항암화학요법을 받는 소화기암 환자의 피로, 불안, 우울, 인지기능이 삶의 질에 미치는 영향 (Impacts of Fatigue, Anxiety, Depression, and Cognitive Function on the Quality of Life in Gastrointestinal Cancer Patients Receiving Chemotherapy)

  • Purpose: This study aimed to investigate the effects of fatigue, anxiety, depression, and cognitive function on the quality of life of gastrointestinal cancer patients receiving chemotherapy. Methods: Data were collected from a total of 141 participants. The measurements used were Functional Assessment of Chronic Illness Therapy for fatigue (FACIT-F), Hospital Anxiety and Depression Scale (HADS) and Functional Assessment of Cancer Therapy for cognitive function (FACT-Cog). Results: Significant correlations were found among fatigue, anxiety, depression, cognitive function, and quality of life. The mean score of quality of life was 59.60 out of 108, and 68% of the variance in QOL was explained by fatigue, anxiety, depression, and cognitive function. Cognitive function was the most influential factor (β=.30), followed by anxiety (β=-.27), depression (β=-.24), and fatigue (β=-.18). We found that the better the cognitive function, the lower the anxiety and depression, and the lower the degree of fatigue, the higher the quality of life. Conclusion: A nursing program for managing the changes in fatigue, anxiety, depression, and cognitive function should be provided to enhance maintenance and the improvement of the quality of life for gastrointestinal cancer patients who receive chemotherapy.

Cognitive Behavioral Therapy in Breast Cancer Patients - a Feasibility Study of an 8 Week Intervention for Tumor Associated Fatigue Treatment

  • Background: Tumor associated fatigue (TAF) or cancer related fatigue (CRF) is not a new concept. Nonetheless, no real headway has been made in the quantitative analysis of its successful treatment via cognitive behavioral therapy. Since 20 to 30% of all breast cancer patients suffer from anxiety and/or depression within the first year of their diagnosis, this issue needs to be addressed and a standard treatment protocol has to be developed. This study focused on developing a simple, reproducible and short (8 weeks) protocol for the cognitive behavioral therapy support of tumor associated fatigue patients. Materials and Methods: Between the year 2011 and 2012, 23 breast cancer patients fulfilled the diagnosis TAF requirements and were introduced into this study. Our method focused on a psycho-oncological support group using a predetermined, highly structured and reproducible, cognitive behavioral therapy treatment manual. Eight weekly, 90 minute sessions were conducted and patients were evaluated before and after this eight session block. Tumor fatigue specific questionnaires such as the multidimensional fatigue inventory (MFI) as well as the hospital anxiety and depression scale (HADS) were used in order to quantitatively evaluate patient TAF. Results: Of the 23 patients enrolled in the study, only 7 patients fulfilled the TAF diagnostic criteria after the psycho-oncological group treatment. This represents a 70% reduction in diagnosable tumor associated fatigue. The HADS analysis showed a 33% reduction in patient anxiety as well as a 57% reduction in patient depression levels. The MFI scores showed a significant reduction in 4 of the 5 evaluate categories. With the exception of the "mental fatigue" MFI category all results were statistically significant. Conclusions: This study showed that a highly structured, cognitive behavioral therapy group intervention will produce significant improvements in breast cancer patient tumor associated fatigue levels after only 8 weeks.

The Interaction of Cognitive Interference, Standing Surface, and Fatigue on Lower Extremity Muscle Activity

  • Background: Performing cognitive tasks and muscular fatigue have been shown to increase muscle activity of the lower extremity during quiet standing. A common intervention to reduce muscular fatigue is to provide a softer shoe-surface interface. However, little is known regarding how muscle activity is affected by softer shoe-surface interfaces during static standing. The purpose of this study was to assess lower extremity muscular activity during erect standing on three different standing surfaces, before and after an acute workload and during cognitive tasks. Methods: Surface electromyography was collected on ankle dorsiflexors and plantarflexors, and knee flexors and extensors of fifteen male participants. Dependent electromyography variables of mean, peak, root mean square, and cocontraction index were calculated and analyzed with a $2{\times}2{\times}3$ within-subject repeated measures analysis of variance. Results: Pre-workload muscle activity did not differ between surfaces and cognitive task conditions. However, greater muscle activity during post-workload balance assessment was found, specifically during the cognitive task. Cognitive task errors did not differ between surface and workload. Conclusions: The cognitive task after workload increased lower extremity muscular activity compared to quite standing, irrespective of the surface condition, suggesting an increased demand was placed on the postural control system as the result of both fatigue and cognitive task.

  • Purpose: The purpose of this study was to test a hypothetical model of chemotherapy-related cognitive impairment (CRCI) and depression in people with gastrointestinal cancer. Methods: A purposive sample of 198 patients undergoing chemotherapy was recruited from November 2014 to July 2015. The instruments were Everyday Cognition (ECog), Hospital Anxiety Depression Scale (HADS), Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F), and M. D. Anderson Symptom Inventory-Gastrointestinal Cancer Module. Data were analyzed using descriptive statistics, correlation, and path analysis. Results: CRCI was directly affected by cancer symptoms (${\beta}=.19$, p=.004) and fatigue (${\beta}=.56$, p<.001)($R^2=47.2%$). Depression was directly affected by fatigue (${\beta}=.48$, p<.001) and CRCI (${\beta}=.27$, p<.001). However, The impact of cancer symptoms on depression was confirmed through the mediating effect of CRCI. Conclusion: Results indicate that in patients with gastrointestinal cancer undergoing chemotherapy along with the direct physiologic effects (fatigue, symptoms) of cancer treatment may have altered cognitive function leading to depression.

대학생의 스마트폰 과다사용이 지각된 인지기능, 피로 및 주간 졸림증에 미치는 영향 (Effects of Smartphone Overuse on Perceived Cognitive Function, Fatigue, and Daytime Sleepiness among College Students)

  • Purpose: This study was aimed to examine the effects of smartphone overuse and individual factors including age, relationship with friends, satisfaction with school life, and time of smartphone usage on perceived cognitive functions, fatigue, and daytime sleepiness among college students. Methods: Three-hundred college students were recruited after confirming the absence of major depressive symptoms. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation, and multiple regression. Results: Smartphone overuse commonly explained the variance in perceived cognitive functions, fatigue, and daytime sleepiness, after controlling for individual factors. In addition, satisfaction with school life was significantly related with better effectiveness of cognitive functions while being male, having good relationships with friends and satisfaction with school life were relatively associated with a lower level of fatigue. Conclusion: The findings of this study revealed that the overuse of smartphone can serve as a psycho-cognitive burden leading to ineffective functioning in daily living. Students who are female and have issues regarding school life and friendship may be a high risk group for experiencing psycho-cognitive dysfunction associated with smartphone overuse. Further investigation is needed to develop effective interventions to improve psycho-cognitive health associated with the excessive use of smartphone in young adults.

Canonical Correlation of 3D Visual Fatigue between Subjective and Physiological Measures

  • Objective: The aim of this study was to investigate the correlation between 3D visual fatigue and physiological measures by canonical correlation analysis enabling to categorical correlation. Background: Few studies have been conducted to investigate the physiological mechanism underlying the visual fatigue caused by processing 3D information which may make the cognitive mechanism overloaded. However, even the previous studies lack validation in terms of the correlation between physiological variables and the visual fatigue. Method: 9 Female and 6 male subjects with a mean age of $22.53{\pm}2.55$ voluntarily participated in this experiment. All participants were asked to report how they felt about their health sate at after viewing 3D. In addition, Low & Hybrid measurement test(Event Related Potential, Steady-state Visual Evoked Potential) and for evaluating cognitive fatigue before and after viewing 3D were performed. The physiological signal were measured with subjective fatigue evaluation before and after in watching the 3D content. For this study suggesting categorical correlation, all measures were categorized into three sets such as included Visual Fatigue set(response time, subjective evaluation), Autonomic Nervous System set(PPG frequency, PPG amplitude, HF/LF ratio), Central Nervous System set(ERP amplitude P4, O1, O2, ERP latency P4, O1, O2, SSVEP S/N ratio P4, O1, O2). Then the correlation of three variables sets, canonical correlation analysis was conducted. Results: The results showed a significant correlation between visual fatigue and physiological measures. However, different variables of visual fatigue were highly correlated to respective HF/LF ratio and to ERP latency(O2). Conclusion: Response time was highly correlated to ERP latency(O2) while the subjective evaluation was to HF/LF ratio. Application: This study may provide the most significant variables for the quantitative evaluation of visual fatigue using HF/LF ratio and ERP latency based human performance and subjective fatigue.

  • 항해사의 업무수행도 감소는 해양사고 발생의 주요 원인 중의 하나로 자주 언급되고 있다. 인터뷰와 항해 기록 분석을 통한 선원의 작업시간 분석에 대한 연구는 일부를 수행되었으나, 작업부하, 작업교대, 스트레스, 수면 등이 선원의 인지능력에 미치는 영향을 분석한 사례 매우 한정적이다. 항공, 철도와 같은 다른 운송 산업에서는 승무원의 근무 패턴을 관리하기 위한 피로관리 도구를 개발하고 있고 있는데, 이러한 도구들은 선원, 항해사 및 육상관리자의 효율적인 근무일정을 계획하는데 도움을 줄 수 있다. 따라서 본 연구에서는 항해사의 인지능력을 평가하기 위해, 해상 충돌상황에서의 종합적 대응능력을 평가할 수 있는 소프트웨어 프로그램을 개발하였다. 항해 전문가를 대상으로 한 실험을 통하여, 평가기준을 개발하였으며, 실습 항해사를 대상으로 하여 시스템에 대한 검증이 이루어졌다. 본 시스템은 선박운항 시 충돌사고를 회피하기 위한 항해사의 인지능력을 평가할 수 있으므로, 당직 전 혹은 승선 전에 짧은 시간 동안의 평가를 통하여 항해사의 전반적 인지능력을 평가하고, 이 결과를 항해사에게 경고메시지 형태로 전달할 수 있다.

  • A fixed type driving simulator is constructed with a car body, beam projector, operation software , driving scenario, and audio equipments. With the simulator, the cognitive effects of fatigue due to two hour continuous driving of a straight road is investigated . The effects of alcohol on driving performance is also studied. The braking operation and lane keeping performance due to fatigue and alcohol are investigated. Changes of vehicle motion due to these effects are verified by computer simulation.

위암환자의 피로와 영양상태와의 관계 (Relationship between Fatigue and Nutritional Status in Patients with Gastric Cancer)

  • Purpose: The purpose of this study was to identify the relationship between fatigue and nutritional status in gastric cancer Design. A correlational and crossectional study design was used. Method: Fatigue was measured using the Revised Piper Fatigue Scale. The parameters for nutritional status consisted of BMI, Hb, Hct, total protein, albumin and lymphocyte count. Result: The mean score of fatigue was $94.31{\pm}22.08(mean\;average\;4.48{\pm}1.00)$. The subdimensional scores of fatigue revealed $34.15{\pm}8.76$ for behavior/severity, $22.00{\pm}7.93$ for cognitive/mood, $21.67{\pm}5.56$ for sensory, and $20.81{\pm}6.69$ for affective. And the level of BMI, Hb, & Hct were under the normal ranges. In the ancillary analysis, there was a significant difference on the fatigue scores by the groups of present diet(F=3.343, P=.026). Age, types of treatments, period after diagnosis and weight change were related to nutritional status. There was a significant negative correlation between fatigue and nutritional status(r=-.371, P=.004). Conclusion: Considering the relationship between fatigue and nutritional status, nurses can identify the risk group most vulnerable to fatigue and malnourishment in order to provide appropriate interventions for them.
