30% of patients with panic disorder (PD) show treatment-resistant and chronic waxing and waning course. Therefore, adequate treatment strategies for PD by evidence based pharmacotherapy and combined cognitive behavioral therapy (CBT) are recommended. Regarding how and why CBT for PD works, three hypotheses include the cognitive theory, anxiety control theory, and behavioral theory were discussed. The recent findings that the altered activation in frontal lobe is normalized after CBT, suggest a reduction of an altered top-down fear processing in the neural correlates of CBT in PD. In order to improve accessibility to CBT, brief CBT and internet based CBT for PD were suggested. Despite limitations of sample sizes and study design, most of studies suggest that brief CBT is more effective than control conditions, and even as equally effective as standard CBT. The evidences suggest that internet based CBT may not be significantly different from face-to-face CBT in reducing anxiety. Several advances within the field of third-wave CBT for PD have led to the development of new techniques based on mindfulness, such as mindfulness-based cognitive therapy and acceptance and commitment therapy. Based on Korean algorithm project for panic disorder, especially the psychological education and cognitive reconstruction components were recommended in CBT with PD.
This study was designed to investigate the effects of Gastrodiae Elata Pharmacopuncure at GB20 on motor control and cognitive dysfunction recovery after mild traumatic brain injury in rats. Rats were divided into three groups; (1) no treatment after traumatic brain injury(experiment I), (2) Treatment with NPA after traumatic brain injury(experiment II), (3) Treatment with GEP after traumatic brain injury(experiment III). In our study, we carried out behavioral test(Rotarod, Morris water maze) and immunohistochemistry study of the change BDNF in the hippocampus(pre, $7^{th}$, $14^{th}$ day). In Rotarod test(motor control function) was significantly increased in the experimental group III as compared with experimental group I, II on $7^{th}$(p<0.01) and $14^{th}$ day(p<0.001). In Morris water maze test(cognitive function) was significantly decreased in the experimental group III as compared with experimental group I, II on $14^{th}$ day(p<0.001). In immunohistochemistric response of BDNF in the hippocampus, the experimental group III was more immune response than the other groups on $14^{th}$ day. These results imply that Gastrodiae Elata Pharmacopuncure at GB20 can play a role in facilitating recovery of motor control and cognitive function after mild traumatic brain injury in rats.

The purpose of this study was to investigate the effects of the horticultural therapy program on patients with mild cognitive impairment and mild dementia depending on the frequency and duration of the interventions. We developed the same 15-session program to improve cognitive functions and life satisfaction and alleviate depression of the elderly women with mild cognitive impairment or mild dementia. Subjects in Longer Treatment group participated in the program once a week for 15 weeks and subjects in Shorter Tratmet group participated twice a week for 7½ weeks. This study conducted pretest-posttest verification of both groups using quasi-experimental design involving 21 subjects. Elderly life satisfaction, Geriatric Depression Scale (short form), and the Korean Version of Consortium to Establish a Registry of Alzheimer's Disease (CERAD-K) were used in the evaluation. As a result, both groups showed an increase in life satisfaction, and a decrease in depression. However, there was a significant difference in the changes of the CERAD-K scores between the two groups (p < .05). In Longer Treatment group, life satisfaction increased significantly (p < .001), and depression decreased at a marginally significant level (p = .068), but no statistically significant change was observed in neurocognitive function. In Shorter Treatment group, life satisfaction increased at a marginally significant level (p = .059), and depression and CERAD-K scores decreased significantly (p < .05). However, in the case of Mini-Mental State Examination (MMSE-K), there was no significant change in both groups. According to these results, when planning a horticultural therapy program for persons with mild cognitive impairment or mild dementia, it is effective to organize and execute the program by determining the duration of intervention as 3 to 4 months or longer, even if this reduces the number of interventions per week.
This study examined the effects of the result of ascertaining predictions on cognitive conflict and conceptual change when students teamed the concept of weightlessness. Participants were 200 pre-service elementary teachers. They answered the pretest composed of two items. Through the demonstration on either of two items of the pretest, they identified whether their predictions were correct or not. In addition, students' cognitive conflicts were measured. After brief instructional treatment, the posttest was conducted. The results of this study are as follows: The more students who identified their own predictions on the experiment were incorrect there were, the more effective it was on cognitive conflict and conceptual change. And cognitive conflicts and conceptual changes of students who identified that their predictions were incorrect were generated meaningfully more than those of students who identified that their predictions were correct. From these results, it is concluded that students who identified that their predictions were correct experience cognitive conflicts, but their cognitive conflicts and conceptual changes were smaller than those of students who identified that their predictions were incorrect.
목적 : 본 연구는 노인들의 인지기능과 구강건강관련 삶의 질 사이의 연관성을 분석하기 위해 수행되었다. 연구방법 : 2020년에 수집된 제8차 고령화연구패널조사에 참여한 지역에 거주하는 45세 이상의 중고령자를 대상으로 인구통계학적 및 임상적 특성을 추출하여 활용하였다. 독립변수는 한국형 간이정신상태검사 점수를 기준으로 분류한 인지기능, 종속변수는 구강건강관련 삶의 질을 측정하기 위한 노인구강건강평가지수이다. 인지기능 그룹 간 공변량의 차이를 보정하기 위해 성향점수 활용법 중 역확률가중치를 적용 후 인지기능과 구강건강관련 삶의 질의 연관성을 분석하기 위해 역확률가중치 적용 전의 다중회귀분석과 적용 후의 다중회귀분석 결과에 차이가 있는지 결과를 비교하였다. 결과 : 연구 대상자는 총 6,116명으로 인지정상군 4,367명, 경도 인지손상군 1,155명, 중증 인지손상군 594명으로 구성되었다. 성향점수 역확률가중치 적용 결과로 표준화된 평균 차이(standardized mean difference)를 확인하여 0.2 이상인 변수를 다시 통제하고 분석한 다중회귀모델에서 인지기능 그룹과 구강건강관련 삶의 질 간의 부정적인 연관성이 있었다(정상 vs. 경도: β = -2.534, p < .0001; 정상 vs. 중증: β = -2.452, p < .0001). 결론 : 본 연구 결과에서 인지손상과 구강건강관련 삶의 질은 음의 연관성을 나타내었다. 성향점수 활용 후 중증 인지손상보다 경도 인지손상에서 더 부정적인 연관성을 나타낸 결과는 인지손상은 저하된 정도와 관계없이 구강건강관련 삶의 질에 부정적인 영향을 미치는 것을 의미한다. 따라서 인지기능의 감퇴가 나타난 모든 환자들은 구강관리의 중요성과 교육이 필요하다는 것을 시사한다.
본 연구는 건강한 중년여성 28명을 대상으로 강도별 유산소운동을 12주간 주 3회로 실시하여 혈압, 레닌-알도스테론계, 신경전달물질 및 인지기능, 작업기억의 변화와 처치 후 인지기능에 영향을 미치는 요인을 분석하고자 실시하였다. 모집된 대상자를 통제집단 9명, 중강도 유산소운동집단 10명(50%V02max), 고강도 유산소운동집단 9명(70%V02max)으로 구분하여 처치 전과 후 반복측정 분산분석과 다중회귀분석을 실시한 결과 다음과 같은 결과가 나타났다. 중강도 유산소운동은 레닌, BDNF, 인지기능과 작업기억을 증가시키고, 알도스테론, 안지오텐신II, 알도스테론-레닌 비율을 감소시켰다. 고강도 유산소운동은 BDNF, 인지기능, 작업기억을 증가시키고 수축기혈압을 감소시켰다. 또한 강도별 유산소운동 후 인지기능에 미치는 요인을 회귀분석한 결과 중강도 유산소운동은 이완기혈압, 레닌-알도스테론 비율의 감소와 작업기억, BDNF의 증가가 인지기능에 영향을 미쳤고, 고강도 유산소운동은 작업기억 BDNF, 세로토닌의 증가가 인지기능에 영향을 미쳤다. 따라서 중강도 이상의 유산소운동은 중년여성의 인지기능 및 작업기억을 향상시키는 것을 알 수 있었다. 또한 인지기능 향상에 영향을 미치는 대사적 요인으로 중강도 유산소운동은 레닌-알도스테론 조절과 작업기억이었고 고강도 유산소운동은 신경전달물질과 작업기억으로 나타났다.
Dementia with Lewy bodies (DLB) is the second most common causes of dementia. It can exhibit a variety of clinical symptoms including cognitive decline, cognitive fluctuation, visual hallucinations, parkinsonism, REM sleep behavior disorder, hypersensitivity to neuroleptics and autonomic dysfunctions. Despite more well-known criteria for DLB, there are often misdiagnosis and inappropriate treatment. It gives a lot of clinical burden to the clinician as well as to patients and families. When reducing the misdiagnosis, the burden of all will be reduced. The special concern and solicitation are needed in order not to miss the diagnosis when the cardinal features of DLB may not be volunteered by patients and the caregivers. To control the symptoms, clinicians must find and reduce drugs that can have the negative effects on DLB symptoms. There is limited evidence about specific interventions but available data suggest cholinesterase inhibitors improve the cognitive and behavioral symptoms and menmantine slightly improves the global impression.
Acceptance and Commitment Therapy (ACT) is a functional contextual intervention approach based on the behavioral theory on human language, which views human suffering as originating in psychological inflexibility fostered by cognitive fusion and experiential avoidance. Thus, the goal of ACT is to enhance psychological flexibility using six core processes including acceptance, cognitive defusion, self-as-context, contact with present moment, values clarification, and committed action. Recent clinical trials have suggested the efficacy for ACT in the treatment of various mental illness and psychological distress. The aim of this review is to offer more knowledge and better understanding of ACT by presenting its underlying principle and an overview of the research field.
Purpose: The purpose of this study was to evaluate a cognitive behavioral anger management intervention in order to increase self-consciousness and decrease anger among Korean military men, Methods: A quasi-experimental design was used for this study. Ninety two soldiers were screened for unhealthy anger expression using a questionnaire. Among them, 26 soldiers who showed unhealthy anger expression were selected for this study. They were matched by rank and assigned to either a treatment or control group. The treatment group received the intervention three consecutive times, once a week and for 120 minutes per session. Participants were assessed before and after the intervention for anger (the State-Trait Anger Scale) and self-consciousness (Self-consciousness Scale). Data was analyzed using descriptive statistics, $x^2-test$, Mann-Whitney test, and Wilcox on signed rank test. Results: There was no statistically significant decrease in anger or increase in self-consciousness between the two groups following treatment. However, there was a significant increase in private self-consciousness in the treatment group after the intervention(p=.006). Conclusion: The cognitive behavioral anger management program improves private self-consciousness in soldiers. However, Additional research is needed to explore whether long-term intervention is more effective for anger control in the military setting. The findings from this study suggest that more attention should be given to mental health care for Korean soldiers.
Purpose: The Purpose of this study was to develop an exercise program for elder people, and to verify the effects of it. Method: A one-group pretest-posttest experimental design was used. The exercise program was developed by researchers, and applied to 20 elders for 3 times a week for 5 weeks. To evaluate the treatment effects, dependent variables such as subjective health status, cognitive function, ADL, IADL, and depression were measured repeatedly 7 times. One-way Repeated Measures ANOVA was used for analysis. Result: There were significant positive changes in subjective health status, cognitive function, ADL, and depression except IADL during the treatment period. The treatment effects decreased significantly during 1-3 weeks after the treatment period. Conclusion: The exercise program, which had developed by the researchers, enhancing subjective health status, cognitive function, and ADL and decreasing depression of elders. Therefore this program is strongly recommended for geriatric nursing care settings.
