A Study on Sasang Constitution Discrimination Using Body Biomarkers - Based on Korean Medicine Cohort Study (신체 생체 지표를 이용한 사상체질 판별 연구 - 한의 코호트 연구를 중심으로)

  • Objective : The study aimed to report the results of workplace-based Korean Medicine cohort, The cohort collected the health information including physical measurements, questionnaires, and biomarkers. Method : This study was conducted on 200 faculty members of two hospitals and two universities in 2017 through 2018. The study analyzed physical measurement, cold-heat questionnaire, body compositions and pulse characteristics. Result : The heat-cold score, waist circumference, visceral fat area, and body water were different according to the Sasang constitution, in order of Taeum > Soyang > Soeum. The pulse energy difference was shown only in women. Conclusion : There were differences in body composition such as heat point, waist circumference, internal fat area and body water content by constitution, and also difference by body mass in total energy in pulse examination. It is believed that there were significant relation between physical, physical and energy metabolic aspects through Sasang constitution.

Population Aging and Consumption Inequality in Korea (인구구조의 고령화와 소비격차)

  • This study aims to analyze the factors affecting consumption inequality in the 10 years following the financial crisis, applying the cohort method on the data for the first to the eleventh wave of the Korean Labor and Income Study produced by the Korean Labor Institute. The study found that consumption inequality increased rapidly immediately following the financial crisis, and then decreased gradually until increasing again from 2005 onward. Analyzed in terms of age-time-cohort effects, there was a significant change in consumption inequality around the age of mid-forties, and the decrease in consumption inequality was smaller in the younger generations than in the older ones. This suggests that as the current younger generations age over time, consumption inequality may become greater. Also, when the factors in population-cohort-age effects from 1998 onward are analyzed, the age effect in consumption inequality becomes smaller, whereas the role of the rising average age due to demographic shifts seems to be increasing. This means that consumption inequality may become a serious problem in the rapidly aging society. Therefore, there is a need to consider ways to bolster social security and to provide further public assistance in the low-income retiree.

An investigation of Residential Overcrowding of Three Asian Ethnic Groups in the US (이중생잔모형을 이용한 아시안 이민자들의 주거밀도 변화추이와 주거과밀 결정요인에 관한 연구)

  • Residential overcrowding, also called density, measured as more than one person per room, is a important variable as a principal indicator of inadequate housing. We investigated how immigrants in the US are assimilated to the host society through a lens of housing density. We estimated the probability of living in overcrowded housing of Korean, Chinese and Japanese immigrants armored with the 1980 and 1990 Public Use Microdata Sample(PUMS, A, 5%) in the US. Along with economic effects, we also consider life-cycle effects on the indicator. We applied "double cohort" method that enables duration of immigration effects to be separated from aging effects, which captures family size fluctuations due to life-cycle effects. The study found that cohort trends sharply changed during 10years. The 1970's immigrants are more likely to live in overcrowded housing than the pre-1970 immigrants. The pre-1970 immigrants are more likely to live in overcrowded housing than native-born persons. This may be explained by different assimilation processes driven by the disparities of individual human capitals or cultural differences among the ethnic groups. Especially, Korean experienced a sharp decline in overcrowding between 1980 and 1990. We also found that the major determinants that affect the level of housing density are years since migration, income, and gender. The present study concluded with some future studies related to the Korean immigrants abroad.ts abroad.

  • The timings of two successive events of interest may not be measurable, instead it may be right censored or interval censored; this data structure is called doubly censored data. In the study of HIV, two such events are the infection with HIV and the onset of AIDS. These data have been analyzed by authors under the assumption that infection time and induction time are independent. This paper investigates the regression problem when two events arc modeled to allow the presence of a possible relation between two events as well as a subject-specific effect. We derive the estimation procedure based on Goetghebeur and Ryan's (2000) piecewise exponential model and Gauss-Hermite integration is applied in the EM algorithm. Simulation studies are performed to investigate the small-sample properties and the method is applied to a set of doubly censored data from an AIDS cohort study.

Development and Validation of a Simple Index Based on Non-Enhanced CT and Clinical Factors for Prediction of Non-Alcoholic Fatty Liver Disease

  • Objective: A widely applicable, non-invasive screening method for non-alcoholic fatty liver disease (NAFLD) is needed. We aimed to develop and validate an index combining computed tomography (CT) and routine clinical data for screening for NAFLD in a large cohort of adults with pathologically proven NAFLD. Materials and Methods: This retrospective study included 2218 living liver donors who had undergone liver biopsy and CT within a span of 3 days. Donors were randomized 2:1 into development and test cohorts. CTL-S was measured by subtracting splenic attenuation from hepatic attenuation on non-enhanced CT. Multivariable logistic regression analysis of the development cohort was utilized to develop a clinical-CT index predicting pathologically proven NAFLD. The diagnostic performance was evaluated by analyzing the areas under the receiver operating characteristic curve (AUC). The cutoffs for the clinical-CT index were determined for 90% sensitivity and 90% specificity in the development cohort, and their diagnostic performance was evaluated in the test cohort. Results: The clinical-CT index included CTL-S, body mass index, and aspartate transaminase and triglyceride concentrations. In the test cohort, the clinical-CT index (AUC, 0.81) outperformed CTL-S (0.74; p < 0.001) and clinical indices (0.73-0.75; p < 0.001) in diagnosing NAFLD. A cutoff of ≥ 46 had a sensitivity of 89% and a specificity of 41%, whereas a cutoff of ≥ 56.5 had a sensitivity of 57% and a specificity of 89%. Conclusion: The clinical-CT index is more accurate than CTL-S and clinical indices alone for the diagnosis of NAFLD and may be clinically useful in screening for NAFLD.

Exploring the Psychosocial Characteristics and Systematic Support of Academically Maladjusted Students (의과대학 학업부적응 학생의 심리사회적 특성과 체계적 지원방안에 대한 탐색)

  • The purpose of this study was to explore the psychosocial characteristics of academically maladjusted students and the College's current status regarding academic maladjustment based on 4-year medical student cohort data collected from 2018 to 2021 and through interviews with 10 medical education experts using the integrated research method. This study included 223 students who consented to inclusion in the student cohort and participated in the emotional behavior test and college adaptability test of whom 65 students experienced academic maladjustment. Academically maladjusted students had significantly higher scores for social stress, anxiety, sense of inadequacy, attention, hyperactivity, and school maladjustment, and significantly lower scores for relationships with parents, interpersonal relationships, self-esteem, self-confidence, and self-strength. Academic and social integrity, emotional stability, university satisfaction, and university service satisfaction were all significantly lower in the academically maladjusted students than in the non-maladjusted group. The expert interviews indicated that academic maladjustment was mainly recognized as a personal problem, with causes such as lack of motivation and learning methods, vulnerability to stress, lack of social relationships and alienation, lack of support from the family, and insufficient resilience. Systematic support other than counseling for academically maladjusted students is required, and an early diagnosis and preventive intervention are important.

Quorum Consensus Method based on Ghost using Simplified Metadata (단순화된 메타데이타를 이용한 고스트 기반 정족수 동의 기법의 개선)

  • Replicated data that is used for fault tolerant distributed system requires replica control protocol to maintain data consistency. The one of replica control protocols is quorum consensus method which accesses replicated data by getting majority approval. If site failure or communication link failure occurs and any one can't get quorum consensus, it degrades the availability of data managed by quorum consensus protocol. So it needs for ghost to replace the failed site. Because ghost is not full replica but process which has state information using meta data, it is important to simplify meta data. In order to maintain availability and simplify meta data, we propose a method to use cohort set as ghost's meta data. The proposed method makes it possible to organize meta data in 2N+logN bits and to have higher availability than quorum consensus only with cohort set and dynamic linear voting protocol. Using Markov model we calculate proposed method's availability to analyze availability and compare it with existing protocols.

Safety and efficacy of transcutaneous bone conduction implant surgery for hearing improvement in microtia patients with bilateral hearing impairment

  • Background In microtia patients with bilateral hearing impairment, hearing improvement is crucial for language development and performance. External auditory canal reconstruction (EACR) has been performed to improve hearing, but often results in complications. We performed transcutaneous bone conduction implant (TBCI) surgery in these patients. This study aimed to evaluate the safety and efficacy of TBCI surgery. Methods A retrospective review was performed of five patients who underwent auricular reconstruction and TBCI surgery and 12 patients who underwent EACR between March 2007 and August 2018. Hearing improvement was measured based on the air-bone gap values using pure-tone audiometry over a 6-week postoperative period. We reviewed other studies on hearing improvement using EACR and compared the findings with our results. The surgical techniques for TBCI were reviewed through case analyses. Results Postoperative hearing outcomes showed a significant improvement, with a mean gain of 34.1 dB in the TBCI cohort and 14.1 dB in the EACR cohort. Both gains were statistically significant; however, the TBCI cohort showed much larger gains. Only three of the 12 patients who underwent EACR achieved hearing gains of more than 20 dB, which is consistent with previous studies. All patients who underwent TBCI surgery demonstrated hearing gains of more than 20 dB and experienced no device-related complications. Conclusions TBCI is a safe and effective method of promoting hearing gains in microtia patients with bilateral hearing impairment. TBCI surgery provided better hearing outcomes than EACR and could be performed along with various auricular reconstruction techniques using virgin mastoid skin.

The Relationship between Parkinson's Disease and Acute Myocardial Infarction in Korea : A Nationwide Longitudinal Cohort Study

  • Objective : The goal of the following statewide age and gender-coordinated cohort study in Korea is to find out if there is a link between acute myocardial infarction (AMI) and Parkinson's disease (PD). Methods : Utilizing the National Health Insurance Sharing Service cohort, patient data were collected. Six thousand four hundred seventy-five individuals with PD were distinguished by utilizing the International Classification of Diseases 10 code G20 and have enrolled in the PD group. The number of participants decreased to 5259 after excluding 1039 patients who were hospitalized less than one time or who visited an outpatient clinic less than twice. Then, 26295 individuals were selected as part of the control group after case control matching was conducted through 1 : 5 age- and gender-coordinated matching. The Cox proportional hazard regression analysis and Kaplan-Meier method were utilized to analyze the likelihood of AMI in PD. Results : After controlling for age and gender, the hazard ratio of AMI in the PD group was 3.603 (95% confidence interval [CI], 2.837-4.577). After that, the following hazard ratio of AMI in the PD group was modified against for co-morbid medical disorders, resulting in 3.551 (95% CI, 2.795-4.511). According to a subgroup analysis, in males and females aged <65 and aged ≥65 and in the non-diabetes and diabetes, hypertension and non-hypertension, dyslipidemia and non-dyslipidemia subgroups, the AMI incidence rates were dramatically higher in the PD group compared to that of the control. Conclusion : Individuals with PD have a greater chance of AMI, according to this cross-national study.

Clinically Significant Cut-off Value of the KS-15 for the Risk of Metabolic Syndrome: Using the Korean Medicine Daejeon Citizen Cohort (KDCC) Study (대전시민코호트 자료를 활용한 대사증후군에 임상적으로 유의한 KS-15의 사상체질 판별 기준)

  • Objectives The purpose of this study is to propose a method to more specifically identify Sasang constitutional risk factors of metabolic syndromes by adjusting the cut-off value of Korea Sasang Constitutional Diagnostic Questionnaire (KS-15). Methods Data of 1997 participants in Korean medicine Daejeon Citizen Cohort study (KDCC) were analyzed. Metabolic syndrome was defined according to the NCEP-ATP III, lifestyle information, and hematologic information including KS-15 and demographic characteristics were used as covariates. Results The 179 subjects with metabolic syndrome accounted for 9.0% of the total. As a result of determining the Sasang constitution for the KS-15 response based on the cut-off values (approximate 0.33), 0.5, and 0.6 of the constitutional score, when performed at the 0.6 cut-off model, the odds ratio of TE was 2.46 which showed a statistically significantly higher risk than the borderline group. For the accuracy of the model and the Area under the curve (AUC), the model accuracy based on the original cut-off of the KS-15 was 0.902 and AUC was 0.737. The accuracy of the model with cut-off of 0.5 and with of 0.6 were 0.904 and 0.902, respectively, and the AUCs were 0.687 and 0.741, respectively. Conclusion In this study, we confirmed that it is effective to increase the cut-off value of KS-15 to 0.6 in the metabolic syndrome risk model. It is expected that this could increase the accuracy of identifying high-risk groups for metabolic syndrome.
