Purpose: The purpose of this study was to investigate the present status of falls among inpatients in general hospitals and to identify the fall risk factors that reflect the characteristics of domestic hospitals. Methods: Data were collected between December 15, 2016 and January 15, 2017 from 32 Korean hospitals having 500 or more beds. First, 42 risk factors were extracted based on literature review and expert opinions. Then the importance of each factor was evaluated by 223 nurses from medical and surgical adult ward and intensive care units in 40 hospitals. Results: The incidence rate of falls in 18 hospitals was 3.87 per 1,000 total discharged patients and 0.55 per 1,000 patient-days, and the rate of injury-related falls was 40.5%. Major risk factors for falling were identified as the following: being over 65 years of age, history of falls during admission, physical mobility disorders requiring assistance, physical factors (dizziness or vertigo, unstable gait, general weakness, walking aids, visual problems), cognitive factors (delirium, lack of understanding on limitations), neurological disease, CNS medications Conclusion: The findings provide information that can be in the development of a fall risk assessment tool for inpatients in general hospitals in Korea.

Obsessive compulsive disorder is a debilitating condition characterized by recurrent obsessive thoughts and compulsive reactions. A great portion of the obsessive compulsive disorder (OCD) patients are managed successfully with psychiatric treatment such as selective serotonin-reuptake inhibitor and cognitive behavioral psychotherapy, but more than 10% of patients are remained as non-responder who needs neurosurgical treatments. These patients are potential candidates for the neurosurgical management. There had been various kind of operation, lesioning such as leucotomy or cingulotomy or capsulotomy or limbic leucotomy, and with advent of stereotaxic approach and technical advances, deep brain stimulation was more chosen by neurosurgeon due to its characteristic of reversibility and adjustability. Gamma knife radiosurgery are also applied to make lesion targeting based on magnetic resonance (MR) imaging, but the complication of adverse radiation effect is not predictable. In the neurosurgical field, MR guided focused ultrasound has advantage of less invasiveness, real-time monitored procedure which is now growing to attempt to apply for various brain disorder. In this review, the neurosurgical treatment modalities for the treatment of OCD will be briefly reviewed and the current state of MR guided focused ultrasound for OCD will be suggested.
This study was to identify HCl design factors for increasing information processing and attention for mentally retarded children. Several factors such as size, location, moving distance were varied for three experiments operated by the amount of information. The results showed that the larger size of target, the greater moving distance of target, and the less amount of information increased mentally retarded children's sensitivity. Also, when the target was displayed at upper left of computer screen, sensitivity of mentally retarded children was high, compared other locations of screen.
Neurofeedback, also known as EEG biofeedback or neurotherapy, is a new therapy differentiated from biofeedback in that brain waves are monitored to study brain itself as well as its functions. Needless to say, it has originated from biofeedback, however it has established itself as an independent and specialized therapy in the field of neurotherapy. In order to have a better understanding of neurofeedback we are going to have a look into its historical and theoretical background first and then we will explain how it can be used clinically. Neurofeedback was clinically introduced for epilepsy for the first time and its effect was proved. Since then it has been used successfully for a number of psychiatric disorders, such as attention deficit hyperactivity disorder(ADHD), anxiety, anxiety and mood disorder, sleep disorder, substance dependency, etc. Furthermore, it has been used for non-psychiatric or psychosomatic symptoms, such as headache, chronic pains, chronic fatigue symptoms, etc. Recently, it has been also used for non-medical areas, such as improvement of peak performance for sports and art activities, enhancement of cognitive function, etc, making the neurofeedback a very promising field for its potential and effectiveness.

The purpose of this study was to evaluate the effectiveness of acupuncture treatment for the symptoms of Alzheimer's disease (AD). There were 11 databases searched for randomized controlled trials using acupuncture treatment for AD. The risk-of-bias assessment tool of the Cochrane Library was used to evaluate the quality of each study. Using the Review Manager (RevMan), a meta-analysis was performed using risk ratio, mean difference, 95% confidence interval, and random effect model. There were 32 studies selected, all of which were conducted in China. There were 16 studies where acupuncture was used as the intervention, and 23 studies used Donepezil as the control group. Acupoint GV20 was most frequently selected during the treatment. The Mini Mental State Examination (MMSE) was the most frequently used outcome variable. A period of 12 weeks was the most common treatment duration. As a result of meta-analysis (n = 25), acupuncture improved the MMSE and activities of daily living scores. Electroacupuncture improved the Alzheimer Disease Assessment Scale-Cognitive Subscale, and scalp acupuncture improved the MMSE score for AD. Acupuncture alleviates the symptoms of AD. However, further research is necessary to provide a better level of evidence.
Purpose: This study was done to identify the patterns and significant predictors influencing hospitalization of Korean men for alcohol use disorder. Methods: A descriptive study design was utilized. Data were collected using self-report questionnaires from 143 inpatients who met the DSM-5 alcohol use disorder criteria and were receiving treatment and 157 social drinkers living in the community. The questionnaires included Alcohol Use Disorders Identification Test (AUDIT), Alcohol Problems, Alcohol Expectancy Questionnaire (AEQ), Life Position, and The Korean version of the Children of Alcoholics Screening Test (CAST-K). Data were analyzed using descriptive statistics, t-test, ${\chi}^2$-test, F-test, Pearson correlation coefficients, and logistic regression with forward stepwise. Results: AUDIT had significant correlations with alcohol problems, alcohol expectancy, and parents' alcoholism. In logistic regression, factors significantly affecting hospitalization were divorced (OR=4.18, 95% CI: 1.28-13.71), graduation from elementary school (OR=28.50, 95% CI: 8.07-100.69), middle school (OR=6.66, 95% CI: 2.21-20.09), high school (OR=6.31, 95% CI: 2.59-15.36), drinking alone (OR=9.07, 95% CI: 1.78-46.17), family history of alcoholism (OR=2.41, 95% CI: 1.11-5.25), interpersonal relationship problems (OR=1.28, 95% CI:1.17-1.41), and sexual enhancement of alcohol expectancy (OR=0.83, 95% CI: 0.72-0.94), which accounted for 53% of the variance. Conclusion: Results suggest that interpersonal relationship programs and customized cognitive programs for social drinkers in the community are needed to decreased alcohol related hospitalization in Korean men.
As the population ages, health problems of the elderly, including functional disorders and cognitive impairment, have become some of the most important social issues in Korea. The purpose of this study is to discover the rate of prevalence and factors related to dementia among the elderly in Jeju Province. 2405 elderly in Jeju Province were interviewed, using a questionnaire consisting of sociodemographic characteristics: health behavior, quality of life, and MMSE-K. Prevalence of dementia was 9.4%(11.7% of females, 4.1% of males). Factors related to dementia, analyzed by multiple logistic regression, were age, working status, subjective health status, social support, and subjective economic status. In community health care for the elderly, factors relating to dementia have to be considered. When planning community health care, priority should be given to the elderly who: need care but live alone; lack social support; have a low standard of living; believe they are not in a good state of health; or whose life satisfaction is low.

Objective : This study was conducted to examine the relationship between the annual changes of suicide methods and suicide rates in South Korea. We compared the suicide methods in South Korea with those in six other OECD countries, Japan, Finland, France, United States, Canada and Australia. Methods : Data of suicide rates and methods since the year 2000 were obtained from the official electronic mortality data published by the seven countries and from the World Health Organization [WHO] mortality database. We compared the correlation between suicide methods and suicidal rates in South Korea using Spearman's correlation test. Results : The proportion of hanging steadily increased, whereas self-poisoning steadily decreased from 2000 to 2011. The increased use of hanging was associated with the increase of suicide rates in South Korea. The preference for jumping as a suicide method in South Korea can be notably compared to that of other countries. Conclusion : Preference use of lethal methods, such as hanging, may play a great role in the increase of suicide rates in South Korea. The limitation of physical and cognitive access to lethal methods can be an effective suicide prevention strategy.

Systemic lupus erythematosus(SLE) is an autoimmune disease which may affect many different organs and disclose various clinical manifestations. Recently central nervous system(CNS) involvement has been recognized as an increasingly significant contributor to morbidity and mortality of SLE. The clinical manifestations of CNS-lupus are highly variable and range from mild cognitive dysfunction, movement disorder, headache, psychosis to life-threatening stroke and coma. Among the neuropsychiatric disorders encountered in patients with SLE, cerebrovascular disease has been a relatively rare complication. The diagnosis and management of CNS-lupus is difficult because of the lack of useful diagnostic methods. If cerebrovascular involvement is suspected, aggressive treatment such as high dose steroid, immunosuppressive therapy, plasma exchange may be required to reduce high mortality rate. We experienced 2 cases cerebrovascular dis eases occurring in SLE patients which presented with various neuropsychiatric manifestations. They were diagnosed as CNS-lupus by neuropsychiatric symptoms, brain MRI, and BEG, and showed good response to high dose steroid pulse therapy.

Objective : The subgenual cingulate cortex, a part of default-mode network, has been known to playa key role in the pathophysiology of depression. The previous studies have reported abnormal functional connectivity between the subgenual cingulate cortex and other brain regions in the patients with depression. The goal of this shldy was to explore the resting-state functional connectivity of the subgenual cingulate cortex between the patients with depression and healthy subjects. Methods : Twenty patients with major depression and age- and sex-matched 20 healthy subjects underwent 5-minute resting state fMRI scans. The functional connectivity map in each subject was acquired using seed-based correlation analysis with the seed located in the subgenual cingulate cortex (Talairach coordinates; x=-10, y=5, z=-10). The functional connectivity maps were calculated using AFNI and compared between the patient and healthy subject group via two-sample T-test using 3dttest++ in AFNI package. Results : Functional connectivity was decreased between the subgenual cingulate cortex and both sides of fusiform gyrus in depressed subjects. Connectivity was also decreased between the subgenual cingulate cortex and the left cerebellum in the patient group. There was no correlation between the severity of depression and the degree of functional connectivity between the subgenual cingulate cortex and the regions showing decreased functional connectivity. Conclusion : Decreased resting-state functional connectivity between the subgenual cingulate cortex and both sides of fusiform gyrus, and decreased connectivity between the subgenual cingulate cortex and the left cerebellum found in the patients with major depression in comparison to the healthy subjects might be related to abnormal emotional and cognitive processing of depressed patients.
