Effects of Computerized Cognitive Training Program Using Artificial Intelligence Motion Capture on Cognitive Function, Depression, and Quality of Life in Older Adults With Mild Cognitive Impairment During COVID-19: Pilot Study (인공지능 동작 인식을 활용한 전산화인지훈련이 코로나-19 기간 동안 경도 인지장애 고령자의 인지 기능, 우울, 삶의 질에 미치는 영향: 예비 연구)

  • Objective : We investigated the efficacy of an artificial intelligence computerized cognitive training program using motion capture to identify changes in cognition, depression, and quality of life in older adults with mild cognitive impairment. Methods : A total of seven older adults (experimental group = 4, control group = 3) participated in this study. During the COVID-19 period from October to December 2021, we used a program, "MOOVE Brain", that we had developed. The experimental group performed the program 30 minutes 3×/week for 1 month. We analyzed patients scores from the Korean version of the Mini-Mental State Examination-2, the Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet for Daily Life Evaluation, the short form Geriatric Depression Scale, and Geriatric Quality of Life Scale. Results : We observed positive changes in the mean scores of the Stroop Color Test (attention), Stroop Color/Word Test (executive function), SGDS-K (depression), and GQOL (QoL). However, these changes did not reach statistical significance for each variable. Conclusion : The study results from "MOOVE Brain" can help address cognitive and psychosocial issues in isolated patients with MCI during the COVID-19 pandemic or those unable to access in-person medical services.

A Systematic Review of the Application Nonpharmacological Cognitive Interventions in Patients With Dementia (치매 환자에게 적용된 비약물적 인지중재방법에 대한 체계적 고찰)

  • Objective : The aim of the study was to investigate the application of nonpharmacological cognitive interventions in patients with dementia. Methods : We searched published studies in KISS, PubMed, and Sciencedirect databases from January 2013 to December 2017. The main keywords used were "Dementia" AND "Cognitive stimulation OR Cognitive rehabilitation OR Cognitive training" and a total of ten studies were selected for analysis from 753 searched articles. Results : Seven of the ten selected studies showed significant improvements in cognitive function after intervention, whereas three studies showed no improvement in cognitive function; however, activation of brain waves, improvement in the relationship between care givers and patients, improvement in the quality of life of care givers, and improvements in visual motor skills were shown. Mini-Mental State Examination(MMSE) was used as the assessment tool for identifying the effects of the cognitive function improvement, and in four studies the quality of life of dementia patients was measured as an intervention effect. The main subject of the cognitive intervention is patients with mild to moderate dementia. Conclusion : The results of this study can be used as a basis for the selection of intervention methods, as well as their duration and assessment, according to the characteristics of dementia patients.

Application of Cognitive Enhancement Protocol Based on Information & Communication Technology Program to Improve Cognitive Level of Older Adults Residents in Small-Sized City Community: A Pilot Study (중소도시 지역사회 거주 노인의 치매예방을 위한 Information & Communication Technology 프로그램 기반 인지향상 프로토콜 적용: 파일럿(Pilot) 연구)

  • Objective : This study, as a preliminary study, applied an Information & Communication Technology (ICT) home-based program to elderly people aged 65 years or older to confirm the effect of the cognitive enhancement program and to find the possibility of remote rehabilitation. Methods : This study from August to October 2022, three subjects were selected and the intervention was conducted for about 2 months. This intervention was conducted using Korean version of Mini-Mental State Examination, Korean version of Montreal Cognitive Assessment (MoCA-K), Computer Cognitive Senior Assessment System, and the Center for Epidemiologic Studies Depression scale to evaluate cognitive improvement before and after the program. The therapist remotely set the level of cognitive training according to the subject's level through weekly feedback. Results : After the intervention, all subjects showed improved scores in most items of the MoCA-K conducted before and after the intervention. In addition, among the items of Cotras-pro, upper cognition, language ability, attention, visual perception, and memory were improved. Conclusion : Cognitive rehabilitation training using an ICT home-based program not only prevented dementia but also made it habitual. Through this study, it was confirmed that remote rehabilitation for the elderly could be possible.

A Systematic Review of Cognitive Orientation to Daily Occupational Performance for Children with Developmental Coordination Disorder (발달성협응장애 아동의 인지기반 작업수행(Cognitive Orientation to daily Occupational Performance; CO-OP) 중재에 대한 체계적 고찰 )

  • Objective : This study was conducted to examine self-selected goals and the outcome measures used in the Cognitive Orientation to daily Occupational Performance (CO-OP) approach for Developmental Coordination Disorder. Methods : Studies published from January 2012 to October 2022 in the PubMed, Embase, ScienceDirect, Cochrance Library databases were searched. Keywords used for search were ('developmental coordination disorder' OR 'DCD') AND ('Cognitive Orientation to daily Occupational Performance' OR 'Cognitive Orientation to Occupational Performance' OR 'CO-OP'). Among 211 searched studies, 7 selected studies that match the thesis of this study were analyzed. Results : The selected studies showed a relatively high level of evidence overall, including two randomized experimental studies, one non-random two-group study, three non-random one-group studies, one single-subject study. The self-selected goals preference of the children was high in the order of play, education, and daily life activities. Most of applicable sessions were conducted 10 times during a 1-h period, and intervention effects showed positive outcomes on the occupation performance motor domain. To measure the effectiveness of CO-OP, the improvement of occupational performance was evaluated using Canadian Occupational Performance Measure (COPM) and Performance Quality Rating Scale (PQRS), and the improvement of motor skills was evaluated using Movement Assessment Battery for Children (M-ABC). Conclusion : This study is expected to be used as basic clinical data when applying the CO-OP approach to Developmental Coordination Disorder.

The Effect of Transcranial Direct-Current Stimulation on Cognitive Function and Depression in Stroke Patient's through a Computerized Cognitive Rehabilitation Program (경두개 직류전류자극과 전산화 인지재활 프로그램이 뇌졸중 환자의 인지기능 및 우울감에 미치는 영향)

  • Purpose : The purpose of this randomized controlled trial study was to examine the effect of transcranial direct current stimulation (tDCS) on cognitive function and depression in stroke patients. Methods : Thirty stroke patients were randomly divided into an experimental group (n = 15) and a control group (n = 15). The experimental group received tDCS while performing computerized cognitive rehabilitation programs, and the control group was provided with sham tDCS while operating the same programs. The 30-minute intervention was implemented five times per week for six weeks. To assess cognitive function before and after the intervention, the Neurobehavioral Cognitive Status Examination was conducted; the Beck Depression Inventory BDI was employed to assess depression. Results : The experimental group showed statistically significant increases in cognitive function and decreases in depression (p < .05 ). Comparing the amount of variation between the groups after arbitration also showed significant differences in cognitive function and depression between the two groups (p > .05). Conclusion : The application of tDCS and computerized cognitive rehabilitation programs for stroke patients may positively affect their cognitive function and depression. Therefore, tDCS used with computerized cognitive rehabilitation programs is positively applicable to the enhancement of cognitive function in stroke patients and reduction of depression.

Survey on community occupational therapy awareness in occupational therapy majors (작업치료 전공자의 지역사회 작업치료 인식도 조사)

  • Objective: This study investigated the awareness of occupational therapy in the community in occupational therapy majors through a survey. This purpose is to investigate the awareness of occupational therapy in the community among occupational therapy majors and establish a theoretical foundation. Methods: The research subjects were surveyed among occupational therapy majors at M University living in Gyeongsangnam-do, and analyzed 141 questionnaires from September 2023 to December 2023. Results: The results of this study that education and awareness improvement are needed to increase awareness of occupational therapy in the community, and it was found that continuing education and case sharing are effective. Activation of home rehabilitation and continuous health management. This institutional development can induce employment activity through community rehabilitation, and activate programs in connection with adult day care centers. For the development of community occupational therapy, participation in education and development of customized treatment are necessary, and patient It should be developed to help with movement and movement, and it has been shown that it can also affect the quality of life of patients. In addition, cutting-edge technologies such as artificial intelligence are expected to be applied to remote support, telemedicine, etc., and are applied to dementia, cognitive patients, and central nervous system patients. In order to institutionalize occupational therapy in the community, it is helpful in daily life, nursing, and management. It was said that this would be helpful for community participation. Conclusion: This study investigated the awareness of occupational therapy in the community among occupational therapy majors. Education and awareness improvement are needed to increase awareness of occupational therapy in the community. Education to improve the professionalism of occupational therapists, strengthening connectivity with other majors, and local organizations. It is believed that collaboration with the local community and institutional supplementation tailored to the needs of the local community were necessary.

Development and Usability of a Cognitive Rehabilitation System Based on a Tangible Object for the Elderly (고령자를 위한 실감객체기반 인지재활 시스템의 개발과 사용성 연구)

  • Objective: To develop and verify the usability of a cognitive rehabilitation system with diverse cognitive functional levels based on tangible objects for the elderly population. Methods: A study was conducted to investigate the system's strengths and weaknesses by upgrading it with responses from two groups of 15 patients and 4 occupational therapists. After undergoing three forms of training - regarding executive function, memory, and concentration for a total of 20-30 min, the participants were asked to answer a structured questionnaire about contents of the three forms of training, hardware including the tablet PC functioning as a CPU and display media and tangible objects, and satisfaction of experiential usage of the system. Results: Both groups responded that the most interesting training area was executive function while the least interesting was concentration. Six participants reported that the size of the screen of the tablet PC was inappropriate, and five responded that the size of the tool was inappropriate. All therapists and 40% of the patients responded that they were satisfied with this system. Conclusion: This system's features include easy manipulation of tangible tools for performing training tasks, easy selection of and training in cognitive areas based on users' needs, and automatic adjustment of difficulty level based on users' performance. The training environment was designed to be similar to the natural environment by using tangible objects in both hands as input devices for the system, and the system was considered as an alternative to the lack of community cognitive rehabilitation specialists.

  • This study was undertaken to investigate the effect of cognitive rehabilitation program (CRP) using virtual content, for recovering patients with cerebrovascular disease (CVD). A total of 34 patients with CVD were divided into a control group and an experimental group, and subsequently subjected to a virtual content based CRP, followed by cognition, ADLs, and upper limbs. The control group completed a universal rehabilitation program (URP), and the experimental group was provided a URP and a virtual content based cognitive rehabilitation program. For both groups, respective programs were conducted twice a day, 5 times a week for 4 weeks. Both groups showed significant improvement in ADLs (p<0.05); but the amount of change in the experimental group 5 point improved significantly (p<0.05). Cognition was significantly different for both groups (p<0.05). However, the experimental group showed a greater change (2 point) than the control group when considering the amount of change (p<0.05). Comparing the amount of change in the upper extremity, the experimental group showed a significant change (0.7 point) than the control group (p<0.05). This study confirms that compared to URP, CRP using virtual content significantly improves ADLs and cognition.

Does the Application Transcranial Direct Current Stimulation and Visual Perception Program Enhance the Visual Perception Function and ADL Performance of Dementia Patients (경두개 직류자극 및 인지훈련 프로그램 적용이 치매환자의 인지기능, 시지각 및 일상생활활동에 미치는 영향)

  • Purpose : There is a lack of research on this field in the Republic of Korea, especially those that have seen the effect of interaction between transcranial direct current stimulation and computerized cognitive rehabilitation therapy. This study divided 30 dementia patients into a treatment group, which received transcranial direct current stimulation and computerized cognitive rehabilitation, and a control group, which received pseudo-transcranial direct current stimulation and pseudo-computerized cognitive rehabilitation. This study evaluated the effects of these treatments on the visual perception, cognition functions, and daily activities of dementia patients. Methods : Fifteen subjects were allocated to the treatment group and the other 15 subjects were allocated to the control group. Treatments were given at intervals of five sessions per week (30 minutes per session) for six weeks (30 times in total). This study used the Neurobehavioral Cognitive Status Examination (NCSE) to examine cognitive functions, MVPT to evaluate visual perception, and FIM to test daily living activities before and after applying the treatments. Results : The results of this study showed that cognitive functions, visual perception, and daily living activities significantly (p<.05) improved after the intervention in the treatment group and the control group. The changes in cognitive functions, visual perception, and daily living activities due to the treatments were significantly different between the groups (p<.05). Conclusion : The results indicated that transcranial direct current stimulation and computerized cognitive rehabilitation therapy improved visual perception and daily living activities by increasing cognitive functions. Consequently, it was found that the simultaneous application of transcranial direct current stimulation in conjunction with a computerized cognitive rehabilitation program was an intervention method that could positively affect the visual perception, cognitive function, and daily living activities of dementia patients. Based on the results of this study, the study of arbitration protocols for demential will have to be more active.

Effect of Home-Visit Occupational Therapy on Community Dwelling Stroke Survivors: A Case Study (지역사회 거주 뇌졸중 환자의 가정방문 작업치료 효과: 사례 연구)

  • Objective : The purpose of this study was to verify the effectiveness of home-visit occupational therapy in stroke patients. Methods : Two patients with stroke who applied for home-based occupational therapy services at a health center in Seoul were enrolled. The home-visit occupational therapy program evaluates the subject's daily living, task performance, cognitive, and emotional functions, sets occupational therapy goals and plans interventions based on a client-centered approach. Occupational therapy programs consisted of 12 sessions based on the client's major problems. Results : COPM scores improved in both cases, there was an improvement in COPM scores, and in Case 2 there were improvements in MBI and K-MMSE scores. There was also an improvement in KGDS scores in Case 1. Conclusion : Home-visit occupational therapy was found to be effective in improving daily activities, cognition, and mental function of stroke patients. During home and community integration, continuous and continuous rehabilitation services need to be activated from institutional rehabilitation to community-based rehabilitation. Active home-visit occupational therapy is needed to promote physical, cognitive, mental and social access in stroke patients discharged from hospitals.
