  • Objectives : This paper aims to understand the meaning and utilitarian values of 'Jongyong(從容)' in medicine by studying its meaning in the 『Huangdineijing』. Methods : Examples of 'Jongyong(從容)' usages in Chinese literature and classical texts were studied first. Next, the similarities and characteristics of its contexts in discourses between Leigong and Huangdi were examined, followed by further examination of the overall meanings of the six chapters, 「Shicongronglunpian(示從容論篇)」, 「Zhuzhijiaolunpian(著至敎論篇)」, 「Shuwuguolunpian(疏五過論篇)」, 「Zhengsishilunpian(徵四失論篇)」, 「Yinyangleilunpian(陰陽類論篇)」, 「Jiejingweilunpian(解精微論篇)」 and the verses that include 'Jongyong(從容).' Results : Generally 'Jongyong(從容)' describes a resilient attitude that responds appropriately and calmly to difficult external situations while maintaining one's character, or refers to one's general behavior. In the 『Huangdineijing』, 'Jongyong(從容)' is only mentioned in discourses between Leigong and Huangdi, which talks about ancient East Asian clinical medicine in general, its transmission and education. Here, the meaning of 'Jongyong(從容)' could be summarized as an expansion of analogical thinking[引比類], medical work that involves the physician's commitment(人事), and detailed measurement of yinyang difference in the body(刑法, 陰陽, 治數). Discussion : In ancient East Asian medicine, the meaning of 'Jongyong(從容)' is closer to measurement of the yinyang difference rather than differentiation of the five element analogy. It is a medical practice that measures the shape, volume, length, area, height, concentration etc. of the human body in numerical figures to allow for the most subtle comparison, which was an important aspect of the physician's work that needed to be demonstrated, educated and transmitted to later generations to enable continuity of the medical profession.

  • Objective: We conducted a survey to understand the current state of pharmacy services and pharmacists' thoughts in the future of the community pharmacy setting. Methods: A questionnaire was distributed to 229 pharmacists and gathered from 95 respondents. Results: We asked to pharmacists about what the ideal pharmacy duty should be in the next 10 years at the community pharmacy setting in Korea. For this question, the respondents said drug counselling (19.4%), dispensing (13.8%), long-term care of chronic disease and healthcare education (12.8%), and OTC counselling (11.5%). When asked about how much time they were spending doing certain tasks in the pharmacy, the main tasks were dispensing (23.5%), counselling (17.1%), prescription review (10.1%), and OTC counselling (5.5%). When asked about what the most important duty of a pharmacist was, medication counselling (45.4%) was the most important task that they identified and the reasons for not being able to fulfill this role properly was time shortage (78.9%) followed by the lack of counselling spaces, up-to-date knowledges, and focus on financial gains over patient counselling. Conclusion: Korean pharmacists are mainly focusing on dispensing in their daily work. Their basic responsibilities can be easily mechanized over the next few years, but the social expectations of disease-prevention and public health promotion both in current and future can not be replaced by such mechanical measures. Therefore, pharmaceutical services in Korea should be developed in more diverse and professional ways.

Methods : The study participants were 18 patients (14 males, 4 females; mean age=38.9 years), all of whom were assessed as meeting the DSM-IV-TR criteria for panic disorder. All participants attended one two-hour session of structured group cognitive behavioral therapy (CBT). Clinical symptoms was assessed before and eight weeks after the single therapy session using the Panic Disorder Severity Scale (PDSS). Results : Eight weeks after a single session of group CBT significant improvements were found in panic attack frequency, distress during panic attacks, severity of anticipatory anxiety, agoraphobic fear/avoidance, panic-related sensation fear/avoidance, impairment in work functioning, impairment in social functioning. Conclusions : One session of group CBT appears to be an effective treatment of panic disorder by reducing the severity of all symptoms assessed on the PDSS. An attempt should be made to replicate the findings of this pilot study in a larger and controlled, comparative clinical trial.

  • The objective of this study was to document how clinical dietitians working at tertiary hospitals spend their time based on several categories of activities using a time measurement study. The questionnaires were distributed to 14 tertiary hospitals, and dietitians answered by classifying their work activities into several categories such as general care, indirect care, direct care, outpatient care, and food service management. A total of 129 clinical dietitians replied and their answers were analyzed according to the categories of activities. The times spent on the categories are as follows: general care (76.7 mins/day, 14%), indirect care (228.4 mins/day, 35%), direct care (120.1 mins/day, 22%), outpatient care (61.5 mins/day, 11%), and food service management (99.0 mins/day, 18%). The total working hours for dietitians was 590.0 mins, which exceeds the standard working hours of 540.0 mins (9 hrs) a day. From this study, we found that clinical dietitians spent very limited time on direct care. Times spent on activities were different according to type of employment and food service. Internship dietitians spent their more time on general care (P<0.001) while irregular dietitians spent more time on outpatient care (P<0.05). In contracted managed food service hospitals, clinical dietitians spent significantly less time on food service management (P<0.001). Regardless of doctors' order and consultation fees, clinical dietitians performed more than 95 percent of free consultation to patients. Entry-level knowledge and skills of dietitians working at hospitals are very important for quality service, but it is equally important to create an administrative and social environment that encourages clinical dietitian to spend more time on direct patient care.

The participants for this study were 15 shift work nurses who were in the age of 25 to 35, the clinical experience of 2 to 14 years and the work department of ICU. ER. ward and delivery room. The data were collected from 2000 to 2003 by using interviews and observations. The contents of the interviews were tape-recorded and were drawn through repeated method. And then were analyzed into the concept, subcategories, and categories with the open coding process and axial coding was done to identify the relationships of the concepts and categories according to the paradigm models. The core category generated, which was a central phenomena of the exhaustion process. The causal condition is change events. The central condition of exhaustion were sorted as physical discomfort, decreasing vigor, psychological instability, feeling of sleeping desire, changing face impression and being heavy body. The intervening condition were discovered as social$\cdot$ psychology$\cdot$physical resist and positive$\cdot$negative interaction strategies. The consequences of the fatigue process is the short term exhaustion relief and long term residual exhaustion. The fatigue process of this study was 'break through exhaustion' of change event-exhaustion-resist-resolve intervention-adaptation. This study offers better understanding on fatigue process of shift work nurses and may facilitate more appropriate interventive strategies to support, information and knowledges according to fatigue process.
