Low Counts of γδ T Cells in Peritumoral Liver Tissue are Related to More Frequent Recurrence in Patients with Hepatocellular Carcinoma after Curative Resection

  • Objectives: TCR-gamma-delta+T cells (${\gamma}{\delta}$ T cells) are non-conventional T lymphocytes that can recognize and eradicate tumor cells. Our previous studies showed that infiltration and function of ${\gamma}{\delta}$ T cells were substantially attenuated in hepatocellular carcinoma (HCC). However, their prognostic value was not clarified. Methods: The association between ${\gamma}{\delta}$ T cells and the clinical outcomes was determined by immunohistochemistry (IHC) in a HCC patient cohort (n = 342). Results:Immunohistochemistry showed decreased infiltration of ${\gamma}{\delta}$ T cells in tumoral tissues compared with paired peritumoral tissues. The counts of ${\gamma}{\delta}$ T cells in peritumoral tissues were negatively correlated with tumor size (P = 0.005). Survival analysis showed that the levels of peritumoral ${\gamma}{\delta}$T cells were related to both time to recurrence (TTR) and overall survival (OS) (P = 0.010 and P = 0.036, respectively) in univariate analysis, and related to TTR in multivariate analysis (P = 0.014, H.R. [95% CI] = 0.682 [0.502-0.927]). Furthermore, the level of peritumoral ${\gamma}{\delta}$ T cells showed independent prognostic value for TTR in Barcelona Clinic Liver Cancer (BCLC) stage A patients (P = 0.038, H.R. [95% CI] = 0.727 [0.537-0.984]). However, tumoral ${\gamma}{\delta}$ T cells did not show independent prognostic value for either TTR or OS in HCC patients. Conclusions: Low counts of ${\gamma}{\delta}$ T cells in peritumoral liver tissue are related to a higher incidence of recurrence in HCC and can predict postoperative recurrence, especially in those with early-stage HCC.

A Time-series Study on Relationship between Visibility as an Indicator of Air Pollution and Daily Respiratory Mortality (대기오염 지표로서의 시정과 일별 호흡기계 사망간의 연관성에 관한 시계열적 연구)

  • There seems to be a consensus among most people that visibility impairment is the most obvious indicator of air pollution. While considerable evidence on the association between air pollution and health outcomes including death and disease have been established, based on industrial complex areas or huge urban cities, time-series, case-crossover and cohort studies, scarce literature exists on the direct evidence for the association between visibility and adverse health outcomes. Our study is assessed the effect of air pollution measured by visibility impairment on respiratory mortality over a period of six years. Relative risks in respiratory deaths were estimated by a Poisson regression model of daily deaths between $1999{\sim}2004$. Daily counts of respiratory deaths as dependent variable was modelled with daily 24-hr mean visibility measurements (kilometers) as independent variable by means of Poisson regression. This model is controlled for confounding factors such as day of weeks, weather variables, seasonal variables and $PM_{10}$. The results in this study is observed the statistically significant association between an inverse health effect and visibility during the study period for respiratory mortality (percentage change in the relative risk for all aged -0.57%, 95% Cl, $-1.01%{\sim}-0.12%$; for $0{\sim}15$ aged -7.12%, 95% Cl, $-13.29%{\sim}-0.51%$; for 65+ aged -0.43%, 95% Cl, $-0.93%{\sim}-0.06%$ per 1 km increased in visibility). The effect size was much reduced during warm season. Visibility impairment resulting from air pollution is strongly associated with respiratory mortality, especially for children may be spent at outdoor. Our result provides a quick and useful indicator for eliciting the contribution of air pollution to the excess risk of respiratory mortality in Seoul, Korea.

The relationship of dietary sodium, potassium, fruits, and vegetables intake with blood pressure among Korean adults aged 40 and older

  • BACKGROUND/OBJECTIVES: The inverse relationships of combined fruits and vegetables intake with blood pressure have been reported. However, whether there are such relationships with salty vegetables has rarely been investigated in epidemiologic studies. We evaluated the relation of combined and separate intake of fruits, vegetable intakes, and salty vegetables, as well as sodium and potassium, with blood pressure among the middle-aged and elderly populations. SUBJECTS/METHODS: The present cross-sectional analysis of a prospective cohort baseline survey was performed with 6,283 subjects (2,443 men and 3,840 women) and free of hypertension, diabetes, cardiovascular diseases, and cancer. Dietary data were collected by trained interviewers using food frequency questionnaire. RESULTS: The significantly inverse linear trend of diastolic blood pressure (DBP) was found in fruits and non-pickled vegetables (81.2 mmHg in the lowest quintile vs 79.0 mmHg in the highest quintile, P for trend = 0.0040) and fruits only (80.9 mmHg in the lowest quintile vs 79.4 mmHg in the highest quintile, P for trend = 0.0430) among men. In contrast, sodium and sodium to potassium ratio were positively related with blood pressure among men (DBP, 78.8 mmHg in the lowest quintile vs 80.6 mmHg in the highest quintile, P for trend = 0.0079 for sodium; DBP, 79.0 mmHg in the lowest quintile vs 80.7 mmHg in the highest quintile, P for trend = 0.0199 and SBP, 123.8 mmHg in the lowest quintile vs 125.9 mmHg in the highest quintile for sodium/potassium). Kimchies consumption was positively related to DBP for men (78.2 mmHg in the lowest quintile vs 80.9 mmHg in the highest quintile for DBP, P for trend = 0.0003). Among women, these relations were not found. CONCLUSION: Fruits and/or non-pickled vegetables may be inversely, but sodium, sodium to potassium, and Kimchies may be positively related to blood pressure among men.

Association of Interleukin 10 Haplotype with Low Bone Mineral Density in Korean Postmenopausal Women

  • Osteoporosis is a disease characterized by exaggerated loss of bone mass, with as much as 50 to 85% of the variation in bone mineral density (BMD) commonly accepted as being genetically determined. Although intensive studies have attempted to elucidate the genetic effects of polymorphisms on BMD and/or osteoporosis in several genes, the genes involved are still largely unknown. The possible associations of genetic variants in five-candidate genes (IL10, CCR3, MCP1, MCP2 and GC) with spinal BMD were investigated in Korean postmenopausal women (n = 370). Fourteen SNPs in five candidate genes were genotyped, and the haplotypes of each gene constructed. The associations of adjusted spinal BMD by age, year since menopause (YSM) and body mass index (BMI), with genetic polymorphisms, were analyzed using multiple regression models. Genetic association analysis of Korean postmenopausal women revealed that IL10 -592A > C and/or IL10 ht2 were associated with decreased bone mass, whereas no significant associations were observed with all polymorphisms in other genes. The levels of spinal BMD in individuals bearing the IL10 -592CC genotype were lower ($0.78{\pm}0.16$) than those in others ($0.85{\pm}0.17$) (P = 0.02), and the BMD of IL10 ht2 bearing individuals were also lower ($0.82{\pm}0.15$) than those in others ($0.85{\pm}0.17$) (P = 0.04). Our results suggest that variants of IL10 might play a role in the decreased BMD, although additional study might need to be followed-up in a more powerful cohort.

Study about the Association between Diabetes and the Targeted SNPs of TCF7L2 and FTO Genes (당뇨병에서 TCF7L2와 FTO 유전자의 특정 단일염기다형성과의 연관성 연구)

  • Diabetes is a disease that contains a high concentration of glucose in blood and due to defects in either insulin secretion or insulin action. Although the distinctive causes and factors of diabetes have not been clarified, the genetic factors are suggested as a main susceptibility until now. SNP (Single Nucleotide Polymorphism), as the most common genetic variation, has an influence on personal susceptibility for diseases. A nonsynonymous SNP, which changes the amino acid of the protein and its function, is especially important. Therefore, this study hypothesized that there are associations between specific SNPs of the targeted genes. Transcription factor 7-like 2 (TCF7L2) and fat mass and obesity associated (FTO) genes were selected as target genes from the results of genome-wide association and other related research studies. Second, four nonsynonymous SNPs (three in TCF7L2 and one in FTO gene) were selected as target SNPs by using public database of NCBI (National Center for Biotechnology Information). The recruited personnel was classified into three subgroups of diabetes, impaired fasting glucose (IFG) and normal groups. The individual genotypes of each group were analyzed by resequencing. None of genetic variations at four targeted SNP sites was revealed in all samples of this study. However, this study found two new SNPs that were not reported in TCF7L2 gene. One is synonymous SNP, which is heterozygous of C/T and no amino acid change of asparagine/asparagines, was located at c1641 and found in one normal person. Another is nonsynonymous SNP, which is heterozygous of G/A, was located at c1501 and found in two samples. This new discovered nonsynonymous SNP induce the amino acid change from alanine to threonine. Moreover, this new nonsynonymous SNP was found among two persons, one of whom was a diabetes patient and the other one was a person at boundary between IFG and normal, suggesting that this variant might be associated with IFG or diabetes. Even if there is a limitation of sample number for statistical power, this study has an importance due to the discovery of new SNPs. In the future study, a large sample number of diabetes cohort will be needed to investigate the frequency and association with new discovered SNP.

Patterns of Korean Women′s Life Course (한국 여성의 생애 유형: 저출산과 M자형 취업곡선에의 함의)

  • This study aims to discover patterns of Korean women's life course in terms of their life time sequence of family roles and economic activity. Primary factors for the change and diversity of family-work role sequence are also examined. Data used in this study is the Fourth Survey of Korean Women's Economic Activity which was conducted by Korean Institute of Women Development (KIWD) in 2002. According to the main results, five distinctive patterns of life course are to be disentangled for ever married women: First, doing simultaneously family and work roles with no maternal leave (13.7%); second, reentry into labor market after maternal leave (M type, 18.6%); third, no reentry into labor market after maternal leave (latent M type, 26.9%); fourth, first job entry after child rearing (23.5%); and finally, no work experience (17.3%). The relative composition of the respective life course has changed over marriage cohorts. M type including latent M type became a dominant life pattern among married women since marriage cohorts of 1980 and later. The share of married women who begin to work first after maternal role or have no work experience has declined with recent marriage cohorts. It is also noted that the share of women with simultaneous family and work roles has increased among marital cohorts of 1990 and later. Marriage cohort differences being controlled, life patterns significantly differ by women's educational level, existence of role model of working mother at growth, women's own and husbands' gender role attitude, and family economy. Finally, some policy concerns for gender role division of family and work are raised.

Analysis of trinucleotide repetitive sequences for Korean patients with spinocerebellar ataxia types 8, 12, and 17

  • Purpose: Spinocerebellar ataxias (SCAs) are progressive neurodegenerative disorders with diverse modes of inheritance. There are several subtypes of SCAs. SCA 8, SCA 12, and SCA 17 are the less common forms of SCAs with limited information available on their epidemiological profiles in Korea. The purpose of this study was to investigate the prevalence of SCA8, SCA12, and SCA17 in Korea. Materials and Methods: Ninety-six unrelated Korean patients were enrolled and showed normal trinucleotide repeats through polymerase-chain reaction (PCR) for the genes ATXN1, ATXN2, ATXN3, CACNA1A, and ATXN7, which correspond to SCA1, SCA2, SCA3, SCA6, and SCA7, respectively. PCR products from patients were further analyzed by capillary electrophoresis using fluorescence labeled primers for the genes ATXN8OS, PPP2R2B, and TBP, which correspond to SCA8, SCA12, and SCA17. Results: Three patients had 104, 97, and 75 abnormal expanded repeats in the ATXN8OS gene, the causative gene for SCA8. None of the patients exhibited abnormal repeats in SCA12 and SCA17. Normal trinucleotide repeat ranges of the cohort in this study were estimated to be 17-34 copies (average, $24{\pm}4copies$) for SCA8, 7-18 copies (average, $13{\pm}3copies$) for SCA12, and 26-43 copies (average, $35{\pm}2copies$) for SCA17. Conclusion: This study demonstrated that SCA8, SCA12, and SCA17 are rare in Korean patients with SCA, and further genetic studies are warranted to enhance the mutation detection rate in the Korean SCA population.

A Descriptive Study of The Way of Life and Health Status of Nursing Students (간호대학생들의 생활습관과 증상호소 및 관련요인)

  • The purpose of this study was to describe the way of life (such as smoking habits, alcohol consumption, exercise, duration of sleep, eating habits etc.), physical. psychological and overall health complaints and to identify the relationship between the way of life and health complaints of the nursing students. Data was collected with the self-administered OMI( Okayama Medical Index Health Questionnaire, devised Cornell Medical Index) from 766 nursing students and was analyzed. For data analysis, descriptive statistics and an ANOVA with an SPSS/PC+ program were used. The results were as follows: 1. The mean age of subjects was 21.0557(2.7618) years old in their 1st, 2nd, 3rd and 4th year-classes of nursing departments and college students in 4 regions of Korea. 62.1% of subjects were of the proper body weight, 2.0% were overweight, and 28.3% were underweight. Most subjects did not have a past history of disease, but 1.8% of subjects had experienced a blood transfusion. 2. 28.3% of the subjects had averaged 7 hours of sleep, 98.5% were nonsmokers, 91.2% were nondrinkers, 74.7% of subjects had milk intake sometimes, 53.7% of the subjects had an average salt intake, 49.6% of the subjects usually had a green vegetable intake, 45.4% of the subjects sometimes had fatty food intake, 64.8% of the subjects had good taste, 29.5% of the subjects enjoyed physical exercise. 3. The highest percentages of 'yes' among physical. psychological and overall health complaints were observed in the question 'Do you have gastric pain?'(60.2%), 'Are you confused of disorderly?' (37.2%), 'Physical body was burdensome and fatigued.'(57.3%). 4. The total O.M.I. score ranged from 1 to 89 with an average of 22.91(SD=12.10). A statistically significant difference was observed for the physical. psychological complaints for alcohol consumption. and for the psychological complaints for salt and fatty food intake. These conclusions should provide grounds for future studies of strategies for health promoting lifestyle for nursing students and their health concerns. Further research is required to investigate the life environmental and psychological factors affecting the way of life for nursing students and their health status, to analyze the health behavior of nursing students related with their culture and consciousness. Also epidemiologic research methodology, such as prospective cohort study should be considered to explore the performing process of nursing student's health behavior related with the occurrence of diseases.

Risk Factors for Allergic Diseases in School-Aged Children (알레르기 질환을 가진 초등학교 고학년의 식생활·환경적 요인 및 성장과의 관련성 분석)

  • Allergies are one of the most common chronic diseases during childhood. While the prevalence of allergic disease in children is increasing dramatically, few studies have evaluated the risk and preventive factors related to this health condition, especially in South Korea. The purpose of this case-control study was to evaluate independent determinants of allergic disease, and to examine whether it is associated with growth and other health conditions in childhood. The current study included 209 cases and 311 age and sex-matched controls (fifth and sixth graders) residing in Daegu, South Korea. Environmental data was collected, including child and family histories of allergies, birth information, and reaction to weaning foods during infancy. In addition, current dietary habits (assessed by 2 day-24 hr food records), health conditions, and anthropometric data were obtained through questionnaires and student health check-ups at the school. Based on chi-square tests, cases had a significantly higher prevalence of having a disease at birth, an allergic reaction to weaning foods, frequent hospital visits, and frequent experiences of the common cold and digestive diseases. In addition, significantly more mothers had a higher education in the cases compared to controls. Based on multivariable conditional logistic regression analysis, factors significantly associated with greater odds for having allergies were parental (OR=21.42) and fraternal (OR=14.40) histories of allergies. The anthropometric measures showed that cases tended to be shorter in height and lighter in weight than the controls, but these differences were not statistically significant. These findings may indicate that current nutrient intakes may not be the only critical factor associated with delayed growth delay in the allergic group. A well-planned, large cohort study is warranted to confirm our findings in the future.

Analysis and Application of Nursing Management Practicum Case Simulation for Developing Performance-Centered Education (성과중심 교육과정 개발을 위한 간호관리실습 사례시뮬레이션 적용 및 내용 분석)

  • The purpose of this study was to develop a nursing management case simulation (NMCS) framework based on the five components of nursing management process and to apply it to clinical nursing practice of nursing college students. The subjects of this study were NMCS reports submitted by the 4th grade 105 nursing students of an university. The research tool is a simulation framework for nursing management practice. It reflects the brainstorming and debriefing process used in the previous simulation exercise based on the five elements of planning, organization, human resource management, directing and control of the nursing management process respectively. As a result of the study, 32 nursing management cases were found to have 79.6% correct rate, 11.6% concept error rate, and 5.6% classification error rate in the first brainstorming and debriefing process for the five components of nursing management process. On the other hand, in the second brainstorming and debriefing process, 94.6% correct rate, 0.0% concept error rate, and 4.4% classification error rate. Based on these results, the NMCS framework developed in this study can be applied to the nursing management theory and practice course of nursing college students as well as simulation based job training and maintenance educations for clinical nurses. Therefore, we propose follow-up studies in various clinical nursing settings and a longitudinal cohort study to investigate the effect of nursing management job skills of nursing college students after graduation.
