Purpose: The purpose of this research was to describe clonorchis sinensis infection experience in high risk populations living in riverside areas. The research question was "How do local residents perceive Clonorchiasis and how are they infected with the parasite". Methods: Qualitative data were collected by focus group interviews with 16 participants from January to February, 2009. All the interviews were tape-recorded, transcribed, and analyzed by the content analysis method. Results: Five main categories were conceptualized, which were "lacking in understanding of Clonorchiasis," "culture of the rural community," "life style," "recovering experience from infection" and "change of health behavior." Conclusion: The result of this study indicates that residents are infected with Clonorchiasis through interaction between individual and group risky factors. Therefore, it is important to develop effective health education programs on both individual and group levels to prevent infection with Clonorchiasis.
The present study aimed to evaluate control efficacy of clonorchiasis by two schemes of repeated treatment with praziquantel at two endemic villages in China. Residents of one village at Guangxi Autonomous Region were treated and examined 6-monthly and of another at Liaoning Province 12-monthly, In residents that took 25 mg/kg x3 (total 75 mg/kg) of Praziquantel every 6 months for one Year the e99 Positive rate showed a significant drop from 69.0% to 17.1%. In contrast, a group of same praziquantel medication once showed a slight marginal decrease in the egg rate from 18.9% to 12.2% after one year Of 39 subjects examined 3 times, 56.4% were cured, 7.7% persistently positive, one (2.6%) reinfected after cure or newly infected, but 25.6% were persistently negative. The present finding suggests that 6-monthly medication with 75 mg/kg of praziquantel should effectively lower the prevalence but incomplete for control of clonorchiasis in heavy endemic areas.
Purpose: This study was done to investigate the knowledge, attitude and prevalence status of Clonorchiasis Sinensis (C.S.) for inhabitants in the Nakdong riverside area. Method: Data were collected from December 16, 2008 to January 10, 2009. Participants were 552 people who responded to a questionnaire and had a stool examination. Results: The overall prevalence of C.S. was 13.8%. As for knowledge of C.S., the Clonorchiasis group had a mean score of 10.78 $({\pm}3.54)$ on a 17-point scale compared to a score of 10.58 $({\pm}4.15)$ for the negative group. The intended behavioral change related to risk of infection, according to presence of infection or not, was not significant but the Clonorchiasis group had a greater desire for behavioral change intention than the negative group. Conclusion: These results suggest that prevalence investigation of C.S. in all areas in the Nakdong riverside is needed and indicates that it is necessary to set up persistent and systematic management programs for patient care and prevention of C.S..

Purpose: This study attempted to investigate the prevalence and related factors of Clonorchiasis among five major riverside residents in South Korea. Methods: This study is descriptive research, nationwide survey, and the subjects are 23,492 residents selected by convenience sampling. Data collection was conducted between March 1 and June 30, 2011, and stool collection and questionnaire survey were conducted by affiliated public health centers in 38 cities and Gun's. Results: The prevalence rates of Clonorchiasis in the five major riversides were as follows: the Guem River 15.2%; the Nakdong River 11.9%; the Seomjin River 10.9%; the Han River 5.7%; and the Yeongsan River 3.9%. The prevalence rates were shown to be significantly high among people who had highly frequent experiences of eating and cooking freshwater raw fish, were diagnosed with liver and/or biliary tract diseases, and drank less than once a month. Conclusion: These results suggest that it is necessary to improve awareness of Clonorchiasis and provide intensive public health education for the riverside residents. And the target groups should be set up by reflecting the characteristics of at-risk groups, and it is necessary to prepare customized strategies for prevention and management of Clonorchiasis.

Chemotherapy of clonorchiasis with praziquantel (PZQ) is effective but about 15% of treated cases have been reported uncured. The present study investigated correlation of single nucleotide polymorphisms (SNPs) of the cytochrome P450 gene, CYP3A5 and cure of clonorchiasis. A total of 346 egg passing residents were subjected and treated by 3 doses of 25 mg/kg PZQ. Reexamination recognized 33 (9.5%) uncured and 313 cured. Numbers of eggs per gram of feces (EPGs) before treatment were significantly lower in the cured group than in the uncured group ($2,011.2{\pm}3,600.0$ vs $4,998.5{\pm}7,012.0$, P<0.001). DNAs of the subjects were screened for SNPs at 7 locations of CYP3A5 using PCR. In the uncured group, the SNP frequencies at g.-20555G > A and g.27526C > T of CYP3A5 were 15.2% and 9.1% while those were 3.8% and 1.0%, respectively, in the cured group. The cure rate was Significantly lower in the cases with SNP at g.27526C > T and EPGs ${\geq}$ 1,000. In conclusion, EPGs and SNPs of CYP3A5 are factors which influence cure of clonorchiasis by PZQ therapy. It is strongly suggested to recommend 2-day medication for individuals with high EPGs ${\geq}$ 1,000.
The Kato-Katz (KK) method is a well-known method of fecal examination for helminthiases. Its diagnostic sensitivity was found very high for clonorchiasis. The present study evaluated the correlation of Clonorchis sinensis egg counts by the KK method with those by direct smear and formalin-ether (FE) technique. The egg counts obtained by the KK method (Y) were correlated with the counts by direct smear (X) with the equation of Y = 659.4 + 0.266X ($r^2$ = 0.738), but not with those by the FE method. The present study demonstrated that the KK method and direct smear were useful for both qualitative and quantitative diagnosis of clonorchiasis, especially in the field.

A survey of an endemic locality at Songjung Ri, Ogok Myun, Goksung County, South Cholla Province for the prevalence of Clonorchis sinensis and Metagonimus yokogawai was primarily done on 53 residence before medication using formalin-ether concentration method of stool examination. After praziquantel medication of all the clonorchiasis patients, 64 humans consisting 82.1% of total inhibitants and including 84.9% of primarily surveyed residents were secondly surveyed after 2-year post-treatment to evaluate the prevalence and reinfection. The prevalence of clonorchiasis was decreased from 35.8% before medication to 10.9% after medication(P<0.01), indicating this disease was not satisfactorily controlled although the rate was significantly lowered. The prevalence of metagonimiasis was dropped from 17.0% to 6.3% on post-treatment. In clonorchiasis cases, sex ratio showed no difference in both pre- and post-treatment, and mean age was the sixties after medication from the forties and fifties before medication, suggesting more aged people of both sexes were infected with this fluke. The reinfection of clonorchiasis and metagonimiasis following 2-year post-treatment was 15.8% and 25.0%, respectively. The proportion of reinfection among the egg positive cases was 50.0% in clonorchiasis and 100% in metagonimiasis. These findings mean that a high fraction of clonorchiasis and metagonimiasis was reinfected with these trematodes. In conclusion, to eradicate the C. sinensis and M. yokogawai in an endemic area, it should be preceded to control the reinfection of these parasites.

Clonorchis sinensis is currently the most important parasite affecting public health problems in the Republic of Korea. We investigated the prevalence of C. sinensis infection among residents living along 5 major rivers in Korea. A total of 42,562 individual stool samples were collected from 37 localities and examined using the formalin-ether sedimentation technique. Helminth eggs were detected in 4,052 (9.5%) residents and 3,586 (8.4%) were infected with C. sinensis. The egg positive rate of C. sinensis in Nakdong, Seomjin, Geum, Yeongsan, and Han River was 11.7%, 9.9%, 6.5%, 3.1%, and 1.0%, respectively. The overall prevalence of clonorchiasis by sex was 11.2% in males and 6.2% in females. The age-prevalence was the highest in the 50-59 years band. It has been reconfirmed that the endemicity of clonorchiasis is higher in southern areas of Korea, especially along Nakdong and Seomjin Rivers. A combination of continuous control programs with health education initiatives is urgently required in these highly endemic areas of clonorchiasis in Korea.
To evaluate the status of clonorchiasis and metagonimiasis of the inhabitants near Talchongang (River) in Chungwon-gun, Chungchongbuk-do, the stools of 67 inhabitants were examined by formalin-ether sedimentation method from August to September, 1993. Also freshwater knish caught in Talchongang were examined by slide compression method. The e99 positive cases of Clonorchis sinenis and Metasonimus sp. were 22 (32.8%), and 14 (20.9%), respectively. Of 17 species of examined fish, 14 species were infected with C. sinensis and 13 species with Metagonimus sp. The adult worm collected from 2 patients after treatment with praziquantel was Metosoninur Miyata type. Also the adult worm obtained from the experimental mice infected with metacercariae from Zacco plntwpus was Metafonimus Miyata type. We found the highly endemic area of clonorchiasis and metagonimiasis along Talchongang.

We evaluated the status of Clonorchis sinensis infection and potential risk factors among residents of riverside areas (Geumgang) in Muju-gun, Jeollabuk-do (Province), Korea. From January to February 2010, a total of 349 (171 males, 178 females) stool samples were collected and examined by the formalin-ether concentration technique. Also, village residents were interviewed using questionnaires to obtain information about C. sinensis infection-related risk factors. Overall egg-positive rate of C. sinensis was 13.2%. Egg-positive rates were significantly higher in males, farmers, and residents who had lived there more than 20 years, and in residents who had eaten raw freshwater fish than in opposite groups, respectively. However, there was no significant difference between age groups, education levels, cigarette smoking, alcohol drinking, health status, past history of infection, and experience of clonorchiasis medication and examination. Logistic regression analysis was performed to determine risk factors for clonorchiasis. On univariate analysis, the odds ratios for males, farmers, those who had lived there more than 20 years, and who had eaten raw freshwater fish were 2.41, 4.44, 3.16, and 4.88 times higher than those of the opposites, respectively. On multivariate analysis, the odds ratio of residents who had eaten raw freshwater fish was 3.2-fold higher than that of those who had not. These results indicate that residents living in Muju-gun, along the Geum River, Korea, have relatively high C. sinensis egg-positive rates, and the habit of eating raw freshwater fish was the major factor for the maintenance of clonorchiasis.
