가상현실기반 인지재활훈련과 컴퓨터기반 인지재활훈련이 회복기 뇌졸중 환자의 기능과 전전두엽 피질에 미치는 영향에 대한 사전연구 (Preliminary Study on the Effects of Virtual Reality-based Cognitive Rehabilitation and Computer-based Cognitive Rehabilitation on Function and Prefrontal Cortex in Convalescent Stroke Patients)

  • PURPOSE: This study compared the effects of computer-based and virtual reality-based cognitive rehabilitation programs on the cognitive function, upper limb function, activities of daily living, and their impact on the prefrontal cortex in convalescent stroke patients. METHODS: Ten recovering stroke patients were assessed for their cognitive function, upper limb function, and daily living activities using the Neurobehavioral Cognitive Status Examination, the Korean version of the Fugl-Meyer Assessment, and the Korean version of the Modified Barthel Index. The prefrontal cortex activity was measured with functional Near Infrared Spectroscopy. The virtual reality-based cognitive rehabilitation group utilized a program of daily living activities delivered via a laptop and Oculus Rift. The computer-based cognitive rehabilitation group performed various cognitive tasks on an all-in-one PC. Both groups underwent cognitive rehabilitation training for 30 minutes per day, three times a week, for six weeks, with identical conventional rehabilitation therapies in the hospital. RESULTS: Both programs positively impacted the cognitive and physical functions. On the other hand, the virtual reality-based cognitive rehabilitation program had a larger influence on improving the cognitive and physical functions of convalescing stroke patients. CONCLUSION: The virtual reality program suggests its potential to enhance cognitive and physical functions in convalescent stroke patients through increased engagement, focus, real-time feedback, and game elements, making it a promising rehabilitation approach.

The Effect of Cognitive Rehabilitation Program Combined with Physical Exercise on Cognitive Function, Depression, and Sleep in Chronic Stroke Patients

  • Objective: This study was conducted to verify the effectiveness of a cognitive rehabilitation program consisting of physical exercises and mental activities for patients with chronic stroke with mild cognitive impairment (MCI). We aimed to investigate how this cognitive rehabilitation program affects patients' cognitive ability, depression, and sleep quality. Design: One group pretest-posttest design Methods: The study was conductedon 12 patients who participated in thecognitive rehabilitation complex exercise program for 16 weeks. The Korean version of the Montreal Cognitive Assessment (MoCA-K), Hamilton Depression Rating Scale (K-HDRS), and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate the measured variables before and after study participation. The cognitive rehabilitation complex exercise program included 30 minutes of cognitive exercise and 30 minutes of Brill Exercise. The Wilcoxon signed-rank test was used to compare the variables before and after program participation. Cronbach's ɑ was used to assess the reliability of the test variables. Results: The post-program assessment showed a statistically significant increase in the MoCA-K score, which measures cognitive function (Z=-2.628, P=0.009). For depression ratings, there was a statistically significant decrease in the K-HDRS score (Z=-2.041, P=0.041). For sleep quality, although there was a numerical increase in the PSQI score, the difference was not statistically significant (Z=-0.702, P=0.483). The reliability test confirmed that all the individual test variables exhibited high reliability (cognitive function, 0.859; depression, 0.872; sleep, 0.822). Conclusions: We found that cognitive rehabilitation program used in this study had a positive effect on the cognitive function and depression in patients with chronic stroke with MCI.

  • Purpose: This study aims to investigate the effects of a group computerization cognitive rehabilitation program on cognitive function in patients with cognitive impairment. Methods: The subjects of this study comprised 34 patients with cognitive impairment (15 males and 19 females) who were randomized into two groups. The experimental group (n = 17) were trained with the group computerization cognitive rehabilitation program, while the control group (n=17) received traditional cognitive therapy. The interventions for both groups were performed for 1 hour per day, once a week for 12 weeks. The cognitive functions of the subjects before and after the experiment were measured using the Korean mini-mental state examination (K-MMSE), global deterioration scale (GDS), clinical dementia rating (CDR), and Lowenstein occupational therapy cognitive assessment (LOTCA). A paired t-test was conducted to examine the intragroup differences before and after the experiment, and ANCOVA was performed to check intergroup differences. Results: The intragroup comparison results showed that the cognitive function of the group that were trained with the group computerization cognitive rehabilitation program improved after the experiment more than for the group that received traditional cognitive therapy. The intergroup comparison results showed significant differences in orientation, visual perception, and visuomotor organization between the two groups. Conclusion: The results of this study confirmed that the group computerization cognitive rehabilitation program was more effective in improving cognitive function than the traditional cognitive therapy in patients with cognitive impairment. Based on the results of this study, the group computerization cognitive rehabilitation program can be used as an effective intervention method for patients with cognitive impairment.

발달장애 아동 부모의 인지재활 경험에 대한 질적 연구: 워드 클라우드 분석과 현상학적 연구 방법 혼합설계 (Parents' Perceptions of Cognitive Rehabilitation for Children With Developmental Disabilities: A Mixed-Method Approach of Phenomenological Methodology and Word Cloud Analysis )

  • 목적 : 본 연구의 목적은 현상학적 방법론과 워드 클라우드 분석을 통해 발달장애 아동 부모의 인지재활 경험에 대한 인식을 알아보기 위함이다. 연구방법 : 인지재활에 대한 부모 인식을 알아보기 위하여 발달장애 아동의 부모 5명을 심층 인터뷰하였다. 아울러 Python을 사용하여 워드 클라우드 분석을 하였고, 출현 빈도 수가 높은 단어를 중심으로 5명의 연구자가 의미 단위와 주제를 현상학적 방법으로 분석하였다. 결과 : 인지재활의 현상에 대한 의미 단위 43개 및 구성요소 9개를 도출하였고, 이로부터 총 3개의 주제를 최종 도출하였다. 도출된 중심 주제는 인지재활의 정의, 인지재활의 문제점, 인지재활 기관 선택 시 고려사항이었다. 인지재활은 발달장애 아동의 학습, 일상생활 기능, 인지기능 향상을 목적으로 하는 치료라는 인식이 있었다. 인지재활의 문제점으로는 치료방식, 치료사의 전문성, 치료비용에 관한 문제인식이 있었다. 마지막으로, 인지재활 기관 선택 시 고려사항으로는 치료사의 전문성, 입소문, 비용과 시간의 적정성이 있었다. 결론 : 발달장애 아동의 부모는 인지재활을 통해 아동의 실질적인 기능 향상을 기대하고 있었고, 이와 더불어 임상에는 현실적인 문제들이 존재하고 있었다. 본 연구는 부모가 인식하는 문제점들을 보완한 보다 나은 인지재활 서비스로 발전되는 데 기초자료가 될 것이다.

경도인지장애 노인의 인지능력 향상을 위한 로봇 콘텐츠 개발 (Development of Robot Contents to Enhance Cognitive Ability for the Elderly with Mild Cognitive Impairment)

  • This paper describes the effect of a robot cognitive rehabilitation program on cognitive functions for the elderly with mild cognitive impairment, and compares it with traditional cognitive therapy programs. Three experiment groups including cognition therapy group, robot cognitive rehabilitation group, and hybrid group have been sampled and one comparative group has been organized for this research. 32 old people whose ages are between 61 and 88 with mild cognitive impairment participated in the programs with an admission of W care hospital. According to the program results, the cognitive therapy program alone had shown a positive effect on the attention function, and the robot cognitive rehabilitation program alone had a positive effect on the total intelligence and memory function. However, a simultaneous operation with both programs had shown a positive effect on the three intelligence areas such as total, basic, and management quotients as well as attention and memory functions as subsidiary factors. This paper has verified that the proposed robot cognitive rehabilitation program makes a positive effect on a cognitive function and plays a complementary role with traditional cognitive therapy programs.

국내 치매 환자를 대상으로 한 전산화 인지 재활 적용 연구에 대한 체계적 고찰 (The Systematic Discussion of a Study on the Application of Computerized Cognitive Rehabilitation on Patient with Dementia in Korea)

  • Purpose : This study aimed to investigate a study on the intervention of computerized cognitive rehabilitation applied to patients with dementia in Korea. Thus, a systematic discussion was conducted. Methods : Studies from January 2000 to December 2016 were searched in the Korea Education and Research Information Service. The main key words used were: "dementia," "computerized cognitive rehabilitation," "computerized cognitive training," "computer assisted cognitive therapy," and "computerized cognitive therapy." Result : A total of five studies were selected. The effects of the computerized cognitive rehabilitation therapy applied to patients with dementia resulted in improved cognitive functions, as well as daily living and activities instrumental to daily living. There was also an increase in brain activity. The qualitative aspects of this study showed a low level of internal and external validities due to the small number of subjects, short period of intervention, and limited study environment. Conclusion : In the future, it is necessary to continue a study in which the level of evidence is high to determine whether various cognitive rehabilitation programs are effectively applied to patients with dementia.

  • 노인과 경증 뇌질환(치매 및 뇌졸중 등) 환자들 중 일정 정도의 자력 행동 능력과 인지 능력이 있는 경우에는 운동과 인지 재활을 병행해야만 하는 경우가 많다. 그러나 기존의 재활 시스템은 대부분 운동 또는 인지 재활로 분리 또는 특화 되어 있는 경우가 많아서 두 가지 모두를 연계하는 형태에 대한 필요성이 있다고 생각된다. 본 논문에서는 ICT 기술의 하나인 CAN 통신 프로토콜을 이용하여 운동과 인지 재활을 병행할 수 있도록 하는 게임형 재활시스템을 제안하고자 한다. 이 시스템은 CAN BUS 구조를 이용하여 재활 시스템을 구성하는 각각의 개별 모듈들을 다양한 형태로 분리 및 결합 사용이 가능하도록 한다. 환자(또는 노인)의 인지 능력과 운동 능력의 정도에 따라 다양한 형태의 재활 환경을 구현할 수 있음을 보이고자 제안된 시스템의 프로토 타입을 실제로 제작하였다. 아울러 이 시스템을 이용한 몇 가지 실험을 통하여 여러 환경에 적용 가능한 운동/인지 병행 재활의 구현 가능성을 보이고 있다.

경두개 직류전류자극과 전산화 인지재활 프로그램이 뇌졸중 환자의 인지기능 및 우울감에 미치는 영향 (The Effect of Transcranial Direct-Current Stimulation on Cognitive Function and Depression in Stroke Patient's through a Computerized Cognitive Rehabilitation Program)

  • Purpose : The purpose of this randomized controlled trial study was to examine the effect of transcranial direct current stimulation (tDCS) on cognitive function and depression in stroke patients. Methods : Thirty stroke patients were randomly divided into an experimental group (n = 15) and a control group (n = 15). The experimental group received tDCS while performing computerized cognitive rehabilitation programs, and the control group was provided with sham tDCS while operating the same programs. The 30-minute intervention was implemented five times per week for six weeks. To assess cognitive function before and after the intervention, the Neurobehavioral Cognitive Status Examination was conducted; the Beck Depression Inventory BDI was employed to assess depression. Results : The experimental group showed statistically significant increases in cognitive function and decreases in depression (p < .05 ). Comparing the amount of variation between the groups after arbitration also showed significant differences in cognitive function and depression between the two groups (p > .05). Conclusion : The application of tDCS and computerized cognitive rehabilitation programs for stroke patients may positively affect their cognitive function and depression. Therefore, tDCS used with computerized cognitive rehabilitation programs is positively applicable to the enhancement of cognitive function in stroke patients and reduction of depression.

인지 재활훈련 평가 시 단기 스트레스가 미치는 영향 연구 (The Evaluation of the Short-term Stress Effect on Cognitive Rehabilitation Training Assessment)

  • The cognitive rehabilitation training is important for treating many cognitive impairment conditions, including Parkinson's disease, stroke, and ADHD. In this study, we developed a new evaluation system to improve the measurement of the conventional evaluation systems for cognitive rehabilitation training. The developed system measured the activity of dopamine(DA) and an autonomic nervous system(ANS) with photoplethysmography and electromyography. The results demonstrated that the cognitive capacity was increased but the activity of DA was decreased with unbalanced ANS by short-term stress. Based on the results, the effect of short-term stress should be recognized for the cognitive rehabilitation training.

  • Purpose: The purpose of this study was to test the effectiveness of a cognitive rehabilitation program for the patients having an acute stroke. The variables evaluated were cognitive function, depression and activities of daily living (ADLs). Methods: A non-equivalent control group pretest-posttest design with sixty-two subjects from two separated institutions was the design for the study. Thirty-three patients were assigned to the treatment group and 29 patients comprised the comparison group. The treatment group participated in the nursing intervention for cognitive rehabilitation for a three week period of time. The study was conducted from September 2010 to April 2011. Results: Following the three week intervention, the cognitive function and activities of daily living of the treatment group were significantly improved compared to the comparison group (t=6.33, p<.001; t=4.57, p<.001). Specifically, depression was significantly decreased in the treatment group (t=3.95, p<.001). Conclusion: The cognitive rehabilitation program was effective in increasing the treatment group cognitive function and activities of daily living along with a decrease in depression scores. Therefore, Cognitive Rehabilitation Program could be expected a beneficial nursing intervention in stroke patients.
