CLINICAL CONSIDERATION ON USING THE ELASTIC 'TIE BACKS' DURING SPACE CLOSURE ('Elastic tie back'을 이용한 발치공간 폐쇄에 관한 임상적 고려)

  • Preadjusted appliance, following the original concept of the Andrews Straight-Wire appliance, became increasingly common in the 1980s. In six phases of treatment, anchorage control, leveling and aligning, overbite control, overjet reduction, space closure, and finishing are very effective with using the preadjusted appliances. Space closure is the phase of treatment in which the difference between standard edgewise and preadjusted mechanics is most noticeable. Orthodontists have been able to reduce the use of closing loops and, because of the level slot lineup, enjoy the advantages of sliding mechanics. In 1990, Dr. John C. Bennett and Richard P. McLaughlin introduced the new space closure system, namely, elastic 'tiebacks'. They found an $.019'\times.025'$ working archwire most effective in an .022'-slot system. Hooks of .024' stainless steel or .028' brass wire are soldered to the upper and lower archwires. The force required for space closure is delivered by elastic 'tiebacks'. An elastic modulo stretched by 2-3mm(to twice its normal length) usually delivers 0.5-1.5mm of space closure per month. Group movement and sliding mechanics are combined for gentle, controlled space closure, so that about 0.5mm of incisor retraction and 0.5mm of mesial molar movement can be seen each month. The tiebacks are replaced every four to six weeks. By using the elastic 'tiebacks', the next two cases were treated during space closure. Even though we found some clinical problems of this mechanics, long treatment time, hard to control of vertical dimension and anchorage, the application method of this system is so simple that orthodontists can manage many patients during short chair time. But we must apply this mechanics after perfect understanding of the biomechanics in tooth movement.

  • Crack tip displacement is originated by tensile stress component, s and shear stress component, t on pure Mode I and pure Mode II. The crack tip displacement(CTD) depends on combined types of different two stress components under mixed-mode loading conditions (MMLC). Thus, the analysis of crack tip displacement must be CTD vector, dv which is composition of ds and dt under MMLC. In this paper, various effects of MMLC on the crack closure are studied experimentally. The crack closure magnitude is calculated from the information of crack tip displacement under MMLC. This information has been obtained from the high resolution optical microscope in direct observations of crack displacement behavior at the crack tip. Observed crack tip displacement is analyzed by using CTD vector to determine crack opening load. The various effects of MMLC on the crack closure are explained using crack opening ratio with crack length and mode mixture. The effective stress intensity factor considering crack closure is also discussed.

  • A new low Reynolds number nonlinear second moment turbulence closure was introduced to analyze a square sectioned 180.deg. bend flow. Inclusion of nonlinear return to isotropy term and cubic mean pressure strain term has brought out a marked improvement in the level of agreement with measured velocity profiles. Optimization of present closure was performed by comparison of computed velocity profiles with the experimental ones with variation of nonlinear return to isotropy term and quadratic and cubic pressure-strain model. Progressive vortex breakdown due to the interaction of primary and secondary flows was well captured by using the optimized second moment turbulence closure.

  • PURPOSES : This study supports the evidence that it is possible to rehabilitate a damaged pavement with a lane closure specifically based on the Gimcheon~Sunsan project. METHODS : The prediction results from the simulation programs were compared with field monitoring, which focused on traffic management planning, congestion (length, time, and passing speed), bypass, and user cost, among others. RESULTS : The research results showed that lane closure application and pavement repair of the aged pavement in Korea were possible, even though the prediction results were minimally different from the field monitoring. The road agency contributes to service life extension of the rehabilitated pavement using this method. CONCLUSIONS : A marginal effect caused by the lane closure was observed on travelling users or vehicles, and the user cost of pavement repair decreased. Therefore, introducing the repair method or rehabilitation in Korea is possible. Information dissemination through various media was properly done to execute the project well. Moreover, the construction area traffic utilized nearby alternative roads. Therefore, improving the repaired pavemen's service life while ensuring that the pavement management agency can provide a road with comfortable user riding quality was possible.

An Analysis of the Variation in the Settling Properties of Cohesive Sediments before and after Closure of the Saemankeum Seadike (새만금 끝막이 공사 전후의 점착성 퇴적물의 침강특성 변화 해석)

  • The primary purpose of this study was to quantitatively examine whether there was a difference in the settling velocities of cohesive sediments in the Saemankeum artificial lake before and after the closure of the Saemankeum seadike. Through in situ sediment sampling and laboratory experiments, the settling velocities were estimated as a function of the suspension concentration and their physico-chemical properties, such as grain size distribution, the percentage of organic contents, mineralogical composition, etc. were also examined. In this study, their inter-relation with settling velocities were also analyzed qualitatively. The result of the settling tests for Saemankeum sediments showed that there was a big difference in the settling velocities before and after the closure of the Saemankeum seadike. Its settling velocities in a flocculated settling region became remarkably larger after the closure compared to those before the closure, while they were similar but relatively smaller in a hindered settling region. This was found to be mainly due to the difference in their grain size.

Closure Duration and Pitch as Phonetic Cues to Korean Stop Identity in AP-medial Position: Perception Test

  • The present study investigated some perceptual phonetic attributes of two Korean stop types, aspirated and lax, in medial position of an accentual phrase. The intonational pattern across syllables (Jun, 1993) is argued to depend on the type of stop (aspirated vs. lax) only in the initial position of an accentual phrase. In Kang & Dilley (2007), we showed that significant differences between aspirated and lax stops in medial position of an accentual phrase exist in closure duration, voice-onset time, and fundamental frequency (F0) values for post-stop vowels. In the present perception experiment, we investigated whether these phonetic attributes contribute to the perception of these two types of stops: The closure durations and/or F0's of post-stop vowels on accentual-phrase medial words were altered and twenty native Korean speakers then judged these words as beginning with an aspirated or lax stop. Both closure duration and F0 significantly affected judgments of stop identity. These results indicate that a wider range of acoustic cues that distinguish aspirated and lax Korean stops in production also plays a role in perception. To account for these results we suggest some phonetic and phonological models of consonant-tone interactions for Korean.

The Experimental Study on the Intraoral Pressure, Closure Duration, and Voice Onset Time(VOT) of Korean Stop Consonants (한국어 파열자음의 인두내압, 폐쇄기 및 Voice Onset Time(VOT)에 관한 실험적 연구)

  • A study to get the data of normal stop consonants production as preliminary study for cleft palate patients was performed. Normal Korean native 21 speakers were pronounced $VCV(V=/a/,C=/P/,/{P^1}/,/P^h/,/t/,/{t^1}/,/{t^h}/,/k/,/{k^1}/,/{k^h}/)$ syllables with natural speech rate and intensity. With intrapharyngeal pressure waveforms by pressure sensor in oropharyngeal cavity, amplitude and time duration of intrapharyngeal pressure were analyzed, and with acoustic waveforms and spectrograms, closure duration and VOT were analyzed. As results, the highest amplitude of intrapharyngeal pressure showed in alveolars and heavily aspirated consonants. Velars and unaspirated consonants were higher than bilabials and slightly aspirated ones each, in intrapharyngeal pressure. Bilabilas, alveolars and velars showed similar rise time of intrapharyngeal pressure build-up, but in decay time and total duration time, bilabials were slightly shorter than alveolars and velars, with no statistic significance. In the aspects of tensity, unaspirated consonants showed the longest rise time, heavily aspirated, the second, and slightly aspirated consonants, the shortest, which were also seen in decay time and total duration time. In closure duration, slightly aspirated consonants had the shortest closure duration, and the heavily aspirated ones, the second, and unaspirated consonants showed the longest. In VOT, heavily aspirated consonants showed the longest, slightly aspirated ones, the second and unaspirated ones showed the shortest.

  • Mediastinitis and sternal wound infection frequently occurred after corrective surgery for complex heart anomalies. Most of the patients are neonate or infant and they have showed low oxygen tension. For that poor condition, application of invasive treatment of mediastinitis is not appropriate and recovery takes a long time. We successfully treated a mediastinitis after Norwood operation using vacuum assisted closure. So we introduce the technique and the modification of neonatal vacuum assisted closure.

  • Ruptured aneurysms of the sinus of Valsalva are relatively rare, and the incidence seems to be higher in oriental than in western countries. Eight patients underwent operative treatment at Chonnam University Hospital from June, 1986 to May, 1992. Six of the patients were male and two female. Age ranged from eight to fifty six years. Associated cardiac lesions were common including AR and VSD in four patients respectively. Diagnosis was made by 2D-Echo and cine-angiogram. In six patients aneurysms of the sinus of valsalva ruptured from the right coronary sinus to the right ventricle and in two from right coronary sinus to the right atrium Direct closure of aneurysmal rupture and patch closure of VSD in four cases, resection of the aneurysm and direct closure in one case, direct closure of the fistula and AVR in two cases, direct closure in one case were performed. One patient combined with VSD, pulmonary hypertention and bacterial endocarditis underwent operation, but he died of sudden cardiac arrest the day after the operation. Operative results were relatively good in the other patients.

Evaluation of echocardiographic markers in dogs with patent ductus arteriosus after ductal closure

  • This study evaluated several known echocardiographic markers related to the assessment of severity in dogs with patent ductus arteriosus (PDA) after the closure of ductus arteriosus (DA). Forty-two dogs with patent ductus arteriosus were enrolled in this study. Evaluated echocardiographic markers were left atrial to aortic root ratio, left ventricular end-diastolic dimension to aortic root ratio, indexed left ventricular end-diastolic and end-systolic dimensions, end-diastolic and end systolic volume index, pulmonic flow to systemic flow (Qp/Qs) ratio, velocities of pulmonary regurgitant and systolic jets, pulmonary flow profiles and the presence of mitral regurgitation. Those markers were evaluated before, 1 day, and 30 days after the closure of DA. Statistically significant changes in some echocardiographic markers (i.e., Qp/Qs) were observed. Although several studies in human and dogs have evaluated the clinical outcome of PDA occlusion using several echocardiographic markers, this study has firstly evaluated all echocardiographic markers known to be useful for assessing the clinical outcome of PDA occlusion in human, and has demonstrated that those markers including the Qp/Qs and pulmonary flow profiles were useful in evaluating of clinical outcome of PDA in dogs and the reduction of LA and LV preload after ductal closure could dramatically reduce after successful ductal occlusion of PDA in dogs.
