A Study on the Determinants of the Incidence and the Transition of Older Adult Disability: Findings from the Korea Longitudinal Study of Aging(KLOSA) (노년기 장애발생과 장애정도의 변화에 미치는 영향요인 연구: KLOSA 1차와 2차 자료를 중심으로)

  • The purpose of this study is to investigate the factors associated with the incidence and the transition of disability among Korean older adults. Samples consist of 1,454(42.7%) men and 2,032(58.3%) women aged 65 and over who participated in the 1st and 2nd wave of the Korea Longitudinal Study of Aging: KLOSA. To estimate the level of disability, ADL and IADL disability indexes are used. As the results, major risk factors for ADL/IADL disability incidence include injury, vision problem, cognitive function, depression, health behavior, socioeconomic characteristics and age. Among the normal older adults, the odds ratio of having dementia symptoms at 2nd wave(2008) are 2.0 times greater for the older adults who have less cognitive function than those who don't have at 1st wave(2006). Among the older adults with chronic diseases, the odds ratio of having disability at 2nd wave are 1.8 times greater for the older persons who have depression than those who don't have at 1st wave. Secondly, concerning the predictors affecting the disability transition among the disabled older adults at 1st wave, the likelihood of remaining at the same level or deteriorating the level of IADL disability, as compared with improving the level, is associated with having less instrumental support or being older. These results indicate that it is necessary to prefer multilevel intervention in order not only to prevent the incidence of disability, but also to prolong the deterioration of disability in the older adults.

A Study on Factors Influencing The State of Adaptation of The Hemiplegic Patients (편마비 환자의 퇴원후 적응상태와 관련요인에 대한 분석적 연구)

  • The purposes of this study are to delineate a profile of the state of a stroke patient's adaptation at 3 months after hospitalization and to explore the relationship between the level of adaptation and the variables which influence the adaptation of hemiplegic patients. To these ends, theoretical framework was derived basically from the stress adaptation model. The basic assumption underlying the level of adaptation is influenced by the presenting focal, contextual and residual stimuli. This group of stimuli is further operationalized and represented by a perception of stress. which is the perceived effect of the disability and by the mediating variables such as sociodemographic factors as an external conditioning variables and perceived social support and hardiness personality characteristics as an internal intervening variables. The dependent varibales in this study is the level of physical, psychological and social adaptation and is hypothesized to be a function of the interaction between 3 sets of variables namely, the perceived disability effect, external conditioning variables and internal intevening varibles. A total of fourty three subjects from 3 general hospitals in Seoul were observed and interviewed with the aid of 7 structured instruments. The data were collected twice on each subject : first at the pre-discharge period arid at 3 months post-discharge from hospital for the second time. The study was carried out for the period from February to August, 1988. The instruments used for the study include 4 existing scales and 3 scales developed by the researcher for this study. They are : 1) The ADL dependency scale and the scale of the clinical physical functions for the assessment of physical adaptation. 2) the SDS(self report of depression) to measure the level of psychological adaptation. 3) The scale for the amount of social activities for the measurement of the level of social adaptation. 4) The scale for the perceived effect of disability for the measurement of the focal stimuli. 5) The health related hardiness scale and the perceived interpersonal support self evaluation list(ISEL) for the measurement of the hardiness personality character and the perceived social support. The data obtained were analyzed using percentage, oneway ANOVA, Pearson coefficients correlation and stepwise multiple regression. The findings provide valuable information about the present level of physical adaptation at 3 months after discharge. The patient revealed a decreased ADL dependency and lowered limitation of physical function as compared with pre - discharge state. Psycholcgically, the average degree of depression at follow up was within normal range of depression. Socially, the amount of social activities was very low. The one way ANOVA and the correlational analysis revealed the relationship between the 3 sets of variables and the adaptation level as follows : 1) The perceived disability effect was related to the degree of the depression and the amount of social activities but was not related to the physical adaptation. 2) Among the sociodemographic variables, sex and education were related to the difference of ADL dependency and the change of physical function. These factors indicate that women more than men and educated more than the less educated were found more independent. The education was also related to the degree of depression suggesting that the higher the educational level, the more well adapted the patients were both physically and psychologically. Age, marital status and job state were not found to be related to the patient's adaptation level. 3) Among the internal intervening variables, the health related hardiness characteristic was related to the differences of ADL dependency, physical functions and the social activities, indicating that the higher the hardiness character the higher the level of physical and social adaptation. 4) The perceived social support, another internal intervening variable, was related to the degree of depression and the social activities. This data suggest that the higher the perception of social support, the better adapted the patients were psychogically and socially. In summarizing the results of the correlational analysis, the level of physical adaptation was influenced by sex, the years of education and the hardiness character. The level of psychological adaptation was influenced by the years of education, the perceived disability effect and the perceived social support. And the level of social adaptation was influenced by the perceived disability effect, the hardiness character and the perceived social support. The stepwise multiple regression analysis shows findings as follows : 1) The most important factor to explain the difference of ADL dependency was sex, indicating females were more independent than males. 2) The most important factor to explain the difference of physical function and the degree of depression was the patient's education level. 3) The strongest explaining factor for the amount of social activities was perceived self esteem(one of the subconcepts of perceived social support). Thus the most important factors influencing the level of adaptation were found to be sex, education, the hardiness character and self esteem. From the above findings, the significance of this study can be delineated as follows : 1) Corroboration of the assumed relationship between the various variables and the adaptation level as suggested in the conceptual model. 2) Support for the feasibility of the cognitive approach for nursing intervention such as hardness character training, counselling and teaching for self-care in the chronic patients.

A Study on Elderly Nursing Home Design Direction Based on Yalom's Existential Psychotherapy (얄롬의 실존주의 심리치료에 의거한 노인요양시설 디자인 방향에 대한 연구)

  • This research comprehended the fundamental cause of elderly nursing home residents' negative feelings of loneliness, depression, fear, and loss of self-usefulness as existential issue, since they already went through loss, death of loved ones, geriatric disease and disability. The purpose of this research is to explore how existentialism and existential psychotherapy theory of Irvin D. Yalom can be applied to nursing home environment design, and to suggest design directions. Based on his framework of human's ultimate interest, death, freedom followed by responsibility, isolation, and meaninglessness, interior design suggestions and applicable spaces were presented. Four cases from Australia, Denmark, Japan and Korea were analyzed according to design suggestions to grasp the current situation and to draw further proposition. The conclusions are as follows. (1)Environment should support nursing home residents to resolve existential issues. (2)Death is the least supported issue on all of the cases. Environmental elements that can induce residents to think and discuss on death, hospice program and space for memorial service is necessary. (3)Regarding to freedom, the environment should support resident's disability to maintain independence as much as possible and residents autonomy and decision should be respected. Single/double rooms, motorized bed, free and safe usage of kitchen and garden are necessary. (4)For isolation, Space and time for meditation should be provided. (5)To find meaning of life, environment should support residents in both physical and cognitive aspect so that they can contribute to others. Space for hobby is necessary for continued creativity and self-realization.

First Korean Case of 16p11.2 Duplication Syndrome Diagnosed by Chromosomal Microarray Analysis

  • A 10-year and 5 month-old girl with developmental delay, intellectual disability, attention deficit hyperactivity disorder, poor weight gain, and microcephaly was transferred to our pediatric clinic for genetic evaluation. Her height was within the 5-10th percentile, and her weight was under the 3rd percentile. On the social maturity scale, her developmental status was scored as 3 years 9 months for social age, and the social quotient was 35.98. A chromosomal microarray analysis was performed and the microduplication at chromosome 16p was observed: arr[GRCh37] 16p11.2 (29580020_30190029)${\times}3$. Currently, the patient is diagnosed with Grade 2 intellectual disability and is attending a computerized cognitive rehabilitation class twice weekly. In addition, nutritional support and growth follow up are also ensured in the Pediatric Gastrointestinal and Endocrinology clinic.

Parents' Perceptions of Cognitive Rehabilitation for Children With Developmental Disabilities: A Mixed-Method Approach of Phenomenological Methodology and Word Cloud Analysis (발달장애 아동 부모의 인지재활 경험에 대한 질적 연구: 워드 클라우드 분석과 현상학적 연구 방법 혼합설계)

  • Objective : The purpose of this study was to investigate parental perspectives on cognitive rehabilitation using a combination of phenomenological research methodology and word cloud analysis. Methods : Interviews were conducted with five parents of children with developmental disabilities. Word cloud analysis was conducted using Python, and five researchers analyzed the meaning units and themes using phenomenological methods. Words with high frequency were considered as a heuristic tool. Results : A total of 43 meaning units and nine components related to the phenomenon of cognitive rehabilitation were derived, and three themes were finalized. The main themes encompassed the definition of cognitive rehabilitation, challenges associated with cognitive rehabilitation, and factors influencing the selection of a cognitive rehabilitation institute. Cognitive rehabilitation emerged as a treatment focused on improving learning, daily functioning, and cognitive abilities in children with developmental disabilities. The perceived issues with cognitive rehabilitation pertained to treatment methods, therapist expertise, and associated costs. In addition, parents highlighted the importance of therapist expertise, humane personality, and affordability of cost and schedule when choosing a cognitive rehabilitation institute. Conclusion : Parents expressed expectations for substantial improvements in their children's daily functioning through cognitive rehabilitation. However, challenges were identified in clinical practices. Going forward, we expect that cognitive rehabilitation will evolve into a better therapeutic support service addressing the concerns raised by parents.

Specialists' Views Concerning the Assessment, Evaluation, and Programming System (AEPS) in Associations for Children with Disabilities in Saudi Arabia

  • To support early intervention, it is necessary to develop programming system tools that enable accurate, valid, and reliable assessments and can help achieve reasonable, generalizable, and measurable goals. This study examined the Assessment, Evaluation, and Programming System (AEPS) used by associations of children with disabilities in Saudi Arabia to assess its suitability for children with intellectual disabilities. A group of 16 specialists with different professional backgrounds (including special education, physiotherapy, occupational therapy, speech therapy and psychology) from 11 associations of children with disabilities took part in semi-structured personal interviews. The study concluded that AEPS is generally suited for use with children with intellectual disabilities. However, its suitability depends on the type and severity of the child's disability. The more severe the disability, the less effective the AEPS is likely to be. On the basis of this finding the researchers formed interdisciplinary teams to organise and integrate the children's learning and assess the benefits of AEPS, including its accuracy and ability to achieve adaptive, cognitive, and social targets, enhance family engagement and learning and develop basic development skills. This study also identified obstacles associated with the use of AEPS. These include the lack of comprehensiveness and accuracy of the goal, lack of precision and non-applicability to large movements and the fact that it cannot be used with all children with intellectual disabilities. In addition, the research showed that non-cooperation within the family is a major obstacle to the implementation of the AEPS. The results of this study have several implications.

Implementation of A Vibration Notification System to Support Driving for Drivers with Cognitive Delay Impairment

  • In this paper, we propose a vibration notification system that combines navigation information and wearable bands to ensure safe driving for the transportation vulnerable. This system transmits navigation driving information to a linked application, converts it into a vibration signal, and provides notifications through a wearable band. Existing navigation systems focus on providing route guidance and location information, so the driver's concentration is dispersed, and safety and convenience are deteriorated, especially for those with mobility impairments, due to standard vision and delayed recognition of stimuli, resulting in an increasingly high traffic accident rate. To solve this problem, navigation driving information is converted into vibration signals through a linked application, and vibration notifications for events, left turns, right turns, and speeding are provided through a wearable band to ensure driver safety and convenience. In the future, we will use cameras and vehicle sensors to increase awareness of safety inside and outside the vehicle by adding a function that provides notifications with vibration and LED when the vehicle approaches or recognizes an object, and we will continue to conduct research to build a safer driving environment. plan.

The Rehabilitation of Gambling Addiction: Comparison with the other psychiatric disorder (도박중독의 재활: 타 정신장애와의 비교)

  • This study reviewed the present state and differences of rehabilitation programs of the gambling addiction by comparing with other psychiatric disorder(including psychotic disability and alcohol addiction). This study also intended to suggest necessities, meanings and inherent fields of the rehabilitation in gambling addiction. First of all, the government and a few gambling industries run clinic centers for gamblers and their families, but have been lacked rehabilitation services for social comeback and adaptation or devaluated rehabilitation services than therapies. Gambling addict didn't have impairments of the cognitive function and their daily abilities was better than any other psychiatric disorders. But Damage of social role or function of gambling addiction was severe. And it is caused by nonadaptive nature of gambling behavior, personality/emotional change through gambling addiction process, and previous personality problem etc. There are many severe failure of social role and its attendant bankrupcy, family's problems and social poverty in gambling addiction, Therefore, important fields in the rehabilitation of gambling addiction should be services for basic social comeback support service, credit recovery support, monetary management, support of rehabilitation of family and vocational rehabilitation. Finally, the significance and critical points of the current study has been discussed as well.

A Review of the Neurocognitive Mechanisms of Number Sense (수 감각의 인지신경학적 기반에 관한 연구 개관)

  • Human and animals are born with an intuitive ability to determine approximate numerosity. This ability is termed approximate number sense (hereafter, number sense). Evolutionarily, number sense is thought to be an essential ability for hunting, gathering and survival. According to previous research, children with mathematical learning disability have impaired number sense. On the other hand, individuals with more accurate number sense have higher mathematical achievement. These results support the hypothesis that number sense provides a basis for the development of mathematical cognition. Recently, researchers have been examining whether number sense training can lead to enhancement in mathematical achievement and changes in brain activity in relation to mathematical problem solving. Numerosity which basically represents discontinuous quantity is expected to be closely related to continuous quantity such as representations of space and time. A theory of magnitude (ATOM) states that processing of number, space and time is based on a common magnitude system in the posterior parietal cortex, especially the intraparietal sulcus. The present paper introduces current literature and future directions for the study of the common magnitude system.

Relationship Between Work Ability, Self-efficacy, Work Satisfaction, Sequelae and Workplace Relationships of Reemployed Workers Who Experienced Industrial Accidents (재취업한 산재 노동자의 업무능력, 자기효능감, 직무 만족, 후유증, 동료관계의 구조적 관계)

  • The current study examined the mediating effect of self-efficacy (SE) in the relationship between work ability (WA) and work satisfaction (WS) perceived by reemployed workers who have experienced industrial accidents based on social cognitive career theory (SCCT), and investigated the effect of Industrial accident sequelae and workplace relationships (WR) on these variables. The data of 976 newly employed workers were selected from the data of the Panel Study of Worker's Compensation Insurance collected in 2018. The data were analyzed by Structural Equation Modeling through SPSS and AMOS programs. It was found that WA had a positive effect on WS through SE. In addition, it was found that the level of pain, the severity of the disability, and the activities of daily living had a negative effect on WS through WA and SE sequentially. Meanwhile, it was found that the WR was affected by the activities of daily living, and had a positive effect on WS through WE. These results emphasize that it is important to provide support to manage sequelae and increase SE in order to help victims of industrial accidents return to the job market and live satisfactorily. It also suggests that SCCT is useful in explaining WS of industrially injured workers.
