Social Readjustment and Relating Factors in Stroke Survivors (뇌졸중환자의 사회적 재적응과 관련요인)

  • Social readjustment is very important in rehabilitation of stroke survivors. The purpose of this study was to examine the level of social readjustment and related factors in stroke survivors. Especially, to find the most useful predictor for social readjustment is a major point of this research. The study included 254 outpatients who were given follow-up care after discharge from. Occupational rehabilitation and resumption of the perceived meaningful social activity prios to the stroke were used to measure social readjustment. The resumptions of first meaningful social activity was the best predictor for life satisfaction in stroke survivors(r=.245, p<.01). The substance of perceived meaningful social activities were job, meeting with friends, hobby and activities for the family. The sum of meaningful social activities (r=.175, p<.01) and occupational rehabilitation (r=.116, p<.05) were significantly related to life satisfaction. There were significant difference in IADL, depression and life satisfaction according to resumption of meaningful social activity. But, occupational rehabilitation was not related to depression. The level of social readjustment was different between occupational rehabilitation and resumption of meaningful social activity. It was 45.7% in the first meaningful social activity and 36.6% in occupational rehabilitation. The related factors with resumption of the first meaningful social activity were that of IADL, depression, illness intrusiveness and cognitive function. And the level of IADL, illness intrusiveness, cognitive function, and age were significantly related to occupational rehabilitation. In conclusion, we suggest that the resumption of the first meaningful activity in stroke survivors is the best predictor of social rehabilitation. Thus, nurses need to work for resumption of meaningful activity as well as occupational rehabilitation.

Correlation Analysis between Cognitive function and Praxis tasks in the Elderly

  • The purpose of this study was to identify differences in cognitive function according to the presence or absence of apraxia and the tasks most relevant to the cognitive function among the various types of tasks in the apraxia test. The subjects were 42 community residents who participated in a cognitive rehabilitation program related to dementia in a Chungbuk area. MMSE-K and BCoS(Birmingham Cognitive Screen) apraxia test were administered to all subjects. The apraxia test includes three types of tasks, gesture production tasks that make meaningful movements according to verbal instructions, gesture recognition tasks that display behavior after make sense of meanings, and meaningless imitation task. Apraxia group(n=30, MMSE-K mean score: 25) showed lower cognitive function than group without apraxia(n=12, MMSE-K mean score: 28). All tasks in the apraxia test showed a significant correlation with cognitive function, but the meaningless imitation task had a negligible correlation. The apraxia test is a good way to assess cognitive function, and it may be more effective to use meaningful behavior to replace cognitive testing.

Developing a Home-based Self-management Support Intervention for Pulmonary Rehabilitation in Patients with Chronic Obstructive Pulmonary Disease (만성폐쇄성폐질환 환자의 호흡재활을 위한 재가 자가관리 증진 중재 개발)

  • Purpose: The purpose of this study was to develop a home-based self-management support intervention (SMSI) for enhancing pulmonary rehabilitation (PR) in patients with chronic obstructive pulmonary disease (COPD) in Korea. Methods: This methodological study was conducted by following these 4 steps: constructing the preliminary draft of a home-based SMSI from existing recommendation and a literature review on PR and self-management interventions; testing content validity with 6 experts in COPD; exploring clinical applicability of the intervention by applying it to 4 COPD patients; determining the final intervention. Results: The final intervention included 4 components and contents identified from the literature review as essentials for self-management of COPD patients: education; exercise training and practice including breathing, muscle strengthening and aerobic exercises; cognitive behavioral strategies including informative self-decision making, collaborative goal setting and supportive advice; and action planning for exacerbation. The intervention was designed to be run by a trained nurse and had 8 weekly sessions consisting of three 60-minute face-to-face sessions and five 20-minute phone-call sessions. Conclusion: The intervention developed in this study incorporates essential components of self-management, i.e. action plan and cognitive behavioral strategies, and will contribute to enhancing and maintaining effects of PR by increasing self-management in COPD patients.

A review of cognitive orientation to daily occupational performance with stroke

  • Objective: The self-decisions of the client regarding the meaningful work as a therapeutic approach of client-orientation. The Cognitive Orientation to daily Occupational Performance (CO-OP) is an occupation-oriented problem-solving approach. The purpose of this study was to describe the goals and intervention protocols of CO-OP in those affected by stroke. Design: A systematic review. Methods: Using EBSCOhost, PubMed, and ProQuest databases, we searched studies published in the past decade that utilized the CO-OP intervention. An initial search revealed 71,171 potential articles. After applying our search criteria to screen the titles, abstracts, and full-text, we included 7 articles that met our inclusion and exclusion criteria. In this study, we used the patient, intervention(s), comparison, outcome method to analyze the 7 selected studies. We analyzed the frequency of goals and intervention protocols. Results: Seven articles met our selection criteria; these studies included participants with an almost normal cognitive function from inpatient and outpatient rehabilitation facilities. CO-OP was used for 237 goals; the most used goal was the instrumental activities of daily living. The training procedure used 3 types of self-selecting goals in the activities. One of the goals was not trained, but was only evaluated to determine the generation effect. The most common outcome measurements included the Canadian Occupational Performance Measure and the Performance Quality Rating Scale. Conclusions: This research provided information about the effectiveness of CO-OP and selecting the correct evaluation tool to assess the efficiency of the intervention. This study suggests that treatment with CO-OP in occupational therapy is effective and that it outlines common protocols.

Analysis of cognitive factors affecting stroke patient's activity of daily living performance -Using the computerized neurocognitive function test- (뇌졸중 환자의 일상 활동 수행에 영향을 미치는 인지요인 분석 -전산화 신경인지기능검사 중심으로-)

  • This research analyzed the cognitive factors affecting stroke patient's activity of daily living performance and suggests the approaches which can contribute to the effective activity of daily living performance in the rehabilitation treatment of stroke patients. In this study, Seoul Computerized NeuroCognitive Function Test (SCNT) and MBI have been performed for 21 patients under extensive rehabilitation treatments, hospitalized in the rehabilitation clinic of A hospital after being diagnosed as a stroke caused by cerebrovascular disorders. To assess the effectiveness of the cognitive factors which affect the stroke patient's activity of daily living performance, activity of daily living performance values were set as dependent variables and 10 cognitive factors were included in the model to carry out analysis of the multiple regression analysis. The results show that stroke patient's activity of daily living performance have statistically significant correlations with divided attention, motor control and selectivir attention. In addition, cognitive factors explained 69.8% of the stroke patient's activity of daily living performance. Consequently, if divided attention and motor control are considered as a focal point of training in the rehabilitation treatment of stroke patients, we can effectively promote the improvement of the activity of daily living performanceroutine activities.

The Relationship Between Activities of Daily Living and Cognitive Score in Stroke Patients (뇌졸중 환자의 인지기능이 일상생활활동에 미치는 영향)

  • The purpose of this study was to examine the effects of stroke patients' cognitive score on their the activities of daily living. The subjects of this study were 30 stroke patients who were admitted to T$\breve{a}$e-Bong hospital from November, 2002 to March, 2003. The subjects were administerd an MMSE as a cognitive assessment and an MBI as a functional assessment upon referral to physical therapy initially. The results were as follows: 1. The MMSE scores for the stroke patients were related to the patients' abilities to perform their activities of daily living. The changes of MBl scores significantly correlated with the changes of MMSE scores (p<.05). 2. The subjects with left hemispheric lesion scored higher in MMSE than those with right hemispheric lesions (p<.05). 3. The hemispheric lesions did not significantly affect the activities of daily living score (p>.05).

Correlations between Cognitive Function and Functional Ability in Strokes using MMSE and FIM

  • The Purpose of this study was to investigate correlations between the Mini-Mental State Examination (MMSE) and the Functional Independence Measure (FIM) in strokes. The data collected retrospectively from fifty-five stroke rehabilitation patients. The MMSE and FIM at the time of admission and discharge were obtained from patients' medical records. Firstly, Pearson correlation coefficients of the MMSE score at the time of admission revealed $0.286{\sim}0.747$ with FIM at the time of discharge (p<0.05). Also, the change of MMSE score significantly correlated with the change of total FIM scores in strokes (r=0.409, p<0.05). Because the MMSE scores at the time of admission and FIM at the time of discharge are correlated, the MMSE scores can be used to predict the FIM at the time of discharge and establish a rehabilitation program.

The Characteristics of semantic association task performance in elderly with subjective memory impairment and mild cognitive impairment (주관적 기억장애 및 경도인지장애 노인의 의미연상과제 수행 특성)

  • The loss of semantic knowledge and impairments in semantic associations by semantic category is gaining increasing attention, as indicators of early-stage cognitive decline. As such, we assigned semantic association task (SAT) to normal elderly (NE) and those with subjective memory impairment (SMI) or mild cognitive impairment (MCI) to examine their performance by semantic subcategories and the differences in error patterns. We found a significant difference in the number of correct response and reaction time under the SAT categories among the three groups, with the highest performance observed in 'function' and the lowest performance in 'superordinate' and 'part/whole'. Moreover, the error frequency was the lowest in NE, followed by those with SMI and MCI, with the latter two groups showing a significant increase in no-response. Our findings demonstrate the varying extent and process of impairments in the semantic network by category over different stages of cognitive decline. Thus, we proposed SAT performance as an indicator to detect and follow-up on cognitive decline in elderly with cognitive disorder.

Effects of Neurofeedback Training and Computer-Assisted Cognitive Rehabilitation on Cognition and Upper Extremity Function in PostStroke. (신경되먹임 뇌파 훈련과 컴퓨터보조 인지 재활훈련이 뇌졸중 환자의 인지와 상지기능에 미치는 영향)

  • Objective : This study was to evaluate the effects of a Neurofeedback(NFB) and Computer Assisted Cognitive Rehabilitation(CACR) training to improve on cognition and affected arm function in stroke subjects. Methods : Participants were randomly allocated to three groups: NFB(n=14), CACR(n=14) and control(n=16). All groups received occupational therapy and physical therapy for 5 session 30 minutes per week during 6 weeks. Also NFB and CACR group practiced additional NeuroComp training and RehaCom training for 30 sessions 30 minutes during 6 weeks. Results were evaluated by cognition, affected arm function. Results : There were significantly increased by CACR training that outcomes of MMSE-K(p<.05). And there weren't significantly difference by NFB and CACR training that outcomes of the affected arm function. And a difference between three groups wasn't found. Conclusion : The NFB and CACR training improves cognitive function. These results suggest that NFB and CACR training is feasible and suitable for individuals with stroke.

A study of the multicomponent therapeutic recreation function intervention strategy by analysis on the operating condition of the cognitive rehabilitation program in dementia care center

  • This study was conducted with 50 elderly people each (5) participating in the cognitive rehabilitation treatment program at the Dementia Care Center in D City to derive the development direction and contents of a multidimensional therapeutic recreation program and a revitalization plan through analysis of the current status and actual conditions of the cognitive rehabilitation program at the Dementia Care Center. aperture) was selected, and 9 people were selected as the subject of expert group opinion collection. The collected data was SPSS ver. Using the 18.0 statistical program, descriptive statistics and the importance and priority of each component were analyzed by hierarchical structure analysis. First, unlike the needs of users, the cognitive rehabilitation support programs currently being provided are not sufficient and require considerable experience. It was found to be low, and the areas for improvement were the expansion of care and protection facilities and the development of various programs to meet the needs of users. Second, the importance and priority of each component of therapeutic recreation were categorized into 6 major categories: exercise therapy , middle category (16 items) behavior-centered approach to exercise therapy, small category (47 items) strength and brain gymnastics, and silver health gymnastics were the highest. This result shows that a multidimensional program plan that considers the priorities of each area must be made when developing a therapeutic recreation program.
