Delayed Bilateral Abducens Nerve Palsy after Head Trauma

  • Although the incidence of unilateral abducens nerve palsy has been reported to be as high as 1% to 2.7% of head trauma cases, bilateral abducens nerve palsy following trauma is extremely rare. In this report, we present the case of a patient who developed a bilateral abducens nerve palsy and hypoglossal nerve palsy 3 days after suffering head trauma. He had a Glasgow Coma Score (GCS) of 15 points. Computed tomography (CT) images demonstrated clivus epidural hematoma and subarachnoid hemorrhage on the basal cistern. Herein, we discuss the possible mechanisms of these nerve palsies and its management.

Selection of Superior Poplar and Willow Clones in Growth Performance and Adaptation Abilities at Sudokwon Landfill Site (수도권매립지에서 생장과 적응력이 우수한 포플러류 및 버드나무 클론 선발)

  • Poplars and willow were planted to identify suitable species and varieties for landfill reclamation at the Sudokwon Landfill Site in 1997. Survival rate, growth performance, vitality, visible foliar injury by pollutants, fungi, and leaf insects, and stem borer damage have been investigated for 10 clones of 4 poplar species and 2 clones of one willow species from 1997 to 2005. The average survival rates of poplar and willow clones were drastically decreased from 90% in 1997 to 53% in 2005. Among poplar species, Populus alba ${\times}$ P. glandulosa showed the highest mean survival rate of 66%, while Populus koreana ${\times}$ P. nigra var. italica and Populus euramericana were the lowest of 41%, respectively in 2005. Clivus, which is one of the clones from Populus alba ${\times}$ P. glandulosa, showed the highest survival rate of 73%. For mean height, Ec028 clone(P. euramericana) showed the highest of $11.2m{\pm}2.1m$ and followed by Clivus of $11.0m{\pm}2.0m$. Clone 131-27(Salix alba) was the lowest of $7.8m{\pm}1.6m$. Vitality, defoliation, visible foliar damage, and stem borer damage were significantly different among clones. Most of Populus alba ${\times}$ P. glandulosa and Salix alba clones seemed to have strong vitality and to be tolerant to various stresses at the site. However, Populus nigra ${\times}$ P. maximowiczii was sensitive to the stress. We have selected 5 clones in total: Clivus as the best clone for waste landfill reclamation, and additionally two Salix clones 131-25, 131-27 and two clones of Populus alba ${\times}$ P. glandulosa (72-9, 72-16) have been selected. These five clones could be supplied for planting at sites having an environment similar to the Sudokwon Landfill Site.

  • Objective : In this study, we aimed to investigate the underlying ethiological factors in chiari malformation (CM) type-I (CMI) via performing volumetric and morphometric length-angle measurements. Methods : A total of 66 individuals [33 patients (20-65 years) with CMI and 33 control subjects] were included in this study. In sagittal MR images, tonsillar herniation length and concurrent anomalies were evaluated. Supratentorial, infratentorial, and total intracranial volumes were measured using Cavalieri method. Various cranial distances and angles were used to evaluate the platybasia and posterior cranial fossa (PCF) development. Results : Tonsillar herniation length was measured $9.09{\pm}3.39mm$ below foramen magnum in CM group. Tonsillar herniation/concurrent syringomyelia, concavity/defect of clivus, herniation of bulbus and fourth ventricle, basilar invagination and craniovertebral junction abnormality rates were 30.3, 27, 18, 2, 3, and 3 percent, respectively. Absence of cisterna magna was encountered in 87.9% of the patients. Total, IT and ST volumes and distance between Chamberlain line and tip of dens axis, Klaus index, clivus length, distance between internal occipital protuberance and opisthion were significantly decreased in patient group. Also in patient group, it was found that Welcher basal angle/Boogard angle increased and tentorial slope angle decreased. Conclusion : Mean cranial volume and length-angle measurement values significantly decreased and there was a congenital abnormality association in nearly 81.5 percent of the CM cases. As a result, it was concluded that CM ethiology can be attributed to multifactorial causes. Moreover, congenital defects can also give rise to this condition.

The Comparative Morphometric Study of the Posterior Cranial Fossa : What Is Effective Approaches to the Treatment of Chiari Malformation Type 1?

  • Objective : The objective of this study was to investigate changes in the posterior cranial fossa in patients with symptomatic Chiari malformation type I (CMI) compared to a control group. Methods : We retrospectively reviewed clinical and radiological data from 12 symptomatic patients with CMI and 24 healthy control subjects. The structures of the brain and skull base were investigated using magnetic resonance imaging. Results : The length of the clivus had significantly decreased in the CMI group than in the control group (p=0.000). The angle between the clivus and the McRae line (p<0.024), as the angle between the supraocciput and the McRae line (p<0.021), and the angle between the tentorium and a line connecting the internal occipital protuberance to the opisthion (p<0.009) were significantly larger in the CMI group than in the control group. The mean vertical length of the cerebellar hemisphere (p<0.003) and the mean length of the coronal and sagittal superoinferior aspects of the cerebellum (p<0.05) were longer in the CMI group than in the control group, while the mean length of the axial anteroposterior aspect of the cerebellum (p<0.001) was significantly shorter in the CMI group relative to control subjects. Conclusion : We elucidate the transformation of the posterior cranial fossa into the narrow funnel shape. The sufficient cephalocaudal extension of the craniectomy of the posterior cranial fossa has more decompression effect than other type extension of the craniectomy in CMI patients.

Craniovertebral Junction Tuberculosis with Atlantoaxial Dislocation : A Case Report and Review of the Literature

  • Craniovertebral junction (CVJ) tuberculosis is a rare disease, potentially causing severe instability and neurological deficits. The authors present a case of CVJ tuberculosis with atlantoaxial dislocation and retropharyngeal abscess in a 28-year-old man with neck pain and quadriparesis. Radiological evaluations showed a widespread extradural lesion around the clivus, C1, and C2. Two stage operations with transoral decompression and posterior occipitocervical fusion were performed. The pathological findings confirmed the diagnosis of tuberculosis. Treatment options in CVJ tuberculosis are controversial without well-defined guidelines. But radical operation (anterior decompression and posterior fusion and fixation) is necessary in patient with neurological deficit due to cord compression, extensive bone destruction, and instability or dislocation. The diagnosis and treatment options are discussed.

Giant Ventral Midline Schwannoma of Cervical Spine : Agonies and Nuances

  • Pure ventral midline giant schwannoma is an extremely rare entity. Spinal intradural extramedullary schwannomas commonly occur posterolateral or anterolateral to the spinal cord. A case of a pure midline ventrally situated giant pan cervical extramedullary schwannoma in an 18-year-old male patient with compressive myelopathy and sphincter involvement is presented. Spinal MR imaging showed a midline ventrally situated extramedullary tumor with severe spinal cord compression extending from clivus to C7 vertebra. It was resected through a posterolateral approach. Histology was consistent with a schwannoma. Post operative MR imaging showed no evidence of the tumor. The radiological features, pathogenesis and surgical strategies in management of these difficult tumors are discussed and the relevant literature is briefly reviewed.

  • 콜레스테롤 육아종은 콜레스테롤 결정이 주위조직에 이물질로 작용하여 육아성 반응 및 골파괴 반응을 유발시키는 질환으로 병리조직학적으로 많은 콜레스테롤 결정이 거대세포를 가진 만성적인 염증성 침윤속에 나타나는것을 특징으로 한다. 이 병변은 아주 드물지만 특징적인 방사선적소견, 병리조직학적 소견과 임상양상으로 쉽게 진단이 된다. 두부에서는 다양한 위치에서 발견되는데 특히 함기화가 잘 된 측두골, 유양동, 고실강에서 많이 보이며 전두골, 상악골, 협골, 인상측두골 및 후두개와에서도 보고되고 있으나 부비동에 발생하는것은 특히 드물다. 부비동에서 발생한 콜레스테롤 육아종의 발생기전은 환기장애설, 배출장애설 및 출혈설 등으로 보고되고 있다. 저자들은 최근 복시와 측두통을 호소하는 50세 여자환자에서 접형동에 발생한 콜레스테롤 육아종이 우측 상안와열 (superior orbital fissure)과 경사대(clivus)를 파괴하며 전교조 (Prepontine cistern)로 확장된 1례를 경험하였기에 임상 증세, 특징적인 전산화단층 및 핵자기공명소견과 병리조직학적인 소견을 문헌 고찰과 함께 보고하는 바이다.

Intradural Invasion of Extradural Clival Chordoma

  • Most chordomas show extradural extension and bone destruction. A 32-year-old man presented with neck pain and progressive paraparesis. He had been diagnosed a clival chordoma and underwent an operation seven years ago. Radiological studies revealed that the tumor was recurred in a retroclival area and invaded into intradural region. We removed the tumor by two staged operations. After surgery, satisfactory results were achieved.

  • Chronic invasive fungal sinusitis (CIFS) is a relatively rare disease which occurs in immunocompetent or immunocompromised patients. We report a case of CIFS in ethmoid sinus with concurrent fungal ball in maxillary sinus. After surgical debridement and antifungal therapy, CIFS recurred with submucosal invasion in nasopharynx and osteomyelitis of clivus. Although CIFS encompasses a slowly progressing invasive fungal disease but potentially aggressive. It should be noted that CIFS could progress and extend through submucosal route without evidence of mucosal lesion.

Experience and successful treatment of craniocerebral gunshot injury at a regional trauma center in Korea: a case report and literature review

  • Craniocerebral gunshot injuries is gradually increasing in the civilian population with a worse prognosis than closed head trauma. We experienced a case of craniocerebral gunshot injury which a bullet penetrating from the submandibular area into the clivus of a patient. The patient did not show any symptom. However, serial laboratory findings showed an increase in blood lead level. We removed foreign bodies without any problems using an endoscopic transnasal transclival approach. Due to the extremely low frequency, guidelines for definitive management of gunshot injuries have not been presented in Korea yet. We introduce our surgical experience of a craniocerebral gunshot injury with an unusual approach for removing intracranial foreign bodies.
