLGYEOK to treat Cold Pattern, there is one method to tonify Fire points and sedate Water points. In Pericardium meridian and Triple Energizer meridian there are some different combination of acupoints depending on literatures. Conclusions: Considering HANGYEOK and YEOLGYEOK in Saam Acupuncture Method, two methods are mainly discussed in HANGYEOK; Water Tonification Fire Sedation and Water Tonification Earth Sedation. To remove the underlying Fire, it is regarded that Water Tonification Earth Sedation is appropriate and close to Saam's thought.

A Study of the Case Record on Dyspnea and Wheezing Asthma Recorded in Xu Ming Yi Lei An ((${\ll}$속명의류안(續名醫類案)${\gg}$에 기재(記載)된 천(喘) 및 효천(哮喘)에 관(關)한 의안(醫案) 연구(硏究))

  • Objectives : Select and analyze the case record of dyspnea and wheezing asthma recorded in Xu Ming Yi Lei An that is the most abundant and wide in contents in existing case records that are systematic, comprehending relatively modern Traditional Chinese Medicine to secure more deep and objective basis of Traditional Chinese Medicine approach for dyspnea and wheezing asthma to analyze and review possibility for clinical application in this study. Methods: The study was conducted with the case records of dyspnea and wheezing asthma in whole Xu Ming Yi Lei An. Pattern identify and classify selected case records and again classified with deficiency syndrome and excess syndrome. Also analyzed prescriptions and herbs used in the case records. Nature of herbs and properties and flavors that were used in the case records were classified and frequency of each nature of herbs were analyzed. Applicable case records were interpreted and suggested prescriptions, pulse feelings, pattern classification were analyzed and described. Results : Among the 5254 case records stated on the complete collection, it is researched that there are 63 case records for the symptom complex of dyspnea as 1.2% of the whole case records, and the case records on the symptom complex of wheezing asthma are 14 as the 0.27% of the total examples. 63 case record examples related with symptom complex of dyspnea were pattern identified and classified. As a result, deficiency syndrome of the Kidney(33 %), deficiency syndrome of the Spleen(26.0%), Wind-Cold(12.3%), phlegm turbidity(12.3%), Heat in the Lung(8.2%), asthenia of the Lung(8.2%) were investigated as above order. 14 case record examples related with wheezing asthma were pattern identified and classified. As a result, phlegm-Heat(26.3%), upper excess and lower deficiency(26.3%), external affections Wind-Cold(15.8%), Dampness-phlegm(10.5%), Lung asthenia(10.5%), Cold phlegm(5.3%), mutual deficiency and detriment of Heart and Kidneys(5.3%) were investigated as above order. Symptom complex of dyspnea has 67.1% of deficiency syndrome, 32.9% of excess syndrome resulting more deficiency syndrome than excess syndrome. Symptom complex of wheezing asthma has 42.1 % of deficiency syndrome and 57.9% of excess syndrome resulting more excess syndrome than deficiency syndrome. In case of symptom complex of dyspnea prescription used in the case record, the order of frequency is as following. Palmijihwang-tang, Bojung-ikgitang, Yungmijihwang-tang, Ijintang, Sojaganggitang, Igongsan. In case of symptom complex of wheezing asthma prescription in the case record, Yungmijihwang-tang, Ohotang, Dodamtang were mostly used. Herbs used in case records of symptom complex of dyspnea are Ginseng Radix, Poria, Glycyrrhizae Radix, Aconiti Iateralis Preparata Radix, Atractylodis Macrocephalae Rhizoma, Dioscoreae Rhizoma, Angelicae Gigantis Radix, Rehmanniae Radix Preparat, Pinelliae Rhizoma, Zingiberis Rhizoma Recens are mostly used. Nature of herb properties used for symptom complex of dyspnea and symptom complex of wheezing asthma are herbs that are warm properties. When the symptom complex of dyspnea and the symptom complex of wheezing asthma were treated. if the patient felt tenderness at Pyesu, doctors conducted pricking blood around the opposite Pyesu or Sipseon acupoint. when the patient didn't have tenderness at Pyesu by soft press, pricking blood was performed both sidees, right and left Pyesu. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, when they got treatment, when the symptom complex of disease is severe, a doctor cauterized the opposite Pyesu while the other Pyesu felt tenderness, and decided how the above treatment is performed whether the degree of the symptom compolex of disease is severe or not. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, if the person felt tenderness at Pyesu and is caught by the Wind-Cold pathogen, slight acupuncture is treated at relevant Pyesu with Fire needling. When patient with symptom complex of dyspnea and symptom complex of wheezing asthma cannot hawk sputum up from the oral and laryngopharynx, suction method is treated. Conclusion : With this study, actual traditional and clinical pattern identification form and characteristics of symptom complex of dyspnea and symptom complex of wheezing asthma were recognized. Modern case report utilizing in clinical application need to be secured and an incurable disease asthma need to be diagnosed and improvement for treatments have to be searched through other case records.

CD206+ dendritic cells might be associated with Heat-pattern and induced regulatory T cells after treatment with bee venom

  • Objectives: Bee venom (BV) is a widely used therapy in Traditional East Asian Medicine (TEAM). We previously reported that BV was clinically effective for treating Parkinson's disease, that phospholipase A2 (PLA2) was the main component of BV, and that it induced regulatory T cells (Tregs) by binding CD206 on dendritic cells (DCs). Therefore, we aimed to reconfirm our findings in human blood samples and investigate the relationship between CD206+ DCs and clinical syndrome differentiation in TEAM. Methods: We surveyed 100 subjects with questionnaires on cold-heat patternization and obtained their blood samples. The obtained human peripheral blood monocytes (hPBMCs) were washed with phosphate-buffered saline (PBS). After resuspension with ex vivo media, numbers of cells were counted. Tregs were counted after culturing the samples in a 37℃ CO2 incubator for 72 h. Results: We divided the subjects into a relatively high CD206+ group or a relatively low CD206+ group. The heat factor scores of high CD206+ group were significantly higher than that of low CD206+ group (high vs low: 239.2 ± 54.1 vs 208.4 ± 55.1, p=0.023). After culturing with PLA2, Tregs increased in the high CD206+ group but decreased in the low CD206+ group. Conclusion: In this study, we reconfirm that CD206+ DCs induced Treg differentiation by incubating human blood samples with PLA2 and that they showed an association with syndrome differentiation, especially with heat patterns, in TEAM. A heat pattern in TEAM might be one indication for PLA2 therapy because its score was elevated in the high CD206+ group.

Efficacy and Safety of Blood Stasis Based Herbal Medicine for Patients with Traffic Accident : A Prospective Observational Study

  • Background: Blood stasis (BS) is commonly used for pattern identification in traumatic injuries, including traffic accidents (TAs). Various studies have identified the efficacy of Korean medicine treatments for TA patients, but studies focusing on the BS-based herbal medicine (BSHM), including Tongdo-san (TDS), are rare. Methods: This was a single-center, prospective observational study, conducted from August 24th, 2018 to December 27th, 2018, which included 40 TA patients. Participants underwent routine Korean medicine treatments including acupuncture, electronic moxibustion, cupping, physical therapy, and herbal medicine. In the herbal medicine treatment, participants took BSHM with more than 3 days including taking TDS. The primary outcome measures were the scores from a 100 mm visual analogue scale (VAS) and numerical rating scale (NRS). Secondary outcome measures included scores from EuroQol-5 dimension (EQ-5D) and EQ-VAS questionnaires, the BS and cold/heat indices, and safety assessments. Results: There were significant improvements in the VAS, NRS, EQ-5D, EQ-VAS scores, and BS index after treatment. In the sub-analysis, VAS, NRS, EQ-5D, and EQ-VAS scores were higher in groups with a higher BS index. The moderate and severe BS index groups showed more improvement than the minor BS index group, and there was a significant difference in the EQ-5D scores. There was no significant differences observed in cold/heat index groups scores. Conclusion: BS is associated with TA-related symptoms. BSHM, including TDS, may significantly reduce BS, pain, and discomfort.

Study on the mixing performance of mixing vane grids and mixing coefficient by CFD and subchannel analysis code in a 5×5 rod bundle

  • Mixing Vane Grid (MVG) is one of the most important structures in fuel assembly due to its high performance in mixing the coolant and ultimately increasing Critical Heat Flux (CHF), which avoids the temperature rising suddenly of fuel rods. To evaluate the mixing performance of the MVG, a Total Diffusion Coefficient (TDC) mixing coefficient is defined in the subchannel analysis code. Conventionally, the TDC of the spacer grid is obtained from the combination of experiments and subchannel analysis. However, the processing of obtaining and determine a reasonable TDC is much challenging, it is affected by boundary conditions and MVG geometries. In is difficult to perform all the large and costing rod bundle tests. In this paper, the CFD method was applied in TDC analysis. A typical 5 × 5 MVG was simulated and validated to estimate the mixing performance of the MVG. The subchannel code was used to calculate the TDC. Firstly, the CFD method was validated from the aspect of pressure drop and lateral temperature distribution in the subchannels. Then the effect of boundary conditions including the inlet temperature, inlet velocities, heat flux ratio between hot and cold rods and the arrangement of hot and cold rods on MVG mixing and TDC were studied. The geometric effects on mixing are also carried out in this paper. The effect of vane pattern on mixing was investigated to determine which one is the best to represent the grid's mixing performance.

Classification of Weather Patterns in the East Asia Region using the K-means Clustering Analysis (K-평균 군집분석을 이용한 동아시아 지역 날씨유형 분류)

  • Medium-range forecast is highly dependent on ensemble forecast data. However, operational weather forecasters have not enough time to digest all of detailed features revealed in ensemble forecast data. To utilize the ensemble data effectively in medium-range forecasting, representative weather patterns in East Asia in this study are defined. The k-means clustering analysis is applied for the objectivity of weather patterns. Input data used daily Mean Sea Level Pressure (MSLP) anomaly of the ECMWF ReAnalysis-Interim (ERA-Interim) during 1981~2010 (30 years) provided by the European Centre for Medium-Range Weather Forecasts (ECMWF). Using the Explained Variance (EV), the optimal study area is defined by 20~60°N, 100~150°E. The number of clusters defined by Explained Cluster Variance (ECV) is thirty (k = 30). 30 representative weather patterns with their frequencies are summarized. Weather pattern #1 occurred all seasons, but it was about 56% in summer (June~September). The relatively rare occurrence of weather pattern (#30) occurred mainly in winter. Additionally, we investigate the relationship between weather patterns and extreme weather events such as heat wave, cold wave, and heavy rainfall as well as snowfall. The weather patterns associated with heavy rainfall exceeding 110 mm day-1 were #1, #4, and #9 with days (%) of more than 10%. Heavy snowfall events exceeding 24 cm day-1 mainly occurred in weather pattern #28 (4%) and #29 (6%). High and low temperature events (> 34℃ and < -14℃) were associated with weather pattern #1~4 (14~18%) and #28~29 (27~29%), respectively. These results suggest that the classification of various weather patterns will be used as a reference for grouping all ensemble forecast data, which will be useful for the scenario-based medium-range ensemble forecast in the future.

A Study on the Relationships between Edema Index of Body Composition and Pathologic Patterns (체성분 분석의 부종지수와 변증설문과의 상관성 연구)

  • Objectives The purpose of this study was to find out whether if there is correlation between the pattern differentiation questionnaire and edema index of body composition analysis. Methods The authors obtained results of pattern differentiation Questionnaires and body composition analysis tests from 195 female patients who visited the Oriental gynecology department in Kyung Hee university Oriental medicine hospital at Gang-dong. The authors conducted correlation analysis between indicators of Body composition analysis and each part of the pattern differentiation Questionnaire. Pearson correlation analysis was performed on SPSS version 13.0 for windows. Results There was negative correlation between the Phlegm Questionnaire and right arm ECF, left arm ECF, and left arm ECW of the Body composition index. There was no correlation between Yin-deficiency Questionnaire and Body composition index. There was no correlation between Heat pattern Questionnaire and Body composition index. There was positive correlation between the Cold pattern Questionnaire and right leg ECF, and right leg ECW of the body composition index. There was no correlation between degree of edema self-awareness and Body composition index. Conclusions The result showed that each Questionnaire score has different correlation or even no correlation with Body composition index and degree of edema self-awareness. Further studies are needed for a better understanding and interpretation of the relationship between edema index on Body composition analysis and Questionnaires.

A Study on the Operating Characteristics by Counter Flow and Parallel Flow in Separate Heat Pipe Exchanger (분리형 히트파이프식 열교환기에서 향류 및 병류유동에 따른 가동특성에 관한 연구)

  • Separate heat pipe exchanger is considered as the high thermal transportation equipment, because evaporator and condenser are separately positioned in the long distance. Its characteristics are that the working fluid is circulated naturally by the position height of two exchangers. But the operating characteristics are restricted by the temperature of hot and cold fluid, flow pattern and diameter of vapor line, etc. in this study, the vapor pressure and the minimum height of two exchangers are studied about the factors restricting the operating characteristics.
