경도인지장애 선별을 위한 전산화 평가 항목 개발 (Development of Computerized Screening Test Items for Mild Cognitive Impairment)

  • 목적 : 본 연구는 경도인지장애 선별을 위한 전산화 평가 항목을 개발하는 것이다. 연구방법 : 문헌고찰을 통해 경도인지장애 선별을 위해 사용되는 기존의 전산화 평가에서 항목들을 추출하였다. 이후 델파이 기법을 실시하였으며, 전문가 패널들의 의견을 취합하여 추출된 항목들을 수정 및 보완하고 최종적으로 중요도 및 적합도를 판단하여 평가 항목들을 선택하였다. 결과 : 문헌고찰을 통해 12개의 전산화 평가에서 37개의 항목을 추출을 하였고 1차적으로 전문가들의 의견을 통해 11개의 항목을 추가하였다. 이후 2차 전문가 모임을 통해 48개의 항목에서 경도인지장애 선별에 부적합한 18개의 항목이 제거되었다. 마지막으로 내용타당도 분석을 실시한 결과, 30개 항목에서 12개 항목이 제거되어 최종적으로 16개의 항목이 선택되었다. 최종 16개의 항목은 주의력, 기억력, 실행기능 영역으로 구성되었다. 결론 : 경도인지장애 선별을 위한 16개의 전산화 평가 항목이 개발되었다. 추후 이 항목들을 이용하여 타당성 연구를 실시하여 경도인지장애 선별에 적합한지 확인할 필요가 있다. 또한 이 전산화 평가를 정상 노인 및 경도인지장애 노인에게 실시하여 전산화 평가에 대한 표준화 연구를 진행하여 임상에서 사용할 수 있도록 해야 할 것이다.

국내 경도인지장애 노인의 비약물적 중재에 대한 통합적 문헌고찰 (An Integrative Review of Non-pharmacological Intervention in Elderly Patients with Mild Cognitive Impairment)

  • 본 연구의 목적은 경도인지장애 노인을 대상으로 비약물적 중재를 수행한 문헌을 검색하고, 이를 분석함으로써 추후, 경도인지장애 노인을 위한 간호학에서의 비약물적 중재프로그램 개발을 위한 기초자료를 제공하기 위함이다. 연구방법으로 Whittemore & Knafl가 제시한 통합적 문헌고찰을 수행하였다. 본 연구결과 경도인지장애 노인을 대상으로 비약물적 중재를 수행한 연구는 최종 11편 이었으며, 연구의 질적 수준은 두 집단 비무작위 연구가 9편, 단일 집단 비무작위 연구가 2편으로 나타났다. 또한 2010년 이후의 연구가 10편이었고, 다양한 분야에서 경도인지장애 노인을 위한 비약물적 중재가 수행되었다. 경도인지장애노인을 위한 비약물적 중재는 인지강화 프로그램 4편, 운동프로그램 6편, 음악프로그램 1편이었다. 경도인지장애 노인을 대상으로 수행한 비약물적 중재의 효과는 각 연구마다 다양한 결과를 나타냈으며, 이러한 연구결과를 바탕으로 추후 지속적인 반복 연구가 필요할 것으로 생각된다.

암 관련 인지 장애의 한의학적 치료에 대한 문헌 고찰 (A review of traditional Korean medical treatment for cancer-related cognitive impairment)

  • Objectives: This article aims to review clinical studies related to traditional Korean medicine (TKM) for cancer-related cognitive impairment (CRCI) up to date. Methods: EMBASE, Pubmed and eight Korean databases were searched for clinical studies about TKM for CRCI up to July 2016. We selected articles about subjects with cancer, using TKM interventions including acupuncture, moxibustion or herbal medicine, containing assessment about cognitive function. Results: Two randomized controlled trials (RCTs) and one randomized controlled feasibility study were included. Selected studies were containing assessments about cognitive function but not as primary outcomes. The three studies included one acupuncture study and two herbal-medicine studies. Only one herbal-medicine study showed improvement in cognitive function. Conclusions: Few clinical studies mainly focusing CRCI treated with TKM have been performed and have shown contradictory results until now. Future studies should be performed considering findings from pre-clinical and clinical studies altogether, and they should be controlled to minimize risk of bias thorough out overall courses of designing, conducting and reporting them.

운동·인지 이중과제 프로그램이 경도인지장애 노인의 인지기능 및 우울에 미치는 영향 (The Effects of Exercise-Cognitive Combined Dual-Task Program on Cognitive Function and Depression in Elderly with Mild Cognitive Impairment)

  • Purpose: This study was to develop and verify the effects of the exercise-cognitive combined dual-task training program on cognitive function and depression of the elderly with mild cognitive impairment (MCI). Methods: A non-equivalent control group pretest-posttest design was used. The participants were assigned into two groups: an experimental group receiving an exercise-cognitive combined dual-task (n=20) and a control group receiving a simple-task (n=18). After 8 weeks of intervention (2 days per week), the change in depression and cognitive functions were compared between the groups. Results: General cognitive function (t=-2.81, p=.011), frontal cognitive function (Z=-3.50, p<.001), attention/working memory function (U=-2.91, p=.004), depression (t=4.96, p<.001) of the experimental group were significantly increased than those of the control group. Conclusion: The findings of the study showed that an exercise-cognitive combined dual-task program for MCI was effective in improving general cognitive function, frontal and executive function, attention/working memory function, and reducing depression.

The Effect of Swiss Ball Program on Lower Extremity Function of Elderly with Mild Cognitive Impairment

  • The purpose of this study was to determine the effects of swiss ball program training on lower extremity function of old elderly with mild cognitive impairment. The subjects were 30 mild cognitive impairment eldrly people aged between 65 and 80, who were divided into the swiss ball program training group(n=15) and the control group(n=15). The swiss ball program training group engaged in a 50-minute exercise session using. Lower extremity function scale of the two groups were measured before and after the intervention. The results are as follows. According to the swiss ball program training conducted to examine the effects of the training on lower extremity function, Moreover, the two groups showed significant differences in lower extremity function. These results indicate that virtual reality training is effective in improving lower extremity function. To conclusion, swiss ball program was found to have a positive effect on elderly lower extremity function. Swiss ball program training can be proposed as a form of fall prevention exercise for the mild cognitive impairment. Swiss ball program may be helpful to reduce the incidence of dementia and behavioral complications.

  • 본 연구의 목적은 지역사회에 거주하고 있는 경도인지장애를 보이는 노인을 대상으로 한 컴퓨터 기반의 인지재활프로그램의 적용이 인지기능과 도구적 일상생활수행능력에 미치는 영향을 알아보고는 것이다. MCI 노인 22명을 실험군 11명, 대조군 11명으로 무작위 할당하여 진행하였으며, 총 10주간, 주 3회, 회기당 30분씩 실험군은 CoTras, 대조군은 전통적 인지 재활을 받았다. 중재 전후로 인지기능과 수단적 일상생활활동의 수행 능력애 대한 변화를 알아보기 위해 NCSE와 K-IADL을 사용하였다. 연구 결과, 실험군은 집중력과 기억력을 포함한 전반적인 인지기능과 수단적 일상생활활동의 수행능력의 향상을 보였다. 지역사회 경도인지장애 노인들을 대상으로 인지기능과 수단적 일상생활활동의 수행능력 증진을 위해 CoTras의 적용을 고려해 볼 수 있을 것이다.

  • 본 연구는 메타분석의 방법으로 경도인지장애 노인을 대상으로 한 컬러테라피 중재 연구에서 인지기능과 우울에 효과가 있었는가를 확인하고자 시도되었다. 본 연구에서는 2000년부터 2022년 10월까지 국내 데이터베이스를 통해 실험연구 최종 8편의 연구에 대한 인지기능과 우울의 효과 크기를 산출하였다. 그 결과 전체 연구의 통계적으로 유의한 평균 효과크기를 확인하였다. 집단 간 효과크기 차이 검정에서도 인지기능과 우울 모두 유의한 효과크기를 확인할 수 있었다. 본 연구를 통해 컬러테라피가 경도인지장애 노인의 인지기능을 향상시키고 우울을 완화시킬 수 있는 효과적인 중재임을 확인하였다. 이 연구결과를 근거로 칼러테라피를 활용한 경도인지장애 노인 대상 중재프로그램의개발이 필요하다고 본다.

산림치유프로그램이 인지기능에 미치는 영향 (The Effects of Forests Healing for Cognitive Function)

  • Objectives : The purpose of this study is to examine the effects of 3 day Forests healing programs for cognitive improvement, which consists of helpful contents of Korean medicine. Methods : This study examined the effects of forests healing for mild cognitive impairment patients, who visited a Korean medicine hospital neuropsychiatry clinic for outpatients. The Forests healing programs for cognitive improvement was run with 24 participants, in which 9 of them quit in the middle of the program, so excluded in data analysis. The Forests healing programs for cognitive improvement was consisted of 3 day helpful Korean medicine contents. The primary outcome measurement was a Computerized Neurocognitive Test (CNT), which measured the cognitive functions with computer programs. Results : The results of this study showed that CNT scores were not significantly changed after 3 day forests healing. General psychological states, including depression, anxiety, and anger, were also checked. Among them, depressive mood significantly improved. Quality of life (QOL) was also enhanced after 3 day program. These results indicate that the 3 day Forests healing programs for cognitive improvement was effective for depressive mood and QOL of mild cognitive impairment patients, but not enough for cognitive functions directly. Conclusions : The 3 day Forests healing programs for cognitive improvement were shown to be an effective intervention in improving some aspects of mild cognitive impairment patients, including depressive mood, and QOL. We suggest that repeated participation of this program can enhance cognitive function of mild cognitive impairment patients.

The Motor-cognitive Training on Cognition and Physical Performance in the Older Adults with Mild Cognition Impairment : A Literature Review

  • Objective: Older adults with mild cognitive impairment (MCI) are more likely to progress to dementia. Motor-cognitive training is applied as a dual-task to improve the cognitive and physical functions of older adults with MCI. The purpose of the study was to know the recent trends in motor training types and outcome measures used for motor-cognitive training in older adults with MCI. Design: Aliterature review Methods: This literature review was conducted in Pubmed, MEDLINE® and Google Scholar with the following key words: older adults, mild cognitive impairment, motor-cognitive training, cognition, and dual-task. The 7 studies were found with the search tool and all studies were randomized controlled trials. Results: In motor-cognitive training in older adults with MCI, 6 studies applied aerobic exercise. And 3 out of 6 studies also applied strengthening exercises. One study applied dual tasks without aerobic exercise. In the 6 studies, overall cognitive and executive function were used as outcome measures, and physical function was evaluated as gait performance. Memory and physical frailty were also used as measurement tools. As a result of all studies, when motor-cognitive training was applied, cognition and physical performance showed significant results. Conclusions: A recent five-year study applied mainly aerobic exercise and strength training to older adults with MCI and found it to improve cognitive and physical performance.

경도인지장애 노인을 위한 지역사회 기반 다면적 인지훈련 프로그램 개발 및 효과평가 (Development and Evaluation of a Community based Multifaceted Cognitive Training Program for the Elderly with Mild Cognitive Impairment)

  • Purpose: The purpose of this study is to develop and evaluate the effects of a multifaceted cognitive training program on cognitive function, depression, and instrumental activities of daily living (IADL) in community dwelling elders with mild cognitive impairment. Methods: A non-equivalent control group pretest-posttest design was used. The subjects were recruited from a community health center in D metropolitan city and were assigned to the experimental or control group. Weekly 50-minute session intervention was delivered to the experimental group over 12 weeks. 8 weeks and 12weeks after intervention, the changes in cognitive function, depression, and IADL in the groups were measured and compared. Data were analyzed with Mann-Whitney U test, Wilcoxon Signed Rank test, and post-hoc test with Bonferroni correction using SPSS/WIN 23.0. Results: When compared with their counterparts (n=15), the elders in the experimental group (n=12) showed significant improvement in cognitive function and depression at week 12. The mean scores of IADL at week 12 were not significantly different between the groups. Conclusion: The results indicate that this multifaceted cognitive training program is effective in improving cognitive function, depression, and avoiding deterioration of IADL among elders with mild cognitive impairment.
